측두하악관절에서 관절융기 형태와 관절원판 변위와의 연관성 연구 (Relation between shape of the articular eminence and disc displacement in the temporomandibular joint)

  • Purpose : To investigate the hypothesis that the morphology of the articular eminence of the temporomandibular joint is a predisposing factor for disc displacement. Materials and Methods: MR images of 126 temporomandibular joints in 94 patients were analyzed to assess for morphology of the articular eminence and disc displacement. The displaced disc was further categorized as disc displacement with reduction (DDWR) and disc displacement without reduction (DDWOR). The morphology of the articular eminence was classified into four types; box, sigmoid, flattened, and deformed. The relationship between the four types of shape of the articular eminence and the two types of disc position was assessed. Results: In the DDWR and DDWOR groups, the morphology of articular eminence were a box type in 40.5%, a sigmoid type in 30.2%, a flattened type in 24.6%, and a deformed type in 4.7%. The box type of the articular eminence were 34.3% in the DDWR group and 42.9% in the DDWOR group. The sigmoid type of the articular eminence were 34.3% in the DDWR group and 28.6% in the DDWOR group. The flattened type of the articular eminence were 28.6% in the DDWR group and 23.1 % in the DDWOR group. The deformed type of articular eminence were 2.9% in the DDWR group and 5.5% in the DDWOR group. Conclusion: Disc displacement is more likely to be found in the temporomandibular joints with a box-shaped articular eminence. It can be considered that shape of the articular eminence is related to the development of disc displacement.

Observation of bilaminar zone in magnetic resonance images of temporomandibular joint

  • Purpose: To observe the relationship of bilaminar zone of temporomandibular joint retrodiscal tissues to the disc condition. Materials and Methods : The upper and lower stratum of bilaminar zone were identified on magnetic resonance open mouth images of 148 joints from 74 patients with disc displacements. Results: Both strata were identifiable in 105 joints which had disc displacement with reduction. Lower stratum was not identifiable in 35 joints which had disc displacement without reduction but 12 of 35 had hyalinized posterior attachment where the disc was. The 8 joints which had partial disc displacement without reduction showed identifiable lower stratum at the reducing site which was medial. Conclusion: Disruption or no identification of lower stratum which corresponds to the condylar portion of posterior attachment may be the sign of disc displacement without reduction.

  • Purpose : To describe the MRI findings of the antero-superior attachment of the disc in patients with internal derangement. Materials and Methods : One hundred fifty-six MR images from 40 normal subjects and 55 abnormal subjects were classified into three study groups of normal, anterior disc displacement with reduction and anterior disc displacement without reduction. On both closed- and open-mouth proton density sagittal images, the depiction of the antero-superior attachment of the disc and its demarcation from the disc were evaluated in three cuts of lateral, central and medial one thirds of the condyle. Results : The depiction of antero-superior attachment was more frequent by the order of normal, anterior disc displacement with reduction and anterior disc displacement without reduction groups, and the significant differences were found on lateral cut of the closed images and lateral and central cuts of the open images. In study for the demarcation between the antero-superior attachment and the disc the lateral cut of the closed images and all three cuts of the open images showed significant differences. Conclusion : Open images are useful to show the difference in depiction of the antero-superior attachment of the disc among the TMJ groups.

자기공명영상에서 관찰되는 측두하악관절의 관절강내 삼출과 동통 및 관절원판 변위와의 관계 (A Relationship between the Joint Effusion and the Presence of Pain and Disc Displacement In the Temporomandibular Joint)

  • The authors assessed if there is a relationship between joint effusion and the presence of pain, and between joint effusion and disc displacement in 446 temporomandibular joints of 223 subjects who visited the Department of Oral Medicine, Kyungpook National University Hospital with the complaints of temporomandibular joint problem. The amount of joint effusion and the position of disc were determined from magnetic resonance images of the temporomandibular joints. The position of disc was classified into normal disc position, disc displacement with reduction (DDWR), and disc displacement without reduction (DDWOR). The amount of joint effusion was graded according to the extent of joint area of high signal intensity on T2-weighted MR images. These findings were correlated with one another and statistically analyzed. The results were as follows; 1. The incidence of temporomandibular joint pain Was higher in the joints with higher amount of joint effusion (P<0.001). 2. The proportion of temporomandibular joints with higher amount of effusion increased in the joints with more advanced articular disc displacement (P<0.001).

측두하악관절 내장증에서의 관절원판의 형태 및 골 변화에 관한 연구 (Evaluation of bone and disc configuration in TMJ internal derangement)

  • Purpose: To investigate bone and disc configuration on MR images in internal derangement related to age. Materials and methods: MR images of 150 TMJs in 107 patients were analyzed to determine the morphologic changes. Two groups were distinguished to be correlated with age. Group I consisted of TMJs that were diagnosed as having anterior disc displacement with reduction (ADDwR), and Group 2 consisted of TMJs that were diagnosed as having anterior disc displacement without reduction (ADDwoR). We assessed the configuration of the articular disc, degree of anterior disc displacement, and osseous changes of TMJs. Results: The third decade (83 of 150 joints) was most frequent in this study. In the ADDwR group biconcave discs was most frequent at all ages except fifth decade, but in the ADDwoR group deformed discs was most frequent at third and forth decades. In the ADDwR group slightly displaced discs was most frequent at all ages, but in the ADDwoR group severely displaced discs was most frequent at second decade, and the degree of disc displacement was increased with aging over 30 years of age. TM joints showed osseous changes in 17% of the ADDwR group, and in 30% of the ADDwoR group. MR findings of osseous changes of the TMJ were not found to be significantly correlated with age. Conclusion: The prevalence of deformation of disc, displacement of disc, and osseous changes of TMJ was higher in the ADDwoR group than in the ADDwR group. MR findings of disc configuration and degree of disc displacement were found to be correlated with age.

악관절장애환자의 하악과두 크기에 관한 연구 (A Study on the Size of TMD Patient's Condyle Head)

  • Objective: In many TMD cases, deformed and reduced condyle heads were frequently observed. This study was prepared to compare the dimensions between normal and symptomatic condyles, using MR images. Materials: One hundred and twenty one patients with clinical signs and MRI-confirmed diagnosis of disc displacement were selected for this study. Thirty eight TMJs from nineteen asymptomatic volunteers who had no clinical symptoms and no disc displacement on sagittal and coronal view of MRI, were served as normal. Methods: Symptomatic condyles were classified according to the severity of the anterior disc displacement. The amount of anterior disc displacement was evaluated at sagittal section, and they were classified into 4 groups as normal(N), little(G0), mild(G1), moderate(G2) and severe displacement(G3). The dimentions of condyle were measured at the 200% magnified view, by digitizing program. All dimensions were compared among each groups on the central section of sagittal and coronal views, and the statistical analysis was performed. Results: The mean value of anteroposterior length of normal condyle was $0.79{\pm}0.13cm$ at sagittal section and mediolateral length was $2.12{\pm}0.22cm$ on coronal section. The mean value of anteroposterior length of symptomatic condyle was $0.67{\pm}0.16cm$ at sagittal section and mediolateral length was $1.97{\pm}0.28cm$ on coronal section. Conclusions: The size of symptomatic condyle was smaller than normal TMJ. The size of condyle was decreased as the amount of the disc displacement was increased. The dimensional change was found on the anterior articular surface of condyle at the mild or moderate disc displacement. And at the case of severe disc displacement, dimensional change was found on the superior articular surface.

측두하악관절내장 환자의 진단분류에 따른 하악운동 특성의 분석 (Analysis of the Mandibular Movements in Patients with Internal Derangement of the Temporomandibular Joint According to Diagnostic Subgroups)

  • The purpose of this study was analyse the mandibular movements in patients with internal derangement of the temporomandibular joint according to diagnostic subgroups. The author classified patients with internal derangement of the temporomandibular joint into 4 diagnostic subgroups by means of the magnet resonance imagings, and evaluated the clinical signs and the mandibular movements with Mandibular Kinesiograph(MKG) in each subgroups. The mandibular movements, measured in this study, were the types of movement in frontal and sagittal plane, velocities in opening and closing movement, and the opening and closing movement, and the opening and closing velocity pattern. The data were compared between the 5 groups including the normal group. The results were as follows : 1. Pain was more frequently observed in the anterior disc displacement without reduction group than in the anterior disc displacement with reduction group. Sound of joint was more frequently observed in the anterior disc displacement with reduction group, and limitation of mandibular opening movement was more frequently observed in the anterior disc displacement without reduction group. Duration of the anterior disc displacement without reduction group was significantly short compared to that of the anterior disc displacement with reduction group, and duration of the unilateral anterior disc displacement without reduction group was shortest in the experimental group. The frequency of Angle's classifications had not significant correlations between the experimental groups. 2. Active and passive range of the opening movement, maximum protrusive movement, maximum lateral movement toward left side were significantly decreased in the experimental groups compared to the control group, but there was no significant difference in the range of the maximum lateral movement toward right side between the control and experiment groups. In unilateral anterior disc displacement without reduction group, the range of maximum lateral movement toward unaffected side was no significant difference in the range of the maximum lateral movement between toward affected side and toward unaffected side. 3. Maximum opening velocity, maximum closing velocity, average opening velocity, average closing velocity and maximum velocity of terminal tooth contact were significantly decreased in the experimental groups compared to control group. There was no significant difference in maximum opening velocity and maximum velocity of Terminal tooth contact between the subgroups of the experimental group each other, but there was significant difference in maximum closing velocity, average opening velocity and average closing velocity between the subgroups each other. 4. In the frontal plane of the MKG, the frequency of complex deviation type(F-2)pattern was significantly increased in the anterior disc displacement with out reduction group compared to the anterior disc displacement with reduction group and the control group. In the sagittal plane, the frequency of coincident type(S-1)was decreased in the same group. 5. In the maximum opening velocity pattern, the frequency of no-peak type (OV-3)in the unilateral anterior disc displacement with reduction group was significantly increased compared to the control group. The frequency of 1-peak type (OV-1) and 2-peak type (OV-2) was decreased in the anterior disc displacement with out reduction group, but the frequency of no-peak type (OV-3)was increased in the same group. In the maximum closing velocity pattern, the frequency of no-peak type (CV-3) was significantly increased in the anterior disc displacement without reduction group. Compared to the anterior disc displacement with reduction group and the control group. The frequency of 1-peak type (CV-1) and 2-peak type (CV-2) in the anterior disc displacement with reduction group was decreased than that in the control group.

직접외상에 의한 급성 비정복성 관절원판 변위의 치험례 (A Case Report on the Treatment of Acute Anterior Disc Displacement without Redution in TMJ Disorders by Direct Trauma)

  • The authors treated a 30-years old female patient who visited the Department of Oral Medicine, PNUH due to the chief complaint of limitation of mouth opening. The magnetic resonance imaging following clinical examination was used for establishing an accurate and reliable diagnosis and the patient was diagnosed as having anterior disc displacement without reduction in the right joint and anterior disc displacement with reduction in the left joint. For managing acute anterior disc displacement without reduction, mandible manipulation was applied first focusing on pain control and then stabilization appliance was used for maintenance of joint stabilization. With time, the sign and symptom was remarkably reduced and an active exercise program was recommended to maintain of normal muscle length, increase joint range of motion and develop normal coordination arthrokinematics. As a result of treatment, the patient did not complain discomfort of normal daily activities and it was difficult to consider that the displace disc was not reduced completely, but the improvement in range of motion and joint mobility were remarkably found. Therefore, an exercise program should be considered to maintain joint mobility and be effective as a self-care.
