Purpose: This study aimed to clarify the needs for hospital-based home care nursing medical services in elder care institutions by analyzing the details and frequency of medical services provided by, and the needs for, hospital based home care nursing in select institutions in Korea. Methods: Seventy-seven staffs at elderly care institutions located throughout the country completed self-report questionnaires between February 1 and May 31, 2009. SPSS ver. 14.0 was used for data analysis regarding frequency and percentage, mean and standard deviation. Results: Forty-eight hospital-based home care nursing medical services in eight domains were identified as being needed in elderly care institutions. The most commonly used medical services were providing instruction in oral drug administration, checking drug beneficial/adverse effects, and administering blood glucose test, while the most needed medical services requiring hospital based home care nursing were complex pressure ulcer care, followed by diabetic foot ulcer management and nutrient injection. Conclusion: The present results should provide fundamental data for better healthcare services with hospital based home care nursing at elderly care institutions as part of a 'win-win' strategy through which medical expenses are reduced, insurance costs are kept stable, and safe and high-quality medical services are provided for residents of elder care institutions. Political decisions intended to promote visits by hospital based home care nurses to elder care institutions would be a prudent course.
Purpose: The purpose of this study was to analyze research trends, using the keyword home health care, in articles published in the Journal of Korean Academic Society of Home Health Care Nursing over the past 10 years. Methods: An analysis was conducted of 50 home health care-based studies chosen from among the 206 studies published in the Journal of Korean Academic Society of Home Health Care Nursing from 2010 to 2019. The analysis focused on research methodology and keyword. Descriptive statistics were used to examine the frequency distribution of research methods and keywords. Results: Study participation was mainly focused on nurses (52.0%). Most of the studies used quantitative methods (96.0%), and 43 studies (86.0%) used self-report structured questionnaires. The most commonly used data analyses methods were descriptive statistics, t-test, analysis of variance, correlation, and regression. Major keywords were home health nursing, elderly care facility, visiting nurse, home care service, home healthcare nurse, home care agencies, long-term care, and home care. Conclusion: The results of this study identified current trends and interests in the Journal of Korean Academic Society of Home Health Care Nursing. This study suggests that future studies include a variety of research methods and maintain appropriate standards of research ethics.
Purpose: We aimed to describe and compare the cognitive function, anxiety, and quality of life for the demented elderly in day care centers or nursing homes. Methods: We selected 44 subjects from day care centers and 63 subjects from nursing homes by performing convenience sampling. The Korean version of Mini Mental Status Examination was used for measuring cognitive status; Rating Anxiety in Dementia, for assessing anxiety; Korean Version of Quality of Life-Alzheimer's Disease, for assessing the quality of life. The data were collected from May to June 2008 and analyzed by SPSS/WIN 14.0. Results: 1) The cognitive function of the subjects in the day care center was higher than that of the subjects in the nursing home. Anxiety in the subjects in the nursing home was higher than that in the subjects in the day care center. 2) The cognitive function and quality of life of the subjects in the day care center showed statistically significant correlation, 3) The anxiety and quality of life of the subjects in the nursing home showed statistically significant correlation. Conclusions: Nursing interventions that improve the quality of life of the demented elderly should be considered and applied according to the degree of cognitive function and anxiety.
The number of residents in elderly institution has been increasing due to the change of the family support system. This study was focused on understanding the process of adjustment of the institutionalized elderly using the Grounded Theory approach. Method: There were seven participants, 4 men and 3 women living in 3 different elderly facilities. The data was collected through in-depth interviews and participant observation from June 20, 1999 to January 10, 2000 and analyzed by the Strauss and Corbin's analysis method. Result: 125 concepts were found and grouped into 30 sub-categories and then grouped into 13 categories. These categories are , , , , , , , , , , , and , which were synthesized into the process of adjustment. being the core category. The adjustment process of the facility elderly consisted of : 1. expressive phase of 'reluctance' 2. control phase of 'reluctance' 3. latent phase of 'reluctance' Conclusion: This study offers better understandings on the adjustment process of the institutionalized elderly and provides more appropriate nursing care to the New Comers of these facilities.
The principal objective of this study was to analyze and conceptualize the socio-psychological burden in utilizing a nursing home for elderly. The subjects were five elderly from a private nursing home located in Seoul and their familial caregivers. An old male and three females were currently staying at the facility, and a female had been discharged already from there. Data were collected through depth interviews, observations and review of records at the facility For analysis, the data were classified by similar contents among significant expressions and factors in common. The subjects' motives to consider admission to the nursing home might be attributed to familial caregivers' burden, a shortage of support, environmental improvement and feeling of helplessness for the case elderly. The concept of burden is including family members' being badly off in living, their weariness, complications among family members, feeling psychological uneasiness, and hospital expenses. The identified image of nursing homes for the elderly in Korea was generally negative at the point of high cost, unreasonable requisites and limitations for admission to the facilities, inferior situations, and especially in that there were few long-term care facilities within the community boundary. From their experience of nursing homes, the interviewees have felt the sentiments of sorry for their old parents, with the thought of being an undutiful, bitterness, and empathy. Additionally, they expressed a sense of anxiety of relative deprivation against the fact that there were no long-term care facilities available for the middle class. On the basis of these, multi-dimensional needs could be identified for the elderly with chronic illnesses.
The purpose of this study is to increase the utility in terms of the elderly's welfare. The data introduced in this paper were collected by the constructed interview method, and 192 aged interviewees were selected from 4 rural areas: Namhaegun, Uiryeong-gun Imsil-gun and Sunchang-gun. From the descriptive analysis of the data, the results derived are as follows: 1) The financial supports are needed to remodel rural village community facilities as senior centers. 2) It is expected that the facility be a center for the aged people's living in a village, and that have multiple functions of general care service network health care, meal support and making friendship spaces for them. 3) Share housing function is also added to the facility in emergency conditions.

The results showed that most of the nurses and caregivers in elderly medical welfare facilities who worked with dementia patients did not have knowledge of their hearing loss. Even the facility managers did not know how to conduct hearing tests for their patients although they did recognize that some of their patients had a hearing loss. Eventually, actual practice was not possible at this moment by the professionals. However, our respondents did have a positive attitude toward screening for hearing loss and help their patients with dementia wear hearing aids if a clinical guideline was provided. We suggest to develop clear and precise clinical guidelines of the hearing screening test for the dementia patients due to the interrelationship between dementia and hearing loss. When these guidelines apply to elderly residents in a medical welfare facility, early diagnosis and treatment of their sensory loss will help alleviate their dementia as well.

Purpose: The purpose of this study was to investigate medication and related factors including: gender, age, duration of hospitalization, MMSE, and ADL in elderly who were hospitalized in long-term care facilities. Methods: A total of 282 elderly people who were hospitalized for more than a month were asked to fill in a structured questionnaire which was used to collect data regarding their gender, age, duration of hospitalization, MMSE, ADL, and medication. Results: The average number of drugs per patient was 7.50. Subjects taking 6-7 medications were the highest ranking (23.7%). According to multiple regression analysis, the most significant factor relating to the number of drugs prescribed for each subject were the ages of the subjects (p=.007). Conclusion: We found that the number of prescribed drugs taken by those who were hospitalized were related to age. A tailored strategy is needed to decrease the number of drugs according to age and to control the medicines prescribed to the elderly who are hospitalized in long-term care facilities.
본 연구는 장기요양서비스의 질 제고를 위해 장기요양서비스 이용 노인들의 신체적 및 정신적 기능 상태를 평가해 보고, 그에 영향을 미치는 관련 요인을 규명하고자 시도하였다. 조사대상은 대전지역에 거주하는 65세 이상 노인 중 장기요양서비스 이용자 350명(재가급여 178명, 시설급여 172명)으로 하였으며, 자료 수집은 2011년 2월 16일부터 3월 31일까지의 기간 동안에 표준화된 무기명식 면접조사용 설문지를 사용하여 면접조사를 통해 이루어졌다. 연구결과, 재가급여 노인의 IADL과 MMSE-K는 시설급여 노인보다 유의하게 높았으나 ADL과 CES-D는 유의한 차이를 보이지 않았다. ADL, IADL, CES-D 및 MMSE-K 상호 간의 상관관계를 보면, 재가 및 시설급여 이용노인 모두 ADL은 IADL 및 MMSE-K와 양의 상관관계를 보인 반면, CES-D와는 음의 상관관계를 나타내었다. IADL은 MMSE-K와 양의 상관관계를 보인 반면, CES-D와는 음의 상관관계를 나타내었다. 다중회귀분석 결과 재가 및 시설 급여 이용 노인 모두 ADL과 IADL에 유의하게 영향을 미치는 변수로는 건망증 증상 유무, 규칙적 운동 및 MMSE-K가 선정되었다. 또한 CES-D와 MMSE-K는 주관적 건강상태, 건망증 증상 유무 및 IADL이 유의하게 영향을 주는 것으로 나타났다. 이상과 같은 결과는 장기요양서비스 이용 노인들의 신체적 및 정신적 기능은 개인의 건강상태나 건강관련행위 특성과 밀접한 관련성이 있음을 시사하고 있다.
