Efficacy of Serum PIVKA-II in the Diagnosis and Follow-up after Treatment of Hepatocellular Carcinoma

  • It is a very important diagnosis and evalution of Hepatocellular carcinoma (HCC) in Korea where hepatitis B-virus is endemic. Protein induced by vitamin K absence or antagonist II (PIVKA-II) appears to be a useful tumor marker. This study was purposed to investigate usefulness of PIVKA-II in the diagnosis and fallow-up after treatment of HCC. A total of 418 patients were included in 187 patients (44.7%) of HCC, 83 patients (19.9%) of liver cirrhosis, 74 patients (17.7%) of chronic hepatitis and 74 patients (17.7%) of other liver diseases with serum PIVKA-II levels by Hicatch PIVKA-II kit. PIVKA-II level were analysed for difference of groups and the comparison of treatment responses. The sensitivity and specificity of PIVKA-II in the diagnosis of HCC were 80.2%, 87.0% at the cut-off value of 40 mAU/mL. There were statistically significant difference between the HCC and other groups (p<0.001), before and after PIVKA-II levels after treatment in HCC (p<0.001). PIVKA-II can be used as a useful tumor marker for patients with HCC, especially early diagnosis in high risk groups, treatment response assesment and monitoring of recurrence.

Early Diagnosis and Intervention Are Needed for a Reasonable Prognosis of Thromboangiitis Obliterans

  • Background: Thromboangiitis obliterans (TAO) poses a higher risk of amputation than atherosclerosis obliterans. It is characterized by onset at a relatively young age. There are currently no clear treatment guidelines for TAO other than smoking cessation. In this study, we aimed to identify factors that could influence a favorable prognosis of TAO. Methods: From January 2009 to December 2019, we retrospectively reviewed the initial symptoms, characteristics, treatments, and disease course of 37 patients (45 limbs) with TAO. Logistic regression analysis was performed to investigate factors affecting the course of symptoms that persisted or worsened despite treatment. Results: Patients' mean age was 37.2±11.4 years, and all patients were men. The mortality rate was 0% during the follow-up period (76.9±51.1 months). All patients were smokers at the time of diagnosis, and 19 patients (51.4%) successfully quit smoking during treatment. When comparing the Rutherford categories before and after treatment, 23 limbs (51.1%) showed improvement, the category was maintained in 11 limbs (24.4%), and 11 limbs (24.4%) worsened. Symptom persistence or exacerbation despite treatment was associated with a higher initial Rutherford category (odds ratio [OR], 1.59; 95% confidence interval [CI], 1.04-2.42; p=0.03) and a higher score of the involved below-knee artery at the time of diagnosis (OR, 2.26; 95% CI, 1.10-4.67; p=0.03). Conclusion: The degree of disease progression at the time of diagnosis significantly affected patients' prognosis. Therefore, early diagnosis and intervention are important to improve the course of TAO.

  • Radiopharmaceuticals targeting prostate-specific membrane antigens (PSMA) are essential for the diagnosis, evaluation, and treatment of prostate cancer (PCa), particularly metastatic castration-resistant PCa, for which conventional treatment is ineffective. These molecular probes include [68Ga]PSMA, [18F]PSMA, [Al18F]PSMA, [99mTc]PSMA, and [89Zr]PSMA, which are widely used for diagnosis, and [177Lu]PSMA and [225Ac]PSMA, which are used for treatment. There are also new types of radiopharmaceuticals. Due to the differentiation and heterogeneity of tumor cells, a subtype of PCa with an extremely poor prognosis, referred to as neuroendocrine prostate cancer (NEPC), has emerged, and its diagnosis and treatment present great challenges. To improve the detection rate of NEPC and prolong patient survival, many researchers have investigated the use of relevant radiopharmaceuticals as targeted molecular probes for the detection and treatment of NEPC lesions, including DOTA-TOC and DOTA-TATE for somatostatin receptors, 4A06 for CUB domain-containing protein 1, and FDG. This review focused on the specific molecular targets and various radionuclides that have been developed for PCa in recent years, including those mentioned above and several others, and aimed to provide valuable up-to-date information and research ideas for future studies.

  • Patients often seek consultation with dentists for their temporomandibular disorders (TMD), especially for pain. Acute pains refer to pains that are of short duration. Common acute TMD are arthralgia and local myalgia. Diagnosis should be made based on careful history taking and clinical examination. Most acute TMD are well controlled by education, cognitive awareness training, and conservative treatment. The aggressive and irreversible treatments should not be applied. Acute TMD should be controlled in the early phase so as not to be proceed to chronic pain.

  • Depression is a frequently developed psychiatric symptom after stroke. Poststroke depression(PSD) causes not only psychological distress related to depression but also other disturbances in many areas such as cognitive function, activities of daily living and quality of life. Therefore, a comprehensive understanding of accurate detection and appropriate treatment of depression is mandatory in patients with stroke. This review focused on the current knowledge of PSD.

3 Stage 2 Switch Application for Transcranial Magnetic Stimulation

  • Transcranial magnetic stimulation utilizes the method of controlling applied time and changing pulse by output pulse through power density control for diagnosis purposes. Transcranial magnetic stimulation can also be used in cases where diagnosis and treatment are difficult since output pulse shape can be changed. As intensity, pulse range, and pulse shape of the stimulation pulse must be changed according to lesion, the existing sine wave-shaped stimulation treatment pulse poses limitations in achieving various treatments and diagnosis. This study actualized a new method of transcranial magnetic stimulation that applies a 3 Stage 2 Switch( power semiconductor 2EA) for controlling pulse repetition rate by achieving numerous switching control of stimulation coil. Intensity, pulse range, and pulse shape of output can be freely changed to transform various treatment pulses in order to overcome limitations in stimulation treatment presented by the previous sine wave pulse shape. The method of freely changing pulse range by using 3 Stage 2 Switch discharge method is proposed. Pulse shape, composed of various pulse ranges, was created by grafting PFN (Pulsed Forming Network) through AVR AT80S8535 one-chip microprocessor technology, and application in transcranial magnetic stimulation was achieved to study the output characteristics of stimulation treatment pulse according to delaying time of the trigger signal applied in section switch.

  • Calcific tendinitis is the leading cause of shoulder pain. Among patients with calcific tendinitis, 2.7%-20% are asymptomatic, and 35%-45% of patients whose calcific deposits are inadvertently discovered develop shoulder pain. If symptoms are present, complications such as decreased range of motion of the shoulder joint should be minimized while managing pain. Patients with acute calcific tendinitis respond well to conservative treatment and rarely require surgery. In contrast, patients with chronic calcific tendinitis often do not respond to conservative treatment and do require surgery. Clinical improvement takes time, even after surgical treatment. This review article summarizes the processes related to the diagnosis and treatment of calcific tendinitis with the aim of helping clinicians choose appropriate treatment options for their patients.

  • Objective : SLAP is rupture of biceps brachii muscle tendon and it's origin, posterior side of superior labral to anterior glenoid fossa. Snapping and Pain, instability are its typical symptoms. SLAP is physical damage so surgeons use arthroscopy. In point of surgeons view, a conservative medicine is not effective for lesion of labrum. So In this article, we report a result of conservative treatment for the amatuer baseball player diagnosis with type 2 superior labral anterior posterior lesion. Methods : In this case, patient played amatuer baseball for 2 years, had diagnosis with type 2 Superior labral anterior posterior lesion by MRI after right shoulder Injury. OS recommened arthroscopy surgery. But he receive conservative Korean medicin treatment in Korean medicine hospital, including Atx, BV, herbal acupunture and rehabilitation excersise. Results : After 6 months, in the end of continuous conservative treatment and rehabilitation excersise, patient can play baseball normally, and felt a little pain. But In physical examination, he still has a some abnormal signs. Conclusion : A Conservative treatment for an amatuer baseball player diagnosis with SLAP type 2 was effective in restore of fuctional activities, but usefulness of this treatment needs more study.

  • A variety of mechanisms may generate pain resulting from injury to the peripheral nervous system. None of these mechanisms is disease-specific, and several different pain mechanisms may be present simultaneously in any one patient. Diagnosis of neuropathic pain is often easily made from the information gathered on neurologic examination and from patient history. Evidence of sensory disturbances elicited by examination combined with laboratory tests confirming injury to peripheral nerve establishes the diagnosis of neuropathic pain. Although treatment of neuropathic pain may be difficult, optimum treatment can be achieved if dentist has a complete understanding of the therapeutic options. Pharmacologic therapy has been the mainstay of treatment. Selection of an appropriate pharmacologic agent is by trial and error since individual response to different agents, doses, and serum level are highly variable. An adequate trial for each agent tried is key to pharmacologic treatment of neuripathic pain. If pharmacologic treatment is not effective, nerve block using lidocaine, steroid and alcohol and neurectomy must be considered for treatment option.
