청소년 성별에 따른 식생활 인식과 권장 식행동 실천 비교 (Gender Differences in Adolescents' Dietary Perceptions and Practices)

  • Objectives: This study attempted to compare adolescents' dietary behaviors and perceptions by gender in order to recommend useful strategies for nutrition interventions. Methods: Subjects were 2,363 middle school (MS) and high school (HS) students. They completed a self-administered questionnaire on their interest in diet and health, dietary perceptions, nutrition knowledge, dietary practices, and dietary environment at home. Data were analyzed using t-test, $x^2$-test, and simple regression analysis by gender and by school groups Results: Overall, girls obtained higher scores than boys did for "interest" (MS: p<0.001; HS: p<0.01), "dietary perceptions" (MS: p<0.001; HS: p<0.01), and "knowledge" (MS: p<0.01; HS: p<0.001). Regarding "dietary practices," no gender differences were observed among MS students, however, among HS students, boys obtained higher scores-reflecting good practices-than girls did (p<0.01). In all subjects, dietary environment at home was strongly associated with dietary practice than other variables (MS boys: ${\beta}$=0.435, p<0.001; MS girls: ${\beta}$=0.492, p<0.001, HS boys: ${\beta}$=0.271, p<0.001; HS girls: ${\beta}$=0.429, p<0.001). Conclusions: We observed gender differences in some of the variables such as knowledge and perception among adolescent students. Educational programs and core strategies that consider these gender differences need to be developed. Specifically, for girls, educational programs should focus on facilitating dietary recommendation adherence, whereas for boys, the program could focus on improving dietary knowledge and perceptions.

Association between dietary fiber intake and kidney stones: results from the National Health and Nutrition Examination Survey (2011-2018)

  • BACKGROUND/OBJECTIVES: Studies on the impact of dietary fiber intake on kidney stones are few, and their results were controversial. This study aimed to explore the association between dietary fiber intake and kidney stones in the nationally representative population of the USA. SUBJECTS/METHODS: This cross-sectional research included 8,588 participants from the National Health and Nutrition Examination Survey, 2011 to 2018. Information regarding dietary fiber intake was obtained from a 24-h recall survey. Participants were categorized into different dietary fiber intake tertiles according to the average of 2 days of dietary recall data. The outcome was self-reported kidney stones. After adjusting for the traditional risk factors, a multivariate logistic regression model was used to examine the association between dietary fiber intake and kidney stones. RESULTS: Eight hundred seventy-two participants had kidney stones. The weighted prevalence (SE) of kidney stones in the lowest tertile, medium tertile, and highest tertile of dietary fiber intake was 11.8% (0.8%), 10.3% (0.8%), and 9.1% (0.8%), respectively. After adjusting for age, sex, race and ethnicity, education level, smoking status, alcohol consumption, physical activity, body mass index, hypertension, diabetes, dyslipidemia, daily water intake, chronic kidney disease stage 3-5, and total energy intake, participants with the highest tertile of fiber intake had a significantly lower risk of kidney stones (odds ratio [OR], 0.68; 95% confidence interval [CI], 0.48-0.95) compared to those in the lowest tertile. Every 5 g/day increment in dietary fiber intake was associated with a significant decrease in risk of kidney stones (OR, 0.90; 95% CI, 0.83-0.98). CONCLUSION: An increase in dietary fiber intake was associated with a lower risk of kidney stones, suggesting adults should be encouraged to maintain an adequate dietary fiber intake to prevent the development of kidney stones. Our results provide evidence to formulate nutrition management strategies for the prevention of kidney stones.

Nutritional education for management of osteodystrophy (NEMO) trial: Design and patient characteristics, Lebanon

  • This study aims to determine the effect of a trained dedicated dietitian on clinical outcomes among Lebanese hemodialysis (HD) patients: and thus demonstrate a viable developing country model. This paper describes the study protocol and baseline data. The study was a multicenter randomized controlled trial with parallel-group design involving 12 HD units: assigned to cluster A (n = 6) or B (n = 6). A total of 570 patients met the inclusion criteria. Patients in cluster A were randomly assigned as per dialysis shift to the following: Dedicated Dietitian (DD) (n = 133) and Existing Practice (EP) (n = 138) protocols. Cluster B patients (n = 299) received Trained Hospital Dietitian (THD) protocol. Dietitians of the DD and THD groups were trained by the research team on Kidney Disease Outcomes Quality Initiative nutrition guidelines. DD protocol included: individualized nutrition education for 2 hours/month/HD patient for 6 months focusing on renal osteodystrophy and using the Trans-theoretical theory for behavioral change. EP protocol included nutrition education given to patients by hospital dietitians who were blinded to the study. The THD protocol included nutrition education to patients given by hospital dietitian as per the training received but within hospital responsibilities, with no set educational protocol or tools. Baseline data revealed that 40% of patients were hyperphosphatemics (> 5.5 mg/dl) with low dietary adherence and knowledge of dietary P restriction in addition to inadequate daily protein intake ($58.86%{\pm}33.87%$ of needs) yet adequate dietary P intake ($795.52{\pm}366.94$ mg/day). Quality of life (QOL) ranged from 48-75% of full health. Baseline differences between the 3 groups revealed significant differences in serum P, malnutrition status, adherence to diet and P chelators and in 2 factors of the QOL: physical and social functioning. The data show room for improvement in the nutritional status of the patients. The NEMO trial may be able to demonstrate a better nutritional management of HD patients.

성인의 구강보건행태에 따른 미각인지역치, 타액분비량이 DMFT, OHIP-14에 미치는 영향 (Effect of Adult's Taste Recognition Threshold and Salivary Flow Amount on DMFT and OHIP-14 depending on Oral Health Behavior)

  • 이 연구는 대구지역 40대 이상 성인을 대상으로 6단계의 용액을 이용하여 미각인지역치(단맛, 짠맛, 신맛, 쓴맛), 흡습지를 사용하여 타액분비량을 측정하여 DMFT index와 OHIP-14와의 관련성을 알아보고자 하였다 그 결과 구강보건실천은 미각인지역치에, 식습관은 타액분비량에 영향을 주고, 미각인지역치 중 단맛과 신맛, 그리고 타액분비량은 DMFT index에 영향을 주어 구강건강상태가 좋으면 OHIP-14도 높아지는 결과를 나타내었다. 구강보건실천도와 식습관이 좋은 사람이 미각과 타액분비량, DMFT index에 영향을 주는 것을 확인할 수 있었다. 이 결과는 OHIP-14를 향상시키기 위해서는 구강보건실천도를 높여 미각인지역치 감수성을 높이고 식습관을 개선하여 타액분비량을 증가시켜 DMFT index를 감소시키는 것이 무엇보다 중요하다고 생각된다. 지금까지는 개인의 미각인지역치 수준의 평가 없이 일률적인 식이교육이 이루어지고 있어 구강보건교육 시 대상자의 미각인지역치를 측정하여 알려줌으로써 스스로 조절할 수 있는 교육 프로그램 개발이 필요할 것으로 생각된다.

Dietary behaviors and body image recognition of college students according to the self-rated health condition

  • This study was done to investigate the relationship between the perception of body image, body weight satisfaction or dietary behavior and self-rated health status in Korean college students. Subjects, 285 college students, were divided into three groups (healthy, normal, and unhealthy) according to the answer for the self-rated health question. Information about demographic status, self-rated health condition, height and weight, perception of body image, satisfaction of body weight, concern for body weight control, dietary behavior, nutritional knowledge, and health-related characteristics collected by a self-reported questionnaire. The proportion of men and women in each group was not significantly different. The academic year, major, experience of nutritional education, and type of residence were not significantly related with self-rated health but the pocket money range was significantly associated (p<0.05) with self-rated health. The proportion of subjects rated their health as unhealthy was the lowest in 210-300 thousand won pocket money range and was increased in less than 210 thousand won or over 300 thousand won pocket money ranges. There were no significant differences for age, height, weight and BMI between the groups. The body image perception and body weight satisfaction levels of healthy group was significantly higher than those of unhealthy group (p<0.01 and p<0.001, respectively), but the level of concern for body weigh1 control in healthy subjects was significantly lower than that in unhealthy subjects (p<0.05). The proportion of subjects reported as healthy was significantly increased with increased frequencies of following food behaviors; weekly use of protein foods (p<0.01), vegetables (p<0.05) and dairy products (p<0.01), and food habits such as "regularity of meal time" (p<0.01), "eating in moderation" (p<0.05), and "eating breakfast" (p<0.001). Overall results suggested that the college students have tended to have a better perception of health when they have better body image perception, body weight satisfaction and dietary behaviors.

Association between dietary omega-3 fatty acid intake and depression in postmenopausal women

  • BACKGROUND/OBJECTIVES: This study aimed to analyze the association between dietary omega-3 fatty acid intake and depression in postmenopausal women using data from the Korea National Health and Nutrition Examination Survey (KNHANES) VI. SUBJECTS/METHODS: The KNHANES is a cross-sectional nationwide health and nutrition survey. Dietary data, including omega-3 fatty acids, were assessed using the 24-h recall method. Depression was evaluated using a survey questionnaire. The association between dietary omega-3 fatty acids and depression was evaluated using multivariate logistic regression analysis. Depression, according to the dietary omega-3 fatty acid intake, was expressed as the odds ratio (OR) with a 95% confidence interval (CI). A total of 4,150 postmenopausal women were included in the analysis. RESULTS: In the fully-adjusted model, the group with the highest dietary omega-3 fatty acid intake significantly showed lower prevalence of depression than the group with the lowest intake (OR, 0.52; 95% CI, 0.33-0.83); a significant linear trend was detected (P for trend = 0.04). According to the dose-response analysis using cubic restricted spline regression, this association was linear and monotonic (P for non-linearity = 0.32). CONCLUSIONS: In this study, the dietary omega-3 fatty acid intake in postmenopausal women was inversely proportional to depression in a dose-response manner. Large cohort studies are needed to verify the causality between omega-3 fatty acids and depression in Korean postmenopausal women.

Association between dietary branched-chain amino acid intake and skeletal muscle mass index among Korean adults: Interaction with obesity

  • BACKGROUND/OBJECTIVES: The branched-chain amino acids (BCAA), including isoleucine, leucine, and valine, promote muscle protein synthesis. However, obesity may interfere with protein synthesis by dysregulating mitochondrial function in the muscles. This study aimed to examine the association between dietary intake levels of BCAA and skeletal muscle mass index (SMI) in middle-aged participants, and the effect of obesity/abdominal obesity on this association. SUBJECTS/METHODS: The data of 3,966 men and women aged 50-64 years who participated in the 2008-2011 Korea National Health and Nutrition Examination Survey were analyzed. Intake levels of energy-adjusted dietary amino acids were obtained using a 24-hour dietary recall. SMI was calculated by dividing the appendicular skeletal muscle mass by body weight (kg) and multiplying the result by 100%. Multivariable general linear models were used to analyze the association of dietary BCAA intake levels with SMI. RESULTS: The beneficial effects of energy-adjusted dietary BCAA intakes on SMI were greater in the non-obesity/non-abdominal obesity groups; however, no significant associations were observed in the obesity/abdominal obesity groups (P > 0.05). CONCLUSIONS: Healthy weight and sufficient intake of dietary BCAA are recommended to maintain muscle mass.

청소년의 식습관이 건강행위실천에 영향을 미치는 요인 (Affecting Factors on Food Habits related Health Behavior Activities of Adolescents)

  • Although adolescents' obesity prevalence increases recently in Korea due to Westernized dietary life, according to social and economic development, and physical activity decline, distorted body image and improper dietary habits are prevalent, because of excessive obsession with weight control, deriving from psychological impacts including appearance supremacy. This study conducted a survey on health oriented awareness and attitude related with dietary habits targeting adolescents, such as high school students and college students, younger than 24, from May 11 to May 29, 2015. In this study, 280 questionnaire copies were collected out of 300 distributed questionnaire copies, and used 269 copies as analysis data, except 11 copies of which responses were inadequate. The results of this study are as follows: First, the high school students showed higher dietary regularity than the college students. Especially, 95.5% of the college students showed very high irregularity of lunch, compared with just 4.5% of the high school students. Second, the adolescents showed distorted perceived body image in comparison with body mass index (BMI). As a result of examining BMI relationship, according to one's own perception on body type, 28.6% of the respondents perceived themselves fat, despite normal BMI, and 40.4% of the respondents perceived themselves normal, despite low BMI. Third, health behavior activities level was higher, as the regularity of breakfast (B=1.093), lunch, (B=0.650), and dinner (B=0.765) was higher. The variable affecting the most ( ${\beta}=0.372$) was the regularity of breakfast. As interest in weight control was higher, health behavior activities was lower. Because over-interest in weight control may be linked with improper dietary habits or weight control, a caution is needed. The results of this study are expected to be utilized as the basic data for the policy and health-oriented program development to improve adolescents' health behavior practice.

Validation of soy isoflavone intake and its health effects: a review of the development of exposure biomarkers

  • BACKGROUND/OBJECTIVES: It is difficult to consistently demonstrate the health effects of soy isoflavones owing to the multitude of factors contributing to their bioavailability. To accurately verify these health effects, dietary isoflavone intake should be measured using a biologically active dose rather than an intake dose. This concept has been expanded to the development of new exposure biomarkers in nutrition research. This review aims to provide an overview of the development of exposure biomarkers and suggest a novel research strategy for identifying the health effects of soy isoflavone intake. MATERIALS/METHODS: We cover recent studies on the health effects of soy isoflavones focusing on isoflavone metabolites as exposure biomarkers. RESULTS: Compared to non-fermented soy foods, fermented soy foods cause an increased concentration of isoflavones in the biofluid immediately following ingestion. The correlation between exposure biomarkers in blood and urine and the food frequency questionnaire was slightly lower than that of corresponding 24-h dietary recalls. Urinary and blood isoflavone levels did not show a consistent association with chronic disease and cancer risk. CONCLUSION: It is crucial to understand the variable bioavailabilities of soy isoflavones, which may affect evaluations of soy isoflavone intake in health and disease. Further studies on the development of valid exposure biomarkers are needed to thoroughly investigate the health effects of isoflavone.

Early adulthood: an overlooked age group in national sodium reduction initiatives in South Korea

  • BACKGROUND/OBJECTIVES: South Korean's sodium consumption level is more than twice the upper limit level suggested by the WHO. Steep increases in the prevalence of hypertension and cardiovascular disease in Korea necessitate more effective sodium reduction programs. This study was conducted in order to compare sodium intake-related eating behaviors and key psychosocial factors according to age group and gender. SUBJECTS/METHODS: Using an online survey, a total of 1,564 adults (20-59 years old) considered to be geographically representative of South Korea were recruited and surveyed. The major outcomes were perceived behaviors, knowledge, intentions, and self-efficacy related to sodium intake. RESULTS: The results show that perceived behavior and level of self-efficacy related to low sodium consumption differed by age and gender. Female participants showed better behavior and intention towards low sodium intake than male counterparts. Young participants in their 20s showed the lowest intention to change their current sodium intake as well as lowest self-efficacy measures. CONCLUSIONS: Future sodium reduction interventions should be developed with tailored messages targeting different age and gender groups. Specifically, interventions can be planned and implemented at the college level or for workers in their early career to increase their intention and self-efficacy as a means of preventing future health complications associated with high sodium intake.
