This research was conducted in order to collect and arrange an oral health information needed the school oral hygiene as a part of a long term local community oral hygiene development to promote the oral health of the residents in Sung-Nam. After gathering the information of oral hygiene of 12-year old schoolchildren in Sung-Nam, these conclusions are reached: (1) The experience of dental caries in permanent teeth of 12-year old schoolchildren in Sung-Nam was revealed 74.7%, proportion of children with one or more caries in permanent teeth was 36.2%, and DMFT index was 2.75. (2) DT rate was 28.4% in total and regional difference has been revealed. In Sujung-Gu, DT rate was 36,8% and this was nearly as twice as 19.8% in Bundang-Gu. (3) For permanent teeth, fillings requirements for one surface were 26.6%, more that two surfaces were 13.3%. In addition, artificial crown treatment requirements were 1.8%, dental pulp treatment was 5.3%, and extraction requirement was 2.2%. In contrast, holders of pit and fissure sealant were only 27.1%. (4) In Sung-Nam, oral hygiene education campaign for 12-year old schoolchildren needs to be continued. Simultaneously, early stage examination and preventive approaches for decayed teeth such as pit and fissure sealant need to be considered for schoolchildren who have high tendency of dental caries and who are in areas which show high percentage of dental caries occurrence.
PURPOSE. The prospective follow-up aimed to assess the performance of lithium disilicate crowns and clinical reasons of adverse events compromising survival and quality. MATERIALS AND METHODS. 58 patients were treated with 375 heat-pressed monolithic crowns, which were bonded with resin cement. Annual recalls up to five years included a complete dental examination as well as quality assessment using CDA-criteria. Any need for clinical intervention led to higher complication rate and any failure compromised the survival rate. Kaplan-Meier-method was applied to all crowns and a dataset containing one randomly selected crown from each patient. RESULTS. Due to drop-outs, 45 patients (31 females, 14 males) with the average age of 43 years (range = 17-73) who had 327 crowns (176 anterior, 151 posterior; 203 upper jaw, 124 lower jaw) were observed and evaluated for between 4 and 51 months (median = 28). Observation revealed 4 chippings, 3 losses of retention, 3 fractures, 3 secondary caries, 1 endodontic problem, and 1 tooth fracture. Four crowns had to be removed. Survival and complication rate was estimated 98.2% and 5.4% at 24 months, and 96.8% and 7.1% at 48 months. The complication rate was significantly higher for root canal treated teeth (12%, P<.01) at 24 months. At the last observation, over 90% of all crowns showed excellent ratings (CDA-rating Alfa) for color, marginal fit, and caries. CONCLUSION. Heat pressed lithium disilicate crowns showed an excellent performance. Besides a careful luting, dentists should be aware of patients' biological prerequisites (grade of caries, oral hygiene) to reach full success with these crowns.

Air abrasion technology can prepare enamel and dentin for bonding, similar to etching by acidic gels and solutions. Longer treatment can excavate pit and fissures, preparing the tooth for immediate placement of bonded resin materials. Although not appropriate for every clinical situation, the air abrasive technology minimizes heat, vibration and bone-conducted noise associated with conventional means of caries removal since the cutting is accomplished by air pressure. Also, patients treated with the air-abrasion technology rarely request anesthesia. Air abrasion technology was more effective in treating early carious lesions and stains compared to lesions where caries had already progressed to produce soft dentin and the strong air stream and noise caused by the evacuation system was a major discomfort to pediatric patients, and the experience and skillfulness of clinician should be required for accurate and proper tooth preparation.
Streptococcus mutans is the primary causative agent of caries, which is one of the most common human diseases. Thus, rapid and early detection of cariogenic bacteria is critical for its prevention. This study investigated the combination of loop-mediated isothermal amplification (LAMP) and microfluid technology to quantitatively detect S. mutans. A low-cost, rapid microfluidic chip using LAMP technology was developed to amplify and detect bacteria at 2.2-2.2 × 106 colony-forming units (CFU)/ml and its detection limits were compared to those of standard polymerase chain reaction. A visualization system was established to quantitatively determine the experimental results, and a functional relationship between the bacterial concentration and quantitative results was established. The detection limit of S. mutans using this microfluidic chip was 2.2 CFU/ml, which was lower than that of the standard approach. After quantification, the experimental results showed a good linear relationship with the concentration of S. mutans, thereby confirming the effectiveness and accuracy of the custom-made integrated LAMP microfluidic system for the detection of S. mutans. The microfluidic system described herein may represent a promising simple detection method for the specific and rapid testing of individuals at risk of caries.

Talon cusp is cusp-like structure that develops additionally in crown of anterior tooth. And it is one of dental anomalies. Talon cusp can cause various diagnostic, functional, aesthetic problems depending on the size and configuration of cusp. there is a tendency for caries to occur in the developmental grooves, and advanced attrition, periodontal problems, irritation the tongue and temporomandibular pain, occlusal interference and displacement of affected tooth may result. Therefore, early diagnosis and appropriate treatment of each case is important so that it minimize local problems, such as caries, periodontal diseases, and malocclusion. These cases which are in permanent and primary anterior teeth are about gradual reduction forming reparative dentine and complete reduction of talon cusp and root canal therapy which is an alternative and effective form of treatment when gradual reduction of talon cusp may not be possible.
Objectives: The resin infiltration technique is a promising alternative therapy for arresting the early dental caries. However, there are very few reports on the safety and biocompatibility of this technique. We evaluated various properties of resin infiltrant (RI) based on a triethylene glycol dimethacrylate (TEGDMA).The water sorption (Wsp) and water solubility (Wsl) was assessed. Additionally, the cytotoxicity of RI against both animal and human fibroblast cell lines was investigated. Methods: The RI of the $Icon^{(R)}$, the first product developed for resin infiltration, is mainly composed of TEGDMA in the resin matrix. The Wsp and Wsl for the RI were measured in accordance with ISO 4049 specifications. Fourier-transform infrared spectroscopy (FTIR) was used for analyzing the polymerization before and after curing of RI. The cytotoxicity of RI against the mouse fibroblasts (L929) and human gingival fibroblasts (hTERT-hNOF) was evaluated using MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide] assay and the data were analyzed using one-way analysis of variance. Results: Wsp and Wsl of the RI specimens were $53.37{\mu}g/mm^3$ and $10.6{\mu}g/mm^3$, respectively. FTIR analysis revealed a slightly higher degree of curing with longer irradiation time. The degree of conversion for RI was high (80.9%) after 40 seconds of light curing. There was a significant decrease in the viability of L929 and hTERT-hNOF cells at RI extraction solution concentrations above 50%, respectively, compared to that in the negative control (p< 0.05). Conclusions: Even though the RI exhibited positive effect on the early prevention of dental caries, the clinicians should also consider the toxicity of RI on periodontal tissues.

The purpose of this study was to get descriptive statistics of the contributing factors for early childhood caries and to predict the relationship of dietary, behavior factors and health status factors of the mother and child at pregnancy and after birth. 411 first caregivers of 5-year-old children in 12 kindergartens in Kangnung city were selected by stratified random cluster sampling. They were asked to fill out questionnaires and 364 of them responded. The obtained results were as follow: 1. Over the three-Fourth of children used nursing bottle or had breast feeding habit beyond the age of 1 year. 2. 8.7% of respondents didn't recognize the necessity of the preventive measures immediate after eruption of primary tooth, and only 35.1% replied that they had begun tooth cleaning. 3. Over 90% of children brush the teeth more than once per day. But over half (614%) of them brush their teeth without parents instruction. Sixty percent of children eat between the meals as often as 1-3 time(s) a day and the remainder at any times. 4. The first time of dental visit was for most children (87%) at over 3 years, recommending the earlier dental visit. Notwithstanding the rate of routine dental visit experience was relatively high(40.2%), implicating positive parents' attitude about oral health at Kangnung area. 5. The relationships between oral health state of the parents and the variables such as the timing of the first tooth cleaning, the frequency of brushing, the time of first dental visit, and the reason of first dental visit were not statistically significant. Together, there was no statistically significant difference between rural and urban area, private and public kindergarten, and boy and girl($x^2-test$, p>.05 or Fisher's exact test, p>.05).

Over the past 20 years, great strides have been made in research regarding the mechanisms involved in the progression of carious lesions, but new equipment and research tools need to be developed to continue these advancements in caries research. Various methods have been applied to reduce the incidence of carious lesions, which have led to a significant decrease in the number of occlusal caries, but a concurrent increase in the proportion of proximal carious lesions. New diagnostic equipment has been developed to detect early stage carious lesions, and these have demonstrated excellent laboratory results and show promise in clinical applications. The research presented here examines the efficacy of the newly developed $DIFOTI^{TM}$ system in detecting proximal carious lesions compared to traditional intraoral exam and bitewing radiography, possible problems or deficiencies of using the system in clinic, possible improvements that can be made to the system, and the efficacy of detecting early, reversible carious lesions that can be remineralized by preventative fluoride applications. The subject pool consisted of 23 grammer school age patients just prior to entering the mixed dentition phase. Each patient was given a thorough oral examination, radiographic examination consisting of bitewing radiographs of the posterior teeth, and $DIFOTI^{TM}$ examination of the anterior and posterior teeth. Each examination was carried out two times by two examiners, and the data were statistically analyzed. The results are as follows: 1. The mean alpha value of reliability test of the visual oral examination was as follows; occlusal surface was 0.8470. mesial surface was 0.6430, distal surface was 0.5727. lingual surface was 0.2807 and distal surface was 0.2339. When the examination was limited to posterior teeth, the mean alpha value was as follows; occlusal surface was 0.8577, distal surface was 0.8211, lingual surface was 0.7728, buccal surface was 0.7152 and mesial surface was 0.6782. 2. The alpha value of reliability test of the radiographic analysis of carious lesions of the occlusal, mesial, and distal surfaces was 0.8500. 3. The alpha value of reliability test of the $DIFOTI^{TM}$ diagnostic analysis of carious lesions of the occlusal, buccal, lingual, mesial, and distal surfaces was determined to be 0.7917. 4. The $DIFOTI^{TM}$ diagnostic system was found to be the most accurate means of detecting occlusal, buccal, and lingual surface carious lesions (p<0.05), while mesial and distal proximal carious lesions were most accurately assessed using bitewing radiography (p<0.05).

The purpose of this study was to assess the cariogenic potential of five infant formulas in vitro. as compared with 10% sucrose solution and whole bovine milk. Buffering capacities were determined by the amount of 0.1 N lactic acid consumed to titrate the 50ml specimen solutions to pH 4.0. The pH of the specimen solution inoculated by Streptococcus mutans was measured by pH meter and the surface microhardness of primary tooth enamel immersed in the specimen solution was measured by the microhardness tester, before and after 48 hours incubation. Also, the solubility of calcium from synthetic hydroxyapatite was evaluated by atomic absorption spectrophotometer The buffering capacity of infant formulas was higher than that of sucrose solution and lower than that of milk, and there were significant differences among infant formulas(P<0.01). The average pH of infant formulas after 48 hours incubation was lower than that of sucrose solution and milk, and there was significant difference between infant formulas and milk(P<0.05). There were no signifiant differences among oops in the microhardness change of primary tooth enamel and in the amount of dissolved calcium ion from synthetic hydroxyapatite after incubation with Streptococcus mutans. In conclusion, infant formulas seemed to fulfill the basic requirments to cause dental caries in primary teeth, and there were significant differences of cariogenic potential among infant formulas. Cooperative efforts of dentistry and manufacturers to reduce the cariogenic potential of infant formulas would be necessary to prevent the early childhood caries in children.
Chlorhexidine has long been used in mouth washes for the control of dental caries, gingivitis and dental plaque. Minimal inhibitory concentration (MIC) is the lowest concentration of an antimicrobial substance to inhibit the growth of bacteria. Concentrations lower than the MIC are called sub minimal inhibitory concentrations (sub-MICs). Many studies have reported that sub-MICs of antimicrobial substances can affect the virulence of bacteria. The aim of this study was to investigate the effect of sub-MIC chlorhexidine on biofilm formation and coaggregation of oral early colonizers, such as Streptococcus gordonii, Actinomyces naeslundii and Actinomyces odontolyticus. The biofilm formation of S. gordonii, A. naeslundii and A. odontolyticus was not affected by sub-MIC chlorhexidine. However, the biofilm formation of S. mutans increased after incubation with sub-MIC chlorhexidine. In addition, cell surface hydrophobicity of S. mutans treated with sub-MIC of chlorhexidine, decreased when compared with the group not treated with chlorhexidine. However, significant differences were seen with other bacteria. Coaggregation of A. naeslundii with A. odontolyticus reduced by sub-MIC chlorhexidine, whereas the coaggreagation of A. naeslundii with S. gordonii remained unaffected. These results indicate that sub-MIC chlorhexidine could influence the binding properties, such as biofilm formation, hydrophobicity and coaggregation, in early colonizing streptococci and actinomycetes.
