TANFIS Classifier Integrated Efficacious Aassistance System for Heart Disease Prediction using CNN-MDRP

  • A dramatic rise in the number of people dying from heart disease has prompted efforts to find a way to identify it sooner using efficient approaches. A variety of variables contribute to the condition and even hereditary factors. The current estimate approaches use an automated diagnostic system that fails to attain a high level of accuracy because it includes irrelevant dataset information. This paper presents an effective neural network with convolutional layers for classifying clinical data that is highly class-imbalanced. Traditional approaches rely on massive amounts of data rather than precise predictions. Data must be picked carefully in order to achieve an earlier prediction process. It's a setback for analysis if the data obtained is just partially complete. However, feature extraction is a major challenge in classification and prediction since increased data increases the training time of traditional machine learning classifiers. The work integrates the CNN-MDRP classifier (convolutional neural network (CNN)-based efficient multimodal disease risk prediction with TANFIS (tuned adaptive neuro-fuzzy inference system) for earlier accurate prediction. Perform data cleaning by transforming partial data to informative data from the dataset in this project. The recommended TANFIS tuning parameters are then improved using a Laplace Gaussian mutation-based grasshopper and moth flame optimization approach (LGM2G). The proposed approach yields a prediction accuracy of 98.40 percent when compared to current algorithms.

  • Learning disabilities (LD), also known as learning disorders, refers to cases in which an individual experiences lower academic ability as compared to the normal range of intelligence, visual or hearing impairment, or an inability to peform learning. Children and adolescents with learning disabilities often have emotional or behavioral problems or co-existing conditions, including depression, anxiety disorders, difficulties with peer relationships, family conflicts, and low self-esteem. In most cases, attention deficit and hyperactivity disorder coexists. As learning disabilities have the characteristics of a difficult heterogeneous disease group that cannot be attributed to a single root cause, they are diagnosed based on an interdisciplinary approach through medicine and education, such as mental health medicine, education, psychology, special education, and neurology. In addition, for the accurate diagnosis and treatment of learning disabilities, the diagnosis, prescription, treatment, and educational intervention should be conducted in cooperation with doctors, teachers, and psychologists. The treatment of learning disabilities requires a multimodal approach, including medical and educational intervention. It is suggested that educational interventions such as the Individualized Education Plan (IEP) and the Response to Invention (RTI) should be implemented.

  • When addressing rural housing issues, it is crucial to consider various aspects and adopt a tailored approach that takes into account the local characteristics and specific needs. Providing guidelines for different stakeholders, whether residents themselves or experts involved in decision-making, is essential for effective management and improvements. In this study, an in-depth analysis of the housing situation in the target area where the rural housing improvement project was implemented was conducted. This involved engaging with residents from three villages through in-depth interviews to gain insights into their perspectives and needs. As a result, a set of housing improvement guidelines were developed, which are specific to each stakeholder group and are tailored to address the criteria of safety, aesthetics, and living environment. These guidelines facilitate informed decision-making by residents and experts alike, ensuring that improvements align with the unique circumstances of each situation, promoting a more holistic and effective approach to addressing rural housing challenges.

  • The use of various adulterants and harmful chemicals is rapidly increasing in various sectors such as agriculture, food, and pharmaceuticals, and they are also present in our surroundings in the form of pollutants. The regular and repeated intake of harmful chemicals often adversely affects human health. The prolonged exposure of living beings to such adverse components can lead to severe health complications. To avoid the unlimited utilization of these chemical components, a sensing technology that is sensitive and reliable for low-concentration detection is beneficial. Surface-enhanced Raman spectroscopy (SERS) is a powerful method for identifying low-range concentrations of analytes, leading to great applications in molecular identification, including various diagnostic biomarkers. SERS in chemical, gas, and biological sensors can be an excellent approach in the sensing world to achieve rapid and multiple-analyte detection, leading to a new and efficient approach in healthcare monitoring.

Hyperparameter Tuning Based Machine Learning classifier for Breast Cancer Prediction

  • Currently, the second most devastating form of cancer in people, particularly in women, is Breast Cancer (BC). In the healthcare industry, Machine Learning (ML) is commonly employed in fatal disease prediction. Due to breast cancer's favorable prognosis at an early stage, a model is created to utilize the Dataset on Wisconsin Diagnostic Breast Cancer (WDBC). Conversely, this model's overarching axiom is to compare the effectiveness of five well-known ML classifiers, including Logistic Regression (LR), Decision Tree (DT), Random Forest (RF), K-Nearest Neighbor (KNN), and Naive Bayes (NB) with the conventional method. To counterbalance the effect with conventional methods, the overarching tactic we utilized was hyperparameter tuning utilizing the grid search method, which improved accuracy, secondary precision, third recall, and finally the F1 score. In this study hyperparameter tuning model, the rate of accuracy increased from 94.15% to 98.83% whereas the accuracy of the conventional method increased from 93.56% to 97.08%. According to this investigation, KNN outperformed all other classifiers in terms of accuracy, achieving a score of 98.83%. In conclusion, our study shows that KNN works well with the hyper-tuning method. These analyses show that this study prediction approach is useful in prognosticating women with breast cancer with a viable performance and more accurate findings when compared to the conventional approach.

Pyogenic granuloma of the hard palate leading to alveolar cleft: a case report

  • This case report describes a rare occurrence of pyogenic granuloma (PG) in the hard palate deviating from its typical gingival location that led to the formation of an alveolar cleft. The aggressive growth pattern of the lesion, with atypical progression from a pedunculated nodule to an alveolar cleft, raised concern. The diagnosis was based on magnetic resonance imaging and computed tomography findings, which revealed a tadpole-shaped lesion originating from the midline hard palate. The differential diagnosis included a minor salivary gland tumor. Surgical excision was performed under general anesthesia and resulted in a mucosal defect without nasolabial fistula formation or bone exposure. The palatal defect was packed with oxidized regenerated cellulose and closed with Vicryl Rapide sutures, both of which contributed to the patient's successful outcomes. Our comprehensive approach, extending across the stages of surgical planning, execution, and postoperative care, demonstrated the advantages of a multidisciplinary strategy for the accurate diagnosis and effective treatment of palatal PGs. This report makes a meaningful contribution to the existing literature on common oral lesions by emphasizing the importance of a broad differential diagnosis and a systematic approach to oral pathologies. It also raises clinical awareness of PGs with atypical presentations and the diagnostic challenge that they pose.

Developing an Integrated Acupuncture Protocol for Treating Medial Tibial Stress Syndrome: A Delphi Consensus Study

  • The present study employs the Delphi method to devise a consensus-based protocol for utilizing integrated acupuncture in treating medial tibial stress syndrome (MTSS). Twenty acupuncture experts contributed opinions across six key themes, including diagnosis, acupuncture points, additional Traditional Oriental Medicine modalities, treatment rationale, treatment duration/frequency, and integration of yoga/naturopathic therapies. Consensus, defined as a 70% agreement or higher, was reached on all themes, reflecting a collective acknowledgment of the necessity for a holistic approach to MTSS management. The final protocol includes six diagnostic criteria, six acupuncture points, one additional modality, two Traditional Oriental Medicine therapies, four treatment rationales, and six yoga/naturopathic therapies. The present comprehensive protocol offers valuable guidance for healthcare professionals seeking an integrated approach to MTSS management.

A comparative study of 3D printing and sol-gel polymer production techniques: A case study on usage of ABS polymer for radiation shielding

  • This study focuses on the comparative analysis of ABS polymer samples produced using two distinct manufacturing techniques: 3D printing and the sol-gel methods. In the first approach, ABS polymer was augmented with rare earth oxides, Er2O3 and Gd2O3, in nano powder form and fabricated into test specimens using 3D printing technology. In the second approach, identical samples were prepared via the sol-gel technique involving mold-based fabrication. Elemental content analysis revealed no significant differences between the samples produced by the two methods. The study proceeds to evaluate the gamma-ray shielding, neutron shielding, temperature resistance, and SEM/EDS pictures of ABS samples generated through both techniques. 3D printing method exhibited more favorable results in terms of structure morphology and thermal stability while there is no significant difference for radiation shielding. The results provide insights into the performance and suitability of each production method for radiation shielding applications. This research not only contributes to enhancing radiation shielding technology but also informs the selection of the most appropriate fabrication method for specific applications in nuclear technologies and diagnostic energy range in medical purposes.

수근관절 활액막염 원인 질환 진단을 위한 다중적 접근법 (Multimodal Diagnostic Approach for Synovitis of the Wrist)

  • 목적: 본 연구의 목적은 수근관절 활액막염 진단하에 활막 절제술을 시행한 환자들을 대상으로 원인 질환 진단을 위한 다중적 접근법을 제시하고자 한다. 대상 및 방법: 2000년 1월부터 2013년 12월까지 서울대학교병원에서 수근관절 활액막염 진단하에 활막 절제술을 시행한 환자 중 수술 전 수근관절 조영 증강 자기공명영상 검사를 시행한 29명의 환자를 대상으로 후향적으로 분석하였다. 이 중 17명의 환자는 수술 전 $Tc^{99m}$ 백혈구 스캔 검사를 시행하였다. 류마티스 관절염 진단 기준에 부합하는 환자의 경우, 수근관절 자기공명영상 및 조직 병리 검사 결과 중 하나 이상에서 류마티스 관절염을 시사하는 소견이 나왔다면 류마티스 관절염으로 최종 진단하였다. 만약 자기공명영상 검사와 조직 검사 소견이 시사하는 진단이 다르게 나타나는 경우에는 조직 검사 소견 결과를 토대로 확진하였다. 결과: 류마티스 관절염 진단 기준을 충족하는 9명의 환자 중 7명의 환자는 류마티스 관절염으로 최종 확진되었으며, 2명의 환자는 결핵성 관절염으로 최종 진단되었다. 류마티스 관절염 진단 기준을 충족시키지 못한 20명의 환자 중 12명은 자기공명영상 검사 및 조직 검사 결과 모두 동일한 질환을 시사하는 것으로 나왔으며 나머지 8명의 환자들은 임상 양상, 자기공명영상 및 조직학적 소견에 근거하여 5명은 비특이적 만성 활액막염, 1명은 류마티스 관절염, 2명은 결핵성 관절염으로 최종 진단되었다. 결론: 자기공명영상 검사 및 백혈구 스캔 검사는 수근관절 활액막염 원인 질환 진단에 많은 도움이 되는 유용한 검사이다. 진단이 어렵거나 약물 치료에 반응이 없는 경우에 활막 절제술을 통한 조직 병리 검사 결과는 매우 유용할 수 있다.

미만성 침윤성 폐질환에 대한 비디오 흉강경 폐생검과 개흉 폐생검의 비교 (A Comparison of Thoracoscopic and Open Lung Biopsy for the Diffuse Infiltrative Lung Disease)

  • 배경: 미만성 침윤성 폐질환의 확진을 위해 많은 환자들에게 폐생검이 시행되고 있다. 개흉 폐생검에서의 작은 절개로 얻을 수 있는 제한된 시야와는 달리, 흉강경을 통한 폐생검은거의 전 폐실질을 관찰한 후 생검할 수 있는 장점과 전통적인 개흉으로 생길 수 있는 유병률을 감소시킬 수 있다는 장점을 가지고 있다. 본 연구에서는 미만성 침윤성 폐질환의 진단에 있어, 흉강경 폐생검(TLB)과 개흉 폐생검(OLB)을 효율성과 안전성의 측면에서 비교하였다. 대상 및 방법: 1993년 3월부터 1997년 8월까지 81명의 환자에게 폐생검을 시행하였으며, 이 중 51명에게 기존의 개흉 폐생검을, 30명에게는 비디오 흉강경 폐생검을 시행하였다. 결과:수술 시간은 TLB에서 63분, OLB에서 79분이었고(p=0.04), 생검 조직 절편의 크기(TLB 7.8 cm3, OLB 6.9 cm3 : p=0.72)와 진단률(TLB 100%, OLB 96%)은 두 군간의 차이가 없었다. 술후 재원 일수는 TLB에서 OLB보다 유의하게 짧았고(TLB 13일, OLB 22일 : p=0.01), 흉관 거치일, 진통제 투여 횟수는 차이가 없었으나 진통제 투여 기간은 TLB에서 유의하게 짧았다(TLB 2.5일, OLB 5.2일, p=0.05). 합병증은 TLB에서 2례(6.67%), OLB에서 4례(7.84%)가 있었으며, 두 군 모두에서 수술로 인한 사망례는 없었다. 결론: 흉강경 폐생검이 미만성 침윤성 폐질환의 진단에 있어서, 기존의 개흉 폐생검의 훌륭한 대안이 될 수 있다고 생각한다.
