Study on the Medical Records of ${\ll}$Gup Yu Bang${\gg}$ ((${\ll}$급유방(及幼方)${\gg}$에 수록(收錄)된 의안(醫案)에 대한 연구(硏究))

  • ${\ll}$Gup Yu Bang${\gg}$ is the first korean book that specialized in pediatrics. It was written by Jo Jeong-jun in 1869 and contains 87 medical records. Medical records is one of the best way to develop one’s abilities of curing a disease without clinical practice and understanding medical condition of the time. These days we are fully aware of the importance of medical records, but there is no sufficient paper or study on medical records. On this study, we investigate 87 medical record of ${\ll}$Gup Yu Bang${\gg}$ by statistical methods. It was analyzed on the base of items such as age, sex, disease, method of medical treatment, treating prescription, modified medicinal substance, progress of disease. ${\ll}$Gup Yu Bang${\gg}$ contain 87 medical records and among of them, 84 records were Jo Jeong-jun own writings of clinical case. Systemic division of disease showed the highest in digestive disease and most frequent case was symptom of vomiting and diarrhea. The distribution of age was showed the highest in the age group under 5 years and the number of male case was more than female case. It showed that he considered observation of the patient's expression in diagnosis methods and have used 87 kinds of treating prescription and the external treatment, acupuncture, moxa. It showed that he quoted a lot of treating prescription from ${\ll}$Dong eui bo gam${\gg}$, ${\ll}$Yi-Hak-Yip-Mun${\gg}$ and ${\ll}$Xiao Er Yao Zheang Zhi Jue${\gg}$. The further study on medical records in traditional medical literatures, would reveal the developmental progress of Korean pediatrics and inform more actual proof on medical condition of the time.

Investigation of Genetic Evidence for Sasang Constitution Types in South Korea

  • In Sasang constitutional medicine, both disease susceptibility and drug response are considered to be related to the characteristics of an individual's physiology and psychology: a theory which is central to traditional Korean medicine. Based on such observable characteristics, Sasang constitutional medicine classifies people into four constitutional types. Genetic studies of Sasang constitution would help reveal the inheritance patterns and models of the typological traits and, moreover, help with traditional medical diagnosis and treatment. To investigate the heritable aspect of Sasang constitution, we collected various pedigrees from South Korea. The study population has 101 pedigrees composed of 593 individuals. The determination of the Sasang constitution type of each individual was performed by doctors who diagnose the Sasang constitutional type of individuals as part of their professional practice. We calculated estimates of familial correlation and heritability. Parent-Offspring pairs showed the strongest familial correlation of Sasang constitutional type, with the correlation values of 0.21 and 0.28, followed by sibling pairs with the value ranging between 0.14 and 0.25. From the heritability analysis conducted with the Variance-Component method, the heritability of TE (Tae-Eum) type, SY (So-Yang) type, and SE (So-Eum) type were 55%, 41%, and 47%, respectively. This pattern of heritability was consistent with different set of analyses, which suggest the robustness of our result. Our result clearly shows that the Sasang constitution type is heritable, and further genetic analysis based on our result will shed light on the biological mechanism of Sasang constitution.

The Research about the Classification System Improvement and Cord Development of Korean Classification of Disease on Oriental Internal Medicine (한국표준질병사인분류중 한방내과영역의 분류체계 개선 및 진단명 구성에 관한 연구)

  • Objectives : It is necessary that the international classification of diseases (ICD) be examined in order to comprise the third revision of the Korean Classification of Disease on Oriental Medicine (KCD-OM) and disease classification in the oriental internal medicine field. It is essential that the selection, classification and definition of disease and pattern names of oriental concepts in internal medicine be clear. Since 2008, the fifth revision of the Korean Classification of Disease (KCD-5) has been used in Korea. It was required to use the reference classification from the Oriental medicine area based on the ICD-10. Methods : In this review, the necessity for, meaning of and content of the third revision are briefly described. The ICD system was reviewed and KCD-OM was reconstructed. How diagnosis in the oriental internal medicine area had changed is discussed. Review and Results : In 1973, the disease classification of oriental medicine was established the basis on the contents of Dongeuibogam. It was irrespective of the ICD. As to the classification system in the Oriental internal medicine field, systemic disease was comprised of wind, cold, warm, wet, dryness, heat, spirit, ki, blood, phlegm and retained fluid, consumptive disease, etc. Diseases of internal medicine comprised a system according to the five viscera and the six internal organs and followed the classification system of Dongeuibogam. The first and second revisions were of the classification system based on the curriculum in 1979 and 1995. In 1979, in the first revision, geriatric disease and idiopathic types of disease were deleted, and skin disease was included among surgery diseases. This classification was expanded to 792 small classification items and 1,535 detailed classification items to the dozen disease classes. In 1995, in the second revision, it was adjusted to 644 small classes and 1,784 detailed classification items in the dozen disease classes. KCD-OM3 did KCD from this basis. It added and comprised the oriental medical doctor's concept names of diseases considering the special conditions in Korea. KCD-OM3 examined the KCD-OMsecond revised edition (1994). It improved the duplex classification, improper classifications, etc. It is difficult for us to separate the disease names and pattern names in oriental medicine. We added to the U code and made one classification system. By considering the special conditions in Korea, 169 codes (83 disease name codes, 86 pattern name codes) became the pre-existence classification and links among 306 U codes of KCD-OM3. 137 codes were newly added in the third revision. U code added 3 domains. These are composed of the disease name (U20-U33, 97 codes), the disease pattern name (U50-U79, 191 codes) and the constitution pattern name of each disease (U95-U98, 18 codes). Conclusion : The introduction of KCD-OM3 conforms to the diagnostic system by which oriental medical doctors examine classes used with the basic structure of the reference classification of WHO and raises the clinical study and academic activity of the Korean oriental medicine and makes the production of all kinds of nation statistical indices possible. The introduction of KCD-OM3 promotes the diagnostic system by which doctors of Oriental medicine examine classes using the association with KCD-5. It will raise the smoothness and efficiency of oriental medical treatment payments in the health insurance, automobile insurance, industrial accident compensation insurance, etc. In addition, internationally, the eleventh revision work of the ICD has been initiated. It needs to consider incorporating into the International Classification of Diseases some of every country's traditional medicine.

Wheel Balance Cancer Therapy in the Treatment of Metastatic Gastric Carcinoma : A Retrospective Analysis of 62 Patients (수레바퀴 암 치료법을 시행받은 진행성 위암환자 62명에 대한 후향적 코호트 분석)

  • Backgrounds : Gastric cancer is one of the most frequent causes of cancer-related deaths worldwide. Gastric cancer patients frequently exhibit distant metastasis at the time of diagnosis and at present, there is no standard regimen after operation or chemotherapy to prevent metastasis and recurrences. Aims : The objective of the study was to compare the overall survival and disease free survival in patients treated with wheel balance therapy (WBT) with patients treated with WBT alone or the combination of WBT and conventional therapy and see if there were any significant improvements in survival between the two groups. Setting & Design : East-West Cancer Center at Daejeon University: retrospective study. Materials & Methods : We retrospectively analyzed 62 patients with stage III (40 patients) and stage IV (22 patients) gastric cancer who have received operation or chemotherapy 2 months prior to beginning WBT. The patients followed the WBT protocol which includes herbal medicine such as PSM capsules, OnePlus syrup, or HAD capsules which prevent metastasis and recurrences. Overall survival and disease free survival were analyzed and the patients were treated for 89.55 weeks and 49.27 weeks (median value) for stages III and IV, respectively. Statistical analysis used : Overall survival and disease free survival were estimated using the Kaplan-Meier method and the WBT and combination treatment arms were compared using the log rank test. Results : For stage III, the 3 year overall survival was 78.5% and disease free survival was 78.3%. Metastasis and recurrence occurred in 13 of 40 patients (32.5%). For stage IV, the 2 year overall survival was 18.2% and disease free survival was also 18.2%. Metastasis and recurrence occurred in 19 of 22 patients (86.4%). No significant difference was found statistically between the WBT alone and combination treatment arms though the combination regimen showed superiority (overall survival p=0.5093, disease free survival p=0.5175). Conclusions : WBT yielded satisfactory results in prolonging survival and preventing metastasis and recurrence in gastric cancer patients. The major treatment unit of WBT is composed of herbal medications (HAD, PSM capsules, OnePlus syrup) and a randomized, prospective study should be carried out using only this treatment unit in the future.

  • After parturition, woman in childbed's health can grow worse. Because when woman in childbed gives birth, Genital organ can be damaged, and lost a lot of blood, and also consume a lot of physical strengths. So, after parturition, woman in childbed is easy to fall in disease if do not take care in health specially. So after parturition, woman in childbed should be careful in stress, sleep, food etc. This thesis described Postpartum Spasm[痙病], Oppressive Feeling and Dizziness[鬱冒], Constipation[便秘], Abdominal Pain[腹痛], Diarrhea[下利], Annoyance[煩亂] and Vomitting Sickness[嘔逆]. Postpartum Spasm[痙病]'s major symptom is rigided body. This disease corresponds to Tetanus in modem medicine, and bacterial infection is assumed for cause. By Oriental medicine, after parturition, when blood is lacking, if patient takes wind, this disease breaks out. Oppressive Feeling and Dizziness[鬱冒]'s major symptom is dizzy head and gloomy eye. After parturition, when blood is lacking, if patient sweats too much and take cold, this disease breaks out. After parturition, if patient consumes a lot of resins of human body, Constipation[使秘] breaks out. There are four causes to Abdominal pain[腹痛]. First, due to deficiency and cold treats by Danggwisaenggangyangyuktang(當歸牛薑羊肉湯). Second, due to stagnation of Gi(氣) treats by Jisiljakyaksan(枳實芍藥散). Third, due to blood stasis treats by Ha-eohyeoltang(下瘀血湯). Fourth, due to lochiorrhea treats by Daeseunggitang(大承氣湯). After parturition, Annoyance[煩亂] and Vomitting Sickness[嘔逆] due to fever of difficiency treat by Jukpidaehwan(竹皮大丸). After parturition, Febrile diarrhea[熱性下利] treats by Baekduonggagamchoagyotang(白頭翁加甘草阿膠湯). This disease is similar with present Bacillary Dysentery. After parturition, Exteroceptive disease due to taking wind treats by Yangdaltang. Due to wind and fever treats by Jukyeoptang(竹葉湯).

  • It is different from the DongEuiBoGam(東醫寶鑑) of Korea traditional medicine that China BeunJungLonChi(中醫辨證論治) was made for the scientific and systematic assortment of China medicine. Symptom(症)is more important than symptom complex(證)in china BeunJungLonChi at DonEuBoGam. And unusual symptoms generally are diseases in DongEuiBoGam. DongEuiBoGam is emphasis on body form, temper, body color, pulse(形氣色脈) and the functional difference of organs by body form and body color. there are many problems in order to unify the theory of Korea traditional medicine by china BeunJungLonChi. I suggest that a new assortment that can combine Sasang constitutional medicine(四象醫學), Constitution-Acupuncture(體質針), and Hyungsang medicine(四象醫學) will be necessary. BeunJungLonChi : diagnosis and treatment based on overall analysis of symptoms and signs, the cause, nature and location of the illness and the patient's physical condition according to the basic theories of traditional Chinese medicine

A Comparative Study on the Concepts of the Chuna(推拿) (추나(推拿)의 개념 비교연구)

  • The Chuna(推拿) in Korea has been developed since 1990's. Korean Chuna which is the only one manual therapy done by medical doctor is riched by absorbing other nation's manual therapy like Chinese Tuina(中國推拿) and Chiropractic therapy of U.S.A. So, We find that Korean Chuna's peculiarity and development by comparing Chinese Tuina, Chiropractic of U.S.A and Japanese manual therapy which influenced establishment of Korean Chuna. We compared each manual therapy's history, development, diagnosis, correction and treatment of human body, frequently contacting disease and medical service of the present state. Korean Chuna has absorbed other nation's manual therapy and advantage of preserving muscle-skeleton and spinal disorder disease. But, for more prosperity, it needs union of various conception and diagnosis and more research and application to more various disease, for example in internal medicine, gynecology or pediatry and founding system as regular academic work inside the college the possibility which objection and it will practice more completeness. And as one of specialty apprentice doctor subject inside the hospital, the possibility of raising a quality improvement of study and research environment.

The Clinical Characteristics and Pharmacologic Treatment of Neuralgic Pain in Maxillofacial Region (악안면 신경통성 동통의 임상적 특징과 약물 치료)

  • This study was performed to provide the information on the clinical characteristics of the most common paroxysmal pain disorder in maxillofacial region, trigeminal neuralgia, and the effects and side effects of carbamazepine. The patients who visited Orofacial Pain Clinic, Dept. of Oral Diagnosis, Seoul National University Dental Hospital for treating paroxysmal pain were studied by history taking, clinical examination, and radiography. Sixty-two patients(male 20, female42) without any clinical and radiological abnormalities were Included. The change of pain, blood tests, and side effects were investigated periodically after administration of carbamazepine. The obtained results were as follows : 1. Almost all patient with trigeminal neuralgia were over the age of forties and it was more common in women. 2. Trigeminal neuralgia was more right sided and the involved nerve was in the order of maxillary n., mandibular n., and ophthalmic n. 3. The mean duration of suffering was 20.7 months. Eighty percent of patients had apparent trigger area. 4. The duration of pain attack was in the older of several seconds, 1 min. to 5 min., more than 10 min., and 5 min. to 10 min. The frequency of pain attack was in the order of more than 10 per day, 6-10 per day, and 1-5 per day. 5. The clinic the patients had visited for reducing neuralgic pain was in the order of dental clinic, neurology, oriental medicine, otolaryngology, and pharmacy. 6. Unnecessary dental treatments for reducing neuralgic pain were performed in 41.9% of the patients. Almosit all treatments were irreversible ones such as endodontic treatment and tooth extraction. 7. The initial mean VAS was 8.6, but it was decreased to 3.8 after 1 month, to 2.7 after 2 months. Almost all patients showed decreased pain with 200-600mg/day of carbamazepine to 6 months. 8. WBC counts, especially neutrophil counts, was decreased in 1 week after administration of carbamazepine but reached initial level after 1 month. SGOT, SGPT, and creatinine did not show any significant change. 9. Blood pressure was not changed significantly after administration of carbamazepine. 10. Almost patients did not show any apparent side effects, but drowsiness, dizziness, skin itching, constipation, and gastric irritation were occurred in some patients.

Review on Prevetive Treatment Disease Program promoted in China (중국의 치미병사업에 관한 고찰)

  • Objective : In Oriental medicine, the concept of Preventive Treatment Disease(治未病, PTD) is intended to actively deal with changes in modern disease patterns due to increase in chronic diseases. In China, preventive health service program based on PTD Theory are being carried out aimed at improving and/or preventing people's health. Method : For the introduction of PTD program based preventive service in Korea for the promotion of people's health, I would like to consider its possibility by reviewing the PTD program being conducted in China. Results : China's preventive health service programs based on PTD Theory started in 2008 for the purpose of promoting people's health and reducing medical expenses by providing medical services tailored to individuals. Regarding the effects of PTD program based preventive health service, improvement of discomforting symptoms comprised 73.04% of responses. As to service items, health guidance, Traditional Chinese Medicinal diagnosis, and preparation of health records were answered as being important. The importance of food and internal medicines for preventing and/or curing PTD Theory are also recognized. Also, as to satisfaction level, 90.64% responded as being satisfied in the order of the level of service providers, their attitudes, service processes, items, costs, and environments. Conclusion : As shown above, according to assessments on China's PTD programs, the PTD Theory is being applied to actual public health programs with highly effective results. Hence, it can be seen that such attempt could also be implemented in Korea as part of a scheme for promoting health by means of Korean medicine health promotion programs. However, further researches into concrete implementation schemes will have to be developed in the future as medical systems are different in Korea from in China.

Implement Traditional Korean Medical Information System of Hypertension through Building Database of Advanced Research Articles (선행연구자료 데이터베이스 구축을 통한 한의학 고혈압 정보 시스템 개발)

  • Objectives: Hypertension is the highest ratio among chronic disease in Korea, and the western medical information about hypertension is provided by many web sites. Advanced researches about hypertension have been conducted in Traditional Korean Medicine(TKM) for decades, but the research results are not arranged. So the results have not been utilized in following research nor contributed to the expansion of public knowledge. Methods : We did this study to improve this situation. In this study, we built database about advanced research articles related hypertension in TKM and implemented TKM information system of hypertension. Results : First, we benchmarked hypertension information systems and designed the TKM information system based on the benchmarking results and comments from TKM doctors. And it was composed of introduce, treatment, and etc. Second, we built prescription, herb, acumoxa, qigong, prevention/management, and pill database which is about 600 data extracted from papers and books. Third, we implemented JAVA/JSP based web information system which provides the database. And we created links for the each papers and books to use more easily. Conclusions : If we provide the research results about TKM hypertension diagnosis and combinational medication of western and oriental medicine, this information system will be more useful. And if we add internal and external project report about hypertension, it will be more worthy.
