Melanoma Incidence Mortality Rates and Clinico-Pathological Types in the Siberian Area of the Russian Federation

  • Russian rates for melanoma incidence and mortality are relatively low as compared to some other white populations but the tumor is of increasing importance. In this paper, data are based on a retrospective descriptive analysis of melanoma epidemiology and clinicopathological characteristics in Krasnoyarsk Territory belonging to the Siberian Federal District of the Russian Federation. The age-adjusted incidence and mortality rates for the period 1996-2009 were determined with subsequent retrospective analysis of clinicopathological data of 103 primary melanoma cases. Our results showed that incidence and mortality rates in the region under consideration match the Russian national trends and correspond to epidemiological data of the countries of Eastern Europe. Stratification of melanoma cases by age, sex, clinicopathological state and localization revealed a prevalence of lesions on the trunk and lower extremities. Most melanomas diagnosed were of superficial spreading type and the third Clark's level of tumor invasion and stage II according to AJCC. In spite of comparatively low rates of incidence and mortality the trend to increase of melanoma cases in the region under consideration obviously calls for more attention and further investigation.

  • Ultrafine particles (< 100 nm in diameter, UFP) are known to be more toxic per unit mass than larger particles and contribute to more than 90% in particle number concentrations in urbanized cities but much less in mass. The major sources of UFP are vehicle emissions in urban areas. Due to their tiny size (the sizes of UFP from vehicle emissions range from 10 to 60 nm depending on engine and fuel types), inhaled UFP can reach the deepest area of respiratory track (e.g., pulmonary alveoli) as well as all of the body via lymph and blood circulation causing various adverse health effects. This article reviews the sources and emission factors of UFP, temporal and spatial distributions in urban areas and their health effects reported by toxicological and epidemiological studies. We also compared the levels of UFP concentrations measured in other countries with those in Korean cities to evaluate the public exposure to UFP in Korea. Ultimately, we expect this study can contribute to developing the risk assessment techniques for public exposure to UFP in the urbanized cities in Korea.

  • To investigate the epidemiological aspects of listeriosis, serotypes of L monocytogenes and antimicrobial susceptibilities of Listeria spp isolated from chicken carcases and chicken slaughter house environmental specimens were determined. Of 28 L monocytogenes strains, 12 strains(42.9%) were serotype 4, and the remaining 16 strains were untypable. Peak distributions of minimum inhibitory concentration$({\mu}g/ml)$ of the isolates were $0.78{\mu}g/ml$ for ampicillin, $0.39{\mu}g/ml$ for erythromycin and penicillin G, $1.56{\mu}g/ml$ for tetracycline and $6.25{\mu}g/ml$ or $12.5{\mu}g/ml$ for chloramphenicol, and $3.13{\mu}g/ml$ to > $100{\mu}g/ml$ for kanamycin and neomycin. Most of 214 isolates were sensitive to ampicillin and erythromycin, but 20. 1~78. 0% of the strains were resistant to tetracycline, kanamycin, penicillin-G and neomycin. Single or double drug resistance were observed in 75.8% of the resistant strains. The most common resistance patterns were Nm P-G(37.4%) in double pattern and P-G(23.7%) in single pattern.

  • To investigate the epidemiological trait of listeriosis, Listeria spp were isolated from poultry meat, products and environmental specimens in chicken slaughterhouse. Also determined were isolation rates by the different enrichment procedures, the biochemical properties of isolates. In a total of 307 samples including poultry meat, liver, feathers, feces, chiller water, scalding water overflow and slaughterhouse floor, Listeria spp were isolated predominantly from scalding water overflow (90.0%), body skin before washing (66.7%), liver (20.0%) and feathers(15.0%) However, few Listeria spp were isolated from body skin after washing and feces. The higher isolation rates were obtained in the secondary enrichment procedure (7.2%) than in the primary enrichment (3.9%); after stored the secondary enrichment cultures for 2 weeks at $4^{\circ}C$, Listeria spp were present in 9.8%. The majority of the isolated Listeria spp were identical to those of the standards strains in biochemical and cultural properties. Overall, Listeria spp were present in 13.4% of the specimens tested, and were in order of prevalence of L innocua(11.1%), L monocytogenes(3.3%), L grayi(0.7%) and L murrayi(0.3%).

Factors Contributing to Low Weight in Community-Dwelling Older Adults (지역사회 거주 노인의 저체중 영향요인)

  • Purpose: The purposes of this study were to describe the prevalence of low weight and to investigate related factors contributing to low weight in community-dwelling older adults. Methods: From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV), the researchers selected 2,964 older adults aged over 65. The original study was a population-based epidemiological survey of health and nutrition with a stratified multistage clustered probability design. Data were analyzed using ${\chi}^2$-test and multiple logistic regression with the SPSS/WIN 18.0 program. Results: The prevalence of low weight in older adults was 4.7%. The prevalence of low weight was different according to demographic characteristics. That is, the rate was higher in male elders, older ones, and rural residents. In the cases of current smoking, frequent alcohol consumption and no chronic disease, the prevalence of low weight was higher. Age, gender, area of residence, and current smoking were found to be contributing factors to low weight in community-dwelling older adults. Conclusion: The result of this study provides important insights into possible reasons for low weight in community- dwelling older adults and suggests that low weight should be considered in promoting older adults' health.

Modeling of transmission pathways on canine heartworm dynamics

  • Canine heartworm disease is a vector-borne disease that is transmitted from dog to dog by mosquitoes. It causes epidemics that disrupt the health environments of dogs and are burdensome for many dog owners. Recent trends of changing temperatures and weather conditions in South Korea may have an impact on the population of mosquitoes, and it affects the population of dogs at risk of heartworm infection. Mathematical modeling has become an important measure for analyzing the epidemiological characteristics of infectious diseases. However, canine heartworm infection transmission has not been reported yet through mathematical modeling. We develop a mathematical model of canine heartworm infection to predict the population of infected dogs depending on the vector (mosquito) population using a susceptible, exposed, infected, and recovered model. Simulation results show that after 1 year, 3,289 dogs out of 73,602 (about 4.5%) are exposed and 134 (about 0.2%) are infected. Only 0.2% of susceptible dogs become infected after 1 year. However, if all exposed dogs are maintained in the same circumstances without any treatment, then the number of infected subjects will increase over time. This may increase the possibility of other dogs, especially dogs that live outside, being infected.

Nasal septum angiofibroma: a rare condition with an unusual onset

  • The characteristics of extra-nasopharyngeal angiofibromas tend to be different from angiofibromas of the nasopharynx according to patient gender, patient age, prevalence, affected site, pathogenesis, and clinical and epidemiological features. We report a case of an extra-nasopharyngeal angiofibroma in a 28-year-old man referred to the ENT Clinic for right-sided epistaxis, airflow impairment and nasal swelling. The right nostril was completely occluded works by a reddish-yellow mass that bled easily. The computed tomography scan revealed an "inhomogeneous solid lesion in the nasal fossa". With the patient under general anesthesia, the formation in the anterior portion of the right side of the nasal septum was removed up to its vascular base. Although electrical cauterization efficiently controlled the bleeding, we abraded the sub-perichondral area to prevent further bleeding as well as recurrence. The histological exam report confirmed the diagnosis of angiofibroma. As in our case, epistaxis is commonly the presenting sign of angiofibroma. Yet its onset was peculiar, given that the bleeding started with a low impact trauma. The nasal swelling was also a relevant feature as well as the breathing impairment. Although uncommon, nasal septal angiofibromas should considered in patients with epistaxis.

Epidemiological characteristics on new cases of notified tuberculosis in Jeju Province, Korea, 2011~2018 (제주도 결핵 신환자 발생의 역학적 특성, 2011~2018)

  • While the notified crude incidence rates(per 100,000 persons, CIR) of new tuberculosis between 2016 and 2017 decreased in nationwide and 16 provinces, except Jeju Province, Korea. The aim of this study was to compare CIRs of Jeju Province and nationwide by calendar years and age groups in order to interpret the increasing trend of CIR in Jeju Province. The data source was the Integrated Diseases and Health Control System of the Korea Centers for Disease Control and Prevention. The selection criteria were the notified new cases as tuberculosis, who had the address as Jeju Province at diagnosis, between 2011 and 2018. The 95% confidence interval of CIR was estimated based on Poisson distribution. There was not statistically significant difference of CIRs between Nationwide and Jeju Province of 2011 and 2018, except 2016. Especially the CIR of group aged over 65 years in Jeju Province, 2016 was significantly lower compared to Nationwide CIR. But, the CIR of group aged 30~50 between 2017 and 2018 showed significantly higher compared to Nationwide CIR. Based on the results, the fact of increasing CIR from 2016 to 2017 in Jeju Province might be interpreted as an introduction of new tuberculosis screening project rather than real increased CIRs.

Epidemiological Characteristics of Rodents and Chiggers with Orientia Tsutsugamushi in the Republic of Korea

  • A survey of rodents and chiggers associated with Orientia tsutsugamushi was conducted in a rural region of the Republic of Korea (Korea) between 2014 and 2018. Overall Apodemus agrarius 15.2% had the highest seropisitive for O. tsutsugamushi, followed by Myodes regulus 11.4%. Monthly risk factors using logistic regression analysis were not associated with O. tsutsugamushi infections in rodents. The overall prevalence rate of O. tsutsugamushi among chiggers was 0.3%. The chigger (Leptotrombidium scutellare) and monthly (October) risk factors were associated with O. tsutsugamushi human infections (P<0.05). Orientia tsutsugamushi infections are endemic in rodents in Korea and people, for example, soldiers who are active outdoors, must employ preventive measures, especially during October (P<0.05). When there are many reports of O. tsutsugamushi infections in Korea. The Boryong strain 85.7% (2/14) was the most common strain detected in chiggers, followed by the Shimokoshi 7.1% (1/14) and Karp 7.1% strains.

  • Methicillin-resistant Staphylococcus aureus (MRSA) is a bacterial pathogen capable of causing human diseases, such as soft tissue infection, bacteremia, endocarditis, toxic shock syndrome, pneumonia, and sepsis. Although the incidence rate of diseases caused by MRSA has declined in recent years, these diseases still pose a clinical threat due to their consistently high morbidity and mortality rates. However, the role of virulence factors in staphylococcal infections remains incompletely understood. Methicillin resistance, which confers resistance to all β-lactam antibiotics in cellular islets, is mediated by the mecA gene in the staphylococcal cassette chromosome mec (SCCmec). Differences in SCCmec types and differences in their sizes and structures serve epidemiological purposes and are used to differentiate between hospital-associated (HA)-MRSA and community-associated (CA)-MRSA. Some virulence factors of S. aureus are also providing a distinction between HA-MRSA and CA-MRSA. These factors vary depending on the presence of toxins, adhesion, immune evasion, and other virulence determinants. In this review, we summarized an overview of MRSA such as resistance mechanisms, SCCmec types, HA- and CA-MRSA, and virulence factors that enhance pathogenicity or MRSA epidemiology, transmission, and genetic diversity.
