Infrequent Hemorrhagic Complications Following Surgical Drainage of Chronic Subdural Hematomas

  • Chronic subdural hematomas mainly occur amongst elderly people and usually develop after minor head injuries. In younger patients, subdural collections may be related to hypertension, coagulopathies, vascular abnormalities, and substance abuse. Different techniques can be used for the surgical treatment of symptomatic chronic subdural hematomas : single or double burr-hole evacuation, with or without subdural drainage, twist-drill craniostomies and classical craniotomies. Failure of the brain to re-expand, pneumocephalus, incomplete evacuation, and recurrence of the fluid collection are common complications following these procedures. Acute subdural hematomas may also occur. Rarely reported hemorrhagic complications include subarachnoid, intracerebral, intraventricular, and remote cerebellar hemorrhages. The causes of such uncommon complications are difficult to explain and remain poorly understood. Overdrainage and intracranial hypotension, rapid brain decompression and shift of the intracranial contents, cerebrospinal fluid loss, vascular dysregulation and impairment of venous outflow are the main mechanisms discussed in the literature. In this article we report three cases of different post-operative intracranial bleeding and review the related literature.

A Cyber-Training & Education Model for Tug-barge Operators

  • The purpose of this study was to create a cyber-training & education program in response to the needs of skippers and crews operating tug-barges within Korean coastal waters and the rapid changers in this industry. Skippers and crews are inclined to operate tug-barges on the basis of experience rather than information. It is not easy to provide useful information whenever they want or to drill them in safety management skills, because of their passive attitude toward education and the few opportunities that exist. In order to increase educational opportunities, efficiency and motivation, the authors have developed this program which consists of a 'tug bridge resource management module, risk perception training module, accident case module, operating module and navigation module', and are hoping that this program will enhance and strengthen all tug-barge operations. We are also putting all our energies into designing up to date animation programs and developing new scenarios concerning the method of evaluation and certification distribution.

  • Conservation laboratory was attached to Keimyung University Museum inMarch 1980 and ever since it has been devoted mainly to the processing andconservation of metal objects. A number of objects have been processed inthis laboratory during the period, including those already in the collection ofthe Museum, those which were discovered during the three major excavationsof Kaya tombs conducted by the Museum, and those processed on commissionfrom other museums in the country,The activities of this laboratory include: (1) conserving the objects againstfurther erosion; (2) raising the archaeological value of the objects by revealingthe structure of such parts of the objects as concealed under rust; and (3)recovering the original shape of damaged objects.The methods adopted by the laboratory include: (1) removing from theobjects the ionized chlorine which usually are the major cause of erosion; (2)strengthening the objects by soaking them in acrylic resins; and (3) applyingresins to the surface of the objects to protect them from further erosion.Chemicals much employed by the laboratory includes the acrylic resin(Ruschot; developed jointly by the Cultural Property Research Institute ofKorea and Samwha Paint Company), the sodium sesquicarbonate, the sodiumhydroxide, the lithium hydroxide, and the benzotriazole.Major apparatus in the laboratory includes the vacuum immersion tank, theairbrasive, the ultrasonic cleaner, the pH-ion meter, the water bath, the zoomstereo microscope, the drying oven, and the drill.

Three Dimensional Dynamic Analysis of Newly Developed Drill and Blast Method with Bi-directions using TBM Pilot Tunnels (TBM 터널을 이용한 이방향 천공 발파공법의 3차원 동적해석)

  • 이방향 천공 발파공법의 실용성을 입증하여 실시공에 착수하기 위해서는 수치해석을 통하여 사전에 종방향 및 방사방향 천공발파에 따른 지표면에서의 지반진동의 영향을 알아보는 것이 필요하였다. 정확한 현장조건과 발파하중이 주어진 상태에서의 정밀해를 구하기 위해서는 시공상의 발파패턴대로 전체 발파공을 모형화하는 것이 필요하나 앞서 언급한 바와 같이 해석목적상 기존공법과 신공법의 지반응답의 차이를 비교하는 것이 주목적이였기 때문에 해석조건을 동일하게 가정하고 발파공도 대표적인 방향성을 가지는 1개공만을 선택하여 해석을 수행하였다. 기존 종방향 천공 발파공법과 신공법인 이방향 천공 발파공법의 상대적 비교를 위한 3차원 동적 해석결과, 관심의 대상이 되는 터널 직상부의 경우 기존의 종방향 천공 발파에 비해 이방향 천공발파는 진동이 두드러지게 감소함을 알 수 있었으며 이는 발파공의 방향성에 따라 지반응답이 크게 영향을 받는다는 것을 의미한다. 따라서 터널굴착시 문제시 되는 지상구조물의 위치에 따라 발파공의 위치를 적절히 조절함으로써 문제발생 예상지역의 진동치를 감소시킬 수 있을 것이라 판단된다. 그러나 이러한 잇점은 확대발파전 파이롯트 터널이라는 자유공간이 존재할 때만 가능하므로 이방향 천공 발파공법은 TBM의 효용성을 극대화시킨 것이라 할 수 있다.

  • The behavior of rock mass and solute(e.g. groundwater, radioactivity) flow in fractured rock can be directly influenced by joint roughness. The characteristics of joint roughness is also a main factor for the rock classification(e.g. RMR, Q system) which is usually used in tunnel design. Nevertheless, most of JRC estimation has been carried out only by the examination with the naked eye. This JRC estimation has a lack of objectivity because each investigator judges JRC by his subjective opinion. Therefore, it will be desirable that the assessment of JRC is performed by a numerical analysis which can give a quantitative value corresponding to the characteristics of a roughness curve. Meanwhile, roughness curves for joint surfaces which are observed in drill cores have been obtained only along linear profiles. Although roughness curves are measured in the same joint surface, they can frequently show diverse aspects in a standpoint of roughness characteristics. If roughness curves can be measured along the elliptical circumferences of joint surfaces from core scanning images or Televiewer images, they will certainly be more comprehensive than those measured along linear profiles for roughness characteristics of joint surfaces. This study is focus on dealing with (1) extracting automatically roughness curves from core scan image or Televiewer image, (2) improving the accuracy of quantitative assessment of JRC using fractal dimension concept.

A study on Actual Quantity of Shotcrete Sprayed in a NATM tunnel (NATM 산악터널의 숏크리트 투입율에 관한 연구)

  • This study has analysed actual overbreak, shotcrete rebound and the ratio between the actual quantity of shotcrete to designed shotcrete measured during a NATM tunnel construction. The measured shotcrete rebound was about 7.2% in average which was about half the allowable rebound (15%), showing shotcrete spraying was performed well. Based on the measurement of excavated tunnel shape, average overbreak was about 28.5cm after tunnel excavation by drill and blasting method. This was about 260% of allowable overbreak. In addition, due to the rebound and overbreak actual amount of shotcrete used in the tunnelling work was about 116.5 % of the designed value. According to the field measurement the ratio of actual shotcrete to designed value showed some relation with standard support pattern, but the size of overbreak did not show the correlation with standard support pattern. Hence current design specifications stating the size of overbreak based entirely on standard support pattern should perhaps be reestablished. The insight into the design guideline regarding overbreak and shotcrete.

Effects of Direct Practice of Newborn Health Assessment on Students' Nursing Clinical Competence and Self-Efficacy (신생아 건강사정 직접실습이 간호학생의 간호수행능력과 자기효능감에 미치는 효과)

  • Purpose: This study was done to examine the effect of direct practice of newborn health assessment on nursing student's clinical competence and self-efficacy and to propose effective strategies for clinical education on newborn care. Methods: Design was a nonequivalent control group quasi-experimental study. The direct practice program was composed of a lecture, demonstration, drill and feedback using a manikin, and repeated direct practice regarding newborn health assessment. Participants were 65 student nurses taking the pediatric nursing practicum in the nursery room at M hospital. The experimental group (n=33) participated in the direct practice program for newborn health assessment and the control group (n=32) received the traditional practice method. Nursing clinical competence was assessed by two nurse investigators and structured questionnaires were used to measure self-efficacy. Results: The experimental group's clinical competence was significantly higher than that of the control group (t=-4.82, p=.000). However no significant difference was found between the two groups for self-efficacy (t=1.264, p=.211). Conclusion: These findings indicate that the direct practice program is effective in improving nursing student's clinical competence, but it was not effective in increasing self-efficacy. Direct practice in various clinical education settings is recommended and longitudinal effects be evaluated.

A Study on the Development of Rotary Ultrasonic Machining Spindle (회전 초음파가공 주축 개발에 관한 연구)

  • Ultrasonic machining (USM) has been considered a new, cutting-edge technology that presents no heating or electrochemical effects, with low surface damage and small residual stresses on brittle workpieces. However, nowadays, many researchers are paying careful attention to the disadvantages of USM, such as low productivity and tool wear. On the other hand, in this study, a high-performance rotary ultrasonic drilling (RUD) spindle is designed and assembled. In this system, the core technology is the design of an ultrasonic vibration horn for the spindle using finite element analysis (FEA). The maximum spindle speed of RUM is 9,600 rpm, and the highest harmonic displacement is $5.4{\mu}m$ noted at the frequency of 40 kHz. Through various drilling experiments on glass workpieces using a CVD diamond-coated drill, the cutting force and cracking of the hole entrance and exit side in the glass have been greatly reduced by this system.

  • When drilling through Inconel 601 nickel-chromium-based alloys, a large amount of cutting oil is required to prevent tools from wear and fracturing due to heat buildup resulting from the high temperature resistance and toughness of this alloy. However, cutting oil supply has become a factor compromising the machining environment, and this has caused attention to shift to a more environmentally friendly cutting fluid supply system called minimum quantity lubrication (MQL). Our aim in this study was to find a more efficient drill processing method using MQL, and to verify its performance. To that end, we proposed a sealed cover, a step feed, and POSS-type cutting oil as measures to increase the effectiveness of MQL in view of the cutting force and tool wear, and established an improvement in efficiency using the proposed measures.

  • To obtain high power, diesel engines continuously increase combustion pressure and mean effective pressure each cylinder, and the excitation sources and noisy sources are increased, too. Moreover, to reduce the costs, shipyards make hull structures weaker than before. As above reasons, it is more difficult to control the vibration phenomenon nowadays. In this study, it was investigated why diesel generator sets reached the vibration allowable limits during the FAT and heavy vibration phenomenon of diesel generator sets using ODS test during onboard tests. Also, it is found out the stiffness of deck and common bed using the test result of their structural impedance. To find out the vibratory characteristics of diesel generator sets, MODAL tests were carried out. From the sensitivity analysis after above tests, it was selected points to be reinforced and studied troubleshooting to solve heavy vibration phenomenon of diesel generator sets.
