Infrequent Hemorrhagic Complications Following Surgical Drainage of Chronic Subdural Hematomas

  • Chronic subdural hematomas mainly occur amongst elderly people and usually develop after minor head injuries. In younger patients, subdural collections may be related to hypertension, coagulopathies, vascular abnormalities, and substance abuse. Different techniques can be used for the surgical treatment of symptomatic chronic subdural hematomas : single or double burr-hole evacuation, with or without subdural drainage, twist-drill craniostomies and classical craniotomies. Failure of the brain to re-expand, pneumocephalus, incomplete evacuation, and recurrence of the fluid collection are common complications following these procedures. Acute subdural hematomas may also occur. Rarely reported hemorrhagic complications include subarachnoid, intracerebral, intraventricular, and remote cerebellar hemorrhages. The causes of such uncommon complications are difficult to explain and remain poorly understood. Overdrainage and intracranial hypotension, rapid brain decompression and shift of the intracranial contents, cerebrospinal fluid loss, vascular dysregulation and impairment of venous outflow are the main mechanisms discussed in the literature. In this article we report three cases of different post-operative intracranial bleeding and review the related literature.

Risk Factors of Prehypertension in Korean Adults: The Korean National Health and Nutrition Examination Survey 2005 (한국 성인의 고혈압 전기 발생 위험요인 분석: 2005년 국민건강영양조사)

  • Purpose: The purpose of this study was to investigate the risk factors related to prehypertension in Korean adults. Methods: The data were obtained from the Korean National Health and Nutrition Examination Survey 2005. The subjects of this study were 3,981 adults aged over 20 years of age. Demographic characteristics, alcohol drinking, smoking, physical activity, stress, BMI, serum lipid profiles and blood pressure were analyzed in this study. Results: Prevalence of prehypertension was 38.9% in this study. As the result of multiple logistic regression, the risk of prehypertension in male, elderly and low income persons was increased. And the risk of prehypertension was increased in the case of problem drinking, alcohol abuse, formal smoker, overweight, obesity and hyperlipidemia in triglyceride. Conclusion: To decrease prehypertension prevalence, it is necessary to detect and manage the influencing risk factors of prehypertension such as alcohol drinking, smoking, obesity, physical activity, stress and serum lipid profiles.

Thiamin and Riboflavin Nutritional Status of Subjects with Alcohol Dependency in Rural Area (농촌지역 알코올 의존자들의 비타민 $B_1$, $B_2$ 영양상태)

  • The prevalence of alcoholism among elderly population is reported to be high in rural areas in Korea. Chronic abuse of alcohol can lead to the development of vitamin B deficiency through inadequate intake, altered absorption and metabolism, and increased excretion. The present study was conducted to assess vitamin B1 and B2 status in seventeen alcohol dependent subjects who do not exhibit any clinical neurological symptoms. Vitamin B1 and B2 nutritional states were determined enzymatically by measurement of transketolase and glutathione reductase activities in erythrocytes, respectively. And dietary intakes of nutrients were determined by a 24-hr recall method. The mean percent activation of erythrocyte transketolase was significantly higher in alcoholics than in alcoholics than in control (p<0.05). The proportion of subjects with a low and borderline status of vitamin B1, was significantly higher in alcoholics than in control (p<0.05). The mean percent activation of erythrocyte glutathione reductase was not different between alcoholics and control. And the proportion of subjects with low and borderline status of Vitamin B2, was higher in alcoholics than in control (p<0.1). Vitamin B1 and B2 status were significantly decreased in alcoholics who were smoking cigarettes compared to non-smoking and non-alcoholic subjects(p<0.05). Whether vitamin supplementation improves the vitamin status of alcohol dependent subjects remains to be researched.

  • Purpose: This study is designed to provide care workers with basic data on the development of job training programs for enhancing professionalism of senior care services by identifying and analyzing job recognition and education requirements according to the type of work place. Methods: Data were collected from April to July 2019 from 177 care workers working at the Elderly Care Facilities, Elderly Housing Welfare Facilities and Rehabilitation Facilities in the B Metropolitan City. It was analyzed using the SPSS Win 21.0 Statistical Program. Results: The results of job recognition of care worker according to the type of work place shows as follows. As for health care welfare facilities for senior citizens and residential welfare facilities for the elderly was the highest level for physical change and mobile care protection belonging to basic care protection technologies. For the rehabilitation and welfare facility, human rights and prevention of abuse among senior citizens which is belonging to occupational ethics and attitudes of care workers showed the highest. Conclusion: It is required for differentiated job training to enhance capacity for care workers according to the type of work place, and it is needed to developed systematic and specific customized hands-on training to perform, not for simple knowledge.

A Study on the Current State of the Integrated Human Rights of the Elderly in Rural Areas of South Korea (농촌지역 거주 노인의 통합적 인권보장 실태에 관한 연구)

  • This study purported to investigate the current state of human rights of older adults residing in rural areas of Korea. The study utilized, as an analytic framework, 4 priority directions (1. "older persons and development", 2. "rural area development", 3. "advancing health and well-being into old age", and 4. "ensuring enabling and supportive environments") with 13 task actions recommended by Madrid International Plan of Action on Ageing (MIPAA). Furthermore, the study examined gender differences in all items included in the analytic framework. Data was collected by the face-to-face survey on 800 subjects aged 65 and over. Statistical analyses were conducted using STATA 13.0 program. The main results were summarized in order of 4 priority directions as follows. First, average working hours per day were 6.2, and men reportedly participated in economic activities and needed job training more than women, while women participated in lifelong education programs more than men. Awareness of fire and disaster prevention facilities was low in both genders. Second, accessibility to the support center for the elderly living alone as well as protective services for the vulnerable elderly was found to be low. IT-based services and networking were used more by men than women, and specifically, IT-based financial transactions and welfare services were least used. Third, medical check-ups and vaccinations were well received, while consistent treatments for chronic illnesses and long-term care services were relatively less given. In addition, accessibility to mental health service centers was considerably low. Fourth, although old house structures and the lack of convenience facilities were found to be circumstantial risk factors for these elders, experiences of receiving housing support services were scarce. The elderly were found to rely more on informal care, and concerns for their care were higher in women than men. Plus, accessibility to elderly abuse services was markedly low. Based on these results, discussed were implications for implementing policies and practical interventions to raise the levels of the human rights for this population.

FAMILY DYNAMICS OF INCEST PERCEIVED BY ADOLESECENTS (청소년이 지각한 근친상간의 가족역동)

  • Family is a primary unit of the major socialization processing for children. Parents among the family members are one of the most important figures from whom the child and adolescent acquire a wide variety of behavior patterns, attitudes, values and norms. An organization of family members product family structural functioning. Abnormal family structure is one of the most important reference models in the learning of antisocial patterns of behavior. Therefore incest and child sexual abuse including spouse abuse, elderly abuse, and neglect occurs in the abnormal family structural setting. In particular, incest, a specific form of sexual abuse, was once thought to be a phenomenon of great rarity, but our clinical experiences, especially over the past decade, have made us aware that incest and child sexual abuse is not rare case and on the increasing trend. Therefore, the aim of this study was to determine the family problem and dynamics of incest family, and character pattern of post-incest adolescent victim in Korea. A total of 1,838 adolescents from middle and high school(1,237) and juvenile correctional institute(601) were studied, sampled from Korean student population and adolescent delinquent population confined in juvenile correctional institutes, using proportional stratified random sampling method. The subjects' ages ranged from 12 to 21 years. Data were collected through questionnaire survey. Data analysis was done by IBM PC of Behavior Science Center at the Korea university, using SAS program. Statistical methods employed were Chi-square, principal component analysis and t-test etc. The results of this study were as follows ; 1) Of 1,071 subjects, 40(3.7%) reported incest experiences(sibling incest : 1.6% ; another type of incest : 2.1%) in their family setting. 2) The character pattern of post-incest adolescent victim was more socially maladjusted, immature, impulsive, rigid, anxious and dependent than non-incest adolescent. Also they showed some problem in academic performance and their assertiveness. 3) The other family members of incest family revealed more psychological and behavioral problem such as depression, alcoholism, psychotic disorder and criminal act than the non-incest family, even though there is no evidence of the context between them. 4) The family dynamics of incest family tended to be dysfunctional trend, as compared with non-incest family. It showed that the psychological instability of family member, parental rejection toward their children, coldness and indifference among family member and marital discordance between the parents had significant correlation with incest.

A Study on the Effects of Health Behavior upon Health Status in Some Old People (일부 노인의 건강행동이 건강상태에 미치는 영향)

  • Elderly problem from being aging society, especially health related problem of the elderly is very serious in many parts of this country. The reason is that most of geriatric disease are chronic and debilitating. The cause of chronic and debilitating disease are bad lifestyle and wrong health habit. Health is affected by a result of interaction of environment and human being. Because of difference of lifestyle between a city and a farm village, health behavior and health status of urban elderly and rural elderly may be dissimilar. Thus the purpose of this study was to grasp health behavior and health status, to identify the factors that effect on health status of the elderly. The subfects for this study, 488 persons aged 60 and over who live in Seoul or Cheonbuk Province. The preliminary survey was carried out from Aug. 19, to Aug. 22, 1996. With complement of questions, main survey was carried out from Sep. 29, to Oco. 10, 1996. The data was analysed by using in SPSS/PC+ program. The results were as follows. 1. General Characteristics 1) In the individual characteritics of the respondents, Seoulites aged 80 and over were 24.7%, the average age was 73.14 years old and rural residents aged 60-69 were 63.7%, the average age was 68.90 years old. In Seoul, 142 men and 101 women were respondents. In Cheonbuk Province, 101 men and 144 women were answered. In Seoul, those who graduated form elementary school were 35.4%, in farming region, illiteracy persons were 44.9%. In Seoul, 47.7% of respondents had spouse and in farming village, 66.1% of respondents had spouse. 39.0% of respondents who's imcome type was independent were Seoulite, and 66.1% of respondents who's income type was independent were rural residents. Employed persons in Seoul and in rural region were 16.9% and 62.0%. 2. Health Behavior 1) For the health behavior total score, the difference by region was not statistically significant. But the score of individual item was different and statistically significant. 2) For the Seoulites, younger person(p〈0.01), the female(p〈0.001) showed better health behavior and for the farming village residents, younger person(p〈0.01), the female(p〈0.01), independent income type(p〈0.05), employed person(p〈0.05) showed better health behavior. 3. Health Status 1) For the self-rated health status total score, the difference by region was statistically significant and individual item score was different and statistically significant. For ADL and IADL total score, the difference by region was not statistically significant, but individual item score was different and statistically significant. 2) For the Seoulites, woman(p〈0.05), lower education(p〈0.00l), independent income type(p〈0.05) showed higher score in self-rated health status. For rural residents, woman(p〈0.05), lower education(p〈0.01), independent Income type(p〈0.001) showed higher score in self-rated health status. For the Seoul residents, younger person(p〈0.001), employed(p〈0.05) showed higher score in ADL and IADL, and for the farm area residents, younger person(p〈0.001), higher education(p〈0.01), having spouse(p〈0.001), family type(p〈0.01) showed higher score In ADL and IADL. 3) For the Seoulites, drinking(p〈0.05), breakfast(p〈0.05), exercise(p〈0.05) and for the rural residents, drinking(p〈0.05), deep sleeping(p〈0.05), exercise(p〈0.01), washing hands before meal(p〈0.01) showed higher score In self-rated health status. For the Seoulites, deep sleeping(p〈0.05), exercise(p〈0.05) and for the farm village residents, fruit(p〈0.05), deep sleeping(p〈0.05), exercise(p〈0.001) showed higher score in ADL and IADL. We carried out this study to analyze the effectiveness through health education program in short term which was performed to use the special subject activities. This study was conducted on 63 students who were first grade in S Junior High School from Dec. 1995 to Feb. 1996. To analyze the effectiveness, we performed the Pretest, 1st Posttest, and 2nd Posttest for learned health knowledge. The results were as follows: 1. Most of the students(69.8%) responded that their health were good, and they got the information for health through Mass Com.. The students who had experience of health education were 15.9%, and the 77.8% of the respondents needed the health education. 2. The means of health knowledge on tests were 18.2(Pretest), 21.5(1st Posttest), and 21.4(2nd Posttest). Increase of health knowledge between Pretest and 1st Posttest was 10.9%. 3. The mean of differences between Pretest and 1st Posttest was 3.26, it was significant(p〈0.01). And the mean of differences between Pretest and 2nd Posttest was 3.19, it was significant(p〈0.01);however, the mean of differences between 1st Posttest and 2nd Posttest was not significant(p=0.2514). 4. The significant main contents were Health Facilities(d=0.42), Pregnancy and Labor(d=0.39), Hygiene(d=0.35), Safety Education(d=0.66), and Drug Abuse(d=0.60)(p〈0.01).

The Health Promotion Programme and Quality of Life in the 21 Century (21세기 삶의 질 향상을 위한 건강증진방안)

  • The ultimate goal of national health promotion services is the improvement of quality of life and health longevity through the implementation of health promotion services. The approach strategy for national health promotion summariged as follows: 1) A model for health promotion should be developed by the level of government. 2) Roles and functions between central government and autonomous local governments should be defined to carry out the health promotion services effectively. 3) New manpower for health promotion such as health educator should be trained and activated at hospitals, health centers, industries, school, and related community agencies. 4) School health education should be strengthened in order to teach: various health subject(smoking & alcohol, drug abuse, accident and safe, nutrition, environmental pollution and preservation, population & family planning, personnel hygiene, physical growth, stress, sex education, communicable disease, physical exercise etc) students through appointing health teachers at school base. 5) Health promotion services in industries should be activated using manpower such as health educator, exercise instructor, dietist and counsellor, 6) Health promotion services for the elderly should be activated. 7) Health screening services in the medical insurance and his/her family should be activated for health promotion services. 8) Health education material development center for health promotion should be established and the materials should be made to distribute to related groups, agencies and institutions (health conte.5, hospitals, schools, pharmacies, industries etc). 9) The pilot health promotion center in each automous local governments(large cities, provinces, Guns and Gu level) should be established and operated for community people. 10) The mass media such as TV, radio, newspapers and magazines should be used effectively. 11) Periodic evaluation of health promotion services should be carried out in order to help effective and successful planning for community health promotion in the future.

Effect of Sociodemographic Factors, Cancer, Psychiatric Disorder on Suicide: Gender and Age-specific Patterns (인구사회적 요인, 암, 일부 전신질환 등이 자살에 미치는 영향: 성별, 연령별 분석)

  • Objectives : We examined the effect of sociodemographic factors, cancer, and psychiatric disorders on suicide by gender and age-specific patterns in South Korea. Methods : The study is a case-control study. Claim data was obtained from the national health insurance database and national death registration database. The number of people who committed suicide was 11,523, which was matched with a control group consisting of ten times as many people at 115,230 selected from the national health insurance and medical aids beneficiaries. The medical utilization of the case group was one year before death and that of the control group was from July 1,2003 to June 30, 2004. Four variables-address, economic status, presence of a psychiatric disease, and cancer-were used in multiple logistic regression analyses. Results : Living in cities or in rural areas showed a greater risk for suicide than living in a metropolitan city. Low economic status, the presence of a psychiatric disorder, and cancer were also statistically meaningful risk factors for suicide. The three major psychiatric diseases, schizophrenia, alcohol abuse, and bipolar disorder, were meaningful in all age groups, but the scale of the odds ratio differed by the age group. Only the psychiatric disorder variable was meaningful in the adolescent group, whereas a psychiatric disorder and economic status were meaningful for the young adult group, and all variables were meaningful for the middle-aged group. A psychiatric disorder and cancer were meaningful in the elderly group, economic status was meaningful for male subjects, and address was meaningful for female subjects. Conclusions : Factors such as living in city or rural areas, low economic status, the presence of a psychiatric disorder, and cancer were statistically meaningful risk factors in suicide. These factors also differed by age group. Therefore, policymakers should establish policies for suicide prevention that are relevant for each age group.

Study on Use of Mung Bean Porridge Reported in Daily Records of Royal Secretariat of Joseon Dynasty (「승정원일기」에 기록된 녹두죽 활용에 대한 고찰)

  • Medicinal porridge (藥粥) is made by putting rice or grains into a pot and boiling with water until a semi-liquid texture is obtained. Porridge is digested fast and absorbed by the body. Children or elderly people with weak digestive ability can eat it without difficulty. The authors of this literature focused on utilization of mung bean porridge (Nokdu-juk) based on SeungjeongwonIlgi (承政院日記), which is the daily record of the Royal Secretariat of the Joseon Dynasty from 1623 to 1910. Four cases were found. The first case used mungbean for treating fever diseases (熱症), the second as a meal alternative, the third for wound recovery, and the last case described the culture of the joseon dynasty. According to the actual cases in SeungjeongwonIlgi (承政院日記), mung bean porridge (Nokdu-juk) was actively used to lower fever and strengthen the spleen and stomach. When suffering from the after effects of a malignant tumor or serious liver disease (肝病), an infectious disease, or a serious physical wound, people ate mung bean porridge (Nokdu-juk). It is suggested that mung bean porridge (Nokdu-juk) can be used as an alternative food treatment for the current medical field due to the prevalence of pain-killers and psychotropic drug abuse.
