Study of Mutual Understanding and Human Body (몸과 소통에 관한 연구)

  • This study searched the philosophical and medical thought of mutual understanding and human body. Mutual understanding is a fundamental problem in all branches of oriental studies. In other words, mutual understanding becomes the existential foundation of heaven-earth-human(三才). So human beings, heaven and the earth can't exist if there is no mutual understanding. It comes out the problem of self-consciousness in philosophy, and qi movement pattern like upward, downward, inward and outward movement in traditional korean medicine. Human beings have mutual understandings with heaven and the earth from a macroscopic standpoint, on the other side the human body from a microscopic standpoint. Qi movement is the mutual understanding and response of qi in human body, so with which the physiological functions and pathologic changes of viscera and bowels comes out. Therefore we want to present how to complete qi movement between viscera-bowels and organs to examine mutual understandings in human body closely. The results was summarized as follows; First, upbearing the clear yang and down bearing the turbid yin of spleen-stomach is main pivot of upward and downward of qi movement, and it is true form of mutual understandings between viscera and bowels, so upward, downward, inward and outward movement of whole viscera and bowels can be controlled by spleen-stomach. Second, by restraining relationships between downward heart fire and upward kidney water, heart fire and kidney water have close communications physiologically and pathologically as upper-lower, yin-yang and water-fire. Third, by restraining relationships between upbearing and effusion of liver and purification and down-sending of lung, liver and lung are outer circles of upward and downward movement, so they have antagonistic functions. Firth, by the relationships between upbearing the clear and down bearing the turbid of spleen-stomach and free coursing of liver, free coursing of liver-gallbladder is the essential requirement that it certify transportation and transformation of spleen-earth, so the disease of liver-gallbladder induce spleen-stomach disease pattern to affect its function of transportation and transformation. Fifth, by spleen and kidney, spleen yang is based on kidney yang, so the weakness of kidney yang can affect the function of transportation and transformation of spleen-stomach as it can't warm spleen earth. Sixth, by homogeny of liver-kidney, essence and blood of liver-kidney and ministerial fire of liver-kidney have mutal generation and limitation physiologically and they mutually are affected in pathologically.

The Use Criteria and Appropriateness of Korean Classification of Diseases(KCD) Focused on Emmeniopathy (월경병을 중심으로 본 한국표준질병사인분류(KCD)의 사용 기준과 적합성)

  • Purpose: KCDO-3(Korean Classification of Diseases(Oriental Medicine)-third edition) being used in January, 2010 accepted the KCD(Korean Classification of Diseases) and added disease pattern and syndrome of oriental medicine. But, the diagnoses of oriental medicine are too uncertain to express in A00-Z99(KCD). In this case, you should choose in U codes under the KCD use guidelines, but U codes are not capable of representing the symptoms too. So, we suggest the use criteria and consider the weakness of the U codes with medical records of patients who visited with amenorrhea or oligomenorrhea. Methods: We referred medical records of patients who visited oriental obstetrics and gynecology from January 1st to December 31st, 2010. From among them, we set up 122 patients who related with emmeniopathy as target group and searched codes distribution based on medical records. And we described that the process of choosing appropriate codes based on the medical records of 49 amenorrhea or oligomenorrhea patients. Results and Conclusions: The emmeniopathy is divided into menstrual disorder, amenorrhea and systemic disorders at the period of menstruation. And emmeniopathy is expressed in some codes such as N91, N92, N93, N94, U321, U77. When a patient visit hospital, a doctor should choose causal codes when there is confirmed diagnosis. Otherwise, a doctor chooses symptom codes. And if there are more than two diagnosis consistent with definition of chief condition, a doctor should code the first listed diagnosis as a chief condition. Because KCD-5 is classified according to western medical diagnosis, it is difficult to choose in KCD-5 when we diagnosed with disease pattern and syndrome of oriental medicine. But U codes are also deficient to express various condition of emmeniopathy. So we should add 'deficiency and detriment of the thoroughfare and conception vessels', 'prolonged menstruation' and various systemic disorders at the period of menstruation.

Locally Advanced Rectal Carcinoma : Curative Surgery Alone vs. Postoperative Radiotherapy and Chemotherapy (국소적으로 진행된 직장암에 대한 근치적 수술 단독 치료군과 수술후 보조적 방사선 및 항암화학요법 병행군의 치료결과 분석)

  • Purpose : To evaluate the effects of postoperative radiotherapy and chemotherapy on the pattern of failure and survival for locally advanced rectal carcinoma, we analyzed the two groups of patients who received curative resection only and who received postoperative radiochemotherapy retro-spectively. Materials and Methods : From June 1989 to December 1992, ninety nine patients with rectal cancer were treated by curative resection and staged as B2-3 or C. Group I(25) patients received curative resection only and group II(74) patients postoperative adjuvant therapy. Postoperative adiuvant group received radiation therapy (4500cGy/25fx to whole pelvis) with 5-FU (500mg/$m^2$, day 1-3 IV infusion) as radiosensitizer and maintenance chemotherapy with 5-FU(400mg/$m^2$ for 5 days) and leucovorin (20mg/m^2$ for 5 days) for 6 cycles. Results : The patients in group I and group II were comparable in terms of age sex, performance status, but in group II $74{\%}$ of patients showed stage C compared with $56{\%}$ of group I. All patients were followed from 6 to 60 months with a median follow up of 29 months. Three year overall survival rates and disease free survival rates were $68\%,\;64\%$ respectively in group I and $64\%,\;61\%$, respectively in group II. There was no statistical difference between the two treatment groups in overall survival rate and disease free survival rate. Local recurrences occurred in $28{\%}$ of group I, $21{\%}$ of group II (p>.05) and distant metastases occurred in $20{\%}$ of group I, $27{\%}$ of group II(p>.05). The prognostic value of several variables other than treatment modality was assessed. In multivariate analysis for prognostic factors stage and histologic grade showed statistically significant effect on local recurrences and lymphatic or vessel invasion on distant metastasis. Conclusion : This retrospective study showed no statistical difference between two groups on the pattern of failure and survival. But considering that group II had more advanced stage and poor prognostic factors than group I, postoperative adjuvant radiochemotherapy improves the results for locally advanced rectal carcinoma as compared with curative surgery alone.

Soil-blending Effect of Eggshell Powder on the Control of Club root Disease and the Growth of Chinese Cabbage in the Field (배추 무사마귀병 발병 억제 및 생육증진을 위한 달걀껍질 토양혼화처리 효과)

  • Before transplanting Chinese cabbage seedlings, two kinds of eggshell powder were blended into the soil of cabbage field where the club root pathogen, Plasmodiophora brassicae, was infested. The incidence of clubroot disease, the shoot and root growth of cabbages, and soil pH were examined four times at 10 to 13 days interval from transplanting Chinese cabbage. As results, the cabbages treated with eggshell powder without membrane showed the fastest growth in above ground part, and the lowest disease index for clubroot disease. The cabbages treated with eggshell powder with membrane showed better growth than the cabbages of non-treated check, but lower growth than those treated with eggshell powder without membrane. Soil pH started to increase from 3 weeks after soil blending of eggshell powder, and it reached to above 8.0. However, the soil pH of non-treated check stayed at around 6.8. In the experiment to compare the effect of eggshell powder with other calcium compounds, soil-blending of $CaCO_3$ resulted the lowest disease incidence of 1.7 and the registered fungicide, 'flusulfamide', and the resistant variety 'CR Green cabbage' followed with the incidence of 1.9. Cabbages of non-treated check scored the highest disease incidence, 3.4, and that of eggshell powder without membrane was as high as 2.7. However, the growth of Chinese cabbage showed the different pattern to the disease incidence. Chinese cabbages treated with eggshell without membrane recorded the highest average growth, around 2.1 kg. On the other hand, the average growth of CR Green Chinese cabbage was about 2.0 kg, that of flusulfamide-treatment plot was 1.7, and that of non-treated check was as low as 1.3 kg. Soil blending of eggshell powder without membrane did not inhibit the development of clubroot, but increased the growth of cabbage to a great extent. Therefore, it was confirmed that soil blending of eggshell powder before transplanting makes the Chinese cabbage culture possible even in the field infested with club root pathogen.

An Analysis of Clinical and Histopathological Pattern of Malignant Melanoma (최근 6년간 경험한 악성 흑생종의 임상병리조직학적 유형 분석)

  • Purpose: Malignant melanoma is a fatal tumor arising in the melanocytic systems of the skin. The incidence of malignant melanoma, formerly considered a rare tumor in Korea, is observed to increase. The aim of this study is to analyze the clinical and histopathological pattern of malignant melanoma in one institute. Methods: Thirty patients with ages ranging from 33 to 80 years, diagnosed as malignant melanoma at our skin tumor department, were enrolled in a retrospective study over a 6 year period(2000. 9-2006. 7). The analyzed data included age, sex, location, duration before diagnosis, clinical type, level of invasion, and stage. Results: The cases were identified and analyzed by clinical and histopathologic study. The male to female ratio was 1:1.7. Lower extremities(especially, feet) were favored sites. The majority of cases were acral lentiginous melanoma(40%), followed by nodular melanoma (36.7%), superficial spreading melanoma(20%) in this study. Clark level IV was predominant in histopathologic study. There was merely narrow gap among each stage by AJCC. Conclusion: The clinicopathological characteristic of melanoma in our patients is quite different with that in the West. In this retrospective study, primary lesions of the foot were predominant with melanoma, and a high percentage of these were classified pathologically as acral lentiginous melanomas. Patients had a more advanced stage of disease at first presentation and a more deeply invasive primary lesion than Western patients. These suggest that malignant melanoma has a worse prognosis in our patients than in the West. So, further organized prospective studies are needed to approach the prudent and accurate diagnosis and management of melanoma in Korea.

Family Linkage Analysis of CCM1 Locus on Chromosome 7q in Familial Cavernous Malformation (가족성 해면혈관종에서 염색체 7q CCM1 염기서열의 가족간 연관성 분석)

  • Although the pathogenesis of cerebral cavernous malformation (CCM) is unknown, a familial predisposition has been recognized, with up to $55\%$ of patients having an affected relatives. Genetic linkage studies have recently mapped a gene causing CCM to a segment of the long arm of chromosome 7 (7q). We report herein a genetic linkage analysis conducted on a Korean three generation family with CCM. It's first report in Korean family. A Korean family in which one member had undergone surgery for ubtracerebrak hematoma (ICH) and confirmed the CCM, was evaluated. They were examined clinically (n=18) and by magnetic resonance (MR) imaging (n=10). Polymorphic markers (D7S1813, D7S1789) spanning the CCM1 locus on 7q were genotyped by the polymerase chain reaction and analysis of linkage was performed in this family (n=17). Six had multiple lesions on brain MR image, one of them being symptomatic, and five were asymptomatic. Seven remaining members were asymptomatic and refused MR image study. One had died of ICH from presumed CCM. Analysis of the pedigree was consistent with an autosomal dominant pattern of inheritance. All affected patients were linked to CCM1. Linkage to CCM1 can account for inheritance of CCM in this family. They had some striking features with a low clinical penetrance and the presence of multiple lesions. These findings have implications for genetic testing of this disorder and represent an important step toward identification of the gene responsible for the pathogenesis of this disease.

A study on pattern of changes of Mibyeong recognition, prevalence and its management plan in Korean public : national survey (일반인의 미병 인식 변화 양상과 관리방안 조사)

  • Objectives : The purposes of this study were to investigate the recognition of Mibyeong, symptom, management plan and correlation between quality of life and health condition, and to suggest a plan for managing Mibyeong status. Methods : Participants were recruited based on the same sampling methods used on the previous study performed in 2013 based on area, gender and age. Questionnaires were collected by Gallup Korea professional surveyor through face-to-face interviews. The questionnaires contain questions about the recognition of Mibyeong, symptom, management plan and the quality of life in accordance with the relevant health conditions. Descriptive statistics were used for data analysis and the results were expressed as percentage ratios (%). Results : 1,100 of people were acquired in 2015. The responses for "First time to hear of Mibyeong" have shown that the percentage were reduced from 80% to 67% compared with 2013 data. The ratio of Mibyeong's symptoms in 2015 were similar to 2013. we suggested some ways to deal with the Mibyeong status including behavior adjustment (non smoking, non alcohol, control sleep pattern), herbal tea, health functional food, exercise, preventive health care (qi-gong, yoga), meditation, home health care medical device (hot-pack, seat device for fumigation, massager), and medical service (hospital, medical clinic oriental medical clinic). Almost people showed that positive opinion with them. There were significant correlation between quality of life with the health status rather than Mibyeong or disease group. Conclusions : This study was performed through scientific questionnaires collected in 2013 and 2015 to investigate people's understanding of Mibyeong as a present condition in Korean public. Some questions had significantly different responds between both years while others showed similar trends for both years. These results suggest that the concept of Mibyeong in oriental medicine could provide a management mechanisms that help people to manage the Mibyeong status.

  • The purpose of the research is to investigate the factors affecting the life quality of oral health according to the knowledge and behavior related with oral health. Total 272 people who work in Gyeong-Nam province participated in this survey. The research shows that major 4 factors are subjective whole body health, ages, dietary pattern and dry mouse. The subjective whole body health and dietary pattern have an positive effect on the life quality of oral health, whereas ages and dry mouse an negative effect on this. As a result, the health and the life quality of oral health should be improved by investigating the factors affecting oral health and thus developing the program enhancing the oral health to prevent oral disease.

Diabetes, Overweight and Risk of Postmenopausal Breast Cancer: A Case-Control Study in Uruguay

  • Obese postmenopausal women increase their risk of developing breast cancer (BC), in particular if they display an android-type pattern of adiposity, which is also associated to increased risks of diabetes mellitus, hypertension and cardiovascular disease. In order to explore the associations among anthropometry (body mass index, body composition, somatotype), some specific items of medical history (diabetes, hypertension, dislypidemias, hyperuricemia) and the risk of BC in Uruguayan women, a case-control study was carried out between 2004-2009 at our Oncology Unit. 912 women of ages between 23-69 years (367 new BC cases and 545 non hospitalized, age-matched controls with a normal mammography) were interviewed. Twenty body measurements were taken in order to calculate body composition and somatotype. Patients were queried on socio-demographics, reproductive history, family history of cancer, a brief food frequency questionnaire and on personal history of diabetes, dislypidemias, hyperuricemia, hypertension and gallbladder stones. Uni- and multivariate analyses were done, generating odds ratios (ORs) as an expression of relative risks. A personal history of diabetes was positively associated to BC risk (OR=1.64, 95% CI 1.00-2.69), being higher among postmenopausal women (OR=1.92, 95% CI 1.04-3.52). The risks of BC for diabetes in postmenopausal women with overweight combined with dislypidemia (OR=9.33, 95% CI 2.10-41.5) and high fat/muscle ratio (OR=7.81, 95% CI 2.01-30.3) were significantly high. As a conclusion, a personal history of diabetes and overweight was strongly associated to BC. The studied sample had a subset of high-risk of BC featured by postmenopausal overweight and diabetic women, who also had a personal history of hypertension and/or dyslipidemia. The present results could contribute to define new high risk groups and individuals for primary as well as for secondary prevention, since this pattern linked to the metabolic syndrome is usually not considered for BC prevention.

Pattern of Lymph Node Pathology in Western Saudi Arabia

  • Background: This study aimed to characterize the histopathological pattern of lymph node pathology among Saudi patients and to highlight the age and gender variations of these lesions as base line data. Materials and Methods: We retrospectively analyzed the data from lymph node biopsy specimens received at the Department of Pathology, King Fahad Hospital, Madinah, Saudi Arabia from January 2006 to December 2013. Results: Of the 289 lymph node biopsy specimens received, 154 (53.3%) were from males and 135 (46.7%) from females giving a male: female ratio of 1.14:1. Age of the patients ranged from 2.5 to 96 years with a mean age 33.9 years. The commonest lymph node group affected was the cervical (30.4%) followed by axillary (9.7%) and inguinal (8.7%). Malignant lymphoma [71 Hodgkin's disease (HD), 57 non Hodgkin's lymphoma (NHL)] 128 (44.3%), reactive hyperplasia 68 (23.5%), and tuberculosis 41 (14.2%) were the common causes of lymph node enlargement. While HD, reactive hyperplasia and tuberculosis were commonest in young adult patients (10-29 years old) and rare above the age of 50 years; NHL was the predominant cause of lymph node enlargement above 50 years. Conclusions: Lymph node biopsy plays an important role in establishing the cause of lymphadenopathy. Among the biopsied nodes, lymphomas were the most common (44.3%) followed by non-specific reactive hyperplasia (23.5%) and tuberculous lymphadenitis (14.2%).
