척추관 협착으로 인한 신경원성 파행 및 하지 근력 저하에 대한 한방복합치료 치험 1례 (A Case of Combination of Korean Medicine Treatments in Neurogenic Claudication and Lower Extremity Weakness due to Spinal Stenosis)

  • Background: To suggest potential of Korean medicine treatments as a conservative management for neurogenic claudication and lower extremity weakness due to spinal stenosis. Case Summary: The patient suffered weakness, pain and numbness of the right leg and difficulty walking with diagnosis of spinal stenosis due to herniated lumbar intervertebral disc. Korean medicine treatments, including herbal medicine, acupuncture, pharmacopuncture and Chuna manual therapy were applied. The Numeric Rating Scale (NRS) of pain and numbness in the right leg decreased from 7 to 4, with an increase in strength of the right leg from 60% to 95% compared to the strength of left leg. Walking duration also increased from less than one minute to more than five minutes. Conclusion: Korean medicine treatment may be considered as an effective conservative management for symptoms of spinal stenosis.

요추 수술 후 환자의 재활 (Postoperative Rehabilitation of Lumbar Spine)

  • We reviewed literatures of western and oriental medicine about postoperative management of spine. Traditionally, with orthoses and bed rest, many physicians restricted activity of daily life and back exercises of patients who had taken spine surgery. More aggressive early mobilization, however, such as strengthening exercise and stretching after 4 weeks of surgery gets grounds nowadays. Physical therapies including manipulation, TENS, MENS, ultrasound and cryotherapy are being used as helpful treatment modalities of postoperative pain and swelling. Failed back surgery syndrome occurs due to wrong patient, diagnosis and surgery and is managed with conservative treatment or reoperation. In oriental medicine, treatments of musculoskeletal injuries including surgical wound are based on the balance of chi(氣) and blood(血).

한방병원에 입원한 두위 현훈 환자의 임상 분석 (Clinical Analysis on the Positional Vertigo Patients treated in an Oriental Medical Center)

  • Background : Vertigo is a very common complaint in clinical practice. The number of patients who complain of vertigo has been increasing due to rapid growth of the elderly population. This study was designed to review the clinical features and success of oriental medical treatment of positional vertigo. Methods : This observation was made on 70 subjects diagnosed with positional vertigo. They were hospitalized in the Semyung University Oriental Medicine Hospital. Results : The results were as follows 1. Female patients (82.9%) were more than male patients. The most common age group was 8th decade and the patients rapidly increased after the 5th decade of age. 2. The most common past history of positional vertigo was hypertension, the second was cerebral infarction, and the third was hyperlipidemia. 3. In the oriental medicine diagnosis, phlegm-dampness syndrome (痰濕交阻 眩暈) was the most common disease (92.9%), and in western medicine, BPPV was the most (82.9%). 4. In herbal medication, Banhabaekchulchunma-tang gami (45.7%) was the most commonly used, the second being Taeksa-tang (30%). 5. The vertigo score of 94.3% patients improved and none got worse. 6. In general characteristics, men over 64 years improved best. Past history had no effect on the improvement of vertigo. 7. The sooner patients visited hospital after onset, the shorter hospitalization time was. Conclusions : Vertigo attacks patients well in advanced age, with various causative diseases. Oriental medical treatments have considerable effects on positional vertigo, especially treated with Banhabaekchulchunma-tang gami and Taeksa-tang.

  • The Meridians are the pathways of the qi and blood, and can be used as the media of the diagnosis and treatments. There has been numerous studies about the meridians, but as the kinesiology develops recently and the attention for the meridian muscles is growing, the meridian muscles are now recognised as the complementary entity of the meridians. In this document, we considered the meridian muscles and the meridia, following the traditional thoughts of the oriental medicine, as the images of the mountains and the rivers.

소아 근시에 대한 임상 연구 고찰 - 최근 중의학 문헌을 중심으로 - (Clinical Trial Study for Childhood Myopia in Traditional Chinese Medical Journals)

  • Objective : The aim of this study was to investigate treatment of childhood myopia in current Traditional Chinese Medical Journals. Material and Method : We searched for treatment of childhood myopia in all Traditional Chinese Medical Journals published from January 2001 to October 2005. Results : Thirteen articles were selected and 7 of them showed total improvement rate was higher than 90% in childhood myopia. Treatment methods were acupuncture, auricular acupuncture, moxibustion, herbal medicine, adhesive plaster. Conclusion : Oriental medical treatments for childhood myopia were presumed to be effective in most trials. But there remains a debate about diagnosis and outcome criteria. Further studies are needed.

  • Bibliographic study on pediatric Chuna in the Myung Chung dynasties resulted as the following. 1. Pediatric Chuna experts in Myung dynasty utilized methods of Chuna to a great extense as means of preventing and treating various kinds of pediatric disorders. 2. Pediatric Chuna experts in Myung dynasty put emphasis on diagnosis through the doctor's inspection. 3. Pediatric Chuna experts in Myung dynasty developed and applied special acupoints for pediatric Chuna. 4. Pediatric Chuna experts in Myung dynasty put emphasis on replenishing and discharging according to hand manipulation. 5. Pediatric Chuna experts in Myung dynasty developed and applied methods of massaging with herbal medicine. 6. Pediatric Chuna experts in Myung dynasty put methods of Chuna into songs for handier use. 7. Pediatric Chuna experts in Chung dynasty corrected the errors of the predecessors and further developed the handed down advantages. 8. Pediatric Chuna experts in Chung dynasty specifically combined the theories of Chuna with characteristics of pediatric physiology. 9. Pediatric Chuna experts in Chung dynasty described treatments by symptoms systematically. 10. Pediatric Chuna experts in Chung dynasty, criticizing the trend at the time of despising pediatric Chuna, endeavored to spread pediatric Chuna. 11. Pediatric Chuna experts in Chung dynasty arranged the relationship between hand manipulation of Chuna and herbal medicine. 12. Pediatric Chuna experts in Chung dynasty arranged various kinds of methods for diagnosis, including diagnosis by palpation of the chest and the abdomen. 13. Pediatric Chuna experts in Chung dynasty combined the method of multiple hand manipulation and method of basic hand manipulation. 14. The theories of pediatric Chuna before Myung Chung dynasties only put emphasis on Ki and Blood, not combining with the basic principles of Korean medicine like Eum/Yang and Five Phases. The scholars of Myung Chung dynasties came to successfully combine the principles of Korean medicine like stability theory, Jang and Bu (internal organs) theory, and Eum-Yang theory with the theories of pediatric Chuna. This combination best characterizes the theories of pediatric Chuna in the Myung Chung dynasties.

수근관 증후군 환자 2례에 대한 증례보고 (The clinical study on 2 cases of patients of carpal tunnel syndrome)

  • Carpal tunnel syndrome is a common peripheral nerve entrapment syndrome that is characterized by pain, numbness, sensory disturbance along the distribution of the median nerve in hand. Diagnosis mainly depends upon careful examination and symptomatology. Treatments have included wrist immobilization, anti-inflammatory drug, local injection of steroid, nerve block and surgical decompression. This is a clinical report about 2 cases of carpal tunnel syndrome patients who undergo oriental medical treatment. Both of two cases, Symptoms were disappeared and physical examinations were improved. The results of this study demonstrate that oriental medicine treatment that including acupuncture and herbal medicine therapy can have noticeable effects in treating the carpal tunnel syndrome.

청신경초종 수술 후유증 환자 증례 1례 (Clinical study on a case of a patient with sequelae caused by removal of vestibular schwannoma)

  • The purpose of this study is to develop an aproach to diagnosis and treatment of sequelae after removal of vestibular schwannoma through Oriental Medicine. An eighty year-old woman with sequelae after removal of vestibular schwannoma was observed and treated. She had difficulty of hearing, facial nerve palsy, dizziness and was generally weak. She was treated with herb medications for these symptoms. Having been treated for 27 days in hospital her symptoms improved. This report provides evidence for application of herbal medicine in treatment of sequelae. However, more cases are required for an in depth study of oriental treatments for this ailment in order to open this method of treatment to general application.

  • 1. Objectives FUO is defined as a temperature higher than $38.3^{\circ}C$ on several occasions and lasting longer than 3 weeks, with a diagnosis that remain uncertain after 1 week of investigation. Diagnosing the cause of FUO is particularly difficult. 2. Methods We have tried researching effect of herbal medicines(Kalkunhaeki - tang treatments) and Acupoint Injection to Taeumin FUO patient. 3. Results A Taeumin patient diagnosed to FUO was treated with Taeumin herbal medicines(kalkunhaeki - tang treatments) and change of patient's fever and general condition were improved. 4. Conclusions The constitutional treatment with Kalkunhaeki - tang may have an effect on management of FUO. So we report the healing process and results of this patient in this study.

형상의학(形象醫學)에 대한 논문 고찰 (Review on Hyungsang Medicine)

  • Objectives: Hyungsang medicine (HM) is an inspection which emphasizes the relationships between pathological patterns and one's facial or bodily shape, color and movement. The purpose of this study was to review previously published study results of the HM. Methods: The authors reviewed a total of 51 HM-related studies published from 2000 to 2012 found on PubMed and various domestic Oriental medicine journals. Each study was classified into theoretical, diagnostic, or treatment-related category. Results: Theoretical studies were mostly based on the Nei-Ching and Dong-Eui-Bo-Gam texts, and dealt with differences in form between types such as masculine and feminine, the five organ image types, the four-type categorization of body essence, vital energy (Qi), mentality, and Blood, Gallbladder and Bladder, the four-type categorization of fish, bird, horse, and turtle types, and the six meridian types. Research on diagnosis has been performed on diagnostic characteristics of HM, correlation between HM and general coordinative manipulation (GCM), and Hyungsang medicine and ante-disease pattern (未病類型). Studies on treatments could largely be classified as treatments for specific diseases using certain acupuncture or herbal prescriptions based on HM. Treatments were mostly evaluated solely through subjective symptom improvement of patients. Conclusions: Our review results suggest that HM-related studies were focused on the constitutional characteristics and clinical utility of HM. To strengthen the theoretical basis of HM and its clinical utility, clinical trials including randomized, treatment-placebo and blind methods are needed.
