Nicotine-poisoning related to the electronic cigarettes (e-cigarette) is increasing worldwide. Moreover, the American Association of Poison Control Centers has advised the public to use caution with e-cigarette devices and highly concentrated liquid nicotine after a surge in related poisonings. We report here the first case of nicotine poisoning from self-injected e-cigarette fluid in Korea. A 17-year-old male patient subcutaneously injected himself with 0.5 ml of nicotine solution for an electronic cigarette via the dorsum of his hand, after which he complained of nausea, vomiting, dizziness and dyspnea. His vital signs were within the normal range, but his mental status was drowsy. He was admitted for observation and the symptoms disappeared the following day. Sinus bradycardia with a rate of 45/min was observed on the third hospital day, but improved after 6 hours. He was discharged without complications.

Acute nicotine poisoning by liquid nicotine for electronic cigarettes is becoming an increasing problem worldwide. On the other hand, there are no regulations regarding its concentration, container or labelling in Korea. This is the first case of a cardiac arrest after liquid nicotine ingestion that was confirmed by plasma nicotine detection in Korea. A 34-year-old male was found with a cardiac arrest at home by the emergency medical services crew, and had a return of spontaneous circulation after 27 minutes of cardio-pulmonary resuscitation. The cause of his cardiac arrest was suspected to be acute nicotine poisoning by the ingestion of liquid nicotine. Toxicology analysis of the National Forensic Service confirmed plasma nicotine, and the pharmacokinetic estimated average concentration of plasma nicotine at the time of the cardiac arrest was 29.7 mg/L, a lethal dose. He was hospitalized for further treatment, but was discharged after 20 days without any improvement. Considering the strong toxicity of nicotine, appropriate policy decisions are required for sales and distribution.

This study will predict risk factors associated with cigarettes in Korea by analyzing the social big data collected from the internet such as blogs, cafes, and SNSes in Korea, using data mining techniques. The key analysis results are as follows. First, when "raising cigarette price"is mentioned online, the negative group (i.e., the proportion of people holding negative views about smoking) increased from 58.6% to 74.8%, and when "lung cancer" is mentioned, it increased to 73.1%. Second, with regard to cigarettes in general, the positive group (i.e., the proportion of people holding positive views about smoking) decreased by 5.6% after the raising of cigarette prices, while the negative group increased by 6.1%. Third, when policies related to "FCTC, raising cigarette price, non-smoking laws, smoking regulations, non-smoking ads, and nonsmoking business" are more frequently mentioned online, the positive group tended to decrease. Finally, when "non-smoking drugs, non-smoking patches, and non-smoking gums" are more frequently mentioned online, the positive group tended to decrease. However, when "electronic cigarettes and supplements" are more frequently mentioned online, the positive group increased.
This study aims to observe changes in heart-rate variability (HRV) indices induced by e-cigarette and conventional-cigarette smoking and to compare the differences in acute cardiac autonomic regulation. All participants (n=41) were exposed to both e-cigarette smoke (ES) and conventional cigarette smoke (CS) in a randomized crossover trial. HRV analysis was performed during each smoking session based on a recorded r-r interval 10 minutes before smoking and at specified recovery periods (REC1, 0-5 min; REC2, 5-10 min; REC3, 10-15 min; REC4, 15-20 min; REC5, 20-25 min; and REC6, 25-30 min). ES led to a significantly increased cardiac sympathetic index (LF/HF ratio) compared with the baseline, and it shifted the sympathovagal balance toward sympathetic predominance, including reduction in the complexity of the interbeat interval (SampEn). In REC1 after ES, only decreases of parasympathetic indices such as rMSSD, pNN50, HF, and SD1 were indicated. CS sessions produced not only an increased LF/HF ratio during smoking and recovery periods (REC1 and REC4) but also enhanced sympathetic predominance on autonomic balance during smoking and recovery periods (REC1, REC2, and REC4). In the CS trials, parasympathetic indices of time and non-linear analysis (rMSSD, pNN50, and SD1) were decreased during smoking and in REC1 to REC5. SampEn was also reduced during smoking and REC1 to REC4. Acute sympathoexcitatory effects induced by e-cigarette use produced statistically significant results. Parasympathetic withdrawal after smoking suggests that e-cigarettes may cause increased cardiovascular risk.

Background: Electronic cigarette (e-cigarette) has rapidly increased. Even though e-cigarettes are actually not smoking cessation alternative supplies, some people perceive them as smoking cessation alternative supplies. The purpose of this study is to identify the relationship between e-cigarette experiences and smoking cessation behaviors among Korean adults. Methods: Cross-sectional data was collected from 7,550 participants of Korea National Health and Nutrition Examination Survey in 2014. A total of 867 participants who were current smokers were enrolled in this study. Multi-variable logistic regression analysis were performed to identify the relationship between the smoking cessation behavior and the e-cigarette experience. Results: The e-cigarette experience rate of current smokers was 22.1%. The e-cigarette experience rate was higher in groups with the higher income, the higher education level, and the white collar jobs. Smokers with experience of smoking cessation (odds ratio [OR], 3.03; 95% confidence interval [CI], 1.67 to 5.76) and with intention to smoking cessation in the future (OR, 1.59; 95% CI, 1.05 to 2.43) were more likely to experience the e-cigarette. Especially, smokers who used smoking cessation alternative supplies were more likely to experience the e-cigarette than smokers who did not use it (OR, 2.87; 95% CI, 1.59 to 5.18). Conclusion: We confirmed that e-cigarette experiences were associated with smoking cessation experience, smoking cessation plan, and use of smoking cessation aid among smokers in Korea. Additional studies should be continued to confirm whether e-cigarette are continuously influencing the smoking cessation of current smokers in South Korea as one of the smoking cessation alternative supplies.

The purpose of this study was to examine how socio-demographic characteristics and delinquent behaviors affect the suicidal ideation among the Korean high school students. Using the 35,668 senior high school student cases of 2009 Korean Youth Health Risk Behavior On-line Survey data, it conducted a secondary data analysis. Frequency analysis, chi-square test and logistic regression analysis was done. Being female, living without parents, low economic status, poor academic record, technical school students, young students, poor academic record, internet addition, problem drinking, smoking cigarettes and experience of drug use significantly increased probability of their suicidal thoughts. Findings may be used for tackle the issues of suicidal thoughts among the Korean senior high school students.
This study aims to determine the effect of e-cigarettes on oral health by investigating the association between the use of different tobacco products and oral health among Korean adults aged 19 years and older. Data from the 2017 Community Health Survey were used for the study. Respondents were divided into four groups: non-smokers, cigarette smokers, e-cigarette smokers, and users of both products. A sample of 228,357 respondents was selected for analysis. Twenty-four questionnaires with missing values (non-response or refusal) were excluded from the sample. A regression analysis was performed with oral as the dependent variable. A multivariate regression analysis showed a significant difference between cigarette smokers and users of both products when compared to the non-smokers. However, e-cigarette users showed a significant when the variables were correlated with age and gender. There was no significant difference in other dependent variables in a multivariate regression analysis. The results of the study indicated no association between e-cigarette use and oral health. More research is needed on factors such as amount and intensity of e-cigarette use.
