건강 상태와 약물오남용 행위 사이에서 자기효능감의 매개효과 (Mediation Effect of Self-efficacy between Health Status and Drug Abuse Behavior)

  • Background: The appropriate use of drugs is very important for a healthy life, as appropriate medication must be taken correctly for successful treatment of a disease. The purpose of this study was to investigate the self-efficacy between health status and drug abuse behavior. Methods: The subjects of this study were selected from 15 workshops located in Seoul, Incheon, Daejeon, and Chungnam, and, convenience sampling was conducted for each employee between September, 2017 and October 2017. A total of 204 questionnaires were analyzed. The questions evaluated health status, self-efficacy, drug abuse behavior, and general characteristics. Results: First, there was no difference in health status according to the general characteristics of the subjects. The self-efficacy varied according to gender, educational background, and monthly income, but not by marital status and residence area. In addition, drug abuse behaviors differed according to gender, residence area, and monthly income, but not by marital status and education level. Second, drug abuse behavior negatively correlated with health status and self-efficacy. Third, regression analysis showed that the self-efficacy between the health status and drug abuse behavior had a partially mediating effect. Conclusion: In patients with poor health status, it is necessary to increase their self-efficacy to reduce reduce drug abuse. Therefore, regular education programs targeting efficacy and drug knowledge are required in the community.

간호 대학생의 약물계산역량에 영향을 미치는 요인 (Predictors of Drug Calculation Competence of Nursing Students)

  • Purpose: The objective of this study was to identify predictors of drug calculation competence of nursing students. Methods: A total of 120 students were recruited from 3 universities from November 10 to 20, 2011. The instruments for this study were drug calculation competence, self-efficacy for drug dosage calculation, anxiety for drug dosage calculation, and the academic self-efficacy scale. The data were analyzed by descriptive analysis, chi-square test, t-test, Scheffe test, partial correlation coefficients, and stepwise multiple regression using the SPSS 18.0 program. Results: The mean score of good competence group was $0.67{\pm}0.08$ and the mean score of no-good competence group was $0.42{\pm}0.10$. The drug calculation competence was positively related to self-efficacy for drug dosage calculation and academic self-efficacy scale, but negatively related to anxiety for drug dosage calculation after controlling personal attributes. The main predictors of drug calculation competence in nursing students were identified as anxiety for drug dosage calculation (${\beta}$=-.25, p=.046), academic self-efficacy (${\beta}$=.19, p=.035). These two factors explained about 10% of variance in drug calculation competence. Conclusion: Based on the results, the strategies reducing the anxiety for drug dosage calculation and improving the academic self-efficacy should be developed and implemented.

Can pursuit eye movements reflect the efficacy of antiepileptic drugs?

  • Background: We evaluated whether eye movements could reflect the efficacy of antiepileptic drugs in patients with epilepsy. Methods: Thirty patients with epilepsy of unknown cause as well as age- and sex-matched normal controls were enrolled in this study. We divided the patients into drug-controlled epilepsy (n = 22) and drug-resistant epilepsy (n = 8) groups according to their seizure controls. We analyzed the differences in the parameters of the eye movements in these two groups compared with normal controls using video-based electro-oculography. In addition, we investigated the differences in the cerebellar volumes of these two groups using whole-brain T1-weighted images. Results: The latency and accuracy of saccade in patients with epilepsy were significantly different from normal controls, but they were not different between patients with drug-controlled epilepsy and drug-resistant epilepsy. However, the gain of pursuit was significantly decreased in patients with drug-resistant epilepsy compared with normal controls (p = 0.0010), whereas it was not different between patients with drug-controlled epilepsy and normal controls (p = 0.9646). In addition, the patients with drug-resistant epilepsy had lower cerebellar volumes than normal controls (p = 0.0052), whereas the cerebellar volumes in patients with drug-controlled epilepsy were not different from normal controls (p = 0.5050). Conclusions: We demonstrated that pursuit eye movements could reflect the efficacy of antiepileptic drugs in patients with epilepsy, a finding that may be related to cerebellar dysfunction.

이미지 활용 약물교육이 노인환자의 약물지식, 복약자기효능감 및 복약오류에 미치는 효과 (Effects of Image-Use Medication Education on Knowledge, Self-Efficacy, and Misuse of Medication in Elderly Inpatients)

  • Purpose: This quasi-experimental study was done to develop image-use medication education for older inpatients and to evaluate the effects on their knowledge, self-efficacy, and misuse of medication. Methods: Fifty nine elders (experimental group - 30, and control group - 29) received medication education once a week for 3 weeks. Data were collected before (pretest), right after (post 1), and 4 weeks after the program finished (post 2). Drug Use Knowledge Scale, Self-efficacy for Appropriate Medication Use Scale, and Drug Misuse Scale were used. Analysis included descriptive statistics, $x^2$ test, repeated measured ANOVA. Results: Findings showed significant differences in knowledge of medication and drug misuse between groups according to time(pretest, post 1, and post 2). At post 1 and post 2, knowledge and self-efficacy levels were significantly higher and misuse scores were lower in the experimental group compared to the control group. Conclusion: Findings indicate that elder-tailored medication education consisting of group education and individual guidance with images is effective and practical for medicine safety in elderly inpatients. Moreover, it could lead to a healthier life for elders, even elders with multiple chronic diseases and taking several medications.

EFFICACY EVALUATION OF THE WHITENING COSMETICS USING IN VITRO TYROSINASE INHIBITION ASSAY

  • We investigated the tyrosinase inhibitory effect using whitening materials such as arbutin, ethyl ascorbyl ether, glabridin, kojic acid, magnesium ascorbyl phosphate and ascorbic acid. Tyrosinase inhibition rate were determined varying the enzyme concentration, reaction time, reaction temperature and pH. The optimal conditions to measure the inhibitory efficacy were as follows. : enzyme concentration 1,500 or 2,000IU/mL, reaction time 15min(for the enzyme concentration 1,500 IU/mL) and l0min(for the enzyme concentration 2,000IU/mL), reation temperature 42$^{\circ}C$, pH 6.5. Under these conditions $IC_{50}$/ of arbutin, ethyl ascorbyl ether, glabridin, kojic acid, magnesium ascorbyl phosphate and ascorbic acid were calculated. In the case of magnesium ascorbyl phosphate, the inhibitory effect of tyrosinase was very low and the $IC_{50}$/ of magnesium ascorbyl phosphate could not be calculated. Other five materials showed good inhibitory effect of tyrosinase and can be used for the whitening materials.

일정한 약물 투여를 통한 HIV 감염자의 long-term Non-Progressor 전환 치료 (Treatment for HIV-Infected Patients to Induce Long-Term Non-Progressor with Constant Drug Dosage)

  • This paper presents a therapy that uses a constant drug dosage for leading HIV-infected patient to LTNP (Long-Term Non-Progressor). Based on analysis of CTLp (Cytotoxic T Lymphocyte precursor) concentration at equilibrium point and its bifurcation, we found the therapy with a drug whose efficacy is less than a certain level brings higher CTLp concentration at the equilibrium point. We observed a treatment with constant drug dosage whose efficacy is less than full treatment may lead HIV-infected patient to LTNP. It turns out that the treatment whose efficacy is less than full treatment is better in the point of performance on controllability.

지프라시돈의 효능 및 안전성 (Efficacy and Safety of Ziprasidone)

  • Ziprasidone is equally effective as haloperidol in treating schizophrenia with fewer side effects and drug interactions. Ziprasidone is an atypical antipsychotic agent and works by blocking serotonin and dopamine receptors in the central nervous system, specifically 5-HT2A and D2 receptors. Low anticholinergic side-effects and low EPS would recommend the drug for use in the elderly. Ziprasidone inhibits reuptake of norepinephrine and serotonin at neurojunction sites in vitro, indicating a potential efficacy for depression and negative symptoms which often follow after exacerbation of schizophrenia. Patients with recent acute myocardial infarction and uncompensated heart failure are contraindicated to the drug due to a possibility of QT prolongation. Although ziprasidone is metabolized by cytochrome P450 3A4, there is no significant drug interaction with the drugs that induce or inhibit the isoenzyme. Ziprasidone is safe with coadministration of lithium and there has been no significant drug interaction reported with oral birth control pills.
