A Giant Sebaceous Epithelioma on the Scalp: A Case Report (두피에 발생한 거대 피지샘 상피종 1례)

  • Purpose: Sebaceous epithelioma (sebaceoma) is a benign tumor with sebaceous differentiation. It presents primarily as a yellowish papule or nodule on the face and scalp. It must be differentiated from basal cell carcinoma and other appendageal tumors. We report a giant sebaceous epithelioma on the scalp and describe the immunohistochemical character of the cells in sebaceous epithelioma to epithelial membrane antigen (EMA). Methods: A 55-year-old-man who presented with 5-cm-diameter 2-cm-height, round shape exophytic ulcerated tumor on his head presented for treatment. The patient had noticed the lesion 40 years prior as a small yellowish plaque and 18 months ago, the plaque started to grow progressively larger. We excised the lesion with 1 cm resection margin, considering the possibility of malignancy because this lesion grossly resembled basal cell carcinoma (BCC). The defect was repaired with the use of a splitthickness skin graft. Results: When we excised the lesion, the margin was clear. Histology showed nodules that consisted of an admixture of basaloid cells and mature adipocytes lacking an organized lobular architecture. Strong expression of EMA on mature adipose cells confirmed the differential diagnosis from BCC with sebaceous differentiation because of the absence of a nuclear palisade pattern and cleft-like spaces on the hematoxylin and eosin (H&E) section. Conclusion: We treated the giant sebaceous epithelioma on the scalp with surgical excision and a split-thickness skin graft. It is important to know that the diagnosis of sebaceous epithelioma should be made based on the histologic pattern of the H&E section. Immunohistochemistry with EMA can help to confirm the differential diagnosis between sebaceous epithelioma and BCC.

Epithelioma of farmed catfish Silurus asotus in Korea (국내산 메기, Silurus asotus의 상피종)

  • Catfish Silurus asotus sampled from a pond farm in Korea, was affected with an epithelial tumor resembling catfish Ameiurus nrbulosus lip and mouth epithelioma. The neoplasm appeared in the form of solitary, multiple, large and fleshy masses upon the skin, fin or barbel. The epithelial tumor proliferation causes papillary folds supported by connective tissue cords. These folds extended above the normal skin surface and into the underlying dermis. In some area, proliferation of mucous cells or club cells were observed. The epithelial cells of the tumor were dispersed with no orderly arrangement. This is the first report on epithelioma of catfish in Korea.

The Effect of Hyangpaeyangyeongtang and Fritillariae Roylei Bulbus Extracts on the Immune Responses to Epithelioma induced by 3-MCA (3-MCA 유발(誘發) 상피종(上皮腫)에 대(對)한 향패양영탕(香貝養榮湯)과 패모단미제(貝母單味劑)가 면역반응(免疫反應)에 미치는 영향(影響))

  • Hyangpaeyangyeongtang, which was first prescribed in the book of Euijonggumgam (醫宗金鑑), has been widely applied to the treatment of scrofula and carbuncle in the neck and tumor as well in the oriental medicine. The Fritillariae roylei bulbus was also used as an anti-tumor medicine. In order to investigate the effect of Hyangpaeyangyeongtang and Fritillariae roylei bulbus extracts on the immune responses to the epithelioma induced by 3-methylcholanthrene and prostaglandin-$F_2{\alpha}$ in mice, the change of epithelioma size was observed and delayed type hypersensitivity, hemagglutinin titer, hemolysin titer, rosette forming cells, natural killer cell activity, lymphocyte transformation, productivity of interleukin-2 and phagocytic activity were also measured. The results were as follows; 1. The epithelioma size was decreased with statistical significance in the Hyangpaeyangyeongtang group at the 14th day, as compared with the control group. 2. Delayed type hypersensitivity was not shown any decrease in the Hyangpaeyangyeongtang group and the Fritillariae roylei bulbus group, as compared with the control group. 3. Hemagglutinin titer was increased with statistical significance in the Hyangpaeyangyeongtang group, but not in the Fritillariae roylei bulbus group as compared with the control group. 4. Hemolysin titer was increased with statistical significance in the Hyangpaeyangyeongtang group, but not in the Fritillariae roylei bulbus group as compared with the control group. 5. Rosette forming cells were not increased in the Hyangpaeyangyeong tang group, but increased with statistical significance in the Fritillariae roylei bulbus group as compared with the control group. 6. Natural killer cell activity at the ratio 100:1 of effector cells to target cells was increased with statistical significance in the Hyangpaeyayeongngtang group and the Fritillariae roylei bulbus group respectively, and the natural killer cell activity at the ratio of 50:1 and 10:1 of effector cells to target cells Hyangpaeyangyeongtang group was increased with statistical significance as compared with the control group. 7. Lymphocyte transformation was increased in the Hyangpaeyangyeong tang group and the Fritillariae roylei bulbus group respectively, as compared with the control group. 8. Productivity of interleukin-2 was increased in the Hyangpaeyangyecing tang group and the Fritillariae roylei bulbus group respectively, as compared with the control group. 9. Phagocytic activity was not shown any decrease in the Hyangpaeyangyeongtang group and the Fritillariae roylei bulbus group, as compared with the control group.

Pilomatricoma (Calcifying Epithelioma of Malherbe) - A case report - (모기질 세포종 - 증례 보고 -)

  • Pilomatricoma is a well defind and characteristic entity histologically, but this tumor may be encountered during aspiration biopsy of subcutaneous mass, and is, occasionally, confused with other neoplasms, particularly squamous cell carcinoma, basal cell carcinoma, and epidermal inclusion cyst. Recently, we have experienced a case of pilomatricoma diagnosed by aspiration biosy. The smear of the aspirate revealed anucleated "ghost" squamous cells, basaloid or basophilic squamous cells and nucleated squamous cells, which lack nuclear features of malignacy. These findings are consistent with pilomatricoma.

Calcifying Epithelioma of Malherbe(Pilomatricoma) in Children (소아의 Malherbe씨 석회화 상피종(Pilomatricoma))

  • A clinical analysis was made of 42 pilomatricoma in 34 children treated, from January 1980 to August 1996 at the Department of Pediatric Surgery, Hanyang University. The gender ratio was 13 to 21 with female preponderance. The average age at presentation was 7 years 3 months(range 8 months to 19 years old). The time average interval from onset to excision was 9.8 months. The predilection site was neck, upper extremities, face, trunk and lower extremities in the decreasing order of frequency. The masses were less than 3cm in diameter and there was no correlation between age at presentation and size of the mass. Of the six children with multiple lesions, two presented with multiple lesions initially and four presented with second lesion metachronously after excision of the primary lesions. Two girls had cysticercosis and von Recklinghausen's disease respectively in association with a pilomatricoma. All lesions were encapsulated subcutaneous masses which were clearly distinguished from surrounding tissue, and 4 of them had pigmentation, ulceration, skin disruption and hemorrhagic changes in the overlying skin. Microscopically the mass was composed of basophilic cells and eosinophilic shadow cells. Calcification, foreign body giant cell, ossification, focal cystic, inflammatory or necrotic changes were also seen.

Vibrio alginolyticus MviN is a LuxO-regulated Protein and Affects Cytotoxicity Towards Epithelioma Papulosum Cyprini (EPC) Cells

  • Vibrio alginolyticus, a Gram-negative marine bacterium, is one of the causative agents of fish vibriosis. Its virulence factors and pathogenesis mechanism are barely known, except for some extracellular products (ECPs) that are known to be regulated by quorum sensing system. Therefore, the present study used a microarray to analyze the transcription profiles of the wild-type V. alginolyticus and a deletion mutant of luxO, the pivotal regulator in Vibrio quorum sensing systems, which resulted in the identification of a putative virulence factor, MviN. Quantitative real-time reverse transcription PCR confirmed that the transcription of mviN was upregulated in the luxO mutant when compared with wild-type, and down regulated in a luxO-con complemented strain. Furthermore, Western blotting indicated that MviN was greatly induced during the late-exponential and stationary phases of growth, indicating that the expression of MviN was cell-density dependent and quorum sensing regulated in V. alginolyticus. Meanwhile, the mviN null mutant displayed a much slower growth rate than the wild type, signifying the essential role of MviN in V. alginolyticus. Western blotting also revealed that MviN was present as an extracellular protein in V. alginolyticus. When epithelioma papulosum cyprini (EPC) cells were treated with the ECPs of the mviN mutant, no cytotoxicity was observed, whereas EPC cells treated with the wild type exhibited pathological changes, which increased with the ECPs concentration and treatment time. Therefore, the results demonstrated that MviN is a LuxO-regulated ECPs component and involved in the pathogenicity of V. alginolyticus.

Effects of Fetal Bovine Serum Concentration on the Propagation of Korean Viral Haemorrhagic Septicaemia Virus in an Epithelioma Papulosum Cyprini Cell Line (우리나라에서 분리된 바이러스성출혈성패혈증 바이러스(Viral Haemorrhagic Septicaemia Virus, VHSV)의 증식에 대한 Fetal Bovine Serum (FBS) 농도의 영향)

  • Fetal bovine serum (FBS) is essential for cell culture and is used in the determination of infectivity titer and propagation of viruses. To clarify the effects of FBS on the propagation of viral haemorrhagic septicaemia virus (VHSV), which is a causative agent of mass mortalities of olive flounder Paralichthys olivaceus in Korea, VHSV was inoculated into an EPC (epithelioma papulosum cyprinid) cell line supplemented with MEMs (minimal essential medium) with FBS concentrations of 0%, 2%, 5%, and 10% (MEM0, MEM2, MEM5, and MEM10), respectively, and infectivity titers were compared. Cytopathic effects were observed in all experimental groups at 2 days post virus inoculation (dpi) and all cells were detached from cell culture flasks at 7 dpi. Infectivity titers increased to 3 dpi, persisted to 7 dpi, and decreased when cells were detached. The titer of VHSV in EPC cells in MEM0 was the lowest while those in the other experimental groups showed similar levels. In conclusion, 2% (v/v) of FBS was sufficient to propagate VHSV in EPC cells and the withdrawal of VHSV from cell culture flasks should be performed before cell detachment.

Detection of viral hemorrhagic septicemia virus (VHSV) in wild marine fishes in the coastal region of Korea (우리 나라 연근해 자연산 해수 어종에서의 Viral Hemorrhagic Septicemia Virus (VHSV)의 검출)

  • In order to analyse the detection of viral hemorrhagic septicemia virus (VHSV) in marine environment surrounding coastal region of Eastern and Southern sea of Korea, the pools of each organ sample of three fish were taken for virus assay from February to May in 2003. The samples comprised 42, taken from 9 species of marine fishes. The VHSV was detected from chub mackerel Scomber japonicus and striped mullet Mugil cephalus in epithelioma papulosum cyprini (EPC) cells. The identity of the virus was confirmed a reverse transcriptase-polymerase chain reaction (RT-PCR). VHSV has previously been reported from chub mackerel, but not from striped mullet. The new isolates was classified as a member of genogroup I (American type) of VHSV and was closely related to the VHSV KVHS'01-l based on comparisons of the partial nucleotide sequence of the glycoprotein (G) gene.

  • Pilomatricoma, calcifying epithelioma of Malherbe, is a benign skin neoplasm of hair matrix origin that is typically occurred in the head and neck region. It usually presents as a superficial, firm, solitary, slow-growing, and painless mass of less than 3 cm in diameter and most often occurs in young age under 20 years. The tumor does not express an aggressive behavior and surgical excision is the treatment of choice. The purpose of this article is to present a case of pilomatricoma of the preauricular region and to review the literature regarding pilomatricomas of the head and neck region.
