Group sequential testing methods for comparing cure rates (임상시험에서 치료율 비교를 위한 집단축차검정법에 관한 고찰)

  • There are many clinical trials where a large portion of the patients are cured of disease. In such a case, one might be more interested in testing the differences in cure rates rather than other types of differences in failure distribution. For ethical and economic reasons, clinical trials must be repeatedly monitored for evidence of treatment benefit or harm. In this article, we examined by simulation the properties of nonparametric group sequential methods for comparing the cure rates between two treatment groups during the trial in a wide range of alternatives, censoring rates and cure rates.

Distribution of Pathogenic Vibrios in the Aquatic Environment Adjacent to Coastal Areas of South Korea and Analysis of the Environmental Factors Affecting Their Occurrence (2016년도 국내 해양환경내 병원성 비브리오균의 분포 및 해양환경인자간의 상관성 분석)

  • Objectives: The pathogenic Vibrios genus denotes halophilic bacteria that are distributed in aquatic environments, including both sea and freshwater. Vibrio cholerae, Vibrio vulnificus, and Vibrio parahaemolyticus are the most important species since they can be potent human pathogens and leading causes of septicemia, wound infections, and seafood borne gastroenteritis. The recent emergence of a potential pandemic clone, V. cholera serotype O1 and the cholera outbreak in South Korea in 2016 indicates the importance of consistent surveillance of pathogenic Vibrio genus within coastal areas. Methods: The present study was undertaken to determine where and how vibrios live in the aquatic environment adjacent to coastal areas of South Korea. For this survey, a total of 838 samples were obtained at 35 different sites in South Korean coastal areas during the period from January 2016 to December 2016. Pathogenic vibrios was determined using the real-time PCR method, and its clones were isolated using three selective plating media. We also monitored changes in seawater and atmospheric temperature, salinity, turbidity, and hydrogen ion concentration at the collection points. Results: The total isolation rates of V. vulnificus, V. cholera (non-pathogenic, non-O1, non-O139 serogroups), and V. parahaemolyticus from seawater specimens in 2016 were 14.2, 13.48, and 67.06%, respectively. Conclusions: The isolation rates of pathogenic vibrios genus showed a positive correlation with temperature of seawater and atmosphere but were negatively correlated with salinity and turbidity.

Serological survey of diseases to poultry and swine in slaughtered ostriches (도축 타조에서 닭 및 돼지 질병에 대한 혈청학적 조사)

  • As all other intensively farmed domestic species, most mortality in ostriches is closely to rearing conditions. While ostriches is also highly sensitive to stress, species-specific infectious disease play only a minor role. But investigation of ostrich's disease is not peformed almost in Korea. The study was performed to investigate the titers of antibody for Newcastle disease(ND), Infectious bronchitis(IB), Egg drop syndrome '76(EDS), Avian influenza(AI), salmonellosis, Mycoplasma gallisepticum infection(MG), Mycoplasma synoviae infection(MS), Infectious bursal disease(IBD), Brucellosis, Toxoplasmosis, Japanese encephalitis(JE), Porcine parvovirus infection, Encephalomyocarditis and Porcine reproductive respiratory syndrome (PRRS). The results obtained in the 62 ostrich sera slaughtered in Gyeongbuk province were summarized as follows: The average of antibody positive rates to ND, IB, EDS, AI(H9Nl), JE, Porcine parvovirus infection and Encephalomyocarditis by HI test were $75.8\%,\;100\%,\;0\%,\;0\%,\;51.6\%,\;50\%\;and\;56.5\%$ respectively. The antibody positive rates to salmonellosis, MG, MS by plate agglutination test were $12.9\%,\;25.8\%,\;and\;0\%$ respectively. Antibodies to disease agent such as IBD and AI by agar gel precipitation(AGP) test, Brucellosis by tube agglutination, toxoplasmosis by latex agglutination test and PRRS by IFA were all negative.

Rabies neutralizing antibody titers in Korean dogs and cats intended for overseas travel

  • Animals imported from abroad are a cause of rabies outbreaks in many countries. Therefore, rabies serology testing for dogs and cats traveling abroad is an important measure to reduce the incidence of rabies. Rabies virus antibodies were measured in sera collected from 2,367 dogs and 894 cats between 2017 and 2021. A serum sample with a value of 0.5 IU/mL or higher was considered a pass. The overall pass rates for rabies virus were 96.4% in dogs and 98.4% in cats. The mean rabies virus neutralization assay titers were higher in cats than in dogs and in female than in male animals. According to age, 6-year-old dogs and 9-year-old cats had the highest virus neutralization assay titers. Of the failure cases, 53.0% (53/100) were dogs or cats less than 1 year old. Although the average failure rates in dogs and cats were low at 3.5% and 1.6%, respectively, the factors influencing failure were age and vaccine manufacturer. Therefore, it is necessary to observe the vaccination interval and timing of blood collection after boosting.

Image Analysis of Diffuse Liver Disease using Computer-Adided Diagnosis in the Liver US Image (간 초음파영상에서 컴퓨터보조진단을 이용한 미만성 간질환의 영상분석)

  • In this paper, we studied possibility about application for CAD on diffuse liver disease through pixel texture analysis parameters(average gray level, skewness, entropy) which based statistical property brightness histogram and image analysis using brightness difference liver and kidney parenchyma. The experiment was set by ROI ($50{\times}50$ pixels) on liver ultrasound images.(non specific, fatty liver, liver cirrhosis) then, evaluated disease recognition rates using 4 types pixel texture analysis parameters and brightness gap liver and kidney parenchyma. As a results, disease recognition rates which contained average brightness, skewness, uniformity, entropy was scored 100%~96%, they were high. In brightness gap between liver and kidney parenchyma, non specific was $-1.129{\pm}12.410$ fatty liver was $33.182{\pm}11.826$, these were shown significantly difference, but liver cirrhosis was $-1.668{\pm}10.081$, that was somewhat small difference with non specific case. Consequently, pixel texture analysis parameter which scored high disease recognition rates and CAD which used brightness difference of parenchyma are very useful for detecting diffuse liver disease as well as these are possible to use clinical technique and minimize reading miss. Also, it helps to suggest correct diagnose and treatment.

Recently epidemiological survey of the viral diseases of broiler chickens in Jeonbuk province from 2005 to 2007 (최근 3년간 (2005-2007년) 전북지역 육계의 주요 바이러스성 질병 발생추이 분석)

  • Recently, the major viral diseases, Newcastle disease (ND), infectious bronchitis (IB), low pathogenic avian influenza (LPAI), avian pneumovirus infection (APV), Marek's disease (MD) and infectious bursal disease (IBD), have led to huge economic losses in chicken industry of Korea. To evaluate prevalence of the major viral disease infections in broiler breeder and broiler farms, epidemiological survey has been conducted in Jeonbuk province from 2005 to 2007 by serological ELISA test for APV, PCR for MD, and RT-PCR for ND, IB, LPAI and IBD, respectively. A total of 424 cases was submitted to our laboratory for diagnosis of the major viral disease from broiler breeder and broiler farms in the above period. The diagnosed results were analysed for the detection rate of infections on basis of years, seasons and ages, respectively. This study was showed that the detection rates of ND and APV were considerably high for every years regardless of seasons and ages in both broiler breeder and commercial broiler. In comparison with detection rates of ND and APV, IB and LPAI were lower but detected around 10% for every years. Especially, detection rate of IB was significantly high in commercial broiler than in broiler breeder. Therefore, to minimize economic losses for broiler breeder and broiler farms, it will need for effective countermeasures to decrease detection rate of the viral respiratory diseases. Although the detection rates of MD and IBD were gradually decreased from 2005 to 2007 in both broiler breeder and commercial broiler, it will continually make an effort about disease control for increasing productivity in chicken industry.

Results of Radiation Therapy for Squamous Cell Carcinoma of the Esophagus (식도 편평세포암의 방사선치료 결과)

  • Purpose: This study was designed to evaluate the effectiveness and prognostic factors for patients treated with postoperative radiation therapy following surgery or with radiation therapy alone for squamous cell carcinoma of the esophagus. Materials and Methods: We retrospectively analyzed 132 esophageal cancer patients treated with postoperative radiation therapy following surgery or patients who were treated with radiation therapy alone at our institution from 1989 to 2006. Thirty-five patients had stage II disease, 88 patients had stage III disease and nine patients had stage IV disease. Tumors were located at the upper esophagus in 18 patients, the mid esophagus in 81 patients and the distal esophagus in 33 patients. Sixty patients were treated with radiation therapy alone and 72 patients were treated with postoperative radiation therapy following surgery. Eight patients received a dose less than 40 Gy and 78 patients received a dose of 40 to 50 Gy. The remaining 46 patients received a dose of 50 to 60 Gy. The majority of patients who underwent postoperative radiation therapy received a dose of 45 Gy. Results: Actuarial survival rates for all of the patients at two years and five years were 24% and 5%, respectively. The median survival time was 11 months. Survival rates for patients who underwent postoperative RT at two years and five years were 29% and 8%, respectively. The corresponding survival rates for patients who received radiation alone were 18% and 2%, respectively. Survival rates at two years and five years were 43% and 15% for stage II disease, 22% and 2% for stage III disease and 0% and 0% for stage IV disease, respectively; these findings were statistically significant. Two-year survival rates for patients with upper, middle and distal esophageal cancer were 19, 29% and 22%, respectively. Although there was a trend of slightly better survival for middle esophageal tumors, this finding was not statistically significant. Complete response to radiation was achieved in 13 patients (22%) and partial response to radiation was achieved in 40 patients (67%) who received radiation alone. No response to radiation was noted in seven patients (12%). A statistically significant difference in survival rates was seen between patients that had a complete response and patients that had a partial response. Two-year survival rates for patients that had a complete response versus patients that had a partial response were 31% and 17%, respectively. There were no survivors for patients with no response as determined at two-year follow-up. Conclusion: We conclude that radiation therapy is an effective treatment for esophageal cancer. Stage and response to radiation therapy were noted to be prognostic factors. A more effective treatment modality is needed to improve long term survival because of the relatively dismal prognosis for this tumor.

A statistical study on infectious diseases of cultured olive flounder, Paralichthys olivaceus in Korea (양식 넙치, Paralichthys olivaceus 질병에 대한 통계적 고찰)

  • The epidemiological study was performed to survey the prevalence of 5 bacterial, 6 viral and 5 parasitic disease of cultured olive flounder, Paralichthys olivaceus in Pohang, Ulsan∙Gijang, Keoje and Wando area of Korea from February to December, 2007. The disease frequency of size groups were as follows; Among the 1,218 fish samples, less than 10 cm (67.6%), 11~20 cm (70.2%), 21~30 cm (73.4%), 31~40 cm (77.6%), above 41 cm (88.2%). The highest detection rates of pathogens were recorded in samples from August and Ulsan. The detection rates of parasites, bacteria or viruses were 43.2%, 38.8% and 24.4%, respectively. The distribution of specific diseases showed that detection rates of diseases occurring the most frequently during the study period were Trichodina (33.1%), viral nervous necrosis virus (16.0%), Vibrio (9.7%), Scutica (10.1%), Ichthyobodo (5.7%).

The serological studies on infectious bursal disease (전염성 F낭병에 대한 혈청학적 연구)

  • This study was carried out to provide the fundamental information for development of proper vaccination program against infectious bursal disease(IBD) to the local chicken farms. The antigen detection was peformed from 8 samples of bursa of Fabricius with agar gel precipitation(AGP) and indirect immunofluorescent assay(IFA), And also, the antibodies in serum samples were detected by the various serological methods such as commercial ELISA assay, AGP and virus neutralization(VN) test. 1. The antigen detection rates were 25% for AGP which is 2 out of 8 farms and 10 out of 40 bursas, and 25% which Is 2 out of 8 farms and 20% 8 out of 40 bursas for IFA, respectively. 2. The mean titer of maternal antibody (>3,000) existed until 10 days of the age with ELISA-GMT. 3. The antibody positive rates which are over 80% showed until 5 days of the age with ELISA and at 10 days of the age with AGP except one, but none of them showed from 1 day of the age. This report came to conclusions that both the protective maternal antibody titers and the antigen positive rates were significant until at the 10 days of the age.

Comparison of Crude and Age-Specific Incidence Rates of Breast, Ovary, Endometrium and Cervix Cancers in Iran, 2005

  • Background: Cancer accounts for 12.6% of total deaths in the world (just after heart disease). Materials and Methods: Frequency and age-specific incidence rates of breast and gynecologic cancers in Iran are calculated based on the dataset of the National Cancer Registry of Iran in 2005. Results: Gynecologic and breast cancer accounted for 7.6% and 25.6% of total cancer cases, respectively. Ovarian cancer was the most frequent gynecologic cancer followed by endometrium. Endometrial cancer revealed the highest age specific incidence rate followed by ovary (after 59 years). Conclusions: Regarding disease burden, breast and gynecologic cases account for 33.4% of total cancer patients. The age specific incidence rate is a useful guide in epidemiologic and future plans.
