Esophageal Stenosis dueto ectopic Trecheobronchial Remnants -2 Case Reports- (Ectopi Tracheobronchial Remnnants에 의한 식도협착증 수술 치험 2례)

  • Esophageal stenosis due to ectopic tracheobronchial remnants is an uncommon anomaly and only few cases were reported in the literature. We have experienced two case of esophageal stenosis due to ectopic tracheobronchial remnants and performed operative correction. The one patient was a five year old male with a chief complaint of swallowing difficulty from birth and the other was a twenty-three year old female with a slowly incresing symptom of dysphagia for twenty years. both of them were performed surgical correction by esophagectomy of the stenotic portion and esophago-gastrostomy with anti-reflux procedures. The resected specimens of the these two patients showed ectopic tracheobronchial chondroepithelial tissue withinthe esophageal wall histipathologically. Postoperative course was uneventful and have been in good condition without any problems.

Obviously Increasing Incidence Trend for Males but Stable Pathological Proportions for Both Genders: Esophageal Cancer in Zhongshan of China from 1970-2007

  • Objectives: To analyze esophageal cancer incidence and pathological data of Zhongshan in China in 1970-2007, and to provide scientific information for its prevention and control. Methods: From Zhongshan Cancer Registry esophageal cancer incident and pathological data were obtained. Pathological proportions and trends were calculated and analyzed. Results: Although there was a continuously and obviously increasing trend for male incidence rates in 1970-2007 in Zhongshan, squamous cell carcinoma (SCC) and adenocarcinoma (AD) incident proportions during 1990-2007 remained relatively stable. Moreover, SCC was the major pathological type, accounting for 70.6 percent of all new cases, while AD were relatively few and accounted for only 2.66 percent throughout the period. Conclusion: The male esophageal cancer incident pattern in Zhongshan in 1970-2007 was quite different from most other domestic areas. The data suggest that etiological analysis should be enhanced for improved control in Zhongshan.

Palliative Intubation in Advanced Esophageal Cancer [including esophagorespiratory fistulas] Using Celestin Tube - Report of 6 Cases - (Celestin Tube 를 이용한 진행된 식도암환자에서의 고식적 수술치료 - 6례보고-)

  • Dysphagia is common symptom in patients with advanced esophageal cancer, which is not to be resected surgically. Especially when esophagorespiratory fistula is complicated, it leads to rapid deterioration and death due to pulmonary infection. Esophageal intubation relieves dysphagia as simple surgical execution and offers rapid effectiveness. For six patients with inoperable esophageal cancer including three esophago-respiratory fistulas, the palliative esophageal intubation was performed in Korea Cancer Center Hospital, in 1988. Traction technique via high gastrotomy with Celestin tube was used. Adequate palliation of dysphagia was achieved in 5 patients, but wound infection was developed in 2 patients, tube migration in 2 patients, and 2 died in hospital due to sepsis on the 16th and 42nd postoperative day, respectively. In 3 patients with esophagorespiratory fistula complicated after radiation therapy, the intubation was performed urgently and the result was satisfactory in 2 of them that the fistula was occluded successfully and aspiration or pulmonary infection was prevented.

Quantitative Analysis of Esophageal Transit of Radionuclide in Patients with Dermatomyositis-Polymyositis (피부근염-다발성근염 환자에서 방사성 동위원소를 이용한 식도 스캔의 정량적 분석)

  • Esophageal transit of radionuclide was quantitatively analyzed in 29 patients with dermatomyositis-polymyositis. Fourteen patients (48.3%) showed retention of tracer in oropharynx. The mean value of percent retention of oropharynx was $15.5{\pm}16.6%$. Esophageal dysfunction was found in 19 patients (65.5%). Among them 4 showed mild, 12 showed moderate and 3 showed severe esophageal dysfunction. Dysphagia was found in 11 patients (37.9%), which was closely related to percent retention of oropharynx.Quantitative analysis of esophageal transit of radionuclide seemed to be a useful technique for evaluation of dysphagia in patients with dermatomyositis-polymyositis.

  • The repair of esophageal atresia with a long gap continues to pose difficulties for the surgeon. There is a general agreement that the child's own esophagus is the best, however, primary repair is not always possible. Foker JE et al. (1997) developed a technique of esophageal lengthening using external traction sutures. We successfully treated one patient with a 4.5cm gap esophageal atresia (4 vertebral spaces) using the external traction suture technique.

  • Intramural esophageal dissection is a rare disorder characterized by a separation of the mucosa and/or submucosa from deeper muscular layers of the esophagus, with or without perforation. Iatrogenic instrumentation such as endoscopy is one of the major causes of IED. We report a case of IED after endoscopy in a patient with hypopharyngeal cancer treated with concurrent chemoradiotherapy, and suggest that a history of chemoradiotherapy can be a risk factor of IED on endoscopy. In this case, chest computed tomography scans show not only typical esophageal double lumen but also eccentric esophageal wall thickening and abnormally thin the other side esophageal wall, and this CT finding may also be important to diagnose IED.

  • Rupture of the esophagus is an uncommon condition that can be a formidable challenge to treat. Unless early treatment is applied, life-threatening complications, such as mediastinitis, sepsis, occurs and continue to be associated with a mortality rate of more than 20%. Definitive repair of esophageal perforation is considered the preferred treatment in the past. In the present study, conservative treatment acquired a good results by help of the development of antibiotics and nutritional supportive methods. Iatrogenic causes constituted most of the injuries, followed by external trauma, spontaneous, ingested foreign bodies, and malignancy. The incidence of perforation following ingestion of foreign bodies is very rare, so recommendations regarding treatment remain controversial. We analyzed the course of 31 patients with esophageal perforation by foreign bodies, concentrating on the diagnostic & treatment delay and methods of treatment, complications.

Case Report of Early Esophageal Carcinoma (조기 식도암 치험 1례 보고)

  • The early esophgeal carcinoma is limited to the mucosa or submucosa without lymph node metastsis which shows good 5-years survival rate. It is nearly 85-90% nowadays,if we just find and manage it.It is really helpful to use the endoscope in management and diagnosis, in addition the biopsy under the endoscopic finding.We experienced one case of early esophageal carcinoma. The patient was 57years old female,who ingested lye 27years ago for suicidal attempt, after 10years, the dysphagia was aggravated slowly. Before admission dysphagia was severely aggravated during 2months. Confirmation of diagnosis was made by endoscopic biopsy.Operation method was substernal colon bypass and total esophagectomy through right thoracotomy. Postoperative course was smooth.We report early esophgeal carcinoma related to lye stricture that was detected by the method of endoscopic examination and biopsy. It is important to screen the patients with longstanding history of benign esophageal stricture by the endoscopic biopsy.

  • This paper investigates the relationship between acoustic characteristics and voice handicap index for 29 males with esophageal speakers. Acoustic characteristics were measured by using a sustained vowel /a/ three times. The stable vocalization for 2 seconds was analyzed by MDVP program. Specifically, relationships between four VHI scores (total, functional, physical, and emotional) and three acoustic characteristics (jitter, shimmer, and NHR) were investigated using the Pearson correlation coefficient. As results, we found no relationship between NHR and VHI scores. However, both jitter and shimmer had statistically significant correlations with all four VHI scores. This research will contribute to establishing a baseline related to speech characteristics in voice rehabilitation with esophageal speakers. Further research could be done to examine the overall quality of life survey, which is widely used as a subjective measure about voice for patients with esophageal speakers.

  • The esophageal perforation with foreign body is very rare. It is usually caused by sharp metallic foreign body or bone piece and rarely caused by long standing of foreign body. The authors observed 5 cases of esophageal perforation with foreign body among 48 cases of esophageal foreign body during the period from Jan. 1976 to Dec. 1977. There were two cases of esophageal foreign body with curved wire. a case with bone piece. a case with safty pin and a case with fish hook.
