A Case Report of Functional Dyspepsia with Abdominal Distention (기능성 소화불량증으로 복부창만(腹部脹滿)이 지속되는 환아 증례 1례 보고)

  • Objectives This study is to report a case that has an important meaning as a result of treating functional dyspepsia with oriental treatment. We investigated functional dyspepsia in 6 years child who had to continue abdominal distention for a month after gastroenteritis. The child has recovered from all dyspepsia symptoms after treating with oriental medicine. Methods The patient had dyspepsia symptoms all day, especially repeated abdominal distention. He sometimes had nausea, vomiting, diarrhea, abdominal pain. For those symptoms, we treated him with herbal medicine and acupuncture. The aim of treatment was to improve functional dyspepsia without western treatment. We measured patient's abdomen circumference for change of abdominal distention. Results The symptoms of functional dyspepsia were vanished and the patient maintains his condition with oriental medicine treatment without western treatment. After herb medicine treatment and acupuncture treatment the patient's dyspepsia symptoms (nausea, vomiting, diarrhea, abdominal pain, and abdominal distention) were vanished and maintained his condition for two months after complete the treatment. Conclusions This study showed that oriental medicine can elevate the functional dyspepsia children's quality of life with continuous health care and treatment. For more accurate studies, further studies would be needed with more cases.

The Effects of Maitland Orthopedic Manual Physical Therapy on the Digestive System

  • The purpose of the present study is to apply Maitland orthopedic manual physical therapy to patients to examine the effects of the therapy on the digestive system through serum gastrin tests that can identify the motility of the digestive system and dyspepsia symptoms and can provide basic data for internal medicinal physical therapy. Maitland orthopedic manual physical therapy was implemented on 20 subjects in total, with 10 in a dyspepsia group and 10 in a control group, for 20 minutes per day, three days per week for three weeks, and the following results were obtained. In the control group, serum gastrin values significantly decreased between before and after treatment(p<.01), and among questionnaire items regarding dyspepsia, those regarding epigastric pain and belching showed significant decreases in these symptoms(p<.05). In the dyspepsia group, serum gastrin values significantly decreased between before and after treatment(p<.01) and all questions regarding dyspepsia showed significant decreases in all symptoms(p<.01). According to the analysis, the dyspepsia group decreased significantly more than the control group in serum gastrin values and all dyspepsia symptoms, except for belching(p<.001). Through the present study, it was identified that Maitland orthopedic manual physical therapy can improve dyspepsia symptoms and is an effective treatment method for internal diseases, such as dyspepsia, by improving gastric motility to become close to the normal state.

  • Objective : The purpose of this study is to determine a distribution of sasang constitution among dyspepsia patients, and to weigh their symptoms score. Methods : We surveyed symptoms of dyspepsia and QSCCII in one hundred dyspepsia patients who had undergone upper gastrointestinal endoscopy. Results : In distribution of sasang constitution in total dyspepsia patients group and distribution according to the result of endoscopy, ratio and symptoms score of So-Eum-tn were higher than the others. However the differences were not statistically significant. According to symptoms score by age, symptoms score of Tae-Eum-In in patients above 35 years old group was higher than the others. In patients group under 35 years old, symptoms score of So- Yang-In is higher than the others. However in neither group was this statistically significant.

The Effects of Interferential Current Therapy on Functional Dyspepsia

  • This study aims to address the effect of interferential current therapy and thereby to provide basic resources to physical therapies for internal medicine by identifying symptoms for functional dyspepsia and serum gastrin level that shows gastric motility. Following results were obtained by performing interferential current therapy to 16 subjects composed of 8 for functional dyspepsia group and 8 for control group for 20 minutes a day, 3 days a week, for 6 weeks. In control group, serum gastrin level was significantly( p<.01), lowered after the therapy whereas there was no significant difference observed in all questions from questionnaire for symptoms of functional dyspepsia between before and after the therapy. In functional dyspepsia group, serum gastrin level was significantly(p<.01), lowered after the therapy and there was also significant(p<.01) reduction in every question from questionnaire for symptoms of functional dyspepsia between before and after the therapy. There was more significant decrease in serum gastrin level and reduction in questionnaire for symptoms of functional dyspepsia in the functional dyspepsia group compared to the control group(p<.01). This study confirms the interferential current therapy as an effective therapeutic method for internal diseases including functional dyspepsia since it not only improves the symptoms of functional dyspepsia but also allows the gastric motility close to normal.

Effect of herb drug medicine Treatment for Functional Dyspepsia: Controlled Trial (기능성 소화불량증 환자에 대한 한약복합제형간의 비교 임상시험: 대조군 연구)

  • Objective : Functional dyspepsia is a prevalent disease. It impedes subjective quality of life. The purpose of this research is to examine the equivalent effect of herb drug medicine treatment(H-D)and Over the Counter(OTC) for functional dyspepsia. Method : In this controlled study, we compared herb drug medicine(H-D) with Over the Counter(OTC) of functional dyspepsia. 30 volunteers who satisfied the requirements were enrolled in study. Severity of dyspepsia was measured by Nepean Dyspepsia Index(NDI-K) before and after treatments. Result : The results are summarized as follows. 1. In Herb drug medicine and Over the Counter groups, total key symptoms score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. 2. In Herb drug medicine and Over the Counter groups, each symptoms score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. 3. In Herb drug medicine and Over the Counter groups, quality of life score of after treatment were significantly decreased and improve rate of key symptoms was higher than before treatment, but there were no statistical significance between two groups. Conclusion : Herb drug medicine treatment(H-D) is effective to improve the symptoms and quality of life in patients with functional dyspepsia.

A Clinical Case Report of Soyangin Patients with Functional Dyspepsia (기능성 소화불량증이 호전된 소양인 비수한표한병 환자 3례 보고)

  • Objectives The purpose of this study was to report significant improvement of functional dyspepsia in 3 patients, who were diagnosed with 'Soyangin Spleen Cold-based Exterior Cold disease' and applied medication based on Sasang Constitutonal medicine.Methods The patients were treated with herbal medications, according to their own constitution. The objective outcome was measured by body composition analyzer and Gastrointestinal Symptom Rating Scale (GSRS), and the subjective sensation was assessed by using Visual Analogue Scale (VAS).Results The symptoms of functional dyspepsia improved remarkedly in all respects, without any side effects. Original symptoms also changed for the better.Conclusions This results show that it is available that functional dyspepsia is treated with medicines according to Sasang Constitutional medicine.

Effect of Herb Medicine Treatment for Functional Dyspepsia: A Randomized Placebo-Controlled and Compared Standard Treatment Trial (기능성 소화불량증 환자에 대한 한약복합제의 치료 효과: 무작위배정 표준치료제 위약 대조군 연구)

  • Objective: Functional dyspepsia is a prevalent disease. It impedes subjective quality of life. The purpose of this study was to examine the equivalent effect of herb medicine treatment (DA-9701) for functional dyspepsia. Methods: In this randomized, single-blinded, placebo-controlled study, we compared a herb medicine (DA-9701) with standard treatment (mosapride) and placebo for the treatment of functional dyspepsia. 42 volunteers who satisfied the requirements were enrolled in study. Severity of dyspepsia was measured by Nepean Dyspepsia Index (NDI) and Functional Dyspepsia Quality of Life (FD-QOL) before and after treatments. Results: 1. In the DA-9701 group, total key symptoms score was significantly lower and improve rate of key symptoms was higher than in the mosapride and placebo groups, but there were no statistically significant differences between three groups. 2. In the DA-9701 and mosapride groups, "nausea" and "bad breath" were significantly lower compared with the placebo group. 3. In the DA-9701 group, NDI Quality of Life scores were significantly higher, but there were no [other] statistically significant differences between the three groups. 4. In the DA-9701 and mosapride groups, FD-QOL scores were higher compared with the placebo group, but there were no statistically significant differences between the three groups. Conclusion: Herb medicine treatment (DA-9701) is effective to improve the symptoms and quality of life in patients with functional dyspepsia.

Could Headache be one of Symptoms induced by Gastric Dysmotility in Patients with Functional Dyspepsia? (기능성 소화불량증 환자의 두통은 위 운동성 장애에 의한 증상이 될 수 있는가?)

  • Objectives : The aim of this study was to investigate the relationship between headache and gastric dysmotility in patients with functional dyspepsia by using electrogastrogram and bowel sounds analysis. Methods : 127 patients(male 40, female 87) with functional dyspepsia were enrolled. By using inquiry and the questionnaire, we examined whether each patient had headache and abdominal pain. All patients were divided into 4 groups according to the above symptoms. Gastric motility was measured during the fasting-postprandial state by using electrogastrogram and bowel sounds analysis. Results : 33 patients complained of headache. There was a significant difference between headache patients and non-headache patients in gastric arrhythmia of the fasting state. Moreover, in headache patients without abdominal pain, postprandial improvement of gastric arrhythmia was poorer than the other groups. So, headache patients without abnormal pain had more severe gastric myoelectrical rhythm disorder than headache patients with abdominal pain. Conclusion : Headache could be one of symptoms induced by gastric dysmotility in patients with functional dyspepsia. Especially, headache in functional dyspepsia was related to gastric arrhythmia.

2 Case Reports of Gastroesophageal Reflux Disease Diagnosed as Soyangin Mangeum (소양인 망음증으로 진단한 역류성식도염 치험 2례)

  • Objectives This study was designed to report treatment of Soyangin Mangeum with gastroesophageal reflux disease. Methods We treated two patients suffering from dyspepsia and chest discomfort. A female patient was treated with Hyeongbangjiwhang-tang. A male patient was treated with Hyungbangsabaek-san. The progress was evaluated with the visual analogue scale for dyspepsia, chest discomfort, and other symptoms. Results A female patient's symptoms such as dyspepsia, chest discomfort were almost cured. but they increased with patient's emotional state repeatedly. A male patient' symptoms such as abdominal pain, dyspepsia, nausea, heart burn was successfully disappeared. Conclusions Hyeongbangjiwhang-tang and Hyungbangsabaek-san can be considered as a treatment for patient with Soyangin Mangyin diagnosed as gastroesophageal reflux disease.

Evaluation for Therapeutic Effectiveness of Banwhasashim-tang in Functional Dyspepsia (기능성 소화불량증 환자에 대한 반하사심탕 엑기스제의 임상효과)

  • Objective : We evaluated the therapeutic effect of Banwhasasimtang dry extract powder on gastrointestinal symptoms and its side effects in patients with functional dyspepsia. Methods : Patients with functional dyspepsia, defined as discomfort in the epigastrium. A total 45 patients participated in this study. Dyspeptic symptoms ratio was obtained by patients, total point of symptom score/total 21 points x 100(%). On each of symptom after 4 weeks administration of Banwhasashim-tang was made by variation of symptom score: If symptoms score of posttreatment is lower than that of pretreatment, it is improved ; Otherwise aggravated, and if there is no change between them, unchangeable. When a full health condition of normal control is assumed 100%, relative health condition of dyspeptic patient could be calculated from 'full health condition-dyspeptic symptoms ratio'. Therefore a whole satisfaction rate on symptom improvement was presented with variation of a relative health condition to dyspepsia between pretreatment and post. Results : Banwhasashimtang significantly improved symptoms in patients. The significance was shown in both all seven items(p<0.01, respectively) and 19.2% (p<0.01) of increase of a whole satisfaction rate on symptom improvement. There is no remarkable side-effect in all laboratory test, excepting 1 case of abdominal pain and another one case of recurred pain of chronic prostatitis. Conclusion : Banwhasashimtang extract provides symptomatic relief in functional dyspepsia and is no remarkable side-effect drug.
