The Comparative Study on the Acupuncture at Affected Limb and Unaffected Limb on Treating Ankle Sprain of Acute Stage (급성기 족관절 염좌 환자에 대한 환측취혈(患側取穴)과 건측취혈(健側取穴)의 치료 효과 비교 연구)

  • Objectives : The purpose of this study is to compare the effects of the acupuncture at affected limb and unaffected limb on treating ankle sprain of acute stage. Methods : From October 1st, 2007 to March 23rd, 2008, the 60 patients who had visited Dongshin mokdong oriental medical hospital and Gwangju oriental medical hospital, Dongshin university with acute ankle sprain were divided into 2 groups ; one group took acupuncture at affected limb, and the other group took acupuncture at unaffected limb. To evaluating the efficiency of each treatment, Visual Analog Scale(VAS) and Ankle-Hindfoot Scale(AHS) were applied before treatment and after 3rd treatment. Results : As a result of evaluating by using VAS, the score of acupuncture at unaffected limb was marked lower than the score of acupuncture at affected limb. As a result of evaluating by using AHS, the score comparison between the two groups had no significance after treatment. Conclusions : Acupuncture at unaffected limb is more effective than acupuncture at affected limb on treating ankle sprain of acute stage.

The Effect of Spinal Cord Stimulation in Patients with Complex Regional Pain Syndrome (복합부위통증증후군 환자에서 척수자극술의 효능)

  • Background: Complex regional pain syndrome (CRPS) is a painful, disabling disorder for which no proven treatment has been established. The purpose of this investigation was to assess the evidence of the efficacy of spinal cord stimulation (SCS) in the management of pain in CRPS patients. Methods: Between March 2004 and June 2006, 11 patients with CRPS were treated with SCS. The visual analog scale (VAS) score for pain (0⁣-10) and pain disability index (PDI) were obtained in all patients prior to treatment, and 1, 3 and 6 months post-implantation. Results: All 11 patients, 5 men and 6 women, with a median age and duration of CRPS of 44 years and 48.8 months, respectively, successfully received a lead implantation for SCS. The mean VAS pain score prior to the treatment was 85.5 out of 100 mm. After SCS implantation, the mean VAS pain scores were 49.5, 57.0 and 56.0 at 1, 3 and 6 months after the procedure, respectively. The mean pain score for allodynia was decreased by 50%, with a significant reduction of the PDI also observed after the treatment. Conclusions: Our current study suggests that SCS implantation is a safe and effective method in the management of CRPS patients.

  • ■Objectives This study is to report the effectiveness of Korean medicine on the treatment for multiple system atropy(MSA-C) patient complaining of autonomic dysfunction. ■Methods The patient was treated with Korean medicine(mainly DokhwalJihwang-tang Gami-bang) for 39 days. The evaluations were performed using UMSARS(Unified Multiple System Atrophy Rating Scale), SCOPA-AUT(The Scale for Outcomes in Parkinson's disease-Autonomic), K-OGS(Korean version of the Orthostatic Grading Scale). ■Results After treatment, the UMSARS score decreased from 25 to 18, the SCOPA-AUT score decreased from 21 to 14, K-OGS score decrease from 15 to 8. ■Conclusion This case suggests that Korean medicine treatment may be effective for MSA-C patients with autonomic dysfuction.

A Case Report on Korean Medical Treatment for a Patient Hospitalized with Acute Compression Fracture (한방병원에 입원한 압박 골절 환자에 대한 한방 치료 1례)

  • Objectives: This study aimed to determine the effects of Korean medical treatment on a patient with compression fractures. Methods: The patient was treated with herbal medicine and acupuncture for 51 days. Symptom changes were measured using the Numerical Rating Scale (NRS), Range of Motion (ROM), Oswestry Disability Index (ODI), and European Quality of Life Five Dimensions (EQ-5D) Scale. Results: The NRS score for lower back pain decreased from 7 to 3, and the ODI score decreased from 51.11 to 22.22. Range of motion (ROM) increased meaningfully in flexion(from 30 to 60), Left/Right lateral bending(from 10/30 to 30/45), Left/Right rotation (from 10/30 to 30/45), and the EQ-5D score also increased from 0.506 to 0.677. Conclusion: The results indicate that Korean medical treatment may be effective for managing patients' acute compression fractures.

A Case Report on Korean Medical Treatment for Delayed Neuropsychiatric Sequelae of Carbon Monoxide Poisoning (일산화탄소중독 이후 발생한 지연성 신경정신과학적 후유증에 대한 한방치료 증례보고 1례)

  • Objective: This clinical study describes the effect of Korean medicine on a patient with Delayed Neuropsychiatric Sequelae after Carbon Monoxide Poisoning. Case presentation: A patient with the delayed neuropsychiatric effects of carbon monoxide exposure was treated with herbal medicine, acupuncture, moxibustion, and cupping. Clinical symptoms were measured with the Korean Mini Mental State Examination (K-MMSE), Functional Independence Measure (FIM), and the Modified Barthel Index (MBI). After 32 days of treatment, the patient's K-MMSE score increased from 11 to 21 points, the FIM score from 52 to 94 points, and the MBI score from 46 to 84 points. There were no side effects. Conclusion: According to this case study, Korean medicine may be considered an effective treatment for delayed neuropsychiatric sequelae, although more studies are needed to confirm its validity.

Case Report: Complex Treatment Using Korean Medicine, Including Jaeumkunbi-tang, Acupuncture, and Limb Activation Exercises for a Patient Suffering Unilateral Neglect due to Intraventricular Hemorrhage Accompanied by Intraventricular Hemorrhage (뇌실내출혈을 동반한 뇌내출혈로 인한 편측무시 증상을 호소하는 환자에게 자음건비탕, 침치료, 사지운동을 포함한 한의복합치료 증례 보고 1례)

  • Objective: This study reports the effectiveness of the Korean medicine treatment of patients with intracerebral and intraventricular hemorrhages suffering unilateral neglect symptoms. Case presentation: The patient was treated using Korean medicine, including acupuncture, herbal medicine, moxibustion, and a limb activation exercise. We measured Unilateral Neglect Symptom progress with a numeric rating scale, Albert's test, Line bisection test, Clock drawing test, and Catherine Bergego Scale. After eight days of combined Korean medicine and rehabilitation exercise, the Unilateral Neglect Symptom intensity reduced from NRS 8 to 6. In addition, Albert's test score decreased from 2.5% to 0%. The Line bisection test score decreased from 19 mm to 12 mm, and the Clock drawing test score improved from 8 to 10. Last, the Catherine Bergego Scale decreased from 12 to 9. No adverse events were observed during treatment. Conclusion: This study suggested that a complex treatment including Jaeumkunbi-tang, acupuncture, moxibustion, and limb activation exercise can be effective for an intracerebral hemorrhage with intraventricular hemorrhage patient suffering from Unilateral Neglect Symptoms. Consequently, more research is needed in the future.

A Comparative Study of Anxiety, Pain and Maternal-fetal Attachment between Women who became Pregnant after Infertility Treatment and became Pregnant Naturally (난임 시술로 임신된 임산부와 자연 임산부의 출산 시 통증 및 태아애착행위 비교연구)

  • Purpose: This study was a comparative study to understand the levels of anxiety, pain and maternal-fetal attachment between women who became pregnant after infertility treatment and became pregnant naturally. Methods: This study used a comparative survey design. Data were collected by 50 couples of natural pregnancy and of who became pregnant after infertility treatment who visited delivery room in C Medical hospital, Seoul. These couples were to have first baby, and cervix dilatation of women was less than 3 cm regardless of diagnosis. Results: The score of anxiety of infertile women was significantly higher than that of naturally pregnant women; however, that of spouses showed no difference. The pain score for infertile women was significantly higher in both the active and transition phases. Pain scores that reported by their spouses did not show differences in either phase. The score of maternal-fetal attachment showed no difference between two groups of women. Conclusion: The result showed the importance of nursing intervention to reduce women's anxiety and pain, through both antenatal-childbirth education programs and assertive nursing interventions. It is necessary to develop and evaluate new intervention which would be more effective for reducing pain and anxiety for couples who became pregnant after infertility treatment.

Effect of Combined Traditional Acupuncture and Applied Kinesiology on Lumbar Diseases

  • Background: Several Korean medical doctors have been practicing applied kinesiology (AK), invented in 1964 by Dr. George J Goodheart, USA. Although the efficacy of traditional acupuncture (TA) and pharmacopucture treatment for lumbar conditions/diseases has been examined, the possible benefits of combining TA and AK have not been reported. Therefore, the aim of this study was to report the effects of combining TA and AK treatment for lumbar disorders using the Japanese Orthopedic Association lumbar score (JOALS) assessment. Methods: There were 21 outpatients treated at Samse Korean Traditional Hospital between March 2018 and September 2018, who presented with L4/L5 or L5/S1 root radiculopathy associated with lumbar spinal stenosis (LSS) and lumbar herniation of intervertebral disk (LHID). They were treated 10×(2 sessions per week, for 5 weeks) with TA and AK approaches that included a category block, manipulation or strain/counterstrain treatments. The primary outcome was mainly assessed using the JOALS score which was used before (0 ×), during (5 ×), and after treatment (10 ×). Results: There were 19 patients diagnosed with LSS and 2 were diagnosed with LHID. Using the JOALS assessment, TA and AK combined approaches improved the lumbar conditions of all 21 patients after 5 × treatmentsand continued to improve after 10 × treatments (p < 0.001). Conclusion: Combined TA and AK treatment was effective in treating spinal conditions/diseases. Prospective, relevant, well-controlled protocols for TA and AK therapies for various conditions are needed.

The Effects of Illustrated Book Information on Post Tonsillectomy Pain and Compliance of Care in Preschoolers (학령전기 아동의 그림동화책 제공이 편도선 수술 후 통증 및 치료이행에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effect of illustrated children's book in providing information about tonsillectomy to improve post tonsillectomy pain control and compliance to treatment for preschoolers. Methods: The design of the study was a quasi-experimental, post-test only design. Subjects were preschoolers aged between 3 and 7 years old who were admitted to a hospital for tonsillectomy. A group of 25 children were assigned to an experimental group, and another 25 were assigned to a control group. The data were analysed using SPSS 12.0 program. The homogeneity between two groups was examined using $x^2-test$. The hypotheses testing was conducted using t-test. Results: The experimental group had lower pain score than control group at 8 hour after tonsillectomy (p<.05). Pain score was also lower in the experimental group after 24 hour after the surgery (p<.05). The experimental group had higher pain questionnaire score than control group at 8 hours after surgery, however the difference was not significant. The experimental group showed better compliance to treatment than the control group (p<.05). Conclusion: Providing illustrated children's book for the purpose of providing information about tonsillectomy was effective intervention for the control of post tonsillectomy pain and improvement of compliance to treatment for preschoolers. Illustrated children's book could be applied as a nursing intervention to relieve post tonsillectomy pain and improve compliance to treatment for preschoolers.

The Effect of Taping Therapy on the Activity of Daily Living of Poststroke-Hemiplegic Patients: A Clinical Study (테이핑이 중풍환자의 마비측 일상생활 동작활동에 미치는 영향)

  • Objectives : In order to study the effect of taping therapy on the activity of daily living of poststroke-hemiplegic patients, a clinical study was performed. Methods : Twenty patients with poststroke-hemiplegia were randomized into two groups. Ten patients treated with kinesio taping. The other ten patients didn't treat with kinesio taping. Tapes were applied to the Deltoid, Supraspinatus, Infraspinatus, Brachioradialis, Gluteus Maximus, Adductor muscles of the hip, Sartorius and Tibialis Anterior. The activity of daily living was measured with a modifted barthel index(MBI). The taping therapy was performed twice a week for 2 weeks. Results : In terms of improvement of activity of daily living, the smaple group showed statistically meningful increase after 2 times treatment compared with control group(p<0.05). In terms of self care score of MBI, the sample group showed statistically meningful increase after 1 time treatment with the control group(p<0.05). In terms of mobility score of MBI, the sample group showed statistically meningful increase after 3 times treatment with the control group(p<0.05). Conclusions : These results support that taping therapy is significantly effective in improvement of the activity of daily living of poststroke-hemiplegic patients.
