  • Objectives: To report effects of M&L psychotherapy combined with Korean medical treatments on a patient with chronic low back pain. Methods: We treated a patient with chronic low back pain using M&L psychotherapy alongside a combination of Korean medical treatments, which included acupuncture, pharmacopuncture, cupping, and herbal medicine. The patient was diagnosed based on the ICD-11 diagnostic criteria for chronic primary pain. To evaluate the patient's progress, we used the Numerical Rating Scale (NRS), Oswestry Disability Index (ODI), Visual Analogue Scale (VAS), Beck's Depression Inventory (BDI), and Beck's Anxiety Inventory (BAI). Results: After three weeks of inpatient treatment and two months of outpatient treatment, the patient showed significant improvement in both pain and psychological symptoms. The NRS score decreased from 7 to 2. The BDI score decreased from 54 to 11 and the BAI score decreased from 33 to 9. Conclusions: This study demonstrates that M&L psychotherapy combined with Korean medical treatments can significantly reduce chronic low back pain and improve psychological symptoms such as depression and anxiety. The patient's notable improvements in NRS, BDI, and BAI scores suggest that an integrated treatment approach addressing both physical and mental health aspects is effective for managing chronic pain. These findings support potential benefits of holistic and multidisciplinary strategies for treating patients with chronic pain.

  • Purpose: The aim of this study was to investigate the level of resilience and related factors in patients with Parkinson's disease (PD) in Korea. Methods: Data were obtained from 148 patients using the Resilience Scale (RS), Beck's Depression Inventory (BDI), and Spielberger's Anxiety Inventory (AI). Results: The mean scores of the RS, BDI, and AI were $127.7{\pm}21.6$, $12.9{\pm}9.3$, and $41.9{\pm}11.1$, respectively. The RS score was strongly correlated with the BDI score (r=-.531, p<.001) and the AI (r=-.572, p<.001). The resilience was significantly revealed by household income (F=4.002, p=.009) and presence of a hobby (t=-3.300, p=.001). In addition, resilience was significantly correlated with age of disease onset (r=.164, p=.046), years of living with PD (r=-.262, p=.001), and the length of treatment with levodopa (r=-.283, p<.001). From the stepwise multiple regression analysis, the most important factors related to the RS score were the AI score, household income, and length of treatment with levodopa. Conclusion: Understanding these factors is essential for developing effective interventions to improve resilience in patients with PD.

  • PURPOSE: This study aimed to determine the changes in the lumbar herniation index, Oswestry disability index (ODI), visual analog scale (VAS), and lumbar flexion range of motion (ROM) following the application of high velocity- low amplitude (HVLA) technique and depression therapy in patients with lumbar herniation discs, and thus to provide a clinical basis for the treatment of lumbar herniation discs. METHODS: We included 45 patients with lumbar herniation discs who were assigned equally to three groups: HVLA technique, depression therapy, and control group. Three times a week for 4 weeks, conservative therapy plus Maitland's lumbar spine rotation technique was applied to the HVLA technique group for 30 min and conservative therapy plus decompression therapy for 30 min to the decompression therapy group, while only conservative therapy was applied to the control group. The lumbar herniation index and Korean version ODI were measured twice before starting and after completing the treatment. The VAS and lumbar flexion ROM were measured before and after each treatment session for twelve. The collected data were analyzed using SPSS software version 21.0. RESULTS: The lumbar herniation index was significantly lower in both the HVLA technique and decompression therapy groups compared to the control group, with decompression therapy being the most effective in reducing the lumbar herniation index. Significant improvements were observed in the ODI, VAS score, and lumbar flexion ROM across all three groups, with HVLA technique being the most effective. CONCLUSION: HVLA Techniqueand decompression therapy were more effective than conservative therapy in reducing the lumbar herniation index, ODI, and VAS scores, and in increasing lumbar flexion ROM. This suggests the importance of combining HVLA technique or decompression therapy along with conservative physical therapy for the effective treatment of lumbar herniation discs.

  • Objectives Hwangnyeonhaedok-tang is a Korean herbal medical treatment that removes toxic heat, fever and inflammation. The purpose of this study was to investigate the effect of Hwangnyeonhaedok-tang treatment on the relief of atopic dermatitis (AD) through regeneration of skin lipid barrier. Methods Male NC/Nga mice (20 g, 6 week age) were used. Each 10 mice were allocated to the control group (Ctrl), the AD-induced with no treatment group (AE), and the group which induced AD after administering Hwangnyeonhaedok-tang extract (HT). To induce AD-like skin lesions, sodium dodecyl sulfate (SDS) (Sigma-Aldrich, USA) was rubbed on the back of each mouse to remove the lipid lamella of the stratum corneum, and Dermatophagoides (D.) farinae crude extract was applied. HT group was orally administered Hwangnyeonhaedok-tang after induction of AD. IL-4 IL-13, $p-I{\kappa}B$, iNOS, Sudan Black B (SB), loricrin, and filaggrin were observed to confirm the effect. Results In HT group, AD skin score was decreased by 46%. The cytokine IL-4 and IL-13, which can identify Th2 differentiation, was reduced by 73% and 58% each. Anti-inflammatory effects were observed in $p-I{\kappa}B$ and iNOS by 69% and 54%, respectively. Finally, SB showed that the regeneration of the lipid layer and the increase of the regeneration power of loricrin and filaggrin were increased by 437% and 464%, respectively. Conclusions From the study result, we observed that Hwangnyeonhaedok-tang treatment alleviates AD by decreasing skin score, reducing Th2 differentiation, inducing anti-inflammatory, and increasing skin lipid barrier regeneration. Thus, Hwangnyeonhaedok-tang treatment would be considered as an effective AD relieving treatment.

  • Objectives : This study reviews the latest articles in Korea and other countries that studied oriental medicine treatment on poststroke depression. Methods : Korean articles were retrieved from the 9 major Korean web article search engines. Foreign articles were retrieved from PubMed. Article published date was from 2000 up to September 2012. There were no restrictions on the types of publication, but articles not available in full text were excluded. The methodological quality was assessed according to Cochrane's assessment of risk of bias and Newcastle-Ottawa quality assessment scale. Results : Twenty-two articles were included in this study. Eleven articles were published in Korea, the rest were published in China. Nine articles were randomized controlled trials (RCT), one article was a non-randomized study (NRS), four articles were case reports, three articles were cross-sectional studies, two articles were comparative studies. In RCT articles, risk of selection bias and performance bias were generally high, risk of detection bias was unclear. The NRS article took four stars in Newcastle-Ottawa quality assessment. Comparison Hamilton rating scale for depression score between oriental medicine treated group and western medicine treated group revealed that there was no remarkable difference in mean score changes after treatment on PSD. Conclusions : The results of this study suggest that oriental medicine treatment is as effective as western medicine treatment for PSD. In the future, more rigorous oriental medicine treatment studies should be conducted.

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.
