Plumbagin Inhibits Expression of Virulence Factors and Growth of Helicobacter pylori (Plumbagin에 의한 헬리코박터 파이로리균의 성장 및 병원성 인자 발현 억제효과)

  • Helicobacter pylori primarily colonizes the human stomach. Infection by this bacterium is associated with various gastric diseases, including inflammation, peptic ulcer, and gastric cancer. Although there are antibiotic regimens for the eradication of H. pylori, the resistance of this species against antibiotics has been continuously increasing. The natural compound plumbagin has been reported as an antimicrobial and anticancer molecule. In this study, we analyzed the inhibitory effect of plumbagin on H. pylori strain ATCC 49503 as well as the expression of various molecules associated with H. pylori growth or virulence by immunoblotting and reverse transcription polymerase chain reaction (RT-PCR) analyses. We demonstrated the minimal inhibitory concentration of plumbagin on H. pylori through the agar dilution and broth dilution methods. Furthermore, we investigated the effect of plumbagin treatment on the expression of the RNA polymerase subunits and various virulence factors of H. pylori. Plumbagin treatment decreased the expression of RNA polymerase subunit alpha (rpoA), which is closely associated with bacterial survival. Moreover, the mRNA and protein levels of the major CagA and VacA toxins were decreased in plumbagintreated H. pylori cells. Likewise, the expression levels of urease subunit alpha (ureA) and an adhesin (alpA) were decreased by plumbagin treatment. Collectively, these results suggest that plumbagin may inhibit the growth, colonization, and pathogenesis of H. pylori by the mechanism demonstrated in this study.

Utilization Policy of the Private Security against Acts of Piracy (해적행위에 대한 민간경비의 활용방안)

  • In recent, the ships of the Republic of Korea have been suffered from acts of piracy at the seas. With the increase of the threats from the acts of piracy and armed robbery against ships, some of the countries have dispatched their navy warships to the international waters and to carry out military operation against the pirates. However, the total eradication of the piracy has not been achieved due to the lack of international cooperation and the limitation of the international maritime convention itself. In addition, the absence of counter-measures to be taken by the ships itself including the employment of the professional maritime security forces should not be overlooked. In this connection, this paper examined the reality of the piracy together with armed robberies at the seas and the problems in the response were also analyzed. Based on the research, it suggested some possible measures with special reference to the private security. The limitation of this paper is that it only focus on the aspect of the private security. In this paper, the cooperation among the countries according to the maritime convention and internationally accepted norms and practices. To achieve the goals of this paper, through the comparative review of the related literatures, the practical measures applicable to the prevention of the piracy were suggested.

On-Site Diagnosis of Fire Blight with Antibody-Based Diagnostic Strips (항혈청 기반 진단 스트립을 이용한 과수 화상병 현장진단)

  • Recently fire blight occurred in the Republic of Korea and eradication program for the disease has been executed since then. Specificity and detection sensitivity of the 2 antibody-based diagnostic strips to Korean isolates of Erwinia amylovora (Ea) and their application for on-site diagnosis were evaluated in this study. Ea AgriStrip, a commercial diagnostic kit, and EB strip, developed in this study, reacted positively to the all tested Korean Ea strains and also to most of Erwinia pyrifoliae (Ep) strains causing black shoot blight. They reacted negatively to all Pusedomonas syringae pv. syringae (Pss) strains that cause shoot blight on apple. Detection sensitivity was similar between the 2 strips. For on-site diagnosis, the two strips reacted positively only to the extractions of the fire-blighted samples on all fire blight occurred orchards except one orchard at which on-site diagnosis was carried out at winter time. In addition, they reacted positively to the black-shoot blighted extractions from the black shoot blight occurred apple orchard. These results suggest that both EB strip and Ea AgriStrip would be useful for on-site diagnosis of fire blight in Korea.

Knowledge and Practice of College Entrants Toward Hepatitis B (일부 대학신입생들의 B형 간염에 대한 지식과 실천)

  • In order to designate a present status necessary for establishment of preventive measures and guidelines of health education against hepatitis B in the course of secondary school education, knowledge and practice toward hepatitis B virus infection was surveyed by a questionnare method on total of 4,855 college entrants in the academic year of 1987 and analyzed the data collected using IBM PC(Trigem 88-II) with SAS package program. About two per cent of college entrants had past history of HBV infections not showing any difference between both sexes and geographical regions. About one third(33.7%) of total students had tested hepatitis B surface antigen(HBsAg), only 4% had tested hepatitis B surface antibody(HBsAb) and vaccination rate amounted to 24.6%, one fourth of total subjects. Both serological tests and vaccination were most commonly performed during adolescence, showing higher rates in female students than in male students. The rates also seemed to be higher in those from urban cities than those from rural cities. Students who had acquired correct knowledge that hepatitis B was infected by virus were amounted to 78.5% of college entrants, and remaining 21.5% had misunderstood that rickettsia, bacteria, fungi or parasites were causal agents. Female students were better aware of the causal agents than male students but there was no difference between places of growth. As for mode of transmission of HBV, 51.5% of male students and 47.7% of female students had correct knowledge. A very few student had known that fact that HBV was transmitted by body fluids such as tear(6.9%), nasal discharge(10.1%) and semen or vaginal secretion(19.2%) and majority(75%) of students had misunderstood that hepatitis B virus would be transmitted per os through food ingestion. Approximately one half(48.9%) of college entrants had knew correctly whom to be vaccinated. Approximately one half of the students knew that hepatr;ma(57.8%) and liver cirrhosis(57.4%) might complicate with hepatitis B virus infection, whereas 12.0% of the students responded that bronchitis was one of the complications of hepatitis B infection. In summary of the above results, we highly recommend that health education program for eradication of hepatitis B virus infection should be introduced in curricula of secondary school education in this country.

Recent Patterns of Intestinal Helminth Infections among the Residents in Taegu City, Korea (대구시민들에 있어서 장내회충류의 최근 감염상)

  • In order to estimate the recent patterns of intestinal helminth infections among the residents in Taegu City, Korea, a survey based on discovery of helminth eggs by formalin-ether sedimentation and modified Graham's scotchtape anal swab techniques, were performed during the period from March to September in 1983. A total of 1, 697 fecal specimens were collected from 854 male and 843 female residents from 5 village (Dong), each of whom were selected randomly to represent 1, 000 persons referred to the census in 1982. Among the specimens examined, one or more species of helminth parasites were found in 473, revealing the overall positive rate of 27.9 per cent. Of them, Trichuris trichiura was found most frequently, in 13.2 percent, followed by Enterobius vermicularis in 7.0 per cen t. Hookworm was the least prevalent. The sex-specific rate of overall helminth infections was a little higher in females than in males, while in cases of Clonorchis sinensis, Metagonimus yokogawai and Taenia species, the infection rates in males was higher than in females. T. trichiura was most prevalent in the 20~29 age group and showed about the same prevalence in males and females of all age groups. C. sinensis infection rate increased with age while those of Ascaris lumbricoides decreased. Single infections were 82.5 per cent of all positive cases. Of these, T. trichiura revealed the highest prevalence in 31.7 per cent, followed by E. vermiculasis in 20.1 per cent and A. lumbricoides in 14.8 per cent. Of the double infections, the percentage of T. trichiura in combination with other intestinal helminths was higher than that of A. lumbricoides. It was concluded that although the infection rates of intestinal helminths among the residents in Taegu City, Korea is still high, eradication of these parasitic diseases seems to be possible with twice-a-year administration of specific anthelmintics, in combination with extensive public health education and improvement of the dietary life.

  • Malaria infection is sensitively influenced by regional meteorological conditions along with global climate change. Remote sensing techniques have become an important tool for extraction of climatic and environmental factors, including rainfall, temperature, surface water, soil moisture, and land use, which are directly linked to the habitat qualities of malaria mosquitoes. Improvement of sensor fidelity with higher spatial and spectral resolution, new multinational sensor development, and decreased data cost have nurtured diverse remote sensing applications in malaria research. In 1984, eradication of endemic malaria was declared in Korea, but reemergence of malaria was reported in mid-1990s. Considering constant changes in malaria cases since 2000, the epidemiological management of the disease needs careful monitoring. Geographically, northmost counties neighboring North Korea have been ranked high in the number of malaria cases. High infection rates in these areas drew special attention and led to a hypothesis that malaria dispersion in these border counties might be caused by north-origin, malaria-bearing adult mosquitoes. Habitat conditions of malaria mosquitoes are important parameters for prediction of the vector abundance. However, it should be realized that malaria infection and transmission is a complex mechanism, where non-environmental factors, including human behavior, demographic structure, landscape structure, and spatial relationships between human residence and the vector habitats, are also significant considerations in the framework of medical geography.

Analysis on the Type of Damaged Land in DeMilitarized Zone(DMZ) Area and Restoration Direction (비무장지대(DMZ) 인근의 훼손지 유형 분석 및 복원방향)

  • Purpose of this study is to classify damaged lands according to the cause of the damage and their influences based on characteristic of the damaged lands in DeMilitarized Zone(DMZ) area, and utilize this study as a fundamental study for establishment of ecosystem restoration system. Literature review and field survey have been conducted to review the damage status of DMZ area. For field survey, first year and second year have been conducted, in which type of the damage has been reviewed in this study. In the result, types of damage have been classified into 6 categories: 'alteration of initial landform', 'loss of surface layer', 'land pollution', 'alteration of soil chemical property', 'decline of vegetation', and 'invasion of foreign species'. Restoration for each damage type is as following. First, for alteration of initial landform, the land is restored to the original landform prior to the damage and connection to surrounding ecosystem is considered. Second, for loss of surface layer, lost surface layer is restored or further loss is prevented with stabilization. Third, for land pollution, source of the pollution is eradicated or self-circulation with purification of polluted land is encouraged. Fourth, for alteration of soil chemical property, soil is restored of its original property with eradication of the pollution source and improvement of soil. Fifth, for decline of vegetation, current vegetation and anticipated alteration in future are considered and number of wild species is to be increased based on structure and characteristic of nearby vegetation. Sixth, for invasion of foreign species, prevention of dominance by risky species and facilitation ecological stability with ecological management are to be considered. Influence according to the cause of damage has occurred in secondary(indirect) influence or simultaneous occurrence of several damage types. Considering all these aspects, when type of the damage is complex, restoration process for each of former mentioned 6 damage types with solitary influence should be considered in unison.

Screening of Anti-Biofilm Compounds from Marine-Derived Fungi and the Effects of Secalonic Acid D on Staphylococcus aureus Biofilm

  • Biofilm formation of Staphylococcus aureus is one of its mechanisms of drug resistance. Anti-biofilm screening of 106 compounds from marine-derived fungi displayed that 12 compounds inhibited S. aureus biofilm formation by >50% at the concentration of $100{\mu}g/ml$, and only secalonic acid D (SAD) and B inhibited by >90% at $6.25{\mu}g/ml$ without inhibiting cell growth after 24-h incubation. Meanwhile, it was found that the double bond between C-1 and C-10 of citrinin derivatives and the C-C connection position of two chromone monomers may be important for their anti-biofilm activities. Moreover, SAD slightly facilitated biofilm eradication and influenced its architecture. Furthermore, SAD slowed the cell growth rate in the preceding 18-h incubation and differentially regulated transcriptional expression of several genes, such as agr, isaA, icaA, and icaD, associated with biofilm formation in planktonic and biofilm cells, which may be the reason for the anti-biofilm activity of SAD. Finally, SAD acted synergistically against S. aureus growth and biofilm formation with other antibiotics. These findings indicated that various natural products from marine-derived fungi, such as SAD, could be used as a potential biofilm inhibitor against S. aureus.

Pseudoepidemic of Mycobacteria Other Than Tuberculosis (MOTT) Due to Contaminated Bronchoscope (기관지경 오염에 의한 비결핵항산균증의 위발생)

  • Background: The development of the flexible fiberoptic broncoscope by Ikeda was an important technologic advance in the diagnosis and management of patients with pulmonary disease. But, cross contamination related to fiberoptic bronchoscope was reported in cases involving tubercle bacilli, MOTT and other agents. Therefore, cleaning and disinfecting of fiberoptic bronchoscope requires careful attention. Methods: From September 1991 to May 1992, medical records of all patients with positive culture for MOTT in bronchial washing specimens were reviewed. Also to evaluate bactericidal effect of 2% glutaraldehyde, culture was performed after inoculum of MOTT, Serratia marsescens and Pseudomonas aeruginosa to the disinfectant solution. Results: In 2% alkaline glutaraldehyde, MOTT was not survived only after 30 minute exposure, but P. aeruginosa and S. marsescens were rapidly inactivated with no survivors after exposure to 2% glutaraldehyde. Since vigorous mechanical cleansing and more than 30 minute of contact time within washing machine, no more outbreak was observed. Conclusions: It is also very important that bronchoscopes must be meticulously cleaned after each procedure and more than 30 minute exposure would be required for eradication of MOTT with 2% glutaraldehyde. However even the most strictly applied infection control measures cannot exclude contamination completly and clinicians have to stay alert to this possibility. Prompt detection of pseudoepidemics is possible if abrupt increase in isolation rates, especially if they involve unusual or generally nonpathogenic organisms, are readily recognized.

Febrile Urinary Tract Infections Caused by Community-Acquired Extended-Spectrum β-Lactamase-Producing and-Nonproducing Bacteria: A Comparative Study (Extended-Spectrum β-Lactamase 생성 균주와 비생성 균주에 의한 지역사회 획득 요로 감염 비교)

  • Purpose: The purpose of this study was to investigate the clinical characteristics and outcome of febrile urinary tract infections (UTIs) caused by community-acquired extended-spectrum ${\beta}$-lactamase (CA-ESBL)-producing and -nonproducing bacteria. Methods: We analyzed febrile UTIs in children hospitalized at Gachon University Gil Medical Center from January 2011 to December 2013 through retrospective data collection from their medical records. Results: Among pathogens causing 374 episodes of UTIs, the proportion of ESBL-producing bacteria was 13.1% (49/374). The proportion of ESBL-producing Escherichia coli and Klebsiella spp. was 13.6% (48/354) and 5.0% (1/20), respectively. There was no significant difference between the CA-ESBL and CA non-ESBL groups in duration of fever ($4.2{\pm}2.7$ vs. $3.7{\pm}2.1$ days, P=0.10) and bacterial eradication rate with empirical antibiotics (100% vs. 100%). The risk of cortical defects on renal scan significantly depended on existence of vesicoureteral reflux rather than ESBL production of pathogen. Conclusions: There was no significant difference between the CA-ESBL and CA non-ESBL groups in renal cortical defects and clinical outcome. Careful choice of antibiotics is important for treatment of community-acquired UTI in children.
