1998년 제주도에서 발생한 볼거리 유행조사 (An Epidemiologic Investigation on Mumps Outbreak in Cheju-do, 1998)

  • Objectives : To describe the characteristics of a mumps epidemic in Cheju-do, 1998 and to identify the risk factors associated with mumps infection. Methods : To estimate attack rate, previously collected data from the Nationally Notifiable Communicable Disease Reporting System and School Health Reporting System, temporarily administered by Division of Education. as well as additional surveillance data were used. In order to identify the clinical characteristics and risk factors associated with mumps, we conducted a questionnaire survey in 17 schools (9 elementary, 4 middle, and 4 high schools) among a population that included healthy students. Results : From March 3 to August 31, 2,195 cases of mumps were identified, and patients under 20 years of age accounted for 2,162 cases (attack rate 13.2, 95% CI 12.6-13.7/1,000). The attack rate for the population under 20 years of age was the highest in Nam county (44.7/1,000), nod in the 7-12 years old sub-group(>20.0/1,000). There was no sexual difference. 80.5% and 59.7% of patients presented periauricular and submandibular swelling respectively. Aseptic meningitis was a complication in 2.9% of cases, orchitis in 1.3%, epididymitis in 0.9% and oophoritis in 0.6% respectively. The overall MMR vaccination rate was 59.1% and it decreased in accordance with increasing age. In students aged 10 years old or below, household contact and MMR vaccination status was significantly associated with infection, and only among students with household contact, the risk of one dose MMR(OR=10.22, 95% CI 2.92-35.78) and non-vaccination (OR=11.62, 95% CI 1.96-68.96) was significantly greater when. compared with that of two dose vaccination. Among students aged 11 years old or above, household contact history was significantly associated and MMR vaccination status was not associated. Conclusions : Low vaccination rate and vaccine failure were thought to predispose the population for this large outbreak. To prevent sustained mumps outbreaks, a second MMR vaccination should be encouraged and catch up vaccinations should be given to elderly children who remain susceptible.

경상북도 거주 고위험군에서 브루셀라 감염에 관한 혈청 역학적 연구 (Seroprevalence of Brucellosis among Risk Population in Gyeongsangbuk-do, 2006)

  • Objectives : Cases of human brucellosis in Korea have recently increased due to the increasing incidence of bovine brucellosis. The authors conducted this study to elucidate the status of brucellosis through seroepidemiologic study. Methods : We selected our study population from a high risk group. We conducted a questionnaire survey and obtained blood samples to determine the seroprevalence of brucellosis antibodies for 10 days in February, 2005. The titers of brucellosis were measured by the combination of standard tube agglutination test (STA) and enzyme-linked immunosorbent assay (ELISA) test. Results : Our study subjects comprised 1,075 cases: 971 livestock workers, 51 veterinarians, and 53 artificial inseminators. In the STA test, 27 cases (2.5%) had titers of greater than or equal to 1:20. Of 1,068 cases (7 cases were excluded due to previous brucellosis), 7 cases of brucellosis were diagnosed with titers of 1:160, giving a seroprevalence of brucellosis of 0.66%. The seroprevalence in the male group was 0.95%, and that of livestock workers, veterinarians, and artificial inseminators was 0.52%, 4.17%, and 0.00%, respectively. The Spearman's correlation coefficient between the positive rate of bovine brucellosis per capita and household and human brucellosis was 0.806 and 0.744, respectively. The concordance rate between the Korea National Institute of Health and the Gyeongsangbuk-do Institute of Health and Environment by the STA and ELISA tests was 94.7% and 100.0%, respectively. Conclusions : The study results indicated in higher seroprevalence rate among veterinarians than among livestock workers and artificial inseminators. Because veterinarians may be exposed to this high risk, effective working guidelines for veterinarians to guard against brucellosis must be developed. Moreover, more extensive epidemiologic research for laboratory workers and meat handlers is needed.

  • Objectives : The purpose of this study is to research on the characteristics of outpatients and distributions of diseases in ophthalmology, otolaryngology of Korean medicine in 2012-2016 and compare the result with previous studies. Methods : This study is based on the analysis of medical records of total 2,319 outpatients in ophthalmology, otolaryngology of Korean medicine at Kyung Hee Korean medicine hospital from January 1, 2012 to December 31, 2016. We searched on the demographic characteristics of outpatients and the distributions of diseases in order of frequency by age and sex. Then, we compared these results to previous results of papers at same hospital in 2000 and 2007. Results : The common diseases in 2012-2016 were dry eye syndrome in ophthalmology(32.14%), tinnitus and hearing loss in otology(72.69%), rhinitis in rhinology(82.20%) and laryngopharyngitis in laryngology(67.77%). The most increasing diseases in the distribution between 2004-2006 and 2012-2016 were retinopathy in ophthalmology(+366.77%), etc including the eustachian tube disorder in otology(+151.55%), olfactory disorder in rhinology(+167.50%) and laryngopharyngitis in laryngology(+126.20%). Conclusions : If we accumulate these kinds of epidemiologic studies on ophthalmology, otolaryngology of Korean medicine, it will be great help to make Korean medical preparations for the increasing diseases on these fields.

한국인 수면장애 환자의 최근 3년간 한방 진료 양태 (Oriental Medical Treatment Pattern of Korean Patients with Sleep Disorders)

  • Objectives: Though there are many studies about sleep disorder, no research has been performed on the utilization of oriental medicine as a treatment. Therefore, the oriental medical treatment pattern of Korean patients with sleep disorders was examined herein using the Health Insurance Review and Assessment Service (HIRAS). Methods: The medical records of patients with sleep disorders (G47) or nonorganic sleep disorders (F51) as a main diagnosis were adopted from the HIRAS database from 2011 to 2013. Analysis was performed on the number of patients and cost per patient, with comparison between oriental and western medicine in terms of gender, age, patient care service type, and hospital type. Results: 1) Regarding sleep disorders, the medical visits and insurance charges have been increasing. Western medicine was utilized 8 times more often than oriental medicine during 3 years. 2) There were 2.5 times more women than men. 3) Among all ages, the 50~59 year group had the highest representation. 4) In comparison of average portions of patient care type over 3 years, outpatients were the majority, while the number of visits of outpatients and hospitalization has been increasing. 5) Comparison of average portion of oriental hospital type over 3 years revealed oriental clinics to be used most. The use of general hospitals was higher in western medicine treatment, while public health centers used oriental medicine more. 6) Regarding average oriental medical cost per patient over 3 years, the total was 88,000 won, with 353,000 won for hospitalization and 85,000 won for outpatients. The outpatient cost has been increasing. 7) In line with 6, oriental medical hospitals cost 126,000 won, local clinics were 85,000 won, and etc. was 95,000 won. Average costs of all types have increased during 3 years, except oriental medical hospitals in 2013. Conclusions: This study provided objective information about the epidemiologic characteristic of oriental medicine used for treatment of sleep disorder. For expansion of oriental medical demand for sleep disorder, this study would be helpful in understanding the recent status.

소아 요로감염의 원인 Escherichia coli 균의 계통 분류와 독성인자 분석 (Phylogenetic Groups and Virulence Factors of Escherichia coli Causing Urinary Tract Infection in Children)

  • 목적: 요로감염은 소아에서 흔한 세균 감염이며, Escherichia coli가 주요 원인균이다. 본 연구는 우리나라에서 소아 요로감염을 일으키는 E. coli의 계통 분류와 독성인자를 분석하고자 하였다. 방법: 2010년 10월부터 2013년 4월까지 요로감염으로 입원한 33명의 소아 환자로부터 검출된 E. coli균주를 대상으로 하였다. 중합효소연쇄반응을 통해 E. coli의 계통 분류 및 5가지 독성인자(fimH, sfa, papA, hylA, and cnf1)를 조사하였다. E. coli의 분자유전학적 특징을 환자의 임상적 진단과 동반된 방광요관 역류에 따라 분석하였다. 결과: 대부분의 요로병원성 E. coli 는 계통 분류에서 B2군(84.8%)에 속했으며, 나머지는 모두 D군(15.2%)에 해당되었다. 독성인자는 fimH (100%), sfa (100%), hylA (63.6%), cnfI (63.6%), 그리고 papA (36.4%)의 분포를 보였다. 임상 진단에 따른 계통 분류에서 급성 신우신염의 경우 B2군이 92.3%, D군이 7.7%를 나타냈으며, 방광염에서는 B2군에서 57.1%, D2군은 42.9%였다. 독성인자는 양 군에서 비슷하게 분포하였다. 급성 신우신염에서 방광요관 역류의 유무에 따른 계통 분류의 분포에는 차이가 없었으나, 독성인자의 경우 papA 유전자가 방광요관 역류가 동반되지 않은 군에서보다 방광요관 역류 군에서 적게 나타났다(43.8% vs. 20.0%, P=0.399). 결론: 본 연구는 국내 소아 요로감염의 원인 E. coli 균주의 분자유전학적 역학 자료를 제시하였으며, 이 결과는 향후 소아 요로감염의 발생 기전을 이해하는 데 기초가 될 것으로 생각된다.

전립선염 증상지수를 이용한 전립선염 증상의 규모와 삶의 질에 미치는 영향 (Size of Prostatitis Symptoms Using Prostatitis Symptom Index(PSI): The Effect of Prostatitis Symptoms on Quality of Life)

  • Objectives : To determine the prevalence of prostatitis symptoms in the general population by questionnaire survey and to measure the effect of prostatitis symptoms on quality of life(QOL). Materials & Methods : A cross sectional community-based epidemiologic study was performed on 2,034 men, living in the Seoul metropolitan area using stepwise random sampling. Out of 2,034 interviewees, 1,356 men who were older than 40 and provided sufficient information were selected for this study. The questionnaires were completed by well trained interviewers. Contents of the questionnaires included demographic data, the Prostatitis Symptom Index(PSI), the International Prostate Symptom Score(IPSS), a general health questionnaire section and a sexual health questionnaire section. The PSI was composed of a sum of the scores from three questions about dysuria, penile pain and perineal pain and it ranged 0 to 12. Incidence of prostatitis symptoms was defined by a score of 4 or more and the reference group was defined as consisting of those with a score of 3 or less. The rate of incidence of prostatitis symptoms was assessed according to age and the difference of QOL between the prostatitis symptoms group and the reference group. Results : The overall positive rate of prostatitis symptoms measured by the PSI, in men older than 40, living in the Seoul metropolitan area, was 4.5%(61/1,356), adjusted to 4.8% by the relative proportion of this age group in the general population of the Seoul metropolitan area as compared to Korea and the World. The proportion of the group with prostatitis symptoms assessed by the PSI did not increase with age although the proportion of participants with moderate to severe lower urinary tract symptoms (LUTS) did increase with age. The group with prostatitis symptoms suffered from a much greater incidence of LUTS compared to the reference group (p<0.05). The QOL scores of the IPSS, and the general health and sexual health status of the group with prostatitis symptoms, were worse than those of the reference group.(p<0.05). Conclusions : The positive rate of prostatitis symptoms in men older than 40, living in the Seoul metropolitan area, was 4.8% and it didn't increase with age. The general QOL of the group with prostatitis symptoms was much worse than that of the reference group.

A genome-wide association study of the association between single nucleotide polymorphisms and brachial-ankle pulse wave velocity in healthy Koreans

  • Purpose: Pulse wave velocity (PWV) is an indicator of arterial stiffness, and is considered a marker of vascular damage. However, a genome-wide association study analyzing single nucleotide polymorphisms (SNPs) associated with brachial-ankle PWV (baPWV) has not been conducted in healthy populations. We performed this study to identify SNPs associated with baPWV in healthy populations in Korea. Materials and Methods: Genomic SNPs data for 2,407 individuals from three sites were analyzed as part of the Korean Genomic Epidemiologic Study. Without replication samples, we performed multivariable analysis as a post hoc analysis to verify the findings in site adjusted analysis. Healthy subjects aged between 40 and 70 years without self-reported history or diagnosis of hypertension, diabetes, hyperlipidemia, heart disease, cerebrovascular disease and cancer were included. We excluded subjects with a creatinine level >1.4 mg/dL (men) and 1.2 mg/dL (women). Results: In the site-adjusted association analysis, significant associations (P<$5{\times}10^{-8}$) with baPWV were detected for only 5 SNPs with low minor allele frequency. In multivariable analysis adjusted by age, sex, height, body mass index, mean arterial pressure, site, smoking, alcohol, and exercise, 11 SNPs were found to be associated (P<$5{\times}10^{-8}$) with baPWV. The 5 SNPs (P<$5{\times}10^{-8}$) linked to three genes (OPCML, PRR35 and RAB40C) were common between site-adjusted analysis and multivariable analysis. However, meta-analysis of the result from three sites for the 11 SNPs showed no significant associations. Conclusion: Using the recent standard for genome-wide association study, we did not find any evidence of significant association signals with baPWV.

신생아 발열 환자에서 중증 감염의 예측 인자 (Predictors of Serious Bacterial and Viral Infections among Neonates with Fever)

  • 목 적 : 본 연구는 발열로 입원한 신생아 중 임상 경과가 불량했던 환자의 특징을 조사하고 입원 후 경과가 양호했던 환자와의 임상증상, 이학적 진찰 소견과 검사 결과를 비교하여 중증 감염의 가능성을 예측하는 척도로 이용할 수 있는지 평가하였다. 방 법 : 2000년부터 2006년까지 입원한 30일 이하의 만삭아 중 액와 체온으로 38$^{\circ}C$ 이상의 발열을 보인 123명을 대상으로 하였다. 산전 위험요인이 있었거나 선천성 기형아, 병원을 방문하기 전까지 항생제를 투여받거나 원내 감염이 의심되는 경우는 제외하였다. 입원기록을 후향적으로 검토하여 진단명과 임상경과를 기준으로 고위험군과 저위험군으로 분류하고 두 군 간에 증상, 진찰 소견 및 검사 결과를 비교하였다. 결 과 : 고위험군에는 30명(24.4%)이 포함되었고 진단명을 빈도순으로 나열하면 무균성 수막염, 요로감염, 세균성 수막염, 감염성 장염, 파종성 혈관내 응고장애를 동반한 패혈증, 균혈증, 폐렴, 연조직 감염, 제대 감염순이었다. 총 백혈구 수의 증가와 혈소판 감소증은 두군 간에 통계적으로 유의한 차이를 보였으나 C-반응 단백은 그렇지 않았다. 경련, 끙끙거림, 맥박수, 입원 전까지의 발열 일수, 총 발열 기간 및 입원시까지의 최고체온은 고위험군과 저위험군 사이에서 차이가 있었다. 결 론 : 본 저자들은 이전의 보고와 달리 배양검사 양성인 경우 외에도 임상적으로 명확한 패혈증이나 무균성 수막염 환자 등을 고위험군으로 포함시켰다. 그러나 이런 대상 선정의 차이에도 불구하고 두 군은 백혈구의 수의 증가, 혈소판 감소증 및 활력징후의 이상에서 유의한 차이를 보였다.

Estimating the urinary sodium excretion in patients with chronic kidney disease is not useful in monitoring the effects of a low-salt diet

  • Background: Several epidemiologic studies have suggested that the urine sodium excretion (USE) can be estimated in lieu of performing 24-hour urine collection. However, this method has not been verified in patients with chronic kidney disease (CKD) or in an interventional study. The purpose of this study was to evaluate the usefulness of estimating USE in a prospective low-salt diet education cohort (ESPECIAL). Methods: A new formula was developed on the basis of morning fasting urine samples from 228 CKD patients in the ESPECIAL cohort. This formula was compared to the previous four formulas in the prediction of 24-hour USE after treatment with olmesartan and low-salt diet education. Results: Most previously reported formulas had low predictability of the measured USE based on the ESPECIAL cohort. Only the Tanaka formula showed a small but significant bias (9.8 mEq/day, P < 0.05) with a low correlation (r = 0.34). In contrast, a new formula showed improved bias (-0.1 mEq/day) and correlation (r = 0.569) at baseline. This formula demonstrated no significant bias (-1.2 mEq/day) with the same correlation (r = 0.571) after 8 weeks of treatment with olmesartan. Intensive low-salt diet education elicited a significant decrease in the measured USE. However, none of the formulas predicted this change in the measured urine sodium after diet adjustment. Conclusion: We developed a more reliable formula for estimating the USE in CKD patients. Although estimating USE is applicable in an interventional study, it may be unsuitable for estimating the change of individual sodium intake in a low-salt intervention study.

충남지역 다중이용시설의 환경수계에서 분리한 레지오넬라균의 특성 분석 (Characterization of Legionella Isolated from the Water System at Public Facilities in Chungcheongnam-do Province)

  • Background: The Legionella case detection and notification rate have increased in public artificial water environments where people visit, including large buildings, public baths, and hospitals. Objectives: In this study, the distribution of Legionella and its epidemiologic characteristics were analyzed in the water systems of public facilities in Chungcheongnam-do Province in South Korea. Methods: Culture and PCR analysis were performed on 2,991 environmental water system samples collected from 2017 to 2019, and associations with year, facilities, seasons, and temperature of water system were statistically analyzed by using R-Studio for Windows. Descriptive data was compared using chi-square tests and independent t-tests. Results: The detection rate of Legionella increased from 3.1% in 2017 to 10.3% in 2019, appearing most frequently in the order of public baths, large-scale buildings, hospitals, and apartments. It was detected mainly in summer from June to August, over 1.0×103 CFU/L on average in 133 cases (66.5%). Lots of germs were detected in bathtub water, cooling tower water, and warm water (p<0.001), and it was detected at higher rates in the cities where multipurpose facilities were concentrated than in rural areas (p=0.018). Conclusions: This study suggests that continuous monitoring and control are required for Legionella in the water system environment of high risk facilities. Moreover, these results will be helpful to prepare efficient management plans to prevent the Legionellosis that occurs in Chungcheongnam-do Province.
