체외순환에서 박동 혈류와 비박동 혈류가 관상동맥 혈류양상에 미치는 영향에 대한 비교 (Comparison of Pulsatile and Non-Pulsatile Extracorporeal Circulation on the Pattern of Coronary Artery Blood Flow)

  • 배경: 심정지와 같은 위급상황에서 관상동맥의 혈류를 유지하는 것은 심장근육의 보존과 회복 및 환자의 생명을 보존하는 데 중요하다. 최근 들어 Extra-Corporeal Life Support System (ECLS)의 기계식 순환장치의 사용으로 심정지 환자의 생명을 보존하고자 하는 노력이 시도되고 있다 본 연구는 체외순환 모델에서 박동성 혈류와 비박동성 혈류가 관상동맥의 혈류량 및 심근에 미치는 영향에 대해 알아보고자 하였다. 대상 및 방법: 실험은 $25\~35Kg$의 돼지 14마리를 각각 7마리씩 두 군으로 나누어 진행하였다. 제 1군은 비박동성 혈류 펌프인 원심펌프를 사용하였고 제2군은 이중 박동형 펌프를 사용하였다. 체외순환은 우심방에서 상행대동맥으로 심폐바이패스를 하고, 9V의 전기 충격으로 심실세동을 만들었다. 체외순환은 2시간 동안 유지하였으며, 펌프량은 두 군 모두 2 L/min로 유지하였다. 초음파를 이용한 관상동맥 혈류 측정장치를 이용하여 좌전하행지의 관상동맥 관류량을 펌프 시작 전(기초치)과 시작 후 20분마다 측정하였다. 또한 관상 정맥동의 혈액을 펌프 시작 전(기초치)과 시작 후 1시간, 2시간에 채취하여 두 군간의 심근효소의 차이와 대사물질의 차이를 비교하였다. 각 관찰지표의 군간 비교는 STATISTICA 통계프로그램(Version 6.0)의 Mann-Whitney U test를 이용하였고 통계적 유의수준은 p값이 0.05 이하인 경우로 하였다. 결과: 관상동맥의 저항지수는 제 2군에서 낮게 나타났으며, 펌프 구동 후 40분, 80분, 100분, 120분에서 통계적으로 의미 있게 나타났다 (p<0.05). 관상동맥의 평균 혈류 속도는 제 2군에서 펌프 구동 후 20분부터 의미 있게 높게 유지되었다(p<0.05). 관상동맥의 혈류량도 제2군에서 높게 유지되었으며, 펌프 구동 후 40분, 60분, 100분, 120분에서 통계적으로 의미 있는 차이를 보였다(p<0.05). 그러나, 관상정맥동의 혈액학적 검사에서는 두 군간에 차이가 없었다. 결론: 박동성 혈류는 비박동성 혈류보다 좌전하행지 관상동맥의 저항지수를 낮추고, 관상동맥의 관류속도를 빠르게 하여, 관상동맥으로의 혈류량을 높게 유지하였다.

단안 선택적 레이저섬유주성형술에서 안압 변동을 보정한 성공예측인자의 분석 (Predictors of Success of Selective Laser Trabeculoplasty Adjusted for Intraocular Pressure Variations)

  • 목적: 선택적 레이저섬유주성형술(selective laser trabeculoplaty, SLT)을 시행 받은 개방각녹내장 환자에서 치료받지 않은 반대안의 안압을 보정하여 SLT의 안압하강 효과 및 성공예측인자를 분석하고자 하였다. 대상과 방법: 단안에 SLT를 시행 받고 1년 이상 관찰한 개방각녹내장 환자 52명 52안을 후향적으로 분석하였다. 시술 전, 시술 후 1, 2, 3개월, 이후 3개월마다 안압을 측정하였다. 시술 후 안압은 치료받지 않은 반대안의 안압변화량으로 보정하여 계산하였다. SLT의 성공은 시술 전 안압보다 20% 이상 감소한 경우로 정의하였다. Kaplan-Meier 생존분석을 이용하여 성공률을 산출하였고, 성공예측인자를 분석하였다. 결과: 시술 전 평균 안압은 $23.17{\pm}6.96mmHg$, 시술 후 1년째 평균 안압하강은 $5.59{\pm}4.78mmHg$ (29.7%)였으며, 반대안 안압을 보정한 안압하강은 $4.70{\pm}4.67mmHg$ (23.9%)였다. SLT의 성공률은 65.4%, 반대안 안압을 보정한 성공률은 53.9%였다. 시술 전안압만이 SLT 성공과 유의한 상관관계를 보였고, 시술 전 안압이 높을수록 시술 후 안압하강이 크게 나타났다(p=0.025). 나이와 시야검사의 mean deviation은 SLT 성공과 유의한 상관관계가 없었다(p=0.066, p=0.464). 결론: 개방각녹내장 환자에서 SLT는 안전하고 효과적인 치료 방법이며, 시술 전 안압만이 SLT의 성공과 유의한 상관관계를 보였다. 정확한 치료 효과 분석을 위하여 반대안의 안압 변동을 고려하여야 하겠다.

인후두 역류질환에서 N-Acetyl Cysteine의 증상 개선 효과에 대한 다기관 평가 (Multicenter Evaluation on the Efficacy of N-Acetyl Cystine in Relieving the Symptoms of Laryngopharyngeal Reflux Disease)

  • Background and Objectives : Laryngopharyngeal reflux disease (LPRD) is relatively common disease. N-acetyl cysteine (NAC) has both mucolytic and antioxidant effect, also may be beneficial in inflammatory airway diseases. The purpose of this study was to evaluate the efficacy and safety of inhaled NAC therapy in LPRD. Materials and Method : We retrospectively reviewed the medical records of 525 LPRD patients at 12 medical centers. Finally 401 patients subjected to inhaled NAC therapy for 2 months were enrolled in the study. We analyzed the change of Reflux Symptom Index (RSI) and Reflux Finding Score (RFS) after use of NAC for 4 weeks and 8 weeks in addition to the patient's compliance of the treatment. Results : The RSI score significantly decreased from $19.87{\pm}6.34$ to $12.78{\pm}6.93$ after 4 weeks and to $10.65{\pm}7.47$ after 8 weeks. The RFS score also significantly decreased from $9.29{\pm}3.4$ to $7.17{\pm}3.41$ after 4 weeks and to $6.1{\pm}3.73$ after 8 weeks (p<0.05). During the treatment periods, 42 patients (10.4%) reported to have 80 episodes of discomfort. Throat discomfort (33%) and nausea (28%) were most common complaints, but the duration of discomfort was usually less than 4 weeks. Conclusion : Inhaled NAC treatment is highly effective for the reduction of both subjective and objective findings in LPRD patients. This study will provide the evidence of new treatment option for patients with LPRD. However, further studies will be needs to assess the real effect of inhaled NAC therapy as a standard treatment regimen of LPRD.

  • Background and Objectives Globus pharyngeus is one of the most common symptoms of patients visiting otorhinolaryngology out-patient clinic, and usually long-lasting, difficult to treat, and frequently recurrent. Mucomyst®, N-acetyl cysteine is an inhalation agent mainly used for mucolysis and reducing inflammation in airway via antioxidative effect. The purpose of this study was to evaluate the efficacy of inhaled Mucomyst® treatment in patients with globus pharyngeus refractory to proton pump inhibitor (PPI). Materials and Method We prospectively evaluated the efficacy of Mucomyst® in relieving symptoms of globus pharyngeus refractory to PPI in nine medical centers. Three hundred and three patients enrolled and finally 229 patients finished the inhaled Mucomyst® therapy for 8 weeks. We analyzed the change of Reflux Symptom Index (RSI), Reflux Finding Score (RFS), Visual Analogue Scale (VAS) for globus, and Globus Pharyngeus Symptom Scale (GPS) after use of Mucomyst® for 4 and 8 weeks. Results The GPS, RSI, RFS, and VAS score significantly decreased serially in patients who finished 8 week-inhalation treatment. The GPS improvement gap was significantly correlated with initial GPS (p<0.001) in multiple regression analysis. Conclusion Inhaled Mucomyst® therapy was effective for the reduction of both subjective and objective findings in refractory globus patients. This study might suggest new treatment option for patients with globus. However, further thorough studies would be needed to assess the real effect of inhaled Mucomyst® treatment as a standard treatment for globus.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made.   • On-X 기계판막은 1997년 처음으로 사용된 이후 전 세계적으로 사용빈도가 증가하고 있다. 본 연구는 이 새로운 기계판막에 대한 유용성과 안전성, 그리고 술 후 혈역학적 변화에 대해 알아보고자 시행되었다. 대상 및 방법: 1999년 4월부터 2002년 8월까지 전남대학교병원 흉부외과에서 On-X 판막을 사용하여 대동맥판막 치환술을 시행했던 52명의 환자를 대상으로 분석하였다. 환자 중 52%가 남자였으며 평균연령은 50$\pm$13세였다. 연구는 판막수술 후 결과보고에 대한 AATS/STS 지침에 따라 분석하였다. 술 전 뉴욕 심장병학회의 기능분류상 class III 이상의 환자가 32명(61.5%)이였으며, 2명의 환자는 이전에 대동맥판막 수술을 받았던 환자였다. 71.1%의 환자에서 동반된 질환에 의해 대동맥판막 치환술과 함께 동반 수술을 시행하였고, 이식된 판막의 크기는 19 mm 13명, 21 mm 26명, 23 mm 10명과 25 mm 3명이었다 평균 외래 추적관찰 기간은 16.6$\pm$10.5 (1∼39)개월이었다. 수술 전과 수술 직후 그리고 3, 6, 12개월에 심초음파를 반복 시행하여 술 후 혈역학적 변화양상을 분석하였다. 결과: 평균 체외순환시간과 대동맥 차단시간은 각각 191$\pm$94.7분과 142$\pm$51.7분이었다. 술 후 조기 및 만기 사망은 없었다. 술 후 12개월간 event-free rate는 혈전색전증 95.6$\pm$6%; 출혈성 합병증 90.2$\pm$4%: 판막 주위누출 92.3$\pm$4%이었으며 전체적인 event-free rate는 76.6$\pm$3%이었다. 수술 후 판막혈전이나 심내막염, 구조적 판막부전은 발생하지 않았다. 술 후 12개월의 평균심박출량은 62.7$\pm$9.8%로 수술 전(55.8$\pm$15.9%)에 비해 의미 있는 증가를 보였다(p=0.006). 좌심실 체적지수와 최대 판막압력차는 수술 전에 247.3$\pm$122.3 g/$\textrm{cm}^2$, 62.5$\pm$38.0 mmHg을 보였으나 술 후 12개월에 각각 155.5$\pm$58.2 g/$\textrm{cm}^2$(p=0.002), 18.0$\pm$10.8 mmHg (p<0.0001)로 감소하는 소견을 보였다. 결론: On-X 기계판막을 이용한 대동맥판막 치환술은 임상적 및 혈역학적으로 만족스러운 조기 결과를 보여주었다. 그러나 대상 환자들에 대한 장기간의 외래 추적관찰과 함께 기존 판막과의 비교가 이루어짐으로써 이 새로운 판막에 대한 유용성과 안정성이 확립될 수 있을 것으로 생각한다.

PL의 브랜드확장이 소비자태도에 미치는 영향에 관한 연구 : 모브랜드 적합도 인식 차이의 조절효과를 중심으로 (The Effect of Brand Extension of Private Label on Consumer Attitude - a focus on the moderating effect of the perceived fit difference between parent brands and an extended brand -)

  • 브랜드확장은 다양한 마케팅 영역 중에서도 전통적으로 활발하게 연구가 진행되어 왔던 영역으로서, 본 연구는 최근 그 중요성이나 활용도가 급증하고 있는 PL(Private Label)제품에 대해 브랜드확장의 개념을 활용하여 차별적으로 접근하고자 하였다. 최근 PL제품에 관한 마케팅연구가 활발하게 진행되고 있으나, 대부분 기존 틀에서 크게 벗어나지 못한 채 단순한 적용에 그치고 있으며, 특히 브랜드확장에 관련된 연구들에서도 PL시장의 특성을 제대로 반영하고 있다고 볼 수 없다. 특히 PL제품의 확장에 있어서는 두 가지 모브랜드가 존재할 수 있는데, 이에 대한 연구는 부재한 상황이다. 이에 본 연구에서는 확장 PL제품의 태도에 영향을 미치는 변수로서 두 가지 모브랜드인 유통업체와 기존 PL제품에 대한 태도를 제시하였다. 또한 개별 모브랜드가 PL제품의 태도에 미치는 영향은 개별 모브랜드와 확장 PL제품간 유사성에 의해 상이할 것이라고 제안하였으며, 유통업체와 기존 PL제품에 대한 태도에 영향을 미치는 변수로서 신뢰와 만족을 제시하였다. 분석결과 유통업체와 기존 PL제품에 대한 태도 모두 확장 PL제품의 태도에 유의한 영향을 미쳤으며 동시에 적합도 정도에 따라 그 영향력이 상이함도 실제 데이터를 통해 검증하였다. 즉 확장 PL제품의 태도는 모브랜드의 적합도가 보다 강하게 형성된 모브랜드의 영향을 더 크게 받는 것을 확인할 수 있었다. 이를 토대로 향후 PL제품을 확장할 경우 소비자가 보다 긍정적인 태도를 갖고 있는 모브랜드에 기초하여 해당 모브랜드와의 연상이 강하게 나타날 수 마케팅 전략을 구사할 필요가 있을 것이다.

심호흡 방법에 따른 상복부 수술환자의 폐 환기능에 미치는 효과 (The Effect of Deep Breathing Methods on Pulmonary Ventilatory Function of Patients Who experiened Upper-abdominal surgery)

  • 본 연구는 수술환자의 술 후 폐환기능 회복을 돕기 위한 효과적인 간호중재의 자료를 제공하고자 부산시내에 소재한 P 대학병원의 일반욋과에 입원하여 전신마취하에 상복부 수술을 받은 환자 46명을 대상으로 하였으며, 자료수집기간은 1993년 11월 1일부터 12월 31일까지로 하였고, 처음 4주간동안 대조군을, 다음 4주간에 실험군을 자료수집하였다. 실험군에는 Incentive Spirometer를 사용한 심호흡법을, 대조군에는 호흡기구를 사용하지 않은 자발적인 심호흡법을 수술 후 의식이 회복된 직 후부터 매 1시간마다 규칙적으로 24시간동안 실시하였다. 폐환기능의 측정도구는 운반이 용이하고 사용이 간편하여 임상적으로 많이 이용되는 Vitalograph Compact를 사용하였으며, 수술 전과 수술 후 24시간, 수술 후48시간, 수술 후72시간, 각 각 4회에 걸쳐 대상자들의 노력성 폐활량과 일초 노력성 호기량을 측정하였다. 여기서 수집한 자료를 SPSS $PC^+$를 이용하여, 백분율, 평균, t검정, $X^2$검정, 일원변량분석으로 통계처리하였다. 본 연구의 결과를 요약하면 다음과 같았다. 1) 대상자들의 일반적특성은 여성이 41.3%, 남성이 58.7%이며, 평균연령은 $48.48{\pm}12.26$, 흡연습관은 대상자 전체의 43.5%가 있는 것으로 나타났으며, 과거 호흡기 질환은 전체의 8.7%가 앓은 경험이 있는 것으로 나타났다. 2) Incentive Spirometer를 사용한 심호흡방법은 수술 후의 노력성 폐활량에서 24시간에 $1.46{\pm}0.54L$, 48시간에 $1.58{\pm}0.60L$, 72시간에 $1.90{\pm}0.61L$로서 유의한 차이를 보였다(F=3.530, P=0.035). 즉 시간경과에 따라 폐환기능의 회복에 효과적이었다. 3) 호흡기구를 사용하지 않은 자발적인 심호흡 방법은 수술 후 노력성 폐활량이 24시간에 $1.19{pm}0.43L$, 48시간에 $1.35{\pm}0.42L$, 72시간에 $1.51{\pm}0.38L$로 유의한 차이를 보였고(F=3.480, P=0.037), 술 후 일초 노력성 호기량도 24시간에 $1.00{\pm}0.28L$, 48시간에 $1.20{\pm}0.37L$, 72시간에 $1.33{\pm}0.33L$로 유의한 차이를 보였으므로(F=6.153, P=0.004), 술 후 폐환기능 회복에 효과가 있다. 4) 실험군과 대조군의 수술 후 노력성 폐활량은 수술 후 72시간에서 실험군이 $1.90{\pm}0.61L$, 대조군이 $1.51{\pm}0.38L$로 유의한 차이를 보였다(t=2.620, P=0.013). 5) 실험군과 대조군의 수술 후 일초 노력성 호기량은 수술 후 24시간에서 $1.33{\pm}0.56L,\;1.00{\ge}0.28L$로 유의한 차이를 보였고(t=2.530, P=0.017), 술 후 72시간에서 $1.72{\pm}0.65L,\;1.33{\pm}0.3L$로 유의한 차이를 보였다(t=2.540, P=0.016). 6) 대상자의 술 후 폐환기능에 영향을 미치는 요인은 성별로 나타났다. 이에 따른 폐환기능의 차이를 보면, 실험군의 술 후 노력성 폐활량이 48시간에 남자($1.78{\pm}0.61L$)가 여자($1.27{\pm}0.45L$)보다 더 높게 나타났으며 (t=2.170, P=0.042), 72시간에도 역시 남자($2.16{\pm}0.56L$)가 여자($1.50{\pm}0.47L$)보다 더 높았다(t=2.910, P=0.008). 술 후 일초 노력성 호기량은 24시간에서 남자가 $1.54{\pm}0.60L$, 여자가 $1.00{\pm}0.30L$으로 유의하였고 (t=2.460, P=0.023), 48시간에 남자가 $1.64{\pm}0.62L$, 여자가 $1.09{\pm}0.41L$로 유의한 차이를 보였고(t=2.360, P=0.028), 72시간에도 남자($1.96{\pm}0.62L$)가 여자($1.33{\pm}0.50L$)보다 더 높게 나타났다(t=2.540, P=0.019). 즉 실험군의 술 후 폐환기능은 남자가 여자보다 더 우세하다는 것을 알 수 있었다. 이상의 요약된 연구결과를 토대로, 연구자는 다음과 같은 결론을 얻었다. Incentive Spirometer를 사용한 심호흡법이 호흡기구를 사용하지않은 자발적인 심호흡보다 술 후 폐환기능 회복에 있어서 더 우수한 방법이었다. 그러므로, 임상간호사는 상복부 수술환자의 술 후 폐환기능 회복을 돕고 나아가 폐합병증을 예방하기 위해 Incentive Spirometer를 사용한 규칙적인 심호흡법을 술 후에 간호중재의 한 방안으로 실시할 것을 권장하며, 아울러 병동의 사정 상 호흡기구의 비치가 가능하지 않다면 호흡기구를 사용하지않은 자발적인 심호흡법을 술후에 꾸준히 적용하는것도 임상간호학발정에 기여하리라고 본다.

유전자 알고리즘을 이용한 다분류 SVM의 최적화: 기업신용등급 예측에의 응용 (Optimization of Multiclass Support Vector Machine using Genetic Algorithm: Application to the Prediction of Corporate Credit Rating)

  • 기업신용등급은 금융시장의 신뢰를 구축하고 거래를 활성화하는데 있어 매우 중요한 요소로서, 오래 전부터 학계에서는 보다 정확한 기업신용등급 예측을 가능케 하는 다양한 모형들을 연구해 왔다. 구체적으로 다중판별분석(Multiple Discriminant Analysis, MDA)이나 다항 로지스틱 회귀분석(multinomial logistic regression analysis, MLOGIT)과 같은 통계기법을 비롯해, 인공신경망(Artificial Neural Networks, ANN), 사례기반추론(Case-based Reasoning, CBR), 그리고 다분류 문제해결을 위해 확장된 다분류 Support Vector Machines(Multiclass SVM)에 이르기까지 다양한 기법들이 학자들에 의해 적용되었는데, 최근의 연구결과들에 따르면 이 중에서도 다분류 SVM이 가장 우수한 예측성과를 보이고 있는 것으로 보고되고 있다. 본 연구에서는 이러한 다분류 SVM의 성능을 한 단계 더 개선하기 위한 대안으로 유전자 알고리즘(GA, Genetic Algorithm)을 활용한 최적화 모형을 제안한다. 구체적으로 본 연구의 제안모형은 유전자 알고리즘을 활용해 다분류 SVM에 적용되어야 할 최적의 커널 함수 파라미터값들과 최적의 입력변수 집합(feature subset)을 탐색하도록 설계되었다. 실제 데이터셋을 활용해 제안모형을 적용해 본 결과, MDA나 MLOGIT, CBR, ANN과 같은 기존 인공지능/데이터마이닝 기법들은 물론 지금까지 가장 우수한 예측성과를 보이는 것으로 알려져 있던 전통적인 다분류 SVM 보다도 제안모형이 더 우수한 예측성과를 보임을 확인할 수 있었다.
