선천성 낭성 폐질환의 수술적 치료 (Surgical Treatment of Congenital Cystic Lung Disease)

  • Pulmonary sequestration, congenital cystic adenomatoid malformation[CCAM], infantile lobar emphysema[ILE], and bronchogenic cysts are four congenital lesions that show abnormal cystic areas within the lung field in early life. They share similar embryologic and clinical characteristics, Therefore they are sometimes difficult to make differential diagnosis each other, and all require surgical treatment. From 1984 to 1993, 20 patients underwent surgical corrections under these diagnostic categories[10 bronchogenic cyst, 4 pulmonary sequestration, 4 CCAM, and 2 ILE] in the department of thoracic & cardiovascular surgery, Inje University, Pusan Paik Hospital. There were 9 females and 11 males, Ages ranged from 26 days after birth to 69 years. Among them 5 cases of bronchogenic cyst were found out incidentally, but remained all 15 cases were noted as symptomatic cases. Recurrent pulmonary infections, respiratory distress and cough with cystic lesions in chest film were the main characteristics of them. Computed tomography and aortography were available for diagnostic conformation. For all the cases surgical resection were performed: 1 pneumonectomy, 2 bilobectomy, 9 lobectomy, 7 cyst resection and 1 mass[extralobar pulmonary sequestration] resection. All surgical treatments were well tolerated with no physical limitation. There was no operative mortality, and only one postoperative complication[empyema thoracis]. All patients were followed up ranging from 4 months to 9 years. A clinical awareness of these related lesions is important for prompt diagnosis and effective surgical treatment.

Clinical Outcome of Breast Cancer BI-RADS 4 Lesions During 2003-2008 in the National Cancer Institute Thailand

  • To determine the clinical outcome of breast cancer BI-RADS 4 lesions and seek a more effective management guideline, we conducted a retrospective study of all BI-RADS4 patients diagnosed between 2003-2008 with follow up time not less than 2 years. A total of 392 cases of BI-RADS 4 were identified and 320 could be sub-categorised as 4a, 4b and 4c. Overall malignant positive results were 7.65, 38.7 and 58.percent, respectively. In all cases assigned to the close follow up group, no malignancy was detectable (P<0.02). The results of the study suggested that BI-RADS sub-categories have benefit for cancer diagnosis and treatment decisions of clinicians and it might be possible to set up a safe follow-up guideline in selected groups of patients to minimize un-necessary tissue biopsy for breast cancer detection.

  • We describe the framework of a data-fuelled, interdisciplinary team-led learning system. The idea is to build models using patients from one's own institution whose features are similar to an index patient as regards an outcome of interest, in order to predict the utility of diagnostic tests and interventions, as well as inform prognosis. The Laboratory of Computational Physiology at the Massachusetts Institute of Technology developed and maintains MIMIC-II, a public deidentified high- resolution database of patients admitted to Beth Israel Deaconess Medical Center. It hosts teams of clinicians (nurses, doctors, pharmacists) and scientists (database engineers, modelers, epidemiologists) who translate the day-to-day questions during rounds that have no clear answers in the current medical literature into study designs, perform the modeling and the analysis and publish their findings. The studies fall into the following broad categories: identification and interrogation of practice variation, predictive modeling of clinical outcomes within patient subsets and comparative effectiveness research on diagnostic tests and therapeutic interventions. Clinical databases such as MIMIC-II, where recorded health care transactions - clinical decisions linked with patient outcomes - are constantly uploaded, become the centerpiece of a learning system.

  • Completion of the human genome project has allowed a deeper understanding of molecular pathophysiology and has provided invaluable genomic information for the diagnosis of genetic disorders. Advent of new technologies has lead to an explosion in genetic testing. However, this overwhelming stream of genetic information often misleads physicians and patients into a misguided faith in the power of genetic testing. Moreover, genetic testing raises a number of ethical, legal, and social issues. Diagnostic genetic tests can be divided into three primary but overlapping categories: cytogenetic studies (including routine karyotyping, high-resolution karyotyping, and fluorescent in situ hybridization studies), biochemical tests, and DNA-based diagnostic tests. DNA-based testing has grown rapidly over the past decade and includes preandpostnatal testing for the diagnosis of genetic diseases, testing for carriers of genetic diseases, genetic testing for susceptibility to common non-genetic diseases, and screening for common genetic diseases in a particular population. Theoretically, once a gene's structure, function, and association with a disease are well established, the clinical application of genetic testing should be feasible. However, for routine applications in a clinical setting, such tests must satisfy a number of criteria. These criteria include an acceptable degree of clinical and analytical validity, support of a quality assurance program, possibility of modifying the course of the diagnosed disease with treatment, inclusion of pre-and postnatal genetic counseling, and determination of whether the proposed test satisfies cost-benefit criteria and should replace or complement traditional tests. In the near future, the application of genetic testing to common diseases is expected to expand and will likely be extended to include individual pharmacogenetic assessments.

Validation of a Diagnostic Model for Core Competencies at the Higher Education Institute in Korea

  • The purpose of this study is to develop and validate a model for diagnosing core competencies at the higher education level. Based on literature reviews, a potential model for core competencies at university was suggested. A tool for validating the model was composed of 24 items, which were delivered to 226 professors and administrative staffs, 730 students, and 134 graduates & external industrial experts. Five constructs (core competencies) were extracted from the data collected among professors and administrative staff responding to the importance of the items. The results of importance and performance surveys on core competencies with students were respectively 3.28 to 3.66 and 2.68 to 3.28 (4-point Likert scale). Statistical differences between importance level and performance level were found in all the sub-categories of core competencies. Borich priority determination formula and Locus for Focus Model were used for the determination of the priority of needs. Importance survey among graduates and external experts showed that the mean of each items ranged from 2.80 to 3.76 (4-point Likert scale). The overall results of the analyses suggest that the final model is appropriate for measuring the core competencies.

Fast Real-Time Cardiac MRI: a Review of Current Techniques and Future Directions

  • Cardiac magnetic resonance imaging (MRI) serves as a clinical gold-standard non-invasive imaging technique for the assessment of global and regional cardiac function. Conventional cardiac MRI is limited by the long acquisition time, the need for ECG gating and/or long breathhold, and insufficient spatiotemporal resolution. Real-time cardiac cine MRI refers to high spatiotemporal cardiac imaging using data acquired continuously without synchronization or binning, and therefore of potential interest in overcoming the limitations of conventional cardiac MRI. Novel acquisition and reconstruction techniques must be employed to facilitate real-time cardiac MRI. The goal of this study is to discuss methods that have been developed for real-time cardiac MRI. In particular, we classified existing techniques into two categories based on the use of non-iterative and iterative reconstruction. In addition, we present several research trends in this direction, including deep learning-based image reconstruction and other advanced real-time cardiac MRI strategies that reconstruct images acquired from real-time free-breathing techniques.

  • Identifying clinical pathways for disease diagnosis and treatment process recommendations are seriously decision-intensive tasks for health care practitioners. It requires them to rely on their expertise and experience to analyze various categories of health parameters from a health record to arrive at a decision in order to provide an accurate diagnosis and treatment recommendations to the end user (patient). Technological adaptation in the area of medical diagnosis using AI is dispensable; using expert systems to assist health care practitioners in decision-making is becoming increasingly popular. Our work architects a novel knowledge-based recommender system model, an expert system that can bring adaptability and transparency in usage, provide in-depth analysis of a patient's medical record, and prescribe diagnostic results and treatment process recommendations to them. The proposed system uses a set of parallel discrete fuzzy rule-based classifier systems, with each of them providing recommended sub-outcomes of discrete medical conditions. A novel knowledge-based combiner unit extracts significant relationships between the sub-outcomes of discrete fuzzy rule-based classifier systems to provide holistic outcomes and solutions for clinical decision support. The work establishes a model to address disease diagnosis and treatment recommendations for primary lung disease issues. In this paper, we provide some samples to demonstrate the usage of the system, and the results from the system show excellent correlation with expert assessments.

정보제공이 입원환자 불안해소에 미치는 효과 (Effect of Informativeness on the Anxiety of Hospitalized Patients)

  • Every patient, who enters the hospital has a potential for becoming anxious. The control of hospitalization anxiety experienced by hospital patients is considered to be an important factor in the process of recovery from illness. This study was conducted to investigate the relationship between informativeness and hospitalization anxiety in order to give basic data for psycho-social aspect of nursing care for hospitalized patients. One hundred patients admitted to Kyungpook National University Hospital during the period of Jan. to June 1975 were sampled and divided into two groups; fifty of experimental and fifty of control group. The set of informations prepared by the investigator were given additionally to experimental group while the control group only received routine informations. Both groups were rated according to the Hospitalization Informativeness Scale which consisted of 24 questionaries and 4 categories and Hospitalization Anxiety Scale one or two days prior to discharge from the hospital. The results of the study were as follows; 1 . Mean values of Hospitalization Informativeness Scale revealed significant differences at 0.01 level with experimental group showing higher mean value. Age, sex ana educational states aid not influence the mean values of Hospitalization Informativeness Scale in both groups. 2 . The length of hospitalization did not influence significantly on the mean value of Hospitalization Informativeness Scale in both groups. 3. Rank difference correlation coefficiently between mean value of Hospitalization Informativeness Scale and the importance of information the patient's perceived were revealed significant at 0.01 level in all 4 categories such as admission discharge, treatment and nursing activities, diagnostic test, diagnosis and prognosis, health teaching for the patient and family in experimental group. While, only two categories such as treatment and nursing activities and health teaching for the patient and family in control group were revealed significant correlation. 4. Mean value of Hospitalization Anxiety Scale revealed significant difference at 0.05 level with the experimental group showing lower Hospitalization Anxiety Scale. 5. Correlation coefficiently between Hospitalization Informativeness Scale and Hospitalization Anxiety Scale were revealed significant at 0.01 level in experimental group but there was no significant correlation in control group.

  • Purpose: The purpose of this study was to identify nurses' perceptions of the importance of concrete items as a basis for nursing practice. A total of 179 concrete items for basic nursing, which clinical nurses must know in order to practice appropriately, were identified. Method: The participants in this study were 225 nurses who had worked for two to five years in university hospitals. The nurses were asked to rate the importance of the 179 items (in 39 middle range categories) on a 5 point scale. Result: The mean age of the nurses was 26 and about 54% were university graduates. Of the 39 middle range categories those rated as the top ten were transfusion (4.682), IV injection (4.492), po medication (4.476), surgery (4.469), infection control (4.438), IM injection (4.413), safety (4.388), oxygenation (4.376), diagnostic test (4.366), and fluid & electrolyte balance (4.359). The categories that had the lowest scores were sexuality (3.449), conceptual nursing (3.465), spirituality (3.527) and personal hygiene (3.548). Conclusion: The results of this study show that most nurses evaluate items which they use frequently in their practice as important.

Reconstruction of Radiation Dose Received by Diagnostic Radiologic Technologists in Korea

  • Objectives: Diagnostic medical radiation workers in Korea have been officially monitored for their occupational radiation doses since 1996. The purpose of this study was to design models for reconstructing unknown individual radiation doses to which diagnostic radiation technologists were exposed before 1996. Methods: Radiation dose reconstruction models were developed by using cross-sectional survey data and the personal badge doses of 8167 radiologic technologists. The models included calendar year and age as predictors, and the participants were grouped into six categories according to their sex and facility type. The annual doses between 1971 and 1995 for those who were employed before 1996 were estimated using these models. Results: The calendar year and age were inversely related to the estimated radiation doses in the models of all six groups. The annual median estimated doses decreased from 9.45 mSv in 1971 to 1.26 mSv in 1995, and the associated dose variation also decreased with time. The estimated median badge doses from 1996 (1.22 mSv) to 2011 (0.30 mSv) were similar to the measured doses (1.68 mSv to 0.21 mSv) for the same years. Similar results were observed for all six groups. Conclusions: The reconstruction models developed in this study may be useful for estimating historical occupational radiation doses received by medical radiologic technologists in Korea.
