Total En Bloc Lumbar Spondylectomy of Follicular Thyroid Carcinoma

  • The presence of distant metastases from differentiated thyroid carcinoma decreases the 10-year survival rates of patients by 50%. This is a report of a 61-year-old female with follicular thyroid carcinoma who presented initially with low back pain. 2-deoxy-2-[18F] fluoro-D-glucose whole-body positron emission tomography/computed tomography (PET/CT) demonstrated a hypointensity lesion in the left thyroid gland with malignant uptake in L1 vertebra and magnetic resonance images revealed paravertebral and epidural extension of mass in L1 vertebra. After thyroidectomy, histopathological study showed a follicular carcinoma. We performed L1 total en bloc spondylectomy with expandable cage for long-term local control. The technical details of total en bloc spondylectomy in follicular carcinoma are described herein.

  • Positron emission tomography-computed tomography (PET/CT) provides both functional and anatomical images of high quality non-invasively with better precision in localization than PET alone. Increase in the use of PET/CT, coupled with increasing concerns about the quality of medical services accrued the demands for accurate evaluation of system performance and quality assurance. Thus, well designed programs for performance evaluation and quality assurance are needed. Widely used protocols for performance evaluation of PET are the methods proposed by National Electrical Manufacturers Association (NEMA) in 1994 and 2001. In addition, in order to maintain high quality of PET/CT images, quality assurance programs including periodic (daily, monthly, and yearly). Therefore, in this article, the methods and present state of performance evaluation and quality assurance of PET/CT are reviewed.

Neurolymphomatosis in patients with mantle cell lymphoma diagnosed by FDG PET-CT

  • Neurolymphomatosis (NL) is characterized by the infiltration of malignant lymphoma cells into peripheral nerves, nerve roots, plexuses, or cranial nerves. This is a very rare complication of mantle-cell lymphoma. Diagnosing NL is made difficult by cerebrospinal fluid cytology and bone-marrow biopsy results often being negative. NL can appear as the only sign of recurrence in a patient with a previous diagnosis of lymphoma. Here we present two cases of NL in patients with mantle-cell lymphoma diagnosed by positron emission tomography with deoxy-fluoro-D-glucose integrated with computed tomography.

Management for locally advanced cervical cancer: new trends and controversial issues

  • This article reviewed new trends and controversial issues, including the intensification of chemotherapy and recent brachytherapy (BT) advances, and also reviewed recent consensuses from different societies on the management of locally advanced cervical cancer (LACC). Intensive chemotherapy during and after radiation therapy (RT) was not recommended as a standard treatment due to severe toxicities reported by several studies. The use of positron emission tomography-computed tomography (PET-CT) and magnetic resonance imaging (MRI) for pelvic RT planning has increased the clinical utilization of intensity-modulated radiation therapy (IMRT) for the evaluation of pelvic lymph node metastasis and pelvic bone marrow. Recent RT techniques for LACC patients mainly aim to minimize toxicities by sparing the normal bladder and rectum tissues and shortening the overall treatment time by administering a simultaneous integrated boost for metastatic pelvic lymph node in pelvic IMRT followed by MRI-based image guided adaptive BT.

신경계 퇴행성 질환에서의 도파민 운반체 영상 (Dopamine Transporter Imaging in Neurodegenerative Disorders)

  • The dopamine transporter (DAT) is responsible for the re-uptake of dopamine from the synaptic cleft and is located on dopaminergic nerve terminals only. DAT single photon emission computed tomography (SPECT) and positron omission tomography (PET) imaging, therefore, offer the unique opportunity to study via striatal uptake the integrity of presynaptic dopaminergic nerve terminals in vivo. In recent years SPECT and PET using specific ligands binding to DAT have evolved as an useful tool for diagnosing and monitoring progression of neurodegenerative disorders affecting dopaminergic systems. This article briefly reviews the literature dealing with DAT SPECT and PET imaging in parkinsonism and other neurodegenerative disorders.

  • Foreign body injections into breasts may produce foreign body reactions, fibrosis, and local swelling of involved lymph nodes, which can be misdiagnosed as metastasis or malignancy. Here, the authors report MR imaging, PET-CT imaging, and pathologic findings of contralateral internal mammary lymphadenopathy suspicious of breast cancer metastasis in a 58-year-old woman with history of left breast cancer, and previous interstitial mammoplasty by paraffin injection in both breasts.

The Role of Imaging in Current Treatment Strategies for Pancreatic Adenocarcinoma

  • In pancreatic cancer, imaging plays an essential role in surveillance, diagnosis, resectability evaluation, and treatment response evaluation. Pancreatic cancer surveillance in high-risk individuals has been attempted using endoscopic ultrasound (EUS) or magnetic resonance imaging (MRI). Imaging diagnosis and resectability evaluation are the most important factors influencing treatment decisions, where computed tomography (CT) is the preferred modality. EUS, MRI, and positron emission tomography play a complementary role to CT. Treatment response evaluation is of increasing clinical importance, especially in patients undergoing neoadjuvant therapy. This review aimed to comprehensively review the role of imaging in relation to the current treatment strategy for pancreatic cancer, including surveillance, diagnosis, evaluation of resectability and treatment response, and prediction of prognosis.

Intraabdominal abscess mimicking gastric cancer recurrence: a case report

  • Surgical site infection is a common healthcare-associated infection that rarely occurs several months after surgery. Herein, a case is described in which an abdominal mass lesion was found at a 6-month follow-up visit after gastrectomy was performed for early gastric cancer. Positron emission tomography-computed tomography revealed a 2.5 cm-sized mass with a high maximal standard uptake value (8.32), located above a previous anastomosis site. Locoregional recurrence of gastric cancer was diagnosed by multidisciplinary team discussion, and explorative laparotomy was performed. However, surgical and pathologic findings revealed that the mass was an intraabdominal abscess. In conclusion, differential diagnosis of delayed abscess formation should be considered if the possibility of tumor recurrence is low, especially after early gastric cancer surgery.

정상 뇌혈류 영상에서 재구성 알고리즘 적용에 따른 섭취율 차이 : 통계적 파라미터 지도를 사용한 분석 (The changes of cerebral blood flow by brain imaging algorithm in the Normal Brains : Analysis by Statistical Parametric Mapping)

  • 뇌 영상분석 알고리즘 적용에 따른 뇌혈류의 변화를 관찰하기 위해 정상 성인 13명(평균연령 39세)을 대상으로 뇌혈류 단 광자 단층촬영(Single Photon Emission Computed Tomography, SPECT)을 시행하였다. 획득된 영상을 여과후 역투영법(Filtered Back Projection, FBP)과 반복적 방법(Ordered Subset Expectation Maximization, OSEM)으로 영상을 재구성하여 통계학적 파라미터 뇌지도법으로 비교하여 방사성 의약품의 뇌 분포양상을 확인하였으며, 혈류의 변화는 크러스트(Cluster)로 표현시켰다. 이에 대한 결과로는 여과후 역투영법이 반복적 방법보다 섭취가 증가된 부위는 우측 전두엽, 대뇌회전하부, 외핵, 좌측대뇌 변엽과 대상이랑이며, 섭취가 감소된 부위는 좌측전두엽, 중간전두이랑, 하측전두이랑, 중심전이랑, 하측전두이랑, 중심전이랑이었다. 이는 영상재구성시 적용되는 알고리즘에 따라 뇌 혈류분포가 다르게 나타난 것을 크러스트(Cluster)로 표현시켰고, 명확한 시각적 표시가 가능하도록 뇌 확룔 지도로 보여주는데 의의가 있다.

  • 심근영상의 SPECT(Single Photon Emission Computed tomography)검사는 감마선을 방출하는 방사성의약품을 환자에게 정맥주사한 후 이 의약품이 심장에 고루 퍼지면 관심부위를 촬영하여 질병으로 인한 변화를 컴퓨터를 이용하여 진단하는 검사법이다. 기능적인 정보를 담고 있는 심근관류 영상은 비침습적인 심근질환 검사에 유용한 방법이지만, 물리적 인자들에 의해 잡음과 낮은 해상도는 판도하는데 어려움을 주게 된다. 본 논문은 심근영상을 레벨 셋 알고리즘을 이용하여 영상을 분할하고 분할된 영역을 3차원으로 구현하여 판독에 도움을 주는 방안을 제안하였다. 판독의 어려움을 해결하기 위하여 레벨 셋을 이용하여 관심부위인 좌심실 영역을 분할하였고 분할된 영역을 3차원영상으로 모델링하였다.
