THE REMINERALIZING EFFECTS OF EARLY ENAMEL CAR10US LESION BY SUPERSATURATED BUFFER SOLUTION UNDER PH CYCLING MODEL (pH 순환 모델에서 과포화 용액의 초기 우식 법랑질에 대한 재광화 효과)

  • Dental caries is the most common oral disease. There are many factors contributing to its development, but complete understanding and prevention are not fully known. However, it is possible to remineralize the early enamel curious lesion by fluoride containing remineralization solution. Recently the pH-cycling model has been used to examine the effect of fluoride solution on remineralization of artificial caries in vitro as it can closely simulate the conditions encountered in vivo within a carefully controlled environment. The aim of this study was to evaluate the remineralizing effects of supersaturated buffer solutions under pH-cycling model. The specimen with 3mm-diameter was made using mature bovine incisors which has no caries and has sound enamel surface. Early curious lesions were produced by suspending each specimens into demineralization solution at pH 5.0 for 33 hours and the specimen whose surface hardness value ranged from 25 to 45 VHN were used. The pH cycling treatment regimen consisted of 5 min soaks of three treatment solutions four times per days for 15 days and the continuous cycling of demineralization and remineralization were carried out for 15 days. Following the pH-cycling treatment regimen, the specimens' surface microhardness were measured by the Vickers hardness test (VHN) and analyzed by ANOVA and Duncan's multiple-range test. 1. The surface microhardness value of supersaturated solution, Senstime, and Gagline groups were increased after pH cycling, and that of supersaturated solution was significantly Increased compared to saline group(P<0.05). 2. The surface remineralization effect of fluoride containing solutions was accelerated by saliva under pH-cycling mode 3. The pH cycling model was considered appropriate to mimic the intra-oral pH changes when evaluating demineralization and remineralization in vitro. Under the results of above study, salivary remineralization effect can be improved by fluoride containing remineralization solution.   • Since it is favorable to include the incipient caries lesion in the diagnostic criteria in adolescence, this study had surveyed dental caries status of adolescents at ages of 13 and 16 by using WHO basic methods and ICDAS-II codes. In this study, mean DMFT index was 3.71, and mean DT index was 1.94. For both indices, the age 16 group showed higher values than the age 13 group. By groups of teeth, DMFT index and DT index exhibited highest to the lowest values in molar, premolar, and anterior teeth, respectively. 77.46% of total numbers of teeth were classified as code 0 in ICDAS-II. Compared to anterior teeth, numbers of decayed teeth were increased in posterior teeth. All caries lesions in anterior teeth and premolars were limited to enamel. ICDAS-II code is an useful method to detect the incipient caries lesion, allowing preventive control on caries management.

A epidemiological study on the oral health in preschool children for the development of community based oral health program in Sungnam city (성남시 보건소 유아구강보건사업 개발을 위한 구강건강실태에 관한 조사연구)

  • Dental caries causes the majority of tooth loss among Koreans. Korea is experiencing an increase in dental caries. And it shows a high prevalence of caries in preschool children, which contrasted with the much-improved situation in developed countries. So, it is important to control caries from preschool periods in Korea. Recently, inspections of the teeth of preschool children for evidence of dental disease became a legislative duty in Korea following Oral Health Act(2000). But, standardized oral health programs in preschool children is not yet proposed. Therefore, it is necessary to develop an community based oral health program in preschool children. The purpose of this study was to obtain basic data for the development of community based oral health program in preschool children of Sungnam city. Oral health status in primary dentition were surveyed in 862 2-6 year-old preschool children of Sungnam city by WHO criteria. From the data, df rate, percentage of preschool child ren with active caries, dft index, dt rate, percentage of preschool children with pit and fissure sealant of each age were calculated, and discussed. The obtained results were as follows : 1. The experience of dental caries in primary dentition showed a tendency to increase quickly between 2 and 3 year old children. So, the oral health program in preschool children should be developed to prevent dental caries and control their oral health under three years of age. 2. In 5 year-old children, the percentage with active caries was 60.8% and dft index was 5.06. The dt rate showed a tendency to decrease as proportion to age, but even though 6 year-old children, the dt rate was 47.7%. Therefore, the annual screening dental examination and oral health education programs at the institution for preschool children should be developed to detect and treat dental caries in primary dentition at early stage. 3. The percentage of preschool children with pit and fissure sealant at six years of age was 10.2%. So, pit and fissure sealant and professional fluoride application programs in Public Health Center should be developed to prevent dental caries. 4. Also, it is necessary to establish oral health goals in preschool children and develop water fluoridation program 10 improve and preserve oral health of preschool children in Seongnam city, effectively.

Applicability Evaluation of Quantitative Light-Induced Fluorescence-Digital and Cariview in Cries Prediction Study (Quantitative Light-Induced Fluorescence-Digital과 Cariview의 우식예측 연구에 대한 활용도 평가)

  • The purpose of this study was to evaluate the correlation between caries experience, Quantitative Light-Induced Fluorescence-Digital (QLF-D) redings and new caries activity test (Cariview) results in preschool children. Fifty-seven healthy kindergarten children (male 28, female 29) were participated this study. The calibrated dentist investigated the caries experience of children and new caries activity test. Cariview samples were incubated in the activated medium at $37^{\circ}C$ for 48 hours. All QLF-D taking and readings were performed by one experienced and trained operator under identical conditions in a dental unit chair located in a darkened room. Analysis range was limited to the maxillary and mandibular anterior teeth. QLF-D redings (white spot and dental plaque) were analysed using QLF system. The dft index had a relatively high correlation with the QLF-D redings (white spot: r=0.617, simple plaque score: r=0.500) (p<0.01). Also, there was significant correlation between dft index and the Cariview score (r=0.286) (p<0.05). However, the Cariview score had no significant correlation between dt index and ft index (p>0.05). QLF-D can be evaluated objectively the initial caries lesions and dental plaque correlated with caries experience. Therefore, QLF-D will be useful to the study of caries prediction.

Influencing factors of dental caries across the life cycle of Koreans (생애주기에 따른 치아우식증 영향 요인)

  • Objectives: This study was conducted in order to identify the various influencing factors of dental caries according to the socio-economic characteristics and oral health behaviors across the life cycle among Koreans. Methods: The data were extracted from the 6th Korea National Health and Nutrition Examination Survey (2013-2015) and a total of 4,871 subjects with ages of 7 and over were selected. The data were analyzed using SPSS 21.0 for ${\chi}^2$-test and multi-logistic regression. Results: Significant differences were observed in the socio-economic characteristics, health behaviors and in the dental caries across the life cycle. The influencing factors of DT includes the type of health insurance (p<0.05) in school aged & adolescence, Oral health examination/year (p<0.01), Residence (p<0.05) in early adults, type of health insurance (p<0.001), Oral health examination/year (p<0.001), use of oral hygiene products (p<0.01) in late Adults, Oral health examination/year (p<0.05) and Gender (p<0.05) in old age. Conclusions: This study suggests that dental health promotion can be enhanced by regular checkup. The government must provide the people with better quality of oral health care and promotion across the life cycle in the near future.

Descending Necrotizing Mediastinitis with Dental Caries -One case report- (충치로 인한 하행 괴사성 종격동염 -1례보고-)

  • Descending Necrotizing Mediastinitis(DNM) is a complication of oropharyngeal infections that can spread to the mediastinum. It is difficult to diagnose early because clinical and radiologic findings appear in the late stage of the infection. late diagnosis is the principal reason for the high mortality in DNM. An 18-year-old female admitted with Ludwig's angina from dental caries. Despite of combined antibiotics, dental extraction and drainge of submental abscess, infection spread to the cervical area. Chest computed tomogram revealed extension of the abscess to the pretracheal and periaortic space and development of bilateral pleural empyema. We performed bilateral cervical mediastinotomy and thoracotomy for drainage and debridement. Tracheostomy to secure the airway and postoperative pleural irrigation were performed. Postoperative course was uneventful and patient was discharged on the 40th postoperative day. It is important to perform chest CT scanning for early diagnosis of DNM when oropharyngeal infection spreads to the cervical area. Improved survival of patients with DNM implies early and radical surgical drainage and debridement via a cervical mediastinomy and thoracotomy.

Assessment of Early Dental Caries by Using Optical Coherence Tomography (Optical Coherence Tomography를 이용한 초기 치아우식 검사)

  • The purpose of this study was to assess the correlation between integrated mineral loss (volume % mineral${\times}{\mu}m$, ${\Delta}Z_{TMR}$) determined using transverse microradiography (TMR) and integrated reflectivity ($dB{\times}{\mu}m$, ${\Delta}R_{OCT}$) determined using optical coherence tomography (OCT) for detecting early dental caries with lesion depth more than $200{\mu}m$. Sixty tooth specimens were made from sound bovine teeth. They were immersed in a demineralized solution for 20, 30, and 40 days. The ${\Delta}R_{OCT}$ was obtained from the cross-sectional OCT image. The ${\Delta}Z_{TMR}$ was obtained from the TMR image. The correlation between ${\Delta}R_{OCT}$ and ${\Delta}Z_{TMR}$ was examined using Pearson correlation. The Bland-Altman plot was constructed using the ${\Delta}R_{OCT}$ and ${\Delta}Z_{TMR}$ values. A significant correlation between ${\Delta}R_{OCT}$ and ${\Delta}Z_{TMR}$ was confirmed (r=0.491, p=0.003). Moreover, most of the difference between ${\Delta}R_{OCT}$ and ${\Delta}Z_{TMR}$ was included in the error section of the Bland-Altman plot. Therefore, OCT could be used as a substitute for TMR when analyzing mineral loss in early dental caries.

Comparison between QraypenTM Imaging and the Conventional Methods of Visual Inspection and Periapical Radiography for Proximal Caries Detection in Primary Molars: An In Vivo Study (유구치 인접면 우식 병소 진단에 있어 QraypenTM과 시진 및 구내 치근단 방사선의 비교)

  • The purpose of this study was to evaluate the efficacy of the newly-developed $Qraypen^{TM}$ (All In One Bio, Korea) system for the diagnosis of early proximal caries by comparing it with the conventional methods of visual inspection and periapical radiography. This study was carried out from July 2015 to April 2016 targeting 32 children aged 7~12 years who visited Y-Dental Clinic for school oral health examinations. Two investigators selected and examined a total of 153 primary molars that had not undergone restorative treatment. Comparisons were carried out between visual inspections, readings of posterior periapical radiography images, and readings of $Qraypen^{TM}$ images. This study revealed that the percentage of interproximal surfaces of primary molar teeth without caries incidence was 83.7% using $Qraypen^{TM}$ imaging and 84.9% using visual inspection and periapical radiography. The differences between the two methods were not statistically significant. Thus, $Qraypen^{TM}$ is expected to be a useful and convenient auxiliary diagnostic device that can facilitate the detection of hidden proximal caries in primary molars.

A simple colorimetric method for the clinical evaluation of caries activity in adults (성인의 치면세균막 내 산생성능력과 치아우식경험과의 연관성)

  • Purpose: The aim of this study was to confirm the validity of the $Cariview^{TM}$, test in adults by assessing correlations between $Cariview^{TM}$, scores and caries experience using a quantitative light-induced fluorescence (QLF) technique. Methods: This retrospective cross-sectional study included 111 patients aged 19-64 years who visited a dental clinic in Seoul from 2015 to 2016, and underwent the $Cariview^{TM}$, test and had their teeth photographed with the Qraycam. Plaque was scraped with sterilized cotton swabs, and then placed in a culture medium and incubated at $37^{\circ}C$ for 48 h. Finally, the degree of color change was evaluated, and scored on a 0-100 point scale. Caries experience index included not only cavitated lesions but also early caries that were detected on white and fluorescence images. Participants were divided into low, moderate and high caries risk groups according to the $Cariview^{TM}$, score, and caries experience was compared between risk groups using ANCOVA. Spearman correlation coefficients were calculated to analyze the associations between $Cariview^{TM}$, scores and caries indices. Result: The mean age of the participants was 40 years, and the mean $Cariview^{TM}$, score was $53.6{\pm}20.7$. The WDMFT and the DMFT indices significantly increased with caries risk, and this increase was significantly different between the low- and high-risk groups. The $Cariview^{TM}$, test scores were significantly correlated with the WDMFT (r = 0.28, P = 0.003) and DMFT (r = 0.29, P = 0.002) indices. Conclusion: The $Cariview^{TM}$, test could be a clinically useful and simple method for assessing caries risk in adults.

Diagnostic Utilization of Laser Fluorescence for Resin Infiltration in Primary Teeth (유치의 레진침투법을 위한 레이저 형광법의 진단적 활용)

  • This study was performed to evaluate clinical use of laser fluorescence (LF) to identify early childhood caries lesions suitable for applying resin infiltration. 20 exfoliated primary molars with proximal caries were selected and cut buccolingually cross the central pit for regarding the mesial and distal surfaces respectively. 27 specimens corresponding to ICDAS code 1 and 2 were selected and the LF values were measured. When infiltrant resin was applied, double staining for microscopy detection has done simultaneously. Tooth samples were sliced with 0.7 mm thick. The maximum lesion depth, maximum penetration depth, and average penetration rate were measured from the confocal scanning laser microscope image. Pearson correlation analysis was performed. The intraclass correlation coefficient of LF values shows excellent agreement. LF values had positive correlation with penetration rate, but not lesion depth and penetration depth. Significant correlation between LF readings and penetration rate was verified in deep enamel caries and dentin caries except shallow enamel caries. Infiltrant resin could penetrate with a higher rate and LF values could be increased in more active caries lesions. In assessing radiologically similar caries lesion, laser fluorescence might be useful for identifying caries activity.
