Evaluation of a theory-based community intervention to increase fruit and vegetable intakes of women with limited incomes

  • The study objectives were to increase both the stage of readiness to eat fruit and vegetables as well as the intakes of women who participated in the Expanded Food and Nutrition Education Program (EFNEP) for families with limited incomes. The intervention was to enhance the currently used curriculum, Eating Right Is Basic III (ERIB3), with stage-specific processes based on the Trans-Theoretical Model of readiness to change. Trained EFNEP community workers taught the enhanced curriculum to 90 mothers in the experimental county and to 53 mothers in the control county. Pre- and post-intervention measures included stages of readiness to eat fruit and vegetables and to intake as assessed by 24-hour dietary recalls and staging questions. Most women recruited into EFNEP were in Action and Preparation Stages (53.5%). Fruit and vegetable intakes showed a linear trend along with the Stage of Change for fruit and vegetable. After intervention, some combination of the ERIB3 and the fruit and vegetables-enhanced ERIB3 resulted in a reported 1/2 servings/day increase in fruits and vegetables in both the control and the experimental counties, although changes were not significant. EFNEP women also moved along the Stage of Change Continuum for fruits and vegetables in both counties. The percentage of people who ate 5 or more servings of fruit and vegetables was significant, however, only in the experimental group. We encourage health professionals to apply lessons learned from this intervention and to continue to pursue theoretically based interventions to change dietary behaviors.

Effect of Nutrition Counseling by Nutrition Care Process on Diet Therapy Practice and Glycemic Control in Type 2 Diabetic Patients (영양관리과정에 의한 영양상담이 제 2형 당뇨병 환자의 식사요법 실천과 혈당 조절에 미치는 영향)

  • Objectives: This study examined the effects of nutrition counseling by the nutrition care process (NCP) on diet therapy practice and glycemic control in patients with type 2 diabetes mellitus. Methods: The survey was conducted on 49 patients whose hemoglobin A1c (HbA1c) level ranged from 6.5% to below 10% among patients aged 30~60s with type 2 diabetes mellitus. Nutrition counseling by the NCP process was carried out twice: first nutrition counseling and follow up counseling. The questionnaires were composed of 54 questions in five fields (general characteristics, health-related behaviors, diet therapy-related items, dietary life, diet therapy-related knowledge, diet therapy-related barriers). Nutrition intervention in nutrition counseling was performed based on the individualized diagnosis of NCP. Results: All the subjects practiced self-monitoring of their blood glucose levels, regular exercise, and diet therapy after NCP-based nutrition counseling. Diet therapy-related knowledge and practice by the subjects were improved after nutrition counseling. While the intake of boiled white rice decreased, the intake of boiled brown rice and barley rice in the subjects increased significantly. After nutrition counseling, the weight and HbA1c of the subjects decreased. Conclusions: These results suggest that personalized nutrition counseling by NCP process is effective for diet therapy compliance and glycemic control of type 2 diabetic patients.

Development of a Nutrition Quotient (NQ) equation modeling for children and the evaluation of its construct validity (어린이 영양지수 (NQ, Nutrition Quotient) 모형 개발과 구성타당도 평가)

  • The objective of this study was to develop and evaluate the construct validity of a Nutrition Quotient (NQ) for children. In a previous report (Kang, et al., 2012), the food behavior checklist for children's NQ, consisting of 19 items, was grouped into a 5-factor structure according to the exploratory factor analysis: balance, diversity, moderation, regularity, and practice. In this study, the construct validity of the NQ was assessed using a confirmatory factor analysis. Elementary school students (n = 1,393) from six large cities completed the NQ test. Indicator tests suggested an adequate model fit (goodness of fit index = 0.9613; adjusted GFI = 0.95; standardized root mean square residual = 0.0464; chi-square test statistics of < 0.001 p-value, 82.1), and item loadings were significant for all subscales (p < 0.05). The standardized path coefficients were used as the weights of the items. The NQ and the 5 factor scores of the student were calculated by the obtained weights of the questionnaire items. Logistic regression was applied to find the significant factors in order to affect a specific nutrient status. The receiver operation characteristic curve analyses were performed in order to find diagnostic cut-off points of the five factors. The food behavior checklist for children's NQ would be a handy and suitable instrument for evaluating dietary behaviors of Korean children.

Effects of Skipping Breakfast on Nutrition Status, Fatigue Level, and Attention Level among Middle School Students in Gyunggi Province, Korea (아침 결식이 경기지역 남녀 중학생의 영양섭취상태, 피로자각도 및 주의집중력에 미치는 영향)

  • Eating breakfast provides crucial nutrition for brain function and helps promote overall health. It is especially critical in growing adolescents, as it is known to form good eating habits and better study habits. This study investigated the effects of skipping breakfast on nutritional state, fatigue level, and attention level. A cross-sectional study was conducted in 2010 on total of 828 adolescents composed of 414 boys and 414 girls. Students who ate breakfast never to twice per week were placed in the breakfast-skipper group while students who ate breakfast more than five times per week were included in the breakfast-eater group. Students performed a self-reported questionnaire on food behaviors, amount of food consumption, fatigue level, attention deficient hyperactivity disease (ADHD) level by Conners-Wells' Adolescent Self-Report Scales, depression scale, and self-esteem level. Statistical analysis was conducted using the SAS program (version 9.1). A total of 135 boys (32.6%) and 138 girls (33.3%) were included in the breakfast-skipper group, whereas 241 boys (58.2%) and 223 girls (53.9%) were included in the breakfast-eater group. The breakfast-skipper group showed irregular food behaviors and lacked nutrients. Specifically, energy (p< .001), protein (p< .001), dietary fiber (p< .001), calcium (p< .01), vitamin A (p< .01), thiamin (p< .05), niacin (p< .001) levels in boy breakfast-skippers were statistically lower compared to boy breakfast-eaters. Intakes of all nutrients except fat in girl breakfast-skippers were statistically lower than in girl breakfast-eaters. Girl breakfast-skippers (41.3%) showed significantly higher fatigue risks compared to girl breakfast-eaters (21.5%). Low attention level was also observed only in girls in the breakfast-skipping group. Moreover, students in the breakfast-skipper group showed higher scores for depression and low self-esteem (p< .001). In conclusion, skipping breakfast has effects on young adolescents' nutrition, manifesting as high fatigue level and low attention level, especially in girls.

The Correlation among Growth Development, Bone Mineral Density of Carpus and Nutrient Intakes in Preschool Children (미취학 아동에 있어 신체발달, 손목 골밀도 및 영양섭취와의 상관성)

  • This study was conducted to examine the correlation among growth development, bone mineral density, and nutrient intakes in preschool children. A total of 62 preschool children measured the anthropometric characteristics and the bone mineral density of carpus using DEXA. The questionnaires categorized to determine health status, life style, dietary behaviors, and dietary intakes were filled out by children's mothers. The average ages of the study subjects were 62.4 months in boys and 62.1 months in girls. The birth height and weight of the subjects were 50.9 cm and 3.4 kg in boys and 50.3 cm and 3.3 kg in girls, respectively. The average height, weight, % body fat, and obesity index were 111.7 cm, 19.6 kg, 15.0%, -2.1% in boys and 109.4 cm, 18.7 kg, 17.5%, 0.2% in girls, respectively. The bone mineral density in carpus was 0.25 g/cm$^2$ in boys and 0.24 g/cm$^2$. In girls. Proportions of children having regular meals were 69.0%, 94.7%, and 69.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were poor appetite and lack of time for breakfast and snacks for dinner. Most of the children(52.6%) had breakfast with the whole family together, and all the children had lunch and dinner with a part of the family. Most of the children answered they had snack more than once a day, and 64.3% had an unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B$_1$, niacin, and vitamin C did not meet the Korean RDAs. Average bone mineral density in carpus was positively correlated to the body weight, the total body water, the lean body mass, and the obesity index, respectively(p<0.01, p<0.01, p<0.01 and p<0.01). The body weight, the total body water, and the lean body mass showed the significant negative correlations with the animal iron intake(p<0.05, p<0.01 and p<0.01). The bone mineral density in the distal carpus was negatively correlated to the animal protein intake and the animal iron intake(p<0.05 and p<0.05). And the bone mineral density in the ultradis carpus and the average bone mineral density in the carpus were positively correlated to the plant calcium intake(p<0.05 and p<0.05). Therefore, more systematic studies to investigate the roles of protein, iron, and calcium in growth development of preschool children were required.

Evaluation of Nutrient Intake, Eating Behavior and Health-Related Lifestyles of Korean College Students

  • The purpose of this study was to investigate the nutrient intake, eating behaviors and health-related lifestyles of Korean non-nutrition major college students after they took a nutrition course. The subjects were 40 male and 147 female students at a university in Inchon. The results are summarized as follows: Average height of male and female students was 176.2 and 162.0 cm, respectively. Average weight was 66.2 and 52.3 kg, respectively. Average intake of calories, protein, vitamin A, vitamin C, vitamin $B_1$, vitamin $B_2$, niacin, and Ca was lower than Korean RDA. Fe intake of female students was lower than Korean RDA. Most students have had dietary problems such as overeating, eating unbalanced meals, and skipping meals. More than 60% of the students skipped breakfast. The main reason for skipping meals was lack of time. Most students didnt exercise regularly. About 40% of the students took vitamin or mineral supplements. As for smoking and alcohol use, 11.3% of the students drank alcohol and smoked, and 74.2% of them only drank alcohol. More than 30% of the students drank alcohol once a week. Most students ate out twice a week, and chose their based on taste rather than nutritional value. The main reason for eating out was simply to enjoy a meal. More than 60% of the students ate at places in or around campus. After taking the nutrition course, intake of milk and other dairy products, vegetables, fruits and protein-rich foods increased in female students. For both male and female students, intake of fat, sugar, processed foods, soft drinks, fried foods and spices decreased. Therefore, nutrition education had effect on non-nutrition major students, suggesting that proper nutrition education encouraged healthy eating habits on the part of college students.

A Survey of Food and Nutrient Intakes in Female College Students (여대생의 식품 및 영양소 섭취량 조사)

  • Purpose: The study was done to investigate Food and Nutrient Intakes of female college students and define differences between teen-age groups and women in their twenties. Method: The subjects of this study were 821 female college students. They were eighteen to twenty nine years old. Nutrient intakes were measured by the Moon, S. J. Scale (1980). Data was collected from June 13 to June, 27. Data was analyzed with the SPSS program by Chi-test, t-test and Pearson's correlation coefficients. Result: Female college students' mean intake of calories, protein, fat, and carbohydrates were $1560.7{\pm}367.71kcal,\;62.0{\pm}19.03g,\;33.1{\pm}9.85g,\;and\;253.7{\pm}57.52g$ respectively. Their mean intake of calcium, phosphorus and iron were $592.9{\pm}221.82mg,\;918.2{\pm}284.24mg\;and\;13.7{\pm}4.50mg$ respectively. Intake of calories, calcium, iron and vitamine B1 were below their Percent of Recommended Intake. Nutrient intakes showed a significant difference between age groups. Teen-age female college students had more nutrient intakes than the women in their twenties. In addition, there was a difference in food consumption behaviors between the teen-age group and the twenty-something group. Conclusion: The nutrient intake of female college students changed by age. Many teen-age female college students ate breakfast well, thus their intake of calories, protein, fat, carbohydrates, minerals such as calcium, phosporus and vitamins was significantly higher than the over twenty years old students. Teen-age female college students like fruits and sugars but over twenty years college students preferred coffee. This study can be useful in directing a dietary consumption status of women to improve nutritional health.

The effects of weight loss by a low-calorie diet and a low-calorie plus exercise in overweight undergraduate students (과체중 대학생에서 저열량식이 또는 저열량식이와 운동병합에 의한 체중 감량 효과)

  • Korean undergraduate students attempt to lose weight but often fail due to utilizing ineffective weight loss strategies. Some diet programs have succeeded, yet, they have not provided adequate skills for long-term weight maintenance. The aim of the study was to determine the effect of a low calorie diet and exercise with nutritional education on weight loss, serum lipid profiles, nutrient intakes, and dietary behavior modification in overweight and obese undergraduate students. The subjects in the low calorie diet group (LCD; n = 12) and the low calorie diet plus exercise group (LCDE; n = 13) had the same goal of losing 4 kg during a 12 week program. Nutrient intakes were assessed by the 24 hour recall method. Also, food habits and dietary behaviors were investigated by self-administered questionnaires before and after the weight control program and one month after completing the program. LCD and LCDE groups lowered body weight by 2 kg and 1 kg, respectively, although they decreased calorie intake by 355 and 287 kcal per day compared to intakes prior to the study. Body fat mass decreased in both the LCD and LCDE groups; however, the decrease was greater in the LCDE group. In addition, only the LCDE group increased muscle mass. The LCD group had a slightly better effect in reducing body weight, body fat, and waist circumference than the LCDE group. However, their decrease was reversed after the mid-study check in the LCD group; the reduction was better maintained and decreased more in the LCDE group. However, serum lipid profiles were already in borderline prior to the study; moreover, they were not modified after losing weight. The dietary behavior program helped students to develop better dietary habits. In conclusion, the combination of a low calorie diet and exercise is necessary in order to maintain longer weight loss by increasing muscle mass and decreasing body fat.

A relationship of healthy eating status and body composition in Korean adults by their constitution type: A cross-sectional study (우리나라 성인의 사상체질에 따른 식생활 상태와 신체구성의 상관성에 관한 단면분석연구)

  • Background Adherence to healthy diet acts as a key role to modify sedentary lifestyle in real life setting. Constitution type of traditional Korean medicine has been prediagnosed to risk factors of cardiometabolic diseases. This study aims to evaluate the associations between body composition and healthy eating status in Korean adults by their constitution type. Methods Of 4046 participants from Korean Medicine Daejeon Citizen Cohort study, Korean Medicine Daejeon Citizen Cohort (KDCC) study were included for analysis. Demographic, health-related behaviors and Korean Medicine (KM) type were surveyed based on a general health-related questionnaire. Anthropometric measurements and dietary factors by using Korean Healthy Eating Index (KHEI) were assessed only in the half of the original participants. Results 50.8% of Taeeum was observed from eligible 1967 participants (66.7%, women). The highest KHEI score was observed in soyang group (52.0±0.3, p=0.006) compared to other two groups. In taeeum group, lower appendicular skeletal muscle mass (ASM)(%)(Taeeum: 38.7±0.1 vs. Soeum/Soyang: 39.2±0.1, p < 0.05), and higher percent of body fat (PBF)(%) (Taeeum: 32.9±0.2 vs. Soeum/Soyang: 29.0±0.2, p < 0.05) by the lowest tertile (T1) of the KHEI score, respectively. When the KHEI score increased by 1 point in taeeum group, a positive relationship was observed, which increased by 0.015% of ASM, however, a negatively related to in which body fat mass (BFM) decreased by 0.022 kg and PBF decreased by 0.024%. Conclusion Customized nutritional management is required that could help maintaining physical health and diet by their constitution type.

Relationship between Fat-Free Mass and Grip Strength, Nutrient Intakes, Exercise Behavior in Middle- and Old-Aged Women (중년기, 노년기 여성에서 무지방조직과 악력, 식이섭취 및 운동습관과의 관계)

  • This study was performed to show change of fat-free mass(FFM), representing mostly the muscle mass change, and muscle strength with increasing age, and relationship between dietary, exercise behaviors and FFM in healthy middle-and old aged women who are of age over 55 years. The FFM and correspondingly hand grip strength showed significant positive correlation with age. But concentration of serum albumin showed no significant relationship with age. The subjects were categorized into groups according to FFM tertile. The anthropometry such as weight, BMI, fat mass, circumferences of waist and hip, WHR, and hand grip strength decreased significantly in the lowest FFM group. But the albumin level showed no change according to FFM level. The FFM showed significant correlation with nutrient intakes such as energy, carbohydrate, protein, Fe, P, Ca. No association, however, was shown with exercise behavior probably because of no case with resistance exercise habits. The variance of FFM was explained 55.2% by height and carbohydrate intake. The variance of height-adjusted FFM could be explained only 16.2% by intake amount of carbohydrate. In conclusion, the decrease of FFM may cause to reduce muscle strength in female elderly. The increasing nutrient intakes were associated with the increased FFM and may protect from risk of sarcopenia. However, only the carbohydrate intake could influence independently the FFM in middle- and old-aged women. The FFM has no association with endurance exercise habits.
