노인에서 치매 조기선별을 위한 시각.금전계산 검사의 유용성 (Availability of the Time and Change Test in Screening for Dementia in the Elderly)

  • Objectives : Dementia has emerged as a leading public health problem in elderly persons, and its early detection is important for the treatment of curable cases, and in the educational support for other family members. Although dementia screening tests are available, they have not gained widespread use in community or primary care settings. Our goal was to validate the Tine and Change (T&C) Test, -including its validity and reliability in patients, and to assess it as a simple, standardized method for the screening of dementia in the rural elderly. Methods : The participants in this study comprised of 59 patients from an urban hospital and 405 persons from a rural community aged 65 years or older. The time test evaluated the understanding of clock hands indicating 11:10, and the change test the ability to make 1,000 Won from a group of coins, consisting of one 500, seven 100, and seven 50 Won coins. The T&C ratings were validated against a reference standard based on the physician's diagnosis of the patients. The convergent validity in relation to other cognitive measure, test-retest agreement, and inter-observer reliability were assessed. To assess the relationship between the Korean Mini-Mental State Exam (K-MMSE) and the T&C Test, the mean K-MMSE scores were compared with the results of the T&C Test in the elderly from a rural community. Results The T&C Test had a sensitivity and specificity of 73.0, and 90.9%, and positive and negative predictive values of 93.1, and 66.7%, respectively. The test-retest and inter-observer agreement rates were both 95%. The K-MMSE scores and T&C Test were significantly related in the elderly from a rural community (p<0.01), The T&C Test was not influenced by the educational status. The Time and Change Tests took a mean of 6.3 and 12.7 seconds, respectively, to complete Conclusion : The T&C Test is a simple, accurate and reliable, performance-based tool in the screening for dementia. Because it is quick, and easy-to-use, it is hoped the T&C Test will be used for the widespread cognitive screening of aging populations.

Knowledge, Attitudes and Practices of Sudanese Women Regarding the Pap Smear Test and Cervical Cancer

  • Background: Despite the established role of the Pap smear test (PST) in prevention and early detection of cervical cancer, it is still rarely practiced in Sudan. Many challenges hinder the establishment of an effective cervical cancer screening program, including socio-cultural factors. Therefore, this study aimed to investigate the knowledge, attitudes and practices (KAP) of Sudanese women with regard to the Pap smear test and cervical cancer. Materials and Methods: A total of 500 married women aged 14 to 58 years were recruited from obstetric clinics, hospitals and universities in Khartoum in 2014. Data were collected using a standardized, pretested questionnaire that inquired socio-demographic characteristics and their KAP about cervical cancer and the PST. Results: More than 52% of participating women were above 30 years of age, and the majority (78.8%) were university degree holders. A total of 486 (97.2 %) of participants were resident in urban areas of Khartoum State. However about 48% of the respondents had never heard about PST, and only 15.8% of the participants had undergone a Pap smear test previously; 46.6% (233/500) knew that the human papilloma virus (HPV) was the causative agent, but only 39.2% (196/500) had heard about HPV vaccination, and only 11.4% (57/500) had received the vaccine. However 68% of the respondents agreed to do Pap smear if properly informed about the test and 75.4% of the respondents agreed to participate in a cervical cancer screening program. Conclusions: Despite a high educational level, less than half of our participants had accurate knowledge about cervical cancer, HPV, and cervical cancer screening. Health education about cervical cancer, HPV and sexually transmitted infections and the role of PST in cervical cancer prevention are crucial when designing interventions aimed at improving cervical cancer screening for Sudanese women.

국내외 알코올사용장애 선혈도구의 비교를 통한 한국성인의 알코올사용장애에 관한 역학조사 (A Epidemiological Study on the Prevalence of Alcohol Use Disorders among the Korean Adult Population)

  • 본 연구조사는 사회복지학계에서는 최초로 국내외에서 개발된 다양한 알코올사용장애 선별도구를 이용하여 신뢰성이 높은 알코올사용장애율을 밝히는 것을 주목적으로 하였다. 조사대상자는 전국에 거주하는 성인으로 충화표집을 이용하여 추출되었고 총 614명이 본 조사에 참여하였다. Alcohol Use Disorder Identification Test, Brief Michigan Alcoholism Screening Test, 한국형 알코올 중독선별 도구를 이용하여 조사대상자의 음주문제를 측정하였고 조사대상자의 음주빈도, 음주 양, 폭음정도를 측정하였다. 본 조사의 주요결과로는 한국성인의 32.2%가 음주와 관련된 문제를 경험한 것으로 나타났으며 전체 성인의 4.2% (한국성인의 134만명)가 알코올사용장애가 있는 것으로 선별되어 우리 나라 성인의 음주문제가 상당히 심각한 수준에 와 있는 것으로 제시하고 있다. 사회복지사는 우리 나라 성인의 음주문제의 심각성을 인식하고 초기면접시 AUDIT나 NAST 같은 선별도구를 사용하여 클라이언트의 음주문제를 항상 파악하여야 할 것이고 알코올 및 약물사용으로 인해 고통 받고 있는 개인들과 그들의 가족을 위한 효과적인 프로그램과 자원을 개발하는데에도 노력을 기울여야 할 것이다. 마지막으로 후속연구를 위한 세 가지 제안점을 언급하고 있다.

치매 선별을 위한 도형모사검사 개발 및 타당화 (Development and Validation of Figure-Copy Test for Dementia Screening)

  • 치매 증상의 진행 지연 및 관리비용의 절감을 위해서는 치매를 조기에 발견하여 관리하는 것이 중요하다. 이에 본 연구에서는 치매와 관련된 인지신경학적 손상을 측정할 수 있는 간단한 그림검사인 도형모사검사를 개발하여, 치매 선별 가능성을 확인하고자 하였다. 또한, 도형모사검사의 이미지 데이터에 대한 기계학습을 통해 검사 채점의 자동화 가능성을 확인하고자 하였다. 이를 위해 270명의 일반 및 손상집단 참가자들에 대하여 도형모사검사, MMSE-DS, 그리고 시계그리기 검사를 수행하였다. 분석 결과, 도형모사검사의 점수는 높은 내적 일치도를 보였을 뿐만 아니라, 다른 두 검사 점수와 유의한 상관을 보여 검사의 타당성을 확인하였다. 세 검사의 치매 선별 정확도를 비교하기 위해 판별분석을 시행한 결과, 다른 두 검사와 비교했을 때 도형모사검사가 일반 및 손상 집단을 각각 90.8% 및 77.1%의 정확도로 예측하여, 집단에 대한 예측 수준이 상대적으로 더 높은 것으로 나타났다. 또한, 신경과 진단을 통한 임상 결과를 통해, MMSE-DS를 통해 선별하지 못했던 치매 사례들을 도형모사검사를 이용하여 선별할 수 있음을 확인하였다. 마지막으로, 도형모사검사의 이미지 데이터를 이용한 기계학습을 수행한 결과, 73.70%의 정확률로 집단을 예측하는 것을 확인하였다. 본 연구는 기존에 사용되어 온 치매 선별 검사의 한계를 보완하여, 시행과 채점이 간편한 새로운 그림검사를 개발하였다는 점에서 의의를 지닌다.

HPV Genotyping Linear Assay Test Comparison in Cervical Cancer Patients: Implications for HPV Prevalence and Molecular Epidemiology in a Limited-resource Area in Bandung, Indonesia

  • Background: Persistent infection with high risk human papillomavirus (hrHPV) is strongly associated with cervical cancer. Normal cervical cells may also harbor hrHPV, and detection of early hrHPV infection may minimize risk of cervical cancer development. This study aimed to compare two commercial HPV genotyping assays that may affordable for early screening in a limited-resource setting in Bandung, Indonesia. Materials and Methods: DNA from cervical biopsies with histologically confirmed as squamous cell cervical cacinoma were HPV genotyped by Linear Assay 1 (Roche Diagnostics, Mannheim, Germany) or Linear Assay 2 (Digene HPV Genotyping RH Test, Qiagen Gaithersburg, MD). In a subset of samples of each group, HPV genotype results were then compared. Results: Of 28 samples genotyped by linear assay 1, 22 (78.6%) demonstrated multiple infections with HPV-16 and other hrHPV types 18, 45 and/or 52. In another set of 38 samples genotyped by linear assay 2, 28 (68.4%) were mostly single infections by hrHPV type 16 or 18. Interestingly, 4 samples that had been tested by both kits showed discordant results. Conclusions: In a limited-resource area such as in Indonesia, country with a high prevalence of HPV infection a reliable cervical screening test in general population for early hrHPV detection is needed. Geographical variation in HPV genotyping result might have impacts for HPV prevalence and molecular epidemiology as the distribution in HPV genotypes should give clear information to assess the impact of HPV prophylactic vaccines.

Socioeconomic Inequalities in Colorectal Cancer Screening in Korea, 2005-2015: After the Introduction of the National Cancer Screening Program

  • Purpose: This study aimed to investigate inequalities in colorectal cancer (CRC) screening rates in Korea and trends therein using the slope index of inequality (SII) and relative index of inequality (RII) across income and education groups. Materials and Methods: Data from the Korean National Cancer Screening Survey, an annually conducted, nationwide cross-sectional survey, were utilized. A total of 17174 men and women aged 50 to 74 years were included for analysis. Prior experience with CRC screening was defined as having either a fecal occult blood test within the past year or a lifetime colonoscopy. CRC screening rates and annual percentage changes (APCs) were evaluated. Then, SII and RII were calculated to assess inequality in CRC screening for each survey year. Results: CRC screening rates increased from 23.4% in 2005 to 50.9% in 2015 (APC, 7.8%; 95% CI, 6.0 to 9.6). Upward trends in CRC screening rates were observed for all age, education, and household income groups. Education inequalities were noted in 2009, 2014, and overall pooled estimates in both indices. Income inequalities were inconsistent among survey years, and overall estimates did not reach statistical significance. Conclusion: Education inequalities in CRC screening among men and women aged 50 to 74 years were observed in Korea. No apparent pattern, however, was found for income inequalities. Further studies are needed to thoroughly outline socio-economic inequalities in CRC screening.

Population-based Cervical Cancer Screening Using High-risk HPV DNA Test and Liquid-based Cytology in Northern Thailand

  • Background: Northern Thailand is a region with a high cervical cancer incidence. Combined high-risk HPV (hrHPV) DNA testing and cytology (co-testing) has increasingly gained acceptance for cervical cancer screening. However, to our knowledge, data from a population-based screening using co-testing have not been available in this region. This study therefore aimed to evaluate the performance of cytology and hrHPV test in women in northern Thailand. Materials and Methods: Cervical samples were collected for hybrid capture 2 (HC2) testing and liquid-based cytology from women aged 30 to 60 years who were residents in 3 prefectures of Chiang Mai in northern Thailand between May and September 2011. Women with positive cytology were referred to colposcopy, while women with positive for HC2 only were followed for 2 years. Results: Of 2,752 women included in this study, 3.0% were positive in both tests, 4.1% for HC2 only, and 1.3% had positive cytology only. At baseline screening, positive HC2 was observed in 70.6% among cytology-positive women compared with 4.3% among cytology-negative women. The prevalence of positive HC2 or cytology peaked in the age group 35-39 years and was lowest in the age group 55-60 years. High-grade squamous intraepithelial lesion or worse lesions (HSIL+) were histologically detected in 23.5% of women with positive baseline cytology and in 9.8% of women with positive baseline HC2 only on follow-up. All women with histologic HSIL+ had positive baseline HC2. Conclusions: The hrHPV test is superior to cytology in the early detection of high-grade cervical epithelial lesions. In this study, the prevalence of histologic HSIL+ on follow-up of women with positive hrHPV test was rather high, and these women should be kept under careful surveillance. In northern Thailand, hrHPV testing has a potential to be used as a primary screening test for cervical cancer with cytology applied as a triage test.

Effects of Group Training Based on the Health Belief Model on Knowledge and Behavior Regarding the Pap Smear Test in Iranian Women: a Quasi-Experimental Study

  • The Pap smear test is recommended for early diagnosis of cervical cancer. The aim of this study was to assess knowledge and behavior regarding the Pap smear test based on the Health Belief Model (HBM) in women referred to premarital counseling classes, Hamadan, Iran. This quasi-experimental study was conducted on 330 women, who were allocated randomly to two case and control groups (n=165). Two educational session classes were performed in the case group. Two stages in before and after intervention groups were evaluated. Analysis of data was performed by SPSS/16.0, using t-test, $x^2$, and McNemar's test. P-values <0.05 were regarded as significant. There was no significant difference between the mean scores of the various structures of this model in two groups before the intervention. However, after the intervention there were significant increase in mean score of knowledge and all variables of HBM in the intervention group(P<0.001). The findings of this study highlight the important role of education about cervical cancer on changing women's beliefs about cervical screening.

The Seoul Neuropsychological Screening Battery (SNSB) for Comprehensive Neuropsychological Assessment

  • The Seoul Neuropsychological Screening Battery (SNSB) is known as a representative comprehensive neuropsychological evaluation tool in Korea since its first standardization in 2003. It was the main neuropsychological evaluation tool in the Clinical Research Center for Dementia of South Korea, a large-scale multi-center cohort study in Korea that was started in 2005. Since then, it has been widely used by dementia clinicians, and further solidified its status as a representative dementia evaluation tool in Korea. Many research results related to the SNSB have been used as a basis for the diagnosis and evaluation of patients in various clinical settings, especially, in many areas of cognitive assessment, including dementia evaluation. The SNSB version that was updated in 2012 provides psychometrically improved norms and indicators through a model-based standardization procedure based on a theoretical probability distribution in the norm's development. By providing a score for each cognitive domain, it is easier to compare cognitive abilities between domains and to identify changes in cognitive domain functions over time. Through the development of the SNSB-Core, a short form composed of core tests, which also give a composite score was provided. The SNSB is a useful test battery that provides key information on the evaluation of early cognitive decline, analysis of cognitive decline patterns, judging the severity of dementia, and differential diagnosis of dementia. This review will provide a broad understanding of the SNSB by describing the test composition, contents of individual subtests, characteristics of standardization, analysis of the changed standard score, and related studies.

유방암 환자에서 Breast Specific Gamma Imaging (BSGI)의 유용성 (Usefulness about BSGI (Breast Specific Gamma Imaging) in Breast Cancer Patients)

  • 목적 : 유방암의 조기진단에 따른 선별 검사는 여러 방법들이 있는데 그 중 유방스캔(Scinti mammography)은 유방촬영에서 정확하게 감별하지 못한 병변을 평가하는데 있어서 매우 유용한 검사 방법으로 잘 알려져 있다. 그러나 이 방법은 원발성 유방암과 재발성 유방암 진단에 민감도와 특이도가 높지만 유방 주변부의 방사능과 검사 장비의 기하학적인 구조로 1 cm 이하의 작은 종양들을 발견하기가 어려웠다. 최근 고 해상도의 BSGI (Breast-Specific Gamma Imaging)도입으로 악성 종양과 양성 종양을 다른 검사 방법들보다 높은 정확도로 구별할 수 있어 불필요한 생검(Biopsy)을 줄일 수 있다. 따라서 이미 알려진 유방암의 조기 진단에 이용하는 단순 유방촬영과 유방초음파 검사, 기존 유방 스캔과 BSGI의 진단적 성능과 검사방법의 숙련도를 알아보고자 본 연구를 수행하였다. 방법 : 2009년 2월 임상적 증상이 없는 지원자 53명을 대상으로 BSGI를 시행하였다. BSGI는 $^{99m}Tc$-MIBI 25 mCi를 주사 후 10분에 $80{\times}80$ Matrix Size, Preset Time Methods으로 CC (Craniocaudal projection), MLO(Mediolateral Oblique projection) 이외에 다각적인 추가촬영법(Additional Views)으로 각각 검사하였다. 결과 : BSGI의 결과는 전체 53명 중 2명이 유방조직 내 종물로 발견되었고, 4명은 명확한 진단이 어려워 재검사를 시행하였다. 재검사를 시행한 지원자는 모두 진음성의 결과를 얻었다. 또한 임상적 유방암 선별검사 결과의 문헌적 고찰은 다음과 같다. BSGI : 민감도 86.5%, 특이도 92.4%, 유방스캔: 민감도 77.8%, 특이도 84.2%, 유방촬영술: 민감도 85~90%, 특이도 20~42%, 유방 초음파검사 : 민감도 66.7%, 특이도 44.2%, MRI: 민감도 88%, 특이도 68%로 각각 나타났다. 결론 : 유방암 조기진단을 위해서는 자가진단과 유방암 선별검사가 필요하다. 그러나 기존의 유방암 선별검사에서 병변을 발견하는데 정확한 결론을 내리기가 쉽지 않았다. 따라서 유방암 선별검사에서 미확정된 경우 명확한 진단을 위해 조직학적 생검이 필수적이다. 유방암 조기검사와 원발성 유방암을 진단하는데 있어서 BSGI는 기존의 유방암 선별검사보다 높은 민감도와 특이도를 나타내어 진단적으로 유용한 정보를 제공하고 불필요한 생검을 줄일 수 있을 것으로 기대된다.
