Immediate Postoperative Epidural Hematomas Adjacent to the Craniotomy Site

  • Objective : The authors present eight cases of immediate post-operative epidural hematomas[EDHs] adjacent to the craniotomy site, describe clinical details of them, and discuss their pathogenesis. Methods : Medical records of eight cases were retrospectively reviewed and their clinical data, operation records, and radiological findings analyzed. Any risk factors of the EDHs were searched. Results : In 5 of 8 cases, adjacent EDHs developed after craniotomies for the surgical removal of brain tumors. Three cases of adjacent EDHs developed after a pterional approach and neck clipping of a ruptured anterior communicating artery aneurysm, a ventriculoperitoneal shunt, and a craniotomy for a post-traumatic EDH, respectively. In all eight cases, brain computed tomography[CT] scans checked immediately or a few hours after the surgery, revealed large EDHs adjacent to the previous craniotomy site, but there was no EDH beneath the previous craniotomy flap. After emergent surgical removal of the EDHs, 7 cases demonstrated good clinical outcomes, with one case yielding a poor result. Conclusion : Rapid drainage of a large volume of cerebrospinal fluid or intra-operative severe brain collapse may separate the dura from the calvarium and cause postoperative EDH adjacent to the previous craniotomy site. A high-pressure suction drain left in the epidural space may contribute to the pathogenesis. After the craniotomy for brain tumors or intracranial aneurysms, when remarkable brain collapse occurs, an immediate postoperative brain CT is mandatory to detect and adequately manage such unexpected events as adjacent EDHs.

Remote Epidural Hematoma Following the Removal of Brain Tumors : Report of Three Cases (뇌종양 제거 후 원격부위에 발생한 뇌경막외혈종 - 증례보고 -)

  • Objective : The authors present three cases of brain tumors in which epidural hematomas(EDHs) were developed postoperatively in the remote areas from craniotomy sites. The preventive tactics as well as possible mechanisms of development of remote EDH are discussed. Material and Methods : The magnetic resonance imagings of three patients revealed a left lateral ventricular mass located just aside of foramen Monro in a 27-year-old male, a large cystic mass in the temporal lobe in a 35-year-old male, and a partially calcified pineal mass in a 27-year-old male patient. The surgical removals of these tumors were performed without any noticeable events during surgery via left frontal transcortical transventricular approach for lateral ventricular tumor, left temporal craniotomy for cystic temporal tumor, and right occipital transtentorial approach for pineal tumor. Results : Postoperative EDHs remote from the sites of craniotomy were detected by the immediate postoperative computerized tomographic scans. We obtained good outcomes without any morbidity in all three patients with emergent evacuation of the hematoma. The pathologic diagnoses were lateral ventricular ependymoastrocytoma, temporal craniopharyngioma and mixed germinoma of the pineal region. Conclusion : It is postulated that a sudden reduction of intracranial pressure(ICP) at the time of tumor removal may strip the dura from the inner table of the skull to cause EDH from the remote site of craniotomy. Gradual reduction of ICP with slow drainage of cerebrospinal fluid before tumor removal as well as lowering the head position of patient during surgery might be helpful for preventing this unusual complication.

  • Silk fibroin is a natural biomaterial that has the biocompatibility and other many advantages. But as a silk fibroin membrane thickness increases, the transparency becomes more opaque. Because the transparency of membranes tissue such as the cornea and dura mater are necessary, transparent membrane is required to replace these transparent membranes. In this study, we fabricated blending silk fibroin membranes that made by mixing the various inorganic salts or polymer in an aqueous solution of silk fibroin. The transparency of the membranes were analyzed. the transparency of these membranes is very different, depending on the mixed materials. Inorganic salts mixed silk membrane was more transparent than the polymer mixed one. Especially, the silk fibroin membrane with calcium chloride was very transparent. We showed the possibility of blending silk fibroin membrane, which can be used in perfect transparent membrane such as the cornea. In the future, we expect that the transparent blending silk fibroin membrane can be used in various medical applications.

Idiopathic Hypertrophic Spinal Pachymeningitis : Report of Two Cases and Review of the Literature

  • Idiopathic hypertrophic spinal pachymeningitis (IHSP) is a rare inflammatory disease characterized by hypertrophic inflammation of the dura mater and various clinical courses that are from myelopathy. Although many associated diseases have been suggested, the etiology of IHSP is not well understood. The ideal treatment is controversial. In the first case, a 55-year-old woman presented back pain, progressive paraparesis, both leg numbness, and voiding difficulty. Initial magnetic resonance imaging (MRI) demonstrated an anterior epidural mass lesion involving from C6 to mid-thoracic spine area with low signal intensity on T1 and T2 weighted images. We performed decompressive laminectomy and lesional biopsy. After operation, she was subsequently treated with steroid and could walk unaided. In the second case, a 45-year-old woman presented with fever and quadriplegia after a spine fusion operation due to lumbar spinal stenosis and degenerative herniated lumbar disc. Initial MRI showed anterior and posterior epidural mass lesion from foramen magnum to C4 level. She underwent decompressive laminectomy and durotomy followed by steroid therapy. However, her conditions deteriorated gradually and medical complications occurred. In our cases, etiology was not found despite through investigations. Initial MRI showed dural thickening with mixed signal intensity on T1- and T2-weighted images. Pathologic examination revealed chronic nonspecific inflammation in both patients. Although one patient developed several complications, the other showed slow improvement of neurological symptoms with decompressive surgery and steroid therapy. In case of chronic compressive myelopathy due to the dural hypertrophic change, decompressive surgery such as laminectomy or laminoplasty may be helpful as well as postoperative steroid therapy.

A SOFT X-RAY STUDY ON THE BONE REMODELLING IN TOOTH MOVEMENT OF DOG (성견의 실험적 치아이동시 골재형성에 관한 연X선학적 연구)

  • The purpose of this study was to observe the pattern of bone remodellings in the periodontal ligament of dog, in which experimental tooth movement was performed A control and 5 experimental dogs, one and half year in age, were studied. Light force (50-75g) was applied by placing open-coil spring between left mandibular premolars ; heavy force (250-300g), between right mandibular premolars. Experimental dogs were sacrificed at 12 hours, 1, 3 ,7 and 14 days after force application, respectively. And soft X-ray films were obtained and read on the sectioned periodontal tissue around mandibular premolars. The results were as follows. 1. New bone formation began to be observed in tension side at 7-day and increased at 14-day , No difference was observed between light force group and heavy force group. 2. Bone resorption was observed as a shape of destruction of lamina dura at 3-day and increased gradually at 7-day, 14-day.

A RADIOGRAPHIC STUDY OF SOLITARY BONE CYSTS (고립골낭에 관한 X선학적 연구)

  • The aim of this study was to evaluate the clinical, radiographic and histopathologic features of 23 cases of solitary bone cyst by means of the analysis of radiographs and biopsy specimens in 23 persons visited the Department of Oral and Maxillofacial Radiology, School of Dentistry, Kyung Hee University and Chunbuk National University. The obtained results were as follows; 1. The incidence of solitary bone cyst was almost equal in males(52.2%) and in females(47.8%) and the prevalent age of the solitary bone cyst were the second decade(47.8%) and the third decade (21.7%). 2. In the signs and symptoms of solitary bone cyst, pain or tenderness revealed in 17.4%, swelling revealed in 13.0%, pain and swelling revealed in 21.7%, paresthesia revealed in 4.4% and 43.5% were asymptom and the tooth vitality involved in the solitary bone cyst, 76.5% were positive and 23.5% were either positive or negative. 3. In the location of the solitary bone cyst, 47.8% present posterior region, 21.7% present anterior region, 21.6% present anterior and posterior region, 4.4% present condylar process area. 4. In the hyperostotic border of the solitary bone cyst, 47.8% were seen entirely, 21.8% were seen partialy, and 30.4% were not seen. 5. In the change of tooth, 59.1% were intact, 18.2% were loss of the alveolar lamina dura, 13.6% were root resorption 4.55% were tooth displacement, 4.55% were root resorption and tooth displacement. 6. In the change of cortical bone of the solitary bone cyst, 39.1% were intact and 60.9% were thinning and expansion of cortical bone. 7. In the histopathologic findings of 9 cases, 33.3% were thin connective tissue wall, 11.1% were thickened myxofibromatous wall, 55.6% were thickened myxofibromatous wall with dysplastic bone formation.

A COMPARATIVE STUDY ON THE ACCURACY OF MASTER CASTS BY IMPLANT IMPRESSION TECHNIQUES (치과 임플랜트 인상채득 방법에 따른 주모형의 정확성에 대한 비교 연구)

  • As the inaccuracy which was made in implant impression prevented passive fit, it needed to solder the sectioned framework at several locations to correct the inaccuracy. Many clinicians have suggested impression techniques which could make passive fit between implant and superstructure. The purpose of this research was to measure and compare the accuracy of three methods of taking impression with the strain amplifier. The experimental groups were classified as follows : Group I : splinted the two parts with $Futar^{(R)}D$ Occlusion. Group II : splinted the two parts with $DuraLay^{(R)}$. Group III ; didn't splint the two parts. The results were as follows ; 1. The values of strain in the vertical and horizontal surfaces were increased in the order of group I, group II and group III. 2. Group 1 showed higher accuracy of the duplicated casts in the vortical and horizontal sur faces than group II and group III (p<0.05). 3. There were no significant differences in the accuracy of the duplicated casts between group II and group III. From the above results, it is considered that the splinting method of impression copings could make an influence on the accuracy of the master casts. To improve the accuracy of the master casts, splinting the squared impression copings with the additional silicone occlusion materials is recommendable.

  • Hypophosphatemia rickets, also known as Vitamin D-resistant rickets(VDRR) and refractory rickets, is a form of rickets which is resistant to the usual doses of vitamin D. VDRR is characterized by decreased renal tubular reabsorption of inorganic phosphate and is easily diagnosed by a normal blood calcium, hypophosphatemia, and slightly elevated alkaline phosphatase. Clinical features of Hypophosphatemia rickets included lateral bowing deformities of the legs, short stature, scoliosis, and enlargement of wrist and ankles. Dental finding in patient with VDRR were spontaneous dental abscesses in caries free teeth and other dental findings included delayed eruption, delayed apical closure, thin and hypoplastic enamel, absent or poorly defined lamina dura, enlarged pulp chambers, and numerous accessory canals and pulp horns that extend up and into the dentinoenamel junction. we reported the clinical feature and treatment of VDRR child who was referred from the department of pediatrics for early loss of primary teeth and its treatment.

  • Thr rickets or osteomalacia, that was induced by nonendocrine osseous or soft tissue tumor, is extremely rare disease and fourteen patients has been reported since 1947. The real nature of this disease is unknown, but postulated that unknown phosphaturic subtance which was elaborated from the tumor affect the renal tubule and produce hypophosphatemia and failure of calcification of osseous tissue. This case presented is that of 41-year-old man who suffered from severe generalized aching pain, severe muscular dystrophy, and shortening of the stature 4 years prior hospitalization. The causal coexisting tumor is walnut sized peripheral giant cell granuloma on the upper gingiva. After surgical removal of the tumor, patient's biochemical findings of the serum and urine were returned to the normal limits 12 days later, and clinical symptoms were marked relieved at 6 weeks later. The dental radiograms which were obtained 4 months later revealed remarkable bone regeneration and newly formed alveolar lamina dura.

CSF Leakage through a Subarachnoid-pleural Fistula after Resection of a Malignant Solitary Fibrous Tumor (악성 고립성 섬유종 제거술 후 발생한 뇌척수액의 흉강내로의 유출)

  • Solitary fibrous tumor is an uncommon submesothelial mesenchymal neoplasm that primarily arises from the pleura. Most solitary fibrous tumors have a benign course, and the single most important predictor of the clinical outcome is the ability to excise the entire lesion. We experienced a case of CSF leakage through a subarachnoid-pleural fistula after resection of a malignant solitary fibrous tumor and the involved rib. We detected CSF leakage via performing CT myelography and we treated this case with hemilaminectomy and dura repair.
