Dosimetric comparison of IMRT versus 3DCRT for post-mastectomy chest wall irradiation

  • Purpose: To compare the dose distribution of three-dimensional conformal radiation therapy (3DCRT) with intensity-modulated radiation therapy (IMRT) for post-mastectomy radiotherapy (PMRT) to left chest wall. Materials and Methods: One hundred and seven patients were randomised for PMRT in 3DCRT group (n = 64) and IMRT group (n = 43). All patients received 50 Gy in 25 fractions. Planning target volume (PTV) parameters-$D_{near-max}$ ($D_2$), $D_{near-min}$ ($D_{98}$), $D_{mean}$, $V_{95}$, and $V_{107}$-homogeneity index (HI), and conformity index (CI) were compared. The mean doses of lung and heart, percentage volume of ipsilateral lung receiving 5 Gy ($V_5$), 20 Gy ($V_{20}$), and 55 Gy ($V_{55}$) and that of heart receiving 5 Gy ($V_5$), 25 Gy ($V_{25}$), and 45 Gy ($V_{45}$) were extracted from dose-volume histograms and compared. Results: PTV parameters were comparable between the two groups. CI was significantly improved with IMRT (1.127 vs. 1.254, p < 0.001) but HI was similar (0.094 vs. 0.096, p = 0.83) compared to 3DCRT. IMRT in comparison to 3DCRT significantly reduced the high-dose volumes of lung ($V_{20}$, 22.09% vs. 30.16%; $V_{55}$, 5.16% vs. 10.27%; p < 0.001) and heart ($V_{25}$, 4.59% vs. 9.19%; $V_{45}$, 1.85% vs. 7.09%; p < 0.001); mean dose of lung and heart (11.39 vs. 14.22 Gy and 4.57 vs. 8.96 Gy, respectively; p < 0.001) but not the low-dose volume ($V_5$ lung, 61.48% vs. 51.05%; $V_5$ heart, 31.02% vs. 23.27%; p < 0.001). Conclusions: For left sided breast cancer, IMRT significantly improves the conformity of plan and reduce the mean dose and high-dose volumes of ipsilateral lung and heart compared to 3DCRT, but 3DCRT is superior in terms of low-dose volume.

동일 기종 선형가속기간 8 MV 광자선에 대한 빔 매칭 정확도 평가 (Evaluation of Beam-Matching Accuracy for 8 MV Photon Beam between the Same Model Linear Accelerator)

  • This study aimed to assess of beam-matching accuracy for an 8 MV beam between the same model linear accelerators(Linac) commissioned over two years. Two models were got the customer acceptance procedure(CAP) criteria. For commissioning data for beam-matched linacs, the percentage depth doses(PDDs), beam profiles, output factors, multi-leaf collimator(MLC) leaf transmission factors, and the dosimetric leaf gap(DLG) were compared. In addition, the accuracy of beam matching was verified at phantom and patient levels. At phantom level, the point doses specified in TG-53 and TG-119 were compared to evaluate the accuracy of beam modelling. At patient level, the dose volume histogram(DVH) parameters and the delivery accuracy are evaluated on volumetric modulated arc therapy(VMAT) plan for 40 patients that included 20 lung and 20 brain cases. Ionization depth curve and dose profiles obtained in CAP showed a good level for beam matching between both Linacs. The variations in commissioning beam data, such as PDDs, beam profiles, output factors, TF, and DLG were all less than 1%. For the treatment plans of brain tumor and lung cancer, the average and maximum differences in evaluated DVH parameters for the planning target volume(PTV) and the organs at risk(OARs) were within 0.30% and 1.30%. Furthermore, all gamma passing rates for both beam-matched Linacs were higher than 98% for the 2%/2 mm criteria and 99% for the 2%/3 mm criteria. The overall variations in the beam data, as well as tests at phantom and patient levels remains all within the tolerance (1% difference) of clinical acceptability between beam-matched Linacs. Thus, we found an excellent dosimetric agreement to 8 MV beam characteristics for the same model Linacs.

유방보존술 후 내유림프절 방사선 조사: 방사선 폐렴과 체적-선량 히스토그램 변수들 (Internal Mammary Lymph Node Irradiation after Breast Conservation Surgery: Radiation Pneumonitis versus Dose-Volume Histogram Parameters)

  • 목적: 방사선 폐렴과 체적-선량 히스토그램(dose-volume hlstogram, DVH) 변수들 사이의 연관성을 평가하고, 내유림프절이 포함된 유방암의 방사선치료에서 방사선 폐렴을 방지할 수 있는 실제적인 지침을 제공하고자 한다. 대상 및 방법: 부분유방절제술을 받은 초기 유방암 환자 20명이 본 연구에 포함되었다. 전체 유방, 상부쇄골림프절, 내유림프절에 총 28회 50.4 Gy가 조사되었다. 방사선 폐렴은 방사선 영상에서의 폐 변화(radiological pulmonary change; RPC)와 증상이 있는 방사선 폐렴(symptomatic radiation pneumonitis)에 의해 평가되었다 DVH 변수들은 grade<2 RPC와 grade${\geq}$2 RPC로 나누어 비교되었다. 이 때, DVH 변수들은 평균 폐 선량(mean lung dose), V10 (10 Gy 이상 받는 폐의 백분율 부피), V20, V30, V40, 그리고 정상 조직 합병증 확률(normal tissue complication probability, NTCP)이다. 결 과: 20명의 환자 중 9명(45%)에서 grade 2 RPC가 발생하였고, 11명(55%)에서는 발생하지 않았다 1명의 환자에서 grade 1의 증상이 있는 방사선 폐렴이 발생하였다. 단변량 분석에서 DVH 변수 중, NTCP가 두 RPC grade군 간에 유의한 차이를 보여주고 있다 (p<0.05) Fisher의 정확한 검증(exact test)은 NTCP값 45%가 RPC의 threshold level로서 적합함을 보여준다. 결 론: 본 연구는 NTCP가 유방암의 내유림프절 방사선치료 후 RPC 예측인자 중 한가지로 쓰일 수 있음을 보여준다. 임상적으로 이는 NTCP 45% 이상에서 RPC가 발생하기 용이함을 의미한다.

선형 가속기를 이용한 정위적 방사선 수술시 병소내 선량분포의 특성조사 (Search of Characteristic for Dose Distribution Presented by Multi­isocentric Stereotactic Radiosurgical Plan Using Linear Accelerator)

  • 방사선 치료 계획의 목적은 정상 조직 부근에서는 최소한의 방사선 조사가 되는 동안 병소에는 동일한 선량이 조사되는 것이다. 선형가속기를 이용한 정위적 방사선 수술시 단일한 구형의 선량분포는 병소에 대하여 균등한 선량분포를 이루고, 병소 내에는 70% 이상의 고선량이 등선량 곡선내에 포함되면서 주위 정상조직에서는 급격히 낮은 선량을 가지게 한다. 또한 이와 같은 방법은 감마나이프를 이용한 정위적 방사선 수술의 경우와 비슷한 치료 계획을 나타낸다. 이처럼 정위적 방사선 수술시 이용되는 구형의 선량분포를 가지는 isocenter는 실제 방사선 수술 계획시 많은 시간과 경험을 바탕으로 수술 계획자에 의해 병소 내에 배치되어 진다. 본 연구는 효율적인 방사선 수술이 수행되도록 수술 계획시 구형 선량분포에 관여하는 빔관련 변수들을 고려하여 병소내 선량분포의 특성을 조사하였다. 이를 위해 불규칙한 형태의 병소를 직육면체형과 원통형으로 가정하여 비교하였고, 동일한 체적의 병소 모델에 대하여 빔관련 변수를 변화시켜 구형 선량분포를 이루는 isocenter들의 위치 및 콜리메이터의 크기를 달리하면서 병소 모델에 대한 선량 분포를 얻었다. 이때, 얻어진 선량분포 Dose Profile과 Dose Volume Histogram (DVH)으로 비교한 결과, 불규칙한 모양의 병소에 대하여 콜리메이터의 크기와 Isocenter의 개수, Isocenter의 간격 등의 빔관련 변수를 최적화함으로서 더 나은 고선량의 등선량 곡선(Isodose Curve)내에 병소를 포함시킬 수 있었다. 이러한 병소내 구형 선량 분포를 가지는 isocenter의 배치에 따른 특성들은 정위적 방사선 수술 계획시 더 효율적이면서, 빠른 수술 계획을 수립하는데 많은 도움이 될 것으로 사료된다.

Comparative Evaluation of Two-dimensional Radiography and Three Dimensional Computed Tomography Based Dose-volume Parameters for High-dose-rate Intracavitary Brachytherapy of Cervical Cancer: A Prospective Study

  • Background: Dosimetric comparison of two dimensional (2D) radiography and three-dimensional computed tomography (3D-CT) based dose distributions with high-dose-rate (HDR) intracavitry radiotherapy (ICRT) for carcinoma cervix, in terms of target coverage and doses to bladder and rectum. Materials and Methods: Sixty four sessions of HDR ICRT were performed in 22 patients. External beam radiotherapy to pelvis at a dose of 50 Gray in 27 fractions followed by HDR ICRT, 21 Grays to point A in 3 sessions, one week apart was planned. All patients underwent 2D-orthogonal and 3D-CT simulation for each session. Treatment plans were generated using 2D-orthogonal images and dose prescription was made at point A. 3D plans were generated using 3D-CT images after delineating target volume and organs at risk. Comparative evaluation of 2D and 3D treatment planning was made for each session in terms of target coverage (dose received by 90%, 95% and 100% of the target volume: D90, D95 and D100 respectively) and doses to bladder and rectum: ICRU-38 bladder and rectum point dose in 2D planning and dose to 0.1cc, 1cc, 2cc, 5cc, and 10cc of bladder and rectum in 3D planning. Results: Mean doses received by 100% and 90% of the target volume were $4.24{\pm}0.63$ and $4.9{\pm}0.56$ Gy respectively. Doses received by 0.1cc, 1cc and 2cc volume of bladder were $2.88{\pm}0.72$, $2.5{\pm}0.65$ and $2.2{\pm}0.57$ times more than the ICRU bladder reference point. Similarly, doses received by 0.1cc, 1cc and 2cc of rectum were $1.80{\pm}0.5$, $1.48{\pm}0.41$ and $1.35{\pm}0.37$ times higher than ICRU rectal reference point. Conclusions: Dosimetric comparative evaluation of 2D and 3D CT based treatment planning for the same brachytherapy session demonstrates underestimation of OAR doses and overestimation of target coverage in 2D treatment planning.

Clinical Impact of Patient's Head Position in Supraclavicular Irradiation of the Whole Breast Radiotherapy

  • Patients with breast cancer can be positioned with their head turned to the contra lateral side or with their head straight during the radiation therapy treatment set-up. In our hospital, patients with locally advanced breast cancer who were receiving radiation therapy have experienced swallowing difficulty after 2 weeks of irradiation. In this pilot study, the impact of head position on reducing dysphagia occurrence was dosimetrically evaluated. Patients were divided into two groups viz., HT (head turned to the contra lateral side of the breast) and HS (head straight) with 10 members in each. Treatment planning was performed, and the dosimetric parameters such as Dmin, Dmax, Dmean, V5, V10, V20, V30, V40, and V50 of both groups were extracted from the dose volume histogram (DVH) of esophagus. The target coverage in the supraclavicular fossa (SCF) region was analyzed using D95 and D98; moreover, the dose heterogeneity was assessed with D2 from the DVHs. The average values of the dose volume parameters were 27.6%, 58.6%, 35.4%, 19%, 13.8%, 14.1%, 11.8%, 8.4%, and 8.1% higher in the HT group compared with those in the HS group. Furthermore, for the SCF, the mean values of D98, D95, and D2 were 42.4, 47.5, and 54 Gy, respectively, in the HS group and 38.9, 45.35, and 55.5 Gy, respectively, in the HT group. This pilot study attempts to give a solution for the poor quality of life of patients after breast radiotherapy due to dysphagia. The findings confirm that the head position could play a significant role in alleviating esophageal toxicity without compromising tumor control.

전립선암의 세기조절 방사선 치료시 밀도보정의 효과 (The Impact of Tissue Inhomogeneity Corrections in the Treatment of Prostate Cancer with Intensity-Modulated Radiation Therapy)

  • 목적: 전립선암의 세기조절 방사선 치료 시, 조직의 밀도보정 여부가 선량분포에 끼치는 영향을 연구한다. 재료 및 방법: 5명의 전립선 암 환자에 대하여 6 MV와 10 MV의 광자선에 대하여 각각 치료계획을 수립하였다. 각각 의 계획에서 7개의 조사선이 설정되었고, 선량계산 시에는 체조직의 밀도의 불균일성을 무시하였다. 선량 처방점인 회전중심점에서의 흡수선량과 계획표적용적(PTV)의 최대선량, 최소선량, 평균선량과 처방점선량의 95% 이상의 받는 부피(V>$_{p95%}$) 등을 측정하였다. 직장과 방광 내에서의 최대선량, 최소선량, 최방선량의 50%, 75%, 90% 이상을 받는 부피를 측정하였다. 동일한 조건에서 조직의 밀도 불균일성을 포함하여 선량분포를 재계산하고, 측정한 모든 물리량을 재 측정하였다. 결과: 밀도보정을 함으로써, 처방점에서의 흡수 선량은 6 MV에서 평균 4.9% 10 MV에서는 평균 4% 감소하였다. V>$_{p95%}$는 6 MV와 10 MV에서 각각 0.8%와 0.9% 감소하였다. PTV의 평균 흡수 선량은 6 MV와 10 MV에서 각각 4.2% 와 3.4% 감소하였다. 직장과 방광에서의 흡수선량은 약 l~2%의 차이를 보였다 결론: 전립선암의 세기변조 방사선치료시에 밀도보정을 무시하는 것은 표적에는 고려할 만한 선량의 차이를 유발하며, 주위의 위험장기에 미치는 영향은 미미하다.

Benefit of volumetric-modulated arc therapy over three-dimensional conformal radiotherapy for stage I-II extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue in the stomach: a dosimetric comparison

  • Purpose: To retrospectively analyze dosimetric parameters of volumetric-modulated arc therapy (VMAT) and three-dimensional conformal radiotherapy (3D-CRT) delivered to extranodal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue in the stomach (gastric MALT lymphoma) to find out advantages of VMAT and conditions for definite benefits of VMAT. Materials and Methods: Fifty patients with stage I-II gastric MALT lymphoma received VMAT (n = 14) or 3D-CRT (n = 36) between December 2005 and April 2018. Twenty-seven patients were categorized according to whether the planning target volume (PTV) overlaps kidney(s). Dosimetric parameters were analyzed by dose-volume histogram. Results: Radiation dose to the liver was definitely lower with VMAT in terms of mean dose (p = 0.026) and V15 (p = 0.008). The V15 of the left kidney was lower with VMAT (p = 0.065). For those with PTV overlapping kidney(s), the left kidney V15 was significantly lower with VMAT. Furthermore, the closer the distance between the PTV and kidneys, the less the left kidney V15 with VMAT (p = 0.037). Delineation of kidney(s) by integrating all respiratory phases had no additional benefit. Conclusions: VMAT significantly increased monitor units, reduced treatment time and radiation dose to the liver and kidneys. The benefit of VMAT was definite in reducing the left kidney V15, especially in geometrically challenging conditions of overlap or close separation between PTV and kidney(s).

Predictors of radiation pneumonitis and pulmonary function changes after concurrent chemoradiotherapy of non-small cell lung cancer

  • Purpose: To evaluate the predictive factors of radiation pneumonitis (RP) and associated changes in pulmonary function after definitive concurrent chemoradiotherapy (CCRT) in patients with non-small cell lung cancer (NSCLC). Materials and Methods: Medical records of 60 patients with NSCLC who received definitive CCRT were retrospectively reviewed. Dose volumetric (DV) parameters, clinical factors, and pulmonary function test (PFT) data were analyzed. RP was graded according to the CTCAE ver. 4.0. Percentage of lung volume that received a dose of threshold (Vdose) and mean lung dose (MLD) were analyzed for potential DV predictors. PFT changes were calculated as the difference between pre-RT and post-RT values at 3, 6, and 12 months after RT. Results: Twenty-two patients (37%) developed grade ${\geq}2$ RP. Among clinical factors, tumor location in lower lobe was associated with RP. Among the DV parameters, only MLD >15 Gy was associated with grade ${\geq}2$ RP. There were statistically significant decreases in PFT at all points compared with pre-RT values in grade ${\geq}2$ RP group. MLD was associated with forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1) changes at 6 and 12 months. V10 was associated with FVC changes at 12 months. V20 and V30 were associated with FEV1 changes at 6 months and FVC changes at 12 months. Conclusion: After definitive CCRT in patients with NSCLC, MLD >15 Gy and lower lobe tumor location were predictors of grade ${\geq}2$ RP. Pulmonary functions were decreased after CCRT and the magnitude of changes was associated with DV parameters.

The effect of geometrical parameters on the radon emanation coefficient and different radon parameters

  • Radon is a radioactive gas produced from the uranium-238 series. Radon gas affects public health and is the second cause of lung cancer. The study samples were collected from one area of the city of Jazan, southwest of the Kingdom of Saudi Arabia. The influence of engineering and physical parameters on the emanation coefficient of gas and other gas parameters was studied. Parameters for radon were measured using a CR-39 Solid-State Nuclear Track Detector (SSNTD) through a sealed emission container. The results showed that the emanation coefficient was affected directly by the change in the grain size of the soil. All parameters of measured radon gas have the same behavior as the emanation coefficient. The relationship between particle size and emanation coefficient showed a good correlation. The values of the emanation coefficient were inversely affected by the mass of the sample, and the rest of the parameters showed an inverse behavior. The results showed that increasing the volume of the container increases the accumulation of radon sons on the wall of the container, which increases the emission factor. The rest of the parameters of radon gas showed an inverse behavior with increasing container size. The results concluded that changing the engineering and physical parameters has a significant impact on both the emanation coefficient and all radon parameters. The emanation coefficient affects the values of the radiation dose of an alpha particle.
