A Study on the Anti-Corrosion Paint(EH 2350) Compatibility Verification for Naval Surface Vessels's Cavitation (캐비테이션 발생에 따른 해군 수상함정 방청도료(EH 2350) 적합성 검증에 관한 연구)

  • The naval surface vessels, which were often exposed to harsh marine environment, tended to be corrosive due to military operations on various sea-areas and courses. Although R.O.K Navy applied various methods to protect further corrosion, the hull corrosion occurred due to cavitation were found on the naval surface vessels at regular and occasional docking. Hull corrosion was a critical factor directly to affect the lifetime of ships and their operational capabilities adversely. In this paper, EH 2350, which was the main anticorrosion paint used by R.O.K. Navy, was compared with DuraTough DL by used by the U.S Navy to collect materials related to anti-corrosion paint. In addition, the paint compatibility verification was conducted through wear abrasion test. Assuming that it was exposed to sea-environment various both abrasion cycle and weight for objective verification. by varying both the abrasion cycles and weights. In this study, the reliability of the EH 2350 conformity, which was used in Naval surface vessels, was secured.

Fatal Subdural Empyema Following Pyogenic Meningitis

  • Subdural empyema is a rare form of intracranial sepsis associated with high morbidity and mortality. The most frequent cause is extension of paranasal sinusitis through emissary veins or of mastoiditis through the mucosa, bone, and dura mater. Development of subdural empyema after pyogenic meningitis is known to be very unusual in adults. We report a rare case of fatal subdural empyema, an unusual complication of pyogenic meningitis. Our bitter experience suggests that subdural empyema should be borne in mind in patient with pyogenic meningitis who exhibit neurological deterioration.

  • The present report addresses the durability related design issues for the structural reinforced concrete of the Busan-Geoje Fixed Link - Bridges and Immersed Tunnel. The overall aim of the durability related issues is to establish the requirements to the design and the construction that will secure the stated and achieve that objective with cost effective construction. To prove that the stated design life of 100 years is valid, a performance and reliability based verification, according on the DuraCrete approach have been performed.

  • As reported in the literature, extraskeletal mesenchymal chondrosarcoma is a rare malignant tumor of soft tissue, and it has a unique, distinctive histologic picture and poor prognosis.The common sites are the orbit, the cranial dura mater, the neck, the thigh, the leg, the chest wall, and the retroperitoneum. Radical excision of the tumor seems to be the primary treatment.We report experience with a very rare case of extraskeletal mesenchymal chondrosarcoma in the posterior mediastinum.

  • Since diaphragmatic injuries are difficult to diagnose, those that missed may present with latent symptoms.Delayed diaphragmatic hernia is very rare and occurs commonly after penetrating or blunt trauma. Recently we had experience with two cases of delayed diaphragmatic hernia after left nephrectomy.They were operated by herniolysis and closure of diaphragm using prosthetic patch[Dura, Dacron through the left thoracotomy. Postoperative courses were uneventful.

Effect of chemical surface treatment on the flexural bond strength of heat curing denture base resin and reliners (화학적 표면처리가 열중합형 의치상 레진과 이장재간의 굴곡결합강도에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the effect of the chemical surface treatment on the flexural bond strength of heat curing denture base resin and reliners. Methods: Denture base resin surface was treated with MMA 95% and TEGDMA 5%, MMA 95% and silane coupling agent 5%, heat curing resin monomer. After denture reliners were injected, flexural bond strength was measured. Results: The repair resin of Vertex SC was higher than Lang, hard reliner of Kooliner was higher than Rebase. Soft reliner of Dura base and Coe-soft showed differently according to the surface treatment. The all chemical treatment groups on Vertex SC were significantly higher than control(p<0.05). In Lang group, 5% MPS treated group showed significantly higher flexural bond strength than others(p<0.05). In Kooliner group, all chemical treatment groups showed significantly higher than control(p<0.05). In Rebase group, the 5% MPS and the monomer denture base resin treated groups showed significantly higher than others(p<0.05). In Dura base group, 5% MPS treated group showed significantly higher flexural bond strength than others(p<0.05). In Coe-soft group, all treated groups were significantly higher than control group(p<0.05). Conclusion: TEGDMA, MPS, and the monomer of heat-cured denture base resin were effective to improve the bond strengths between denture base and denture relining materials. Especially, 5% MPS expected to strengthen effectively the bonding property of denture base and denture reliners within the results of this study.

A Gunshot Wounds to the Cervical Spine and the Cervical Spinal Cord: A Case Report (총상으로 인한 경추부 및 척수손상: 증례 보고)

  • Gunshot wounds are rare in Korea, but they have tended to increase recently. We experienced an interesting case of penetrating gunshot injuries to the cervical spine with migration the fragments of the bullet within the dural sac of the cervical spine, so discuss the pathomechanics, treatment and complications of gunshot wounds to the spine and present a review of the literature. A 38-year-old man who had tried to commit suicide with a gun was admitted to our hospital with a penetrating injury to the anterior neck. the patient had quadriplegia. A Computed tomography (CT) scan and 3-dimensional CT of the spine showed destruction of the left lateral mass and lamina of the 5th cervical vertebra; the bullet and fragments were found at the level of the 5th cervical vertebra. The posterior approach was done. A total laminectomy and removal of the lateral mass of the 5th cervical vertebrae were performed, and bone fragments and pellets were removed from the spinal canal, but an intradurally retained pellets were not totally removed. A dural laceration was noted intraoperatively, and CSF leakage was observed, so dura repair was done watertightly with prolene 6-0. The dura repair site was covered with fibrin glue and Tachocomb$^{(R)}$. Immediately, a lumbar drain was done. Radiographs included a postoperative CT scan and X-rays. The postoperative neurological status of the patient was improved compared with the preoperative neurological status. however, the patients developed symptoms of menigitis. He received lumbar drainage(200~250 cc/day) and ventilator care. After two weeks, panperitonitis due to duodenal ulcer perforation was identified. Finally, the patient died because of sepsis.

Idiopathic Hypertrophic Spinal Pachymeningitis with an Osteolytic Lesion

  • Idiopathic hypertrophic spinal pachymeningitis (IHSP) is a chronic, progressive, inflammatory disorder characterized by marked fibrosis of the spinal dura mater with unknown etiology. According to the location of the lesion, it might induce neurologic deficits by compression of spinal cord and nerve root. A 58-year old female with a 3-year history of progressive weakness in both lower extremities was referred to our institute. Spinal computed tomography (CT) scan showed an osteolytic lesion involving base of the C6 spinous process with adjacent epidural mass. Magnetic resonance imaging (MRI) revealed an epidural mass involving dorsal aspect of cervical spinal canal from C5 to C7 level, with low signal intensity on T1 and T2 weighted images and non-enhancement on T1 weighted-enhanced images. We decided to undertake surgical exploration. At the operation field, there was yellow colored, thickened fibrous tissue over the dura mater. The lesion was removed totally, and decompression of spinal cord was achieved. Symptoms improved partially after the operation. Histopathologically, fibrotic pachymeninges with scanty inflammatory cells was revealed, which was compatible with diagnosis of idiopathic hypertrophic pachymeningitis. Six months after operation, motor power grade of both lower extremities was normal on physical examination. However, the patient still complained of mild weakness in the right lower extremity. Although the nature of IHSP is generally indolent, decompressive surgery should be considered for the patient with definite or progressive neurologic symptoms in order to prevent further deterioration. In addition, IHSP can present as an osteolytic lesion. Differential diagnosis with neoplastic disease, including giant cell tumor, is important.

Non-Watertight Intermittent Dural Closure in Neurological Surgery (뇌수술의 비방수성 경막 봉합)

  • Objective : In neurosurgical practice, it has been generally accepted that when the dura is opened, it should be watertightly closed, and traditionally non-watertight closure has not been performed. We clinically tried non-watertight closure, analyzed the frequency of CSF leakage and evaluated the possible clinical application of non-watertight closure. Methods : After classifying our cases with supratentorial and infratentorial approach, we tried non-watertight and watertight closures and compared the results. We also analyzed the cases with or without dural graft. Results : In supratentorial approach, the rate of cerebrospinal fluid leakage noted in non-watertight closure was similar to that of watertight closure. In infratentorial approach, except microvascular decompression(MVD), the rate of cerebrospinal fluid leakage in non-watertight closure was higher than that of watertight closure. Dura graft application did not seemed to influence the cerebrospinal fluid leakage. Conclusion : Since the frequency of cerebrospinal fluid leakage was not higher in non-watertight closure than that of watertight closure, non-watertight closure can be applied in supratentorial approach. In infratentorial approach, non-watertight closure may be applied in surgery with relatively short dural incision, such as MVD. However, non-watertight closure doesn't seem to be appropriate in surgery that requires wide dural incision, such as skull base surgery.

Comparative study on tissue responses of 3 resorbable membranes in rats (흡수성 차폐막의 조직반응에 관한 비교연구)

  • The purpose of this study is to evaluate histologically the resorption and tissue response of various resorbable collagen membranes used for guided tissue regeneration and guided bone regeneration, using a subcutaneous model on the dorsal surface of the rat. In this study, 10 Sprague-Dawley male rats (mean BW 150gm) were used and the commercially available materials included acellular dermal matrix allograft, porcine collagen membrane, freeze-dried bovine dura mater. Animals were sacrificed at 2,6 and 8 weeks after implantation of various resorbable collagen membranes. Specimens were prepared with Hematoxylin-Eosin stain for light microscopic evaluation. The results of this study were as follows: 1. Resorption : Inner portion of porcine collagen membrane was resorbed a lot at 6 weeks, but its function was being kept for infiltration of another tissues were not observed. Freeze-dried bovine dura mater and acellular dermal allograft were rarely resorbed and kept their structure of outer portion for 8 weeks. 2. Inflammatory reactions : Inflammatory reaction was so mild and foreign body reaction didn't happen in all of resorbable collagen membranes, which showed their biocompatibility. 3. In all of resorbable collagen membranes, multinuclcated giant cells by foreign body reactions were not observed. Barrier membranes have to maintain their function for 4-6 weeks in guided tissue regeneration and at least 8 weeks in guided bone regeneration. According to present study, we can find all of the resorbable collagen membranes kept their function and structure for 8 weeks and were rarely resorbed. Foreign body reaction didn't happen and inflammatory reaction was so mild histologically. Therefore, all of collagen membranes used in this experiment were considered proper resorbable membranes for guided tissue regeneration and guided bone regeneration.
