A Systemic Review of Studies for Traditional Chinese Treatment of Intestinal Metaplasia - Research using CNKI (장상피화생(腸上皮化生) 치료에 대한 최신 임상 연구 동향 - CNKI 검색을 중심으로)

  • Objectives: The purpose of this research is to investigate recent clinical studies of Intestinal Metaplasia (IM) based on a search of the China National Knowledge Infrastructure (CNKI). Methods: We investigated recent clinical studies of Oriental medicine therapies in traditional Chinese medical journals for IM through a CNKI search. 20 clinical articles published from 2010 to 2016 were analyzed. This study examined the authors, publication years, types of studies, criteria for diagnosis and evaluation, periods, purposes of study and articles classified by methods of treatment. Results: Most of articles were classified as randomized controlled trial. IM was diagnosed using gastroscopy, pathological examination and clinical symptoms in a high proportion of articles. The criteria for evaluation most frequently used were the effective ratios of clinical symptoms, gastroscopy and pathological tests. The methods of treatment in all studies were Chinese herb medicine. Acupuncture, acupoint injection therapy, and embedding therapy were also used in several studies. Conclusion: In this study, the Chinese treatment for IM was markedly effective. To improve the treatment of IM in Korea, many clinical studies and case reports are required, based on Chinese medicine.

A Study on Association of DSOM Symptom Scores for Women infertility in Oriental Medicine (한방진단시스템을 이용한 여성불임(女性不姙)의 한의학적 인자(因子) 연구)

  • Purpose : We have carried out this study to investigate association of DSOM scores and women infertility in oriental medicine for systematization diagnosis and treatment of women infertility. Methods : We analyzed the results of medical records for 151 patients who were diagnosed as women infertility. We investigated the possession of symptom index of women infertility, recognition association of DSOM. Results : 1. In comparison with 151 clinical trial's data. deficiency of Yin(陰虛), coldness(寒), spleen(脾) are associated positively and deficiency of Yang(陽虛), phlegm(痰) are associated negatively. 2. In pathogenesis(病機) indicator of average, cold inside the body is associated positively and diarrhea, kidney are associated negatively. 3. In pathogenesis five score-point of value, Heat(熱), kidney(腎) are associated negatively. 4. In pathogenesis(病機) indicator. constipation(便秘) is associated positively and hardness of lower abdomen, tingly weakness of lower back and knee question are associated negatively. 5. In 140 questions, nine questions are associated positively and ten question are associated negatively.

  • The followings are the conclusions drawn from the clinical cases of apoplexy; The main cause of apoplexy is the deficiency of both Jung and Ki. dam typed persons are stricken with paralyses because of the insufficiency of the kidney water. On the other hand, bangkwang typed persons' apoplexy comes from the deficiency of Ki and dump-phlegm. The prevention of apoplexy is very important so that porpe medical care should be taken at the appearance of premonitory symptoms like vertigo, dim sight tinnitus, stiff neck, numbness and others. It appears very reasonable both clinically and pathologically that Li dongyuen classified the apoplexy in to three groups : the first group is apoplexy involving meridians ; the second. involving Bu ; the third, involving Jang. The accurate diagnosis of apoplexy regulates com prehension consideration of four factors configuration color, pulse and symptoms and distinction from the similar diseases. Apoplexy is the up wand floating of Yaug in deficiency due to the deficiency of genuine Yim. In its early stages it should be treated by eliminating the excess in the upper pant. In the lower pant becomes the fundamental treatment.

  • Observing after enforcing the medication and the acupuncture theraphy on the CVA patient sick with the cerebral embolus by organic pathology of the heart and whose desease was from heat symptoms caused by exessive pathogenic factors of which caused from affection due to exogeneous factors, we obtained the results as follows; 1. Serious condition could be relieved by removing acute symptoms according to the principle of the 'In emergency causes treat the acute symptoms first, when these being relieved treat it's fundament cause'. 2. The lesion could be disable for which the traditional medication theraphy through the differentiation of symptoms-complexes had removed the symptoms caused by organic pathology of heart. 3. The acupuncture stimulation through the differentiation of symptoms seemed to influence electric conduction system. Through this case, If we objectify the diagnosis and the medical treatment, the part of the curing diseases can progress more effectively.

A Study on the Correlation between Iris Markings and Obesity Indices (홍채표지와 비만지표와의 상관성 연구)

  • Objectives This study analyze the correlation between iris markings and the obesity level shown in body composition indices. And It quantitatively evaluates iris markings according to the level of obesity to improve a clinical application plan for the iris analysis. Methods This study analyzed the iris and body composition through the quantization of iris markings for 28 female patients. It calculated descriptive statistics by using SPSS 13.0 and investigated correlation coefficients of non-parametric Spearman in order to analyze the relation between iris markings and body composition indices. Results Considering the correlation between iris markings and body composition indices, an area of the pupil had a significant positive correlation with height and with fat-free amount such as body water, protein, and minerals. BMI showed significant positive correlations with the length of the autonomic nerve wreath and the ratio of the autonomic nerve wreath, and the area of the autonomic nerve wreath. Moreover, the number of lacunas in the pancrea area had a significant positive correlation with weight and BMI. Conclusions This study objectifies the quantification of iris markings to identify the correlation between iris markings and the level of obesity. The study results will be used as a basic data to make standards for the diagnosis of obesity, the treatment of related diseases, and the diagnosis of iris markings for prevention.

A Case Study on Atopic Dermatitis with the Treatise on Febrile Diseases (상한론 처방을 이용한 아토피성 피부염 환자 1례 보고)

  • Introduction : Atopic dermatitis is very epidermic disease in these days. Atopic dermatitis is difficult for patient to be treated because there is no specific treatment. The oriental method of atopic dermatitis is varied as patient situation and doctor's view point. The Treatise on Febrile Diseases(傷寒論) may be a hint for atopy disease. Object: Although we use various method such as acupucture, aroma therapy, a medicine for external use, we use the two decoctions in the Treatise on Febrile Diseases according to differentiation of syndromes and abdominal diagnosis. Result : After inpatient treatment in 21 days, the itching, redness, exudation, crust, lichen get better and insomnia and constipation was removed quickly. The SCORAD index is decreased from 84.2 to 25.9. Conclusion : These decoction can treat the atopic dermatitis although there is no symptom in atopy because we cure the all situation of the patient. We can cure the atopic dermatitis not only by routine remedy for various symptoms but also by diagnosis and treatment based on overall analysis of symptoms and signs.

  • The result of this paper runs as follows: 1. The theory of preclinic phase (=mibyung) was scientifically completed as one basic philosophy in "NAEKYUNG(內經)" and on influenced in the coming generations. Two principles for mibyung is to grow good energy and to avoid etiological cause. 2. So far oriental medicine has responded to already diseases, while to recognize the importance of mibyung is to convert it into preventive medicine which study and improve health. In spite of the opinion that no disease is health and no health is disease, the contrite of medical approch by the relative importance is necessary by understanding the steps of mibyung between health and disease with subdividing the steps of the occurance therefore, the scope of oriental medicine may be recognized from every disease to mibyung, that is, health. 3. Diagnosing and treating in the step of mibyung has more important meaning than suffering step because the checkup of mibyung means early examination and treatment. Mibyung can make an opportunity that improve scientific contradiction and defect of oriental medicine. However, scince the theory and practice lack the arrangement and study, much exertion and discussion is necessary.4. The diagnosis and cure in mibyung doesn't have many methods for treating, its index and standard isn't nified, and related theory is of small quantity. But the most prominent means of solution. with combination with other sciences and through the convertion into modem clinical examination, is to accomplish moderization, objectivity and indexation, etc. 5. The representive mibyungs are a hereditary disease, immune lack, mutation, early tumor, incubation of hepatitis and each infectious diseases, stress, etc. Since every science is the product of the times, it has the historical limits. As the times develop, the desire for good health is growing. Therefore we should consider above request in this times.

  • l. Backgrounds of Studies Cerebrovascular accident. one of the three major causes of death among audults with cardiopathy and malignancy, has been on the increase in korea while it is on the decrease in European countries and Japan. Types of stroke undergo changes caused by prolongment of life expectancy. and social and economic variety. More patients of ischemic stroke show a tendency to increase now than those of hemorrhagic stroke in the past. Many clinical studies on medical cerebrovascular and oriental stroke of paralysis have been published. but few clinical studies on therapeutics of integrated oriental and western medicine are to be found. So I have made an attempt to study clinical observations and therapeutic responses of ischemic stroke under integrated oriental and western medicine. 2.Methods. Among the patients admitted into the clinic of Joong-Poong, Woo-Suk University Hospital from May 1. 1993 until April 30. 1994 those 56 patients who were diagnosed as ischemic stroke on Computed Tomography(CT) and showed no dubious symptom after examination of coagulation and bleeding time were classified into the following six steps and treated: l)diagnosis 2)emergency treatment 3)basic treatment 4)treatment of risk and provoking factors. and preceeding disease 5)complications and conservative therapy 6)rehabilitation. For a period of basic treatment both herb medication and urokinase therapy were applied at the same time. Intravenous injection has been given at a unit of 300.000 dosage a day as urokinase therapy during basic treatment. If they showed any dubious symptom in glucose tolerance test. fructose 500ml and urokinase 300.000 dosage were mixed and injected. In case of no symptom 5% DW 500ml was mixed with urokinase 300.000 unit. and injected at a speed of 15gtt per minute. 3. Results and Conclusions 1) The level of ambulation has been improved from 42.9% when admitted to 73.2% when discharged in the degree of recovery. The level of severe function disorder has been remarkably decreased from 55.4% when admitted to 19.6% when discharged. 2) The treatment effect on the basis of therapeutic response of clinical and subjective symptom shows as follows: 7.1% Excellent. 35.7% Good. 37.5% Effective. 10.7% Stationary. and 8.9% Aggravated. The total recovery above effective shows 80.3%. Judging from the above results I think it proper to develop the model of better preventing and treating ischemic stroke through effective therapeutic and clinical studies of integrated oriental and western medicine.

Comparison of Clinical Characteristics by Types of Enuresis in Children Utilizing Questionnaires (병력취득 설문지를 이용한 야뇨증의 형태에 따른 임상양상의 비교)

  • Purpose : It is important to take a precise history of patients to diagnose enuresis. However, this is difficult to perform in the outpatient department, because the consultation time is limited. So by taking the medical histories with questionnaires, we classified the enuresis patients into monosymptomatic and non-monosymptomatic groups and compared the clinical characteristics between the two groups in order to determine whether this would aid in the diagnosis and treatment of enuresis. Methods : Thirty-four enuresis patients seen in the Department of Pediatrics at Yeungnam University Hospital from 2004 January to 2005 July were enrolled in this study. Two to three weeks before starting medical treatment, questionnaires about the patients' urination habits and past medical history were taken from the Parents with consents. Results : There were 21 patients in the monosymptomatic group and 13 patients in the non-monosymptomatic group. There was no difference in the delivery mode, birth weight, birth order, sleep habit, constipation, and treatment results between the two groups. Urinary infection was more frequently observed in the non-monosymptomatic group. There was a higher incidence of daytime incontinence and a past medical history of allergy in the non-monosymptomatic group. Oriental medicine was the most common treatment of enuresis before visiting the hospital, but the treatment effect was minimal. Conclusion : Comparison between the monosymptomatic and non-monosymptomatic group with the questionnaires was helpful for detailed diagnosis and treatment of enuresis. A more effective education for enuresis patients is needed, since before visiting the hospital many of them wasted their efforts with ineffective oriental medicine treatment. A Study on the Severity Classification in the KDRG-KM (Korean Diagnosis-Related Groups - Korean Medicine) (한의 입원환자분류체계의 중증도 분류방안 연구)

  • Backgrounds: Inpatient Classification System for Korean Medicine (KDRG-KM) was developed and has been applied for monitoring the costs of KM hospitals. Yet severity of patients' condition is not applied in the KDRG-KM. Objectives: This study aimed to develop the severity classification methods for KDRG-KM and assessed the explanation powers of severity adjusted KDRG-KM. Methods: Clinical experts panel was organized based on the recommendations from 12 clinical societies of Korean Medicine. Two expert panel workshops were held to develop the severity classification options, and the Delphi survey was performed to measure CCL(Complexity and Comorbidity Level) scores. Explanation powers were calculated using the inpatient EDI claim data issued by hospitals and clinics in 2012. Results: Two options for severity classification were deduced based on the severity classification principle in the domestic and foreign DRG systems. The option one is to classify severity groups using CCL and PCCL(Patient Clinical Complexity Level) scores, and the option two is to form a severity group with patients who belonged principal diagnosis-secondary diagnosis combinations which prolonged length of stay. All two options enhanced explanation powers less than 1%. For third option, patients who received certain treatments for severe conditions were grouped into severity group. The treatment expense of the severity group was significantly higher than that of other patients groups. Conclusions: Applying the severity classifications using principal diagnosis and secondary diagnoses can advance the KDRG-KM for genuine KM hospitalization. More practically, including patients with procedures for severe conditions in a severity group needs to be considered.
