선천성 식도 폐쇄 수술 후 시행한 재 수술의 성적 (Result of Secondary Surgery after Primary Surgery for Esophageal Atresia Anomalies)

  • We reviewed the records of 25 patients who were re-operated upon after primary repair of esophageal atresia with or without fistula at the Department of Pediatric Surgery, Seoul National University Children's Hospital, from January 1997 to March 2007. Types of the esophageal atresia anomalies were Gross type A in 5 patients, C in 18, and E in 2. The indications for re-operation were anastomosis stricture (n = 14), tracheo-bronchial remnant (n = 4), persistent anastomosis leakage (n = 3), recurrent tracheo-esophageal fistula (n = 2) and esophageal web (n = 2). The interval between primary and secondary surgery was from 48 days to 26 years 5 months (mean: 2 years and 4 months). Four patients required a third operation. The interval between the second and third operation was between 1 year 1 month and 3 year 10 month (mean: 2 years 5 months). Mean follow up period after last operation was 35 months (1 years-8 years 6 months). The secondary surgery was end-to-end esophageal anastomosis in 15, esophagoplasty in 5, gastric tube replacement in 5. After secondary operation, 6 patients had anastomosis stricture (4 patients were relieved of the symptoms by balloon dilatation, 2 patients underwent tertiary operation). Five patients had leakage (sealed on conservative management in all). Two patients had recurrent tracheo-esophagel fistula (1 patient received chemical cauterization and 1 patient underwent tertiary operation). Currently, only one patient has feeding problems. There were no mortalities. Secondary esophageal surgery after primary surgery for esophageal atresia was effective and safe, should be positively considered when complications do not respond to nonoperative therapy.

Preoperative administration effect of metronidazole, ceftriaxone sodium and their combination on stenotic index of jejunal anastomotic segment of dogs

  • The jejunum is the longest part of the small intestine and its lumen is mainly involved in the absorption of the nutrients. The present study was conducted to evaluate the effects of metronidazole, ceftriaxoine sodium and their combination on the stenotic index of the end to end jujunal anastomotic site. To accomplish this, 20 healthy stray dogs were subjected to end to end jejunal ansastmosis. Dogs in Group A (control) underwent jejunal anstomosis with no antibiotic prophylaxis, while those in Group B received surgery and metronidazole alone at 50 mg/kg, those in Group C received ceftriaxone sodium intravenously at 30 mg/kg body weight prior to surgery and dogs in Group D were given metronidazole in combination with ceftriaxone sodium at 50 mg/kg and 30 mg/kg, respectively, 2 h before surgical intervention. No significant difference (p > 0.05) in the stenotic index was observed at 14 days after jejunal anastomosis. These findings indicate that prophylactic administration of metronidazole and ceftriaxone sodium alone or in combination had no significant effect on the stenotic index of the jejunum.

수술후성 기관협착증에 관한 임사적 고찰 (The clinical study for the postoperative tracheal stenosis)

  • 문명의 발달로 나날히 복잡해지는 사회 환경속에서 우리는 생명을 위협하는 수 많은 사고를 당하게 되며, 이럭 경우에 상기도를 유지하기 위한 기관절재술을 많이 시행하게 되는데, 때로는 기관협착등 이에 따른 여러가지 합병증을 유발하게 되는 경우가 있으며, 임상가들에게도 곤란한 문제를 갖어다 주고있다. 그러나 이러한 합병증은 수술방법의 개선과 항생제의 출현등으로 그 빈도가 많이 감소되기는 하였으나, 1969년 Lindholm은 술후성 기관협착증의 빈도를 1.5%내지 10%라고 했으며, 1969년 Donnelly, Mulder와 Rubush, 그리고 1971년 Andrew와 Pearson 등은 이들 원인의 대부분이 cuff 가 있는 기관삽입관이나, 기관케뉼(tracheal canula)의 부적합한 사용이나, 또는 감염에 의한 압박괴사(pressure necrosis)로 형성된다고 하였고, 협착부위로서 1972년 Bryce는 절개공(stoma)주위가 가장 많으며, 응급기관절개술시에 의사의 부정요법(mal-practice)도 원인이 될 수 있다고도 주의한 바 있다. 기관협착증의 치료로서 협착이 경미한 경우에는 일차적으로 세심한 관찰을 실시하면서 실리콘관(silicon tube)이나 스텐트(stent)를 사용하거나, 비강을 통한 삽관법(naso tracheal intubation)으로 기계적인 확장을 시도하며 (Schmiegelow, 1929, Montgomery, 1965), 육아조직이 형성되었을 경우에는 기관경검사하에서 이를 제거한 후에 steroid를 병용하는 편이 좋은데(Birck, 1970) 그밖에도 기관개찰술(Fenestration method, 백·홍 1974)이나 재수술(Revision)을 하기도 한다. 이러한 방법으로서도 치료가 불가능한 경우에는 그 협착 부분을 절제한 후에 단단문합술(End-to-End Anasto-mosis)을 시행하는 수도 있다. 저자들은 1967년 10월부터 1977년 3월까지 10연년간 세브란스병원에서 기관절개술을 받았던 1514례를 대상으로 일련의 조사를 실시하여 이들 중에서 기관협착증을 유발한 23례를 치료하였으며, 여기서 몇가지 지견을 얻었기에 보고하는 바이다.

근,원위부 기관 식도루를 가진 식도 폐쇄증 - 1예 보고 - (Esophageal Atresia with Double Tracheoesophageal Fistula - A Case Report -)

  • Esophageal atresia with double tracheoesophageal fistula is a very rare anomaly and is difficulty to diagnose preoperatively. We treated a full term baby with esophageal atresia with double tracheoesophageal fistula. At the first operation, only the distal tracheoesophageal fistula was identified and ligated. When the upper esophageal pouch was opened, intermittent air leakages in sequence with positive bagging were noticed. However, intraoperative bronchoscopy did not identify a fistula in the proximal pouch, and the operation was completed with end to end anastomosis of the esophagus. On the $7^{th}$ postoperative day, esophagography showed another tracheoesophageal fistula proximal to the esophageal anastomosis. A wire was placed in the fistula preoperatively under bronchoscopy. At the 2nd operation through the same thoracotomy incision the proximal fistula was identified and ligated. On the $12^{th}$ postoperative day, esophagography showed neither stricture nor leakage.

Direct Open Venous Drainage: An Alternative Choice for Flap Congestion Salvage

  • In this report, we present a scalp defect reconstruction with lateral arm free flap. We highlight the difficulty in obtaining a recipient vein and the venous drainage managed through an open end of the donor vein. A 52-year-old woman presented with a pressure sore on the left scalp. A lateral arm free flap was transferred to cover this $8{\times}6cm$ defect. The arterial anastomosis was successful, but no recipient vein could be identified within the wound bed. Instead, we used a donor venous end for the direct open venous drainage. In order to keep this exposed venous end patent, we applied heparin-soaked gauze dressing to the wound. Also, the vein end was mechanically dilated and irrigated with heparin solution at two hour intervals. Along with fluid management and blood transfusion, this management was continued for the five days after the operation. The flap survived well without any complication. Through this case, we were able to demonstrate that venous congestion can be avoided by drainage of the venous blood through an open vessel without the use of leeches.

좌측 주기관지 외상성 단절의 지연복원 -1례 보고- (Delayed Repair of Completely Transected Left Main Bronchus-A report of one case-)

  • A complete transection of left main bronchus was repaired by end to end anastomosis 5 months after the chest trauma in the Yeungnam University Hospital. The patient was a 36 years old male who had been injured bluntly by a heavy package on the left chest. The initial symptoms and signs were dyspnea, chest pain and subcutaneous emphysema on the left neck, but on admission at our hospital his chief complaint was only mild left chest discomfort. The preoperative chest X-ray findings 5 months after the trauma revealed total collapse of the left lung, deviation of trachea to the left, elevation of left diaphragm, abrupt discontinuation in the course of an air-filled left main bronchus and bronchoscopy showed that the left main bronchus was completely occluded, without any signs of inflammation, approximately 4 cm from the carina. The operation was performed through standard posterolateral thoracotomy incision at the fifth intercostal space. There was not any suppuration within the transected lung but plenty of white mucus which was removed by forceful suction. The transected bronchial edges were debrided and anastomosed primarily by end to end with interrupted nonabsorbable sutures. The suture line was reinforced with a pleural flap. The postoperative course was uneventful and pulmonary function following operation improved progressively and proved the delayed repair to have a reasonable decision.

기관삽입관에 의한 기관협착증 의 외과적 치료: 4례 보고 (Circumferential Resection and Reconstruction of The Mediastinal Trachea Without Prosthesis for Tracheal Stenosis: A Report of 4 Cases)

  • The present treatment of respiratory failure, using cuffed endotracheal and tracheostomy tube has produced, apparently with increasing frequency, three lesions which have serious ceminical manifestations such as tracheal stenosis, tracheomalasia, and localized tracheal erosion. Extensive resection and reconstruction of the trachea must be necessary because conservative treatment has generally failed in the fully developed stenotic lesion. of the mediastinal trachea following extensive resection is best accomplished by direct anastomosis of the patient`s own tracheobronchial tissue. Any replacement of the mediastinal trachea must be air tight and laterally rigid, and must heal dependably. A variety of materials has been used for substitution following circumferential excision of tracheal segments within the mediastinum. These attempts have often failed because of early leak or late stenosis. We have successfully performed circumferential resection and primary end-to-end anastomosis of the trachea for 4 cases of post-intubation tracheal stenosis located a few centimeter below the tracheostomy stoma in the period of 3 years between 1974 and 1976. The lesion in one patient was found in the upper trachea which was approached anteriorly through a cervicomediastinal incision with division of the upper sternum. Other three located in the lower half of the trachea were operated through a high transthoracic incision with appropriate hilar mobilization in addition to cervical flexion for the development of the cervical trachea into the mediastinum. There were no hospital death, but suture line granulations occurred in two patients were managed by bronchoscopic removal of granulations without difficulties.

대동맥축착증의 외과적치료 (Surgical Treatment of Coarctation of the Aorta)

  • 1992년 5월부터 1996년 6월까지 외과적으로 치료한 31례의 대동맥축착증의 중단기 성적을 검토하였다. 모두 31명의 환아중 19명(61.3%)이 신생아였으며 26명(83.9%)이 생후 3개월이내의 유아였다. 동반기형에 따라 세군으로 나누었는데 중요한 기형이 동반되지 않은 군(group I)이 9명, 심실중격결손을 동반한 군(group II)이 15명, 복잡심기형이 동반된 군(group III)이 7명이었다. 35.5%(11명)의 환아에서 대동맥궁의 형성부전이 동반 되었다. 수술방법으로는 17명의 환아에서 확장단단문합술, 7명에서 단단문합술과 쇄골하동맥편성형술을 함 께 시행(combined resection and flap pmcedure)하였고, 나머지 7례에서는 쇄골하동맥편성형술을 시행하였다. 술후 잔존협착은 술후 쟌존협착을 측정하였던 28례중 7례(25%)에서 발생하였고 쇄골하동맥편성형술후 6례중 2례(33.3%), 단단문합술과 쇄골하동맥편성형술을 함께 시행한후 7례중 없었으며, 확장단단문합술을 시행한 15례의 환아중 5례(33.3%)에서 발생하였다. 생존환아에서 평균 20.5개월의 추적기간후 술후 대동맥축착은 모 두 3례로 12%(3/25)였다. 이를 수술방법에 따른 빈도를 보면 쇄골하동맥편성형술후 6 恪\ulcorner2례(33.3%), 단단문 합술과 쇄골하동맥편성형술을 함께 시행한 7례중 없었으며, 확장단단문합술을 시행한 12례의 환아중 1례 (8.3%)에서 발생하였다. 대동맥축착증의 수술과 관련된 사망은 3례(9.7%)로 모두 복잡심기형을 동반한 군에서 발생하였다. 결론적으로 복잡기형을 동반하지 않은 경우(1, ll군)는 대동맥축착증의 수술과 관련된 수술사 망은 없었으며 복잡기형을 동반한 대동맥축착증의 수술은 높은 사망률을 보였다. 또한 대동맥궁형성부전증 이 있었던 경우에서 없었던 경우보다 더 높은 술후 잔존정착의 빈도를 보여 이의 적절한 치료가 요구되었다.

Influences of Geometric Configurations of Bypass Grafts on Hemodynamics in End-to-Side Anastomosis

  • Background: Although considerable efforts have been made to improve the graft patency in coronary artery bypass surgery, the role of biomechanical factors remains underrecognized. The aim of this study is to investigate the influences of geometric configurations of the bypass graft on hemodynamic characteristics in relation to anastomosis. Materials and Methods: The Numerical analysis focuses on understanding the flow patterns for different values of inlet and distal diameters and graft angles. The Blood flow field is treated as a two-dimensional incompressible laminar flow. A finite volume method is adopted for discretization of the governing equations. The Carreau model is employed as a constitutive equation for blood. In an attempt to obtain the optimal aorto-coronary bypass conditions, the blood flow characteristics are analyzed using in vitro models of the end-to-side anastomotic angles of $45^{\circ}$, $60^{\circ}$ and $90^{\circ}$. To find the optimal graft configurations, the mass flow rates at the outlets of the four models are compared quantitatively. Results: This study finds that Model 3, whose bypass diameter is the same as the inlet diameter of the stenosed coronary artery, delivers the largest amount of blood and the least pressure drop along the arteries. Conclusion: Biomechanical factors are speculated to contribute to the graft patency in coronary artery bypass grafting.
