Elevated Mean Platelet Volume is Associated with Presence of Colon Cancer

  • Background: Colon cancer is the second most common cancer in developed countries. Activated platelets play a key role in inflammation and atherothrombosis, with mean platelet volume (MPV) is an early marker of platelet activation. The aim of the study was to clarify the relevance of MPV in patients with colon cancer. Materials and Methods: We measured MPV levels in 128 patients with colon cancer before and after surgery, and 128 controls matched for age, gender, body mass index (BMI) and smoking status. The odds ratios (ORs) and 95% confidence intervals (CIs) for colon cancer were calculated using multivariate logistic regression analyses across MPV quartiles. Results: Patients with colon cancer had higher MPV compared with controls. Surgical tumor resection resulted in a significant decrease in MPV levels (11.4 fL vs 10.7 fL; p<0.001). A positive correlation between MPV and tumor-nodule-metastases (TNM) stage was found. Furthermore, after adjusting for other risk factors, the ORs (95%CIs) for colon cancer according to MPV quartiles were 1.000, 2.238 (1.014-4.943), 3.410 (1.528-7.613), and 5.379 (2.372-12.198), respectively. Conclusions: The findings show that patients with colon cancer have higher MPV levels compared with controls, and these are reduced after surgery. In addition, MPV was found to be independently associated with the presence of colon cancer. Further studies are warranted to assess the utility of MPV as a novel diagnostic screening tool for colon cancer.

Diagnostic Values of the Measurement of Fungiform Papillae in Patients with Burning Mouth Syndrome (구강작열감증후군 환자에서 심상유두 수 측정의 진단적 가치)

  • Objectives: This study evaluated the diagnostic values of measuring the number of fungiform papillae in patients with burning mouth syndrome (BMS). Methods: Seventy participants (35 BMS patients and 35 Healthy control) participated in this study. The number of fungiform papillae (FP) was measured on the anterior part of the tongue within an area of $9mm^2$ using a digital camera. The subjects were evaluated on their yin deficiency and blood stasis conditions using the Yin-Deficiency Questionnaire (YDQ) and Blood Stasis Questionnaire (BSQ). Moreover, the severities of subjective tongue pain in the BMS patients were assessed using Visual Analog Scale. Results: There were significant differences in the number of FP and the YDQ and BSQ scores between the two groups. The number of FP and the YDQ scores in the BMS group showed statistically significant correlations with the VAS score of tongue pain. In the bivariate logistic regression analysis, the factor that most strongly contributed to BMS was the number of FP. The optimal cut-off value of the number of fungiform papillae was calculated as 5.5 with 71.4% sensitivity and 82.9% specificity. There was no statistically significant correlation between the number of FP and the YDQ or BSQ score. Conclusions: According to these findings, measuring the number of FP could be a valuable evaluation indicator of BMS.

Basic Tongue Diagnosis Indicators for Pattern Identification in Stroke Using a Decision Tree Method

  • Objectives: The purpose of this study was to specify major tongue diagnostic indicators and evaluate their significance in discriminating pattern identification subtypes in stroke patients. Methods: This study used a community based multi-center observational design. Participants (n=1,502) were stroke patients admitted to 11 oriental medical university hospitals between December 2006 and February 2010. To determine which tongue indicator affected each pattern identification, a decision tree analysis of the chi-square automatic interaction detector (CHAID) algorithm was performed. The chi-squared test was used as the criterion in splitting data with a p-value less than 0.05 for division, which is the main procedure for developing a decision tree. The minimum sample size for each node was specified as n =10, and branching was limited to two levels. Results: From the 9 tongue diagnostic indicators, 6 major tongue indicators (red tongue, pale tongue, yellow fur, white fur, thick fur, and teeth-marked tongue) were identified through the decision tree analysis. Furthermore, each pattern identification was composed of specific combinations of the 6 major tongue indicators. Conclusions: This study suggests that the 6 tongue indicators identified through the decision tree analysis can be used to discriminate pattern identification subtypes in stroke patients. However, it is still necessary to re-evaluate other pattern identification indicators to further the objectivity and reliability of traditional Korean medicine.

The concentration of serum amyloid A and haptoglobin of pigs infected with Aujeszky′s disease virus (오제스키병에 감염된 돼지의 serum amyloid A와 haptoglobin의 농도 변화)

  • The acute phase serum protein response is a well-known general indicator of inflammation, trauma or other pathological conditions and its relevance for the monitoring of the health status of domestic animals is being increasingly realized. The changes in serum protein composition which occur after tissue damage represent a part of the systemic response of the injured animals which is mediated by pro-inflammatory cytokines such as TNF-$\alpha$, IL-6 and IL-1. These responses play a vital role in containing the tissue damage and enhancing the processes of repair and resolution. From a clinical perspective, the assay of acute phase proteins can provide a method for detecting inflammation. In animals, the most sensitive acute phase proteins are haptoglogin, serum amyloid A and at-acid glycoprotein in response to inflammatory condition. The aim of this study was to assess the diagnostic value of the concentrations of serum amyloid A(SAA) and haptoglobin(HP) in serum of pigs infected with Aujeszky's disease virus(ADV). Fifty pigs infected with ADV and 5 normal pigs were used in this experiment. The mean serum concentration of Shh of pigs infected with ADV was 96.8 $\pm$ 7.1 $\mu\textrm{g}$/㎖(range, 36.0∼187.5 $\mu\textrm{g}$/㎖) and that of normal pigs was 42.9$\pm$3.3 $\mu\textrm{g}$/㎖(range, 17.3∼127.8 $\mu\textrm{g}$/㎖). The mean serum concentration of HP of pigs infected with ADV was 1,164.4 $\pm$ 96.9 $\mu\textrm{g}$/㎖ (range, 790.2∼l,769.2 $\mu\textrm{g}$/㎖) and that of normal pigs was 675.4 $\pm$ 56.3 $\mu\textrm{g}$/㎖ (range, 650.0-690.4 $\mu\textrm{g}$/㎖). The mean concentrations of SAA and HP in serum of pigs infected with ADV compared with those of normal pigs showed approximately a two-fold. It was concluded that the concentrations of Shh and HP in serum may proved to be diagnostic marker of Aujeszky's disease.

Plant Leave as an Indicator for Pollution by Hydrocarbons and Heavy Metals in Al-Zubair City, Southern Iraq

  • The potential sources and spatial distribution of heavy metals and polycyclic aromatic hydrocarbons (PAHs) were investigated in the leaf plants of Al-Zubair city. A total of 14 samples of conocarpus lancifolius plant leaf were collected and analyzed for their heavy metals and PAHs content using inductively coupled plasma mass spectrometry (ICP-MS) and a 7890 Agilent capillary gas chromatograph (GC) respectively. Bioaccumulation factor calculation revealed the highest pollution of heavy metals , due to the activity of a petrochemical in the area. The diagnostic ratio of Ant/(Phe+Ant), BaA/BaA+Chr), In/(In+BghiP), Flu/Pyr, FlA/FlA+Pyr), FlA/FlA+Pyr), ∑LMW/∑HMW are commonly used for determining the origin and source of PAHs in various environmental media. The diagnostic ratio indicated the anthropogenic origin. PAHs with five-to-six membered rings were dominant in the plant leaf, which likely results from anthropogenic activities. The leaves of C. lancifolius have a preponderance of high molecular weight PAHs compared to low molecular weight PAHs, indicating a combustion origin (car exhaust, petroleum emissions, and fossil fuel). C. lancifolius leaves are a reliable indication of atmospheric PAHs absorption. The background level of heavy metals in the city (or the near environment) is in the order of Fe > Cu > Ni > Cr. On the other hand, the bioaccumulation in plant leaves showed greater tendencies as follows: Co>Cd>Zn=As>Cu>Mn>Ni>Pb>Cr>Fe. Cobalt showed high bioaccumulation, indicating strong uptake of Co by plant leaves. These findings point to human activity and car emissions as the primary sources of roadside vegetation pollution in Al-Zubair city.

  • This study was conducted to review data utilization plans and suggest solutions based on the need for tools that can objectively diagnose problems in rural areas. The study extracted items through prior research and was carried out as a process of reviewing data capable of constructing spatial data. The results performed in the above manner are as follows. First, prior research and expert surveys were conducted to select diagnostic items to derive details of a total of three items that should be considered for rural space plans such as regional revitalization and regeneration using regional characteristics such as environment, society, and culture. Second, as a result of listing data available for regional diagnosis, 6 types of environment, 15 types of society, and 18 types of culture were selected out of a total of 529. Finally, it was suggested to establish a national standard spatial unit by grasping the limitations of public data such as omission of spatial data and accuracy. This study is significant in that it presented implications for data utilization as well as selection of items for rural diagnosis and reviewed data utilization based on rural specialized districts of "The Act on Support for Restructuring and Regeneration of Rural Spaces" to be implemented in 2024. This is considered to be valuable as a study for sustainable rural diagnosis if processes such as spatial data construction and weight setting are carried out in the future.

Assessment of radiographic left atrial dimension and C-reactive protein in dogs with myxomatous mitral valve disease

  • Radiographic left atrial dimension (RLAD) is a valuable metric for assessing left atrial enlargement in dogs. While there have been studies on the use of RLAD and the increase in C-reactive protein (CRP) levels based on heart disease stages, there has been no prior research on the correlation between RLAD and CRP. In this study, the objective was to investigate the relationship between the rise in RLAD as myxomatous mitral valve disease (MMVD) stages advance and the increase in CRP levels with MMVD stage progression. In this study, a total of 30 small-breed dogs were included as subjects. These dogs were diagnosed with MMVD at the American College of Veterinary Internal Medicine (ACVIM) stage B1 or B2, or stage C, based on a comprehensive assessment including physical examination, thoracic radiography, and echocardiography. Measurements of VHS and RLAD were compared to assess any significant differences. There were significant differences in RLAD between dogs with MMVD ACVIM stage B1 and those with stage C. The monocytes and CRP levels showed significant differences between ACVIM stage B1, B2 and ACVIM C. Additionally, a significant correlation was observed between the RLAD and VHS measurements. This underscores the notable association between MMVD stage advancement and elevated monocyte and CRP levels. The RLAD scores exhibited a significant difference among dogs with ACVIM stages B1, B2, and C, and significant variations were also observed in monocyte and CRP levels. These results suggest that monocyte and CRP levels may be a valuable diagnostic indicator for heart disease in dogs during the diagnostic evaluation.

CT-Based Fagotti Scoring System for Non-Invasive Prediction of Cytoreduction Surgery Outcome in Patients with Advanced Ovarian Cancer

  • Objective: To construct a CT-based Fagotti scoring system by analyzing the correlations between laparoscopic findings and CT features in patients with advanced ovarian cancer. Materials and Methods: This retrospective cohort study included patients diagnosed with stage III/IV ovarian cancer who underwent diagnostic laparoscopy and debulking surgery between January 2010 and June 2018. Two radiologists independently reviewed preoperative CT scans and assessed ten CT features known as predictors of suboptimal cytoreduction. Correlation analysis between ten CT features and seven laparoscopic parameters based on the Fagotti scoring system was performed using Spearman's correlation. Variable selection and model construction were performed by logistic regression with the least absolute shrinkage and selection operator method using a predictive index value (PIV) ≥ 8 as an indicator of suboptimal cytoreduction. The final CT-based scoring system was internally validated using 5-fold cross-validation. Results: A total of 157 patients (median age, 56 years; range, 27-79 years) were evaluated. Among 120 (76.4%) patients with a PIV ≥ 8, 105 patients received neoadjuvant chemotherapy followed by interval debulking surgery, and the optimal cytoreduction rate was 90.5% (95 of 105). Among 37 (23.6%) patients with PIV < 8, 29 patients underwent primary debulking surgery, and the optimal cytoreduction rate was 93.1% (27 of 29). CT features showing significant correlations with PIV ≥ 8 were mesenteric involvement, gastro-transverse mesocolon-splenic space involvement, diaphragmatic involvement, and para-aortic lymphadenopathy. The area under the receiver operating curve of the final model for prediction of PIV ≥ 8 was 0.72 (95% confidence interval: 0.62-0.82). Conclusion: Central tumor burden and upper abdominal spread features on preoperative CT were identified as distinct predictive factors for high PIV on diagnostic laparoscopy. The CT-based PIV prediction model might be useful for patient stratification before cytoreduction surgery for advanced ovarian cancer.

  • This study tried to suggest ways to improve the indicator system to strengthen the personal information protection. For this purpose, the components of indicator system are derived through domestic and foreign literature, and it was selected as main the diagnostic indicators through FGI/Delphi analysis for personal information protection experts and a survey for personal information protection officers of public institutions. As like this, this study was intended to derive an inspection standard that can be reflected as a separate index system for personal information protection, by classifying the specific IT technologies of the 4th industrial revolution, such as big data, cloud, Internet of Things, and artificial intelligence. As a result, from the planning and design stage of specific technologies, the check items for applying the PbD principle, pseudonymous information processing and de-identification measures were selected as 2 common indicators. And the checklists were consisted 2 items related Big data, 5 items related Cloud service, 5 items related IoT, and 4 items related AI. Accordingly, this study expects to be an institutional device to respond to new technological changes for the continuous development of the personal information management level diagnosis system in the future.

Inflammation and hyponatremia: an underrecognized condition?

  • Timely diagnosis of hyponatremia is important for preventing potential morbidity and mortality as it is often an indicator of underlying disease. The most common cause of eurvolemic hyponatremia is the syndrome of inappropriate antidiuretic hormone (SIADH) secretion. Recent studies have demonstrated that proinflammatory cytokines such as interleukin (IL) $1{\beta}$ and IL-6 are involved in the development of hyponatremia, a condition that is associated with severe inflammation and is related to antidiuretic hormone (ADH) secretion. Serum sodium levels in hyponatremia are inversely correlated with the percentage of neutrophils, C-reactive protein, and N-terminal-pro brain type natriuretic peptide. Additionally, elevated levels of serum IL-6 and IL-$1{\beta}$ are found in inflammatory diseases, and their levels are higher in patients with hyponatremia. Because it is significantly correlated with the degree of inflammation in children, hyponatremia could be used as a diagnostic marker of pediatric inflammatory diseases. Based on available evidence, we hypothesize that hyponatremia may be associated with inflammatory diseases in general. Understanding the mechanisms responsible for augmented ADH secretion during inflammation, monitoring patient sodium levels, and selecting the appropriate intravenous fluid treatment are important components that may lower the morbidity and mortality of patients in a critical condition.
