Osteopetrosis is a rare bone disease characterized by systemic osteosclerosis due to an osteoclast dysfunction that decreases bone resorption. This report demonstrates two cases of adult osteopetrosis with secondary osteomyelitis of the maxilla, in siblings who are 43-year old female and 55-year old, male respectively. The common radiographic features of these cases were increased radiopacity in skull, rib and vertebra. The radiographic features that differed between these two cases were the osteosclerotic pattern of the jaw bones, that is, diffuse patterns in the female case, while the male case showed nodular patterns that were confined to the root apices. The diagnosis of osteopetrosis may be complicated due to the varying degree of osteosclerosis on panoramic radiograph. Additional radiographs such as the chest and skull radiograph may be helpful for the diagnosis of osteopetrosis.
It is becoming increasingly apparent that periodontitis consists of mixture of diseases, most of which respond favorably to traditional mechanical therapy. Among these variants of the disease, some appear to be associated with unusual microbial infections and defective host defenses. Many of these fail to respond to conventional treatment. The recognition that some forms of periodontitis are refractory to standard periodontal therapy has given rise to a new classification of peridontitis. A series of 1692 subgingival microbial samples sent to a diagnostic microbiology laboratory included 738 samples that could be identified as compatible with a clinical diagnosis of refractory or recurrent periodontitis. In descending order of prevalence the associated microbiota included Bacteroides forsythus(85%) ,Fusobacterium species(78%), Spirochetes(67%), Campylobacter rectus(64%), Porphyromonas gingivalis(59%), Peptostreptococcus micros(58%), motile rods(46%), Prevotella intermedia(33%), Eikenella corrodens(13%), Capnocytophaga species(12%) ,and Actinobacillus actinomycetemcomitans(6%). Antibiotic resistance to tetracycline, penicillin G, or metronidazole was particularly noticeable for Fusobacterium species, Capnocytophaga species, and Actinobacillus actinomycetemcomitans. It was largely absent for Campylobacter rectus. No antibiotic data were obtained for Porphyromonas gingivalis or Bacteroides forsythus, as these species were detected by immunofluorescence. The results indicate that a substantial number of microorganisms associated with refractory periodontitis are variably resistant to commonly-used antibiotics. Diagnostic microbiology must be considered an essential adjunct to the therapist faced with periodontal lesions refractory to conventional treatment.

Cherubism is an uncommon, benign fibrosseous lesion of the jaws that causes a progressive, painless, symmetrical expansion of the maxilla and mandible. It's autosomal dominance pattern of inheritance has been confirmed. A characteristic deformity is specific to this disease: hypertrophy of the mandible, swelling of cheeks and sometimes hypertrophy of maxilla with eyes tending to look up ; that looks like the Renaissance cherubs. It usually makes figures during childhood between 2-4 years of age and progresses until puberty, after which it spontaneously regresses 10 most cases. As a result of this case review, the management strategies for cherubismic children in standpoint of pediatic dentistry can be summarized as follows: 1. It can be detected early in childen through its characteristic clinical and radiographic features, which is confirmed by histopathological examination and familial history. 2. The supervision of arch space is required against its frequent sequelae, the early missing of primary teeth and eruption disorders of permanent teeth. 3. It is highly recommended to continue the periodic check-up with clinical and radiographic examination, leading to surgical intervention in cases of aggravation.
Sleep is associated with definite changes in respiratory function in normal human beings. During sleep, there is loss of voluntary control of breathing and a decrease in the usual ventilatory response to both low oxygen and high carbon dioxide levels. Especially, rapid eye movement (REM) sleep is a distinct neurophysiological state associated with significant changes in breathing pattern and ventilatory control as compared with both wakefulness and non-rapid eye movement (NREM) sleep. REM sleep is characterized by erratic, shallow breathing with irregularities both in amplitude and frequency owing to marked reduction in intercostal and upper airway muscle activity. These blunted ventilatory responses during sleep are clinically important. They permit marked hypoxemia that occurs during REM sleep in patients with lung or chest wall disease. In addition, sleep-disordered breathing (SDB) is more frequent and longer and hypoventilation is more pronounced during REM sleep. Although apneic episodes are most frequent and severe during REM sleep, most adults spend less than 20 to 25% of total sleep time in REM sleep. It is, therefore, possible for patients to have frequent apneas and hypopneas during REM sleep and still have a normal apnea-hypopnea index if the event-rich REM periods are diluted by event-poor periods of NREM sleep. In this review, we address respiratory physiology according to sleep stage, and the clinical implications of SDB and hypoventilation aggravated during REM sleep.
Foreign bodies in esophagus and tracheobronchus should be sometimes treated with emergency and are used to be considered as an importment disease in otolaryngologic field. Interestingly, variety of the incidence and object of foreign bodies have been showed as being changed the mode af life pattern and civilization. Our department had already reported the statistical survey at first in 1954, successively in 1992, 1963 and 1968, respectively according to the age, sex, variety af foreign bodies, location and duration of the lodgement. Now, we have experienced the new 186 cases during the consecutive 5 year period from January 1973 to December 1977 and reported as a 5th report comparing with previous reports.
Essential hypertension is a multifactorial disease, and has been shown to be associated with insulin resistance. The relationship between the genetic variation of the insulin receptor (INSR) gene and essential hypertension In Korean population was investigated by the Nsi 1 restriction fragment length polymorphism (RFLP) pattern of this gene. The observed genotype frequencies of INSR gene were not deviated from those expected for the Hardy-Weinberg equilibrium (HWE), but a significant association was observed between essential hypertension and N1 allele of Nsi 1 RFLP at the INSR gene ($X^2$-test; P<0.05). Moreover, the frequency of N1 allele was significantly different between normotensives and essential hypertensives in subgroups that were not obese ($X^2$-test; P<0.05). These data suggest that the Nsil RFLP of INSR gene may be a useful genetic marker for essential hypertension in Korean population.

Objective: Low back pain is a primary of source of dysfunction and economic costs. Gluteus medius muscle co-activation and activity pattern change caused the low back pain. Active release technique (ART) is a patented, non-invasive, soft tissue treatment process that both locates and breaks down the scar tissue and adhesions. The purpose of this study was to assess the effects on chronic low back pain using ART on gluteus medius so that suggest usable treatment method for treating chronic low back pain. Design: One group pretest-posttest design. Methods: Twelve patients with chronic low back pain were participated in this study. Subjects in ART group were received 2 times a week for 3 weeks treatments with either ART on gluteus medius muscle trigger points. Outcome measures were conducted by pain intensity with a pain visual analogue scale and pressure pain threshold on gluteus medius. Results: Completion of the intervention, the visual analogue scale was decreased in ART group (p<0.05). Also pressure pain threshold was decreased in ART group (p<0.05). Conclusions: Our results suggest that the response to ART may be usable to treat low back pain. ART was presented to reduce pain level of low back in people with chronic low back pain. Further study is required to management for low back pain due to gluteus medius and more ART study.
Epstein-Barr virus-associated gastric carcinoma (EBVaGC) is one of the four subtypes of gastric carcinoma (GC), as defined by the novel classification recently proposed by The Cancer Genome Atlas. EBVaGC has several clinicopathological features such as longer survival and higher frequency of lymphoepithelioma-like carcinoma (LELC) and carcinoma with Crohn's disease-like lymphoid reaction that distinguish it from EBV-negative GC. The intensity and pattern of host cellular immune response in GC have been found to significantly correlate with the prognosis of patients with GC, suggesting that immune reaction and tumor microenvironment have critical roles in the progression of GC, and in particular, EBVaGC. Here, we reviewed the cellular and molecular mechanisms underlying prominent immune reactions in patients with EBVaGC. In EBVaGC, deregulation of the expression of immune response-related genes promotes marked intra-or peritumoral immune cell infiltration. The expression of programmed death receptor-ligand 1 is known to be increased in EBVaGC, and therefore, it has been proposed as a favorable prognostic factor for patients with EBVaGC, albeit some data supporting this claim are controversial. Overall, the underlying mechanisms and clinical significance of the host cellular immune response in patients with EBVaGC have not been thoroughly elucidated. Therefore, further research is necessary to better understand the role of tumor microenvironment in EBVaGC.
Sometimes in clinic, we have found that cast crown construction is necessary to fit the existing partial denture clasp, when the abutment is involved with caries, a failing restoration, periodontal disease, or fracture. This is important not only to protect the abutment tooth from missing, but to restore retention, bracing and support for partial denture. For constructing the cast crown, several methods have been reported by many of previous authors. The number of techniques reported during the past are ideal but have uncertain results However, with more improved and practical technic developed by me, the problem becomes more successful. One method in which does not need the laboratory technecian is the wax pattern has been made in chairside waxing by doctor himself. The other in which requires the laboratory technician is to use special impression method and articulator for laboratory waxing. In one case of the reported here, we have successful results in restoring the abutment with cast crown for original denture clasp.
Phytophthora rot on broad bean(Vicia faba) occurred in the experimental field at Gyeongsangnam-do Agricultural Research and Extension Services from 2004 to 2006. The fungus isolated from the diseased plants grew well on potato dextrose agar and showed an arachnoid or rosaceous colony pattern. Sporangia were conspicuously papillated, noncaducous, ovoid to globose, and $25-64{\times}18-44{\mu}m$ in size. Oogonia and oospores were spherical and measured as 20-32 ${\mu}m$ and 16-28 ${\mu}m$ in size, respectively. Oospores were relatively small and aplerotic. Antheridia were amphigynous, spherical, and unicellula. Chlamydospores were globose and 18-40 ${\mu}m$ in size. Optimum temperature for growth was about $28^{\circ}C$ on potato dextrose agar. The disease occurred in all parts of the plant including roots, stems, leaves and pods in the field. The symptoms similar to those of naturally infected plants were induced by artificial inoculation and the pathogen was re-isolated from the plant. On the basis of mycological and pathological characteristics, the causal pathogen of broad bean rot was identified as Phytophthora nicotianae. This is the first report of Phytophthora rot of broad bean caused by P. nicotianae in Korea.
