  • The purpose of this study was to identify the effects of music therapy on anxiety in neurotic patients. The subjects of the study were 41 patients that had been diagnosed as having anxiety disorder, neurotic depression, or somatization disorder and were admitted to one general hospital in Seoul. The 41 research subjects were assigned to an experimental (22 clients) and a control (19clients) group. Data were gathered from September. 25, 1995 to December. 15, 1995 using a questionnaire and physiological measurement tool. Data were analyzed with the SAS package using frequency, t-test, paired t-test and Pearson correlation coefficients. The results of this study are as follows ; 1. There were significant differences between two groups on systolic and diastolic blood pressure, and pulse rate after treatment. In the experimental group, Systolic and diastolic blood pressure, and pulse rate decreased significantly after Music Therapy. 2. There were no significant differences between the two groups on the pre and post psychological anxiety score. But, after music therapy, experimental group had a lower psychological anxiety score than the control group. From these results, it is concluded that the music therapy can be effective in decreasing anxiety in neurotic patients.

  • As patients on bed rest perform repeated Valsalva maneuver, it is necessary for them to prevent the-danger inherent in repeated Valsalva maneuver through intelligent rehabilitative nursing care. In this regard, it seems to be important to furnish a scientific rationale underlying rehabilitative nursing care. This study was undertaken to find the effects of repeated Valsalva maneuver upon circulation of nor-mal men. The subjects for this study were twenty normal and healthy college students of age from 19 to 26. For the first time, the ECG of standard 12 leads was recorded and the blood pressure was measured under the resting state. And the subjects performed Valsalva maneuver for 10 seconds, then expired air for 2 seconds. After the subjects carried out in this menner for 1 minute, the ECG and the blood pres-sure were recorded and measured again. The Changes of heartbeats, systolic and diastolic pressures after the practice of repeated Valsalva maneuver were compared with those of the letting state. The results obtained were as follows : 1. The heartbeats after the practice of repeated Valsalva maneuver generally increased but did not show statistical significance. 2. The systolic pressure measured after the practice of repeated Valsalva maneuver was higher than that of the resting state. 3. The diastolic pressure measured after the practice of repeated Valsalva maneuver was higher than that of the resting state.

  • Purpose: The purpose of this study was to evaluate the hemodynamic changes in degree and duration that occur during Valsalva maneuver (VM). Furthermore, we wanted to investigate the patterns and mechanisms of physiological hemodynamic control. Method: Thirty six healthy college students were recruited from Y university. Each participant was provided with written informed consent. Blood pressure (BP), heart rate (HR), cardiac output (CO) were continuously recorded using the Finometer. Result: During the phase I of VM, means of systolic and diastolic pressures were increased by 32.15% and 38.28%, respectively, compared with basal values. HR and CO were decreased by 9.91% and 13.01%, respectively. Immediately after the maneuver (phase III), systolic and diastolic pressures were decreased by 5.05% and 6.24%, respectively, compared with those obtained in the phase II. HR and CO were elevated by 13.33% and 11.93%, respectively, compared to the levels of earlier phases. BPs were represented with overshoot in the phase IV, and recovered by baseline values about 20 sec after VM. Conclusion: These results demonstrated that hemodynamic changes are variable in the event of VM even in healthy humans. It will be valuable to accumulate more quantitative hemodynamic information in special populations such as the elderly and the patients with cardiovascular problems.

  • Blood pressure (BP) measurement plays a pivotal role in veterinary medicine for diagnosing cardiovascular disorders and monitoring anesthesia of animals. Although indirect BP measurement has been widely applied to monitor BP because of its convenience and non-invasiveness, it is still unclear whether indirect BP measurement is compatible with direct BP measurement in minipigs. In addition, the effect of animal posture during BP measurement is not well understood in minipigs despite its importance to cardiovascular performance. Therefore, both systolic and diastolic arterial BPs in minipigs were measured via femoral artery catheterization for direct BP measurement and using a compressive cuff as an indirect BP measurement under the dorsal or right lateral recumbent postures. Numerical values were processed by the Bland-Altman method to calculate the bias ${\pm}$ SD and the limits of agreement (LOA). In accordance with the American College of Veterinary Internal Medicine guidelines, the results between direct and indirect BP measurements were determined as apparent disagreements in both systolic and diastolic arterial BPs under all postures because of large bias ${\pm}$ SD and wide LOA. The results of the present will help prevent misinterpretation of the anesthetized patient's condition during monitoring of BP by indirect measurement.

  • Surgical treatment of partial endocardial cushion defect was accomplished in Feb. 1984 in this department. The 5 year old male patient had history of frequent upper respiratory tract infection and since his age of 3 years dyspnea on exertion and palpitation were noted but there were no cyanosis and clubbing. A thrill was palpable on the apex and grade IV/IV harsh systolic ejection murmur and diastolic murmur was audible on it. Liver was palpable about 3 finger breadths and no ascites. Chest X-ray revealed increased pulmonary vascularity, moderate cardiomegaly [C-T ratio; 0.69], and enlarged left atrium. EKG showed first degree heart block, RVH, LVH, and LAD. Echocardiogram showed paradoxical ventricular septal movement and abnormal diastolic movement of the anterior leaflet of mitral valve. Right heart catheterization resulted left to right shunt [Qp:Qs:2.1:1 ] and moderate pulmonary hypertension [60/40 mmHg]. Left ventriculogram showed mitral regurgitation [Grade III/IV] and filling of left atrium and right atrium nearly same time. Operative findings were: 1.Primum type atrial septal defect [3x2 cm] 2.Cleft on the anterior leaflet of mitral valve. 3.No interventricular communication and cleft of tricuspid valve leaflet. The mitral cleft was repaired with 4 interrupted sutures. The primum type atrial septal defect was closed with Dacron patch intermittently at endocardial cushion and continuously remainder. The post operative course was uneventful and discharged on 22nd postoperative day in good general conditions.

  • Twenty six patients were operated a total correction of tetralogy of Fallot between Jan., 1984 and July, 1985 at the Dept. of thoracic and cardiovascular surgery, Chonnam University Medical School, and a comparison between the survived group [n = 18] and the dead group [n = B was performed to detect factors influencing laboratory data, cineangiographic findings, operative findings and methods, and pump time. Following results were obtained, 1. There was no significant difference between two groups in the preoperative P.O2 and hematocrit level. 2. The size of the interventricular defect was not related to the operative mortality. 3. There was a significant difference in mortality rate between combined type of stenosis of pulmonary artery, valve and infundibulum and other types of right ventricular outflow tract stenosis. 4. There was a significant difference in mortality rate between the transannular patch reconstruction and other types of operative procedure. 5. There was no significant difference between two groups in total bypass time and aortic time. 6. There was no significant difference between two groups in left ventricular end diastolic volume and right ventricular end diastolic volume. 7. The operative mortality was related to the ratio of the diameter of the pulmonary valve annulus or each pulmonary artery to the ascending or descending aorta in cineangiographic findings, but there was no statistical significance of which probably is due to the lack of the total number of patients.

  • Purpose: The purpose of this study is to explore the effects of a group movement exercise program on psychophysiological variables in the institutionalized elderly. Methods: This research adopted the non-equivalent control group pretest-posttest design. Twenty elders were selected as an experimental group, whereas twenty-two elders were as a control group. The group movement program consisted of 35 minutes of dance and three days a week for eight weeks. Resting systolic blood pressure, diastolic blood pressure and heart rate were measured in a relaxed sitting position. The range of flexion and extension of joint was measured with a gonimeter. Life satisfaction and self-efficacy were measured by self-report. Data were analyzed through $x^2$-test, t-test and ANCOVA using SPSS/WIN program. Results: The results were as follows: Resting systolic blood pressure, diastolic blood pressure and resting heart rate decreased significantly by the group movement training. The range of flexion and extension of the shoulder and hip joints increased significantly. The scores of life satisfaction and self-efficacy increased remarkably by the group movement. Conclusion: The results suggest that group movement training can be an effective intervention to improve psychophysiological variables and the range of motion for the institutionalized elderly.

  • In order to investigate the relationship between plasma renin activity and metabolism of Ca and Na in blood pressure, the habitual dietary intakes of Na and Ca urinary excretion of Ca, Na and K, and plasma level of renin activity, aldosterone, and indices of Ca metabolism were measured in 27 untreated hypertensive women and 30 age-matched normal women on a free diet. Hypertensive subjects were classified into high renin hypertensive (HH), medium renin hypertensive(HM) and low renin showed no significant difference among normotensive, LH, MH and HM groups. It appeared that 25-(OH) Vit D3 level of HH group was significantly higher than LH group(p<0.05). There was significant difference in habitual intake of Ca between normotensive and LH groups. However, habitual intake of Na showed no significant difference among normotensive, LH, MH and HH group. Positive correlation of systolic and diastolic blood pressure with PTH(r=0.324, r=0.375) and urinary Ca(r=0.496, r=0.278) and a negative correlation of systolic blood pressure with habitual Ca intake(r=-0.371) existed(p<0.05). A relative magnitude of factors affecting hypertension was analyzed by multiple regression analysis. Overall results about relative influence of independent variables to dependent variable (systolic blood pressure) indicated that urinary Ca was the higher correlation in all subjects(p<0.0001), followed by age and aldosterone. PTH showed a significant correlation for relative influence on diastolic blood pressure in all subjects. The above results indicated that renin-aldosterone system and Ca regulating hormone had a mutual relationship in hypertension.

  • This study was desiged to investigate the effects of calcium supplementation on the metabolism of sodium and potassium and blood pressure in seven healthy college women, aged from 19 to 21 years old. For this purpose, metabolic studies were conducted for two weeks. During the first week, the subjects ate experimental diet of which nutrients composition was similar to their usual intake. And during the consecutive second week, they ate the same experimental diet supplemented with 500mg or calcium daily. The results were summarized as follows : 1) Urinary excretion of sodium was significantly increased(p<0.05), but fecal excretion and retention of it was not affected by supplementary intake of calcium. 2) Potassium balance was not changed after additional intake of calcium. 3) Serum sodium and potassium level decreased significantly(p<0.05), but aldosterone and renin levels in serum were not changed by additional intake of calcium. 4) Systolic blood pressure(SBP) was not affected, but diastolic blood pressure (DBP) was significantly decreased (p<0.05) by supplementation of calcium. The above results showed that daily supplementary intake of calcium can be effective to decrease diastolic blood pressure through inducing the change of sodium metabolism in young women eating usual Korean diets.
