경피적외선 조사요법과 침치료로 병행 치료한 알레르기성 접촉 피부염 환자 치험 1례 (The Clinical Study on 1 Case of Allergic Contact Dermatitis Treated with Infra-red Therapy and Acupuncture)

  • 알레르기성 접촉 피부염을 진단 받고 양방치료를 받았으나 별무 호전하던 환자에게 침치료와 양격산가감방을 복용하여 약 2개월간 증상이 초기보다 증상이 완화되는 효과가 있었다. 그러나 약물 치료를 배제하고 침치료만 시행하던 중 환자가 상열감 및 소양감은 거의 소실되었으나 홍조 증상이 정체 양상을 보였고 이에 저자는 치료후기를 혈조 상태로 변증하고 환자의 환부에 경피적외선조사요법을 시행하여 정체중이던 안면홍조 상태가 최근 약 1달간 10회의 치료로 환자가 만족할만한 효과가 있었다.

서울 지역을 대상으로 모집된 성인 아토피피부염 환자의 삶의 질과 경제적 비용 부담에 관한 조사 (Quality of Life and Economic Impact of Adult Atopic Dermatitis Patients in Seoul)

  • Objective : Atopic dermatitis (AD) may profoundly affect patient's quality of life (QOL), and also cause economic impact. The aim of our study was to evaluate the quality of life and the economic impact of adult atopic dermatitis patient in Seoul. We also evaluate the relationship of QOL and economic impact with severity of AD. Methods : 30 adult atopic dermatitis patient were included and evaluated by using the SCORAD Index and EASI. Patients were asked to fill in the questionnaires about their quality of life and financial costs during the past year by AD. Data about sleep disturbance and pruritus were also obtained. Pearson's correlation were used for statistical analysis. Results : 1. Among 30 patients, women were 19(63.3%), men were 11(36.7%). The mean age of the patients were 27.3 years old, patients between the ages of 17 and 30 years were 23, over 30 were seven. 2. The mean score of Objective SCORAD was $32.89{\pm}7.30$, Subjective SCORAD was $8.13{\pm}3.53$ and EASI was $9.15{\pm}6.90$이었다. 3. The mean score of Skindex-29 was $28.26{\pm}7.58$, DLQI was $10.17{\pm}5.55$. 4. By analyzing the questionnaire, a monthly average cost of 583,200 won for each patient was determined. Direct cost was 236,800 won and indirect cost was 346,300 won. 5. By analyzing the correlation between the severity of AD and QOL, Objective SCORAD and EASI were positively correlated with QOL(Skindex-29, DLQI) but not significant, meanwhile subjective SCORAD were significantly and positively correlated with QOL(Skindex-29, DLQI). 6. By analyzing the correlation between the severity of AD and economic impact, Objective SCORAD were significantly and positively correlated with direct cost and indirect cost. Also, EASI were significantly and positively correlated with direct cost and oriental medical hospital visits. Conclusions : The above results show that the QOL of the patients with atopic dermatitis is significantly related to their disease severity. Atopic dermatitis patients pay 583,200 won a month, and the economic impact of the patients is significantly related to their disease severity.

두피 지루피부염과 화폐상 습진을 동반한 아토피피부염 환자 치험 1례 (A Case of Atopic Dermatitis Accompanying Seborrheic Capitis and Nummular Eczema)

  • Objectives : The objective of this study is to report a case of a male atopic dermatitis patient accompanying seborrheic capitis and nummular eczema improved by Korean medicine therapy and lifestyle modification. Methods : A male patient was hospitalized for eczematous lesions in the head, face, and both hands which relapsed on March 2021. For 15 days, he took Korean medicine therapy including acupuncture, Bangpungtongseongsan-gagam, pharmacopuncture, and wet dressing with Hwangryunhaedok-tang. Simultaneously, lifestyle correction also conducted during administration. On the other hand, corticosteroid and antihistamine were prescribed from internal medicine of our hospital for the first 10 days because of severe skin lesions. As an outpatient, he was continuously treated by the same Korean medicine therapy except herbal decoction weekly for about 7 months after discharge. To assess symptoms, scoring atopic dermatitis(SCORAD) index, taking photos, and numerical rating scale(NRS) were used. Results : After 15 days of hospitalization, the SCORAD index decreased to 30.0, which was about a half of the initial SCORAD index(61.2). NRS score also dropped from 6 to 3. Despite stopping western medicine administration, skin lesions and subjective symptoms of the patient were steadily improved without aggravation. For 7 months of continued outpatient treatment, atopic dermatitis were steadily ameliorated despite temporary aggravation and improvement of symptoms, and seborrheic capitis was not relapsed. The final SCORAD index and NRS on November 9th, 2021 were 24.7 and 1, respectively. Conclusions : These results suggests that Korean medicine therapy contributes to improving SCORAD index, subjective symptoms, and skin lesions of the patient. Furthermore, lifestyle modification is also important as much as proper treatment for caring atopic dermatitis patients.

A Case of Cyclosporine Treatment in Cat with Allergic Dermatitis, Nonresponsive to Prednisolone Treatment

  • A 2-year-old spayed female Persian cat presented to Kangwon National University Veterinary Medical Teaching Hospital with pruritus and erythema on the tips of both ears, around the eyes, and in the caudal abdomen. This patient had previously been prescribed prednisolone, but did not respond positively to the treatment. A skin screening test revealed that there were no fleas or fungi, and that only cocci were present. Blood testing revealed no remarkable findings. The patient was prescribed antibiotics (amoxicillin-clavulanic acid 25 mg/kg for 2 weeks) with no prednisolone. After 2 weeks, clinical signs were alleviated and the skin screening test showed no signs of cocci. However, clinical signs recurred even with the prescription of antibiotics. Four weeks after the steroid-free interval, Malassezia spp. hypersensitivity was detected upon a serum allergy test, and pathological analysis confirmed eosinophilic and mastocytic superficial dermatitis in the caudal abdomen. Based on these results, we suspected allergic dermatitis and prescribed 7 mg/kg cyclosporine A once a day. After 3 weeks, clinical signs were resolved. Seven weeks after the first trial with cyclosporine A, we reduced the cyclosporine A dose to 7 mg/kg every other day. The patient's symptoms have since been well controlled for 6 months. This study suggests that cyclosporine A can be a good choice for treating cats with suspected allergic dermatitis that has not responded positively to steroid treatment.

  • 패혈증이 동반된 아토피피부염 환자를 19일간의 입원치료기간동안 한약치료와 항생제치료를 병행하여 치료하였다. 향후 적절한 감염 관리가 동반된다면, 2차 감염이 병발된 아토피피부염을 적극적으로 치료할 수 있을 것으로 사료된다.

  • This report describes the effectiveness of mahaenggamseog-tang(麻杏甘石湯) on the patients who suffered from atopic dermatitis and psoriasis. We administered mahaenggamseog-tang with heat-clearing and lung-dispersing(淸熱宣肺) effect to two dermatitis patients who showed sweating with panting(汗出而喘), vexation and thirst(煩渴), aversion to heat(惡熱), agitation(煩燥), swelling of the face and eyes(面目浮腫). The severity of dermatitis was evaluated by visual analogue scale(VAS), pictures, SCORAD index(case 1), PASI(case 2). After the treatment, VAS/SCORAD index of case 1 changed from 9/71.5 to 0/6.1; VAS/PASI of case 2 from 8/36.8 to 1/6.8. Mahaenggamseog-tang could improved the symptoms of atopic dermatitis and psoriasis in this study. This study offers an approach to managing chronic dermatitis more effectively using Mahaenggamseog-tang which used mainly for bronchial diseases and obesity.

개인 건강 기록을 통한 아토피 피부염 환자의 사상체질 특성에 관한 연구 (A Study on Sasang Constitutional Characteristics of Atopic Dermatitis Patients Based on Atopic Dermatitis Personal Health Record)

  • Objective The purpose of this study is to analyze the daily life records of atopic dermatitis patients according to Sasang Constitution based on the data recorded using Personal Health Record (PHR) platform, and to find basic data for the Sasang Constitutional daily life management and prevention. Method Thirty patients with atopic dermatitis under 19 years old were included. The data recorded using PHR platform by the patient or the parents for 6 weeks and allergic blood tests were analyzed according to Sasang Constitution. Result As a result of analyzing the atopic dermatitis PHR related to Sasang Constitutional characteristics, there were no significant differences due to the small number of subjects. There were a tendency associated with the Sasang Constitutional characteristics in the emotional state, urine, stool and weakness score of five viscera. Allergic reactions tended to occur in those with higher weakness score of Viscera. There was a significant correlation between quality of life and SCORing Atopic Dermatitis(SCORAD) score. Conclusion This study can be considered as a primary study on items that tend to be associated with Sasang constitutional characteristics in patients with atopic dermatitis.

아토피피부염의 한의학 진료 지침 개발을 위한 사전 연구 (A study on the Development of Traditional Korean Medicine Clinical Practice Guideline for Atopic Dermatitis)

  • Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.

  • Background: Infantile atopic dermatitis involves the face, with erythematous weepy patches. This weeping eczema has severe itching and causes eczema secondarily. Also it has a profound effect on the quality of life. Many infantile AD patient and parent suffer troubles because of weeping eczema. Objective: The purpose of this study was to evaluate the efficacy of Gyejigahwanggitang(桂枝加黃?湯) in infantile AD patient with weeping eczema. Method: I treated them with Gyejigahwanggitang(桂枝加黃?湯) for balancing YoungWi(調和榮衛). I also used acupuncture therapy.

  • Objective : The objective of this study is to analyze an increasing rate, difference of attack rate in age, relationship between atopic dermatitis and breast-feeding, the relationship between atopic dermatitis and the Sasang constitution, and various treatments of atopic dermatitis. Methods : This clinical study was carried out with 22 theses which are related with diagnosis, treatments, prognosis and control of atopic dermatitis. The 22 theses are carried on J Korean Oriental Pediatrics, J Korean Oriental Ophthalmology, J Korean Oriental Med, J Korean Academy of Pediatric Allergy and Respiratory Disease, J Korean Acad Fam Med and Korean J Food & Nutr. Results : The prevalence rate, attack rate and diagnosis rate of atopic dermatitis are increased in the year of 2000, compared with those of 1995. Comparing age of patient between the year of 1992 and 2002, the attack rate of atopic dermatitis is increased quickly over 7 years old. Specific immunoglobulin E(IgE) antibodies detected in patients under 1 year old was exclusively caused by food. But for the age over 7 years old, food and inhalant allergen are detected in the year of 2002 in compare with that of 1992. Because of breast-feeding, intemperate diet adjustment during the period of maternity of family history of atopic dermatitis, the attack rate of atopic dermatitis is increased in infant. The types of patients are categorized according to Sasang constitution and Soeumin group was largest. The Oriental medicine treatments of atopic dermatitis are bath & skin hydration, avoidance from antigen, dietetic treatment, external treatment, medication and phototherapy. Conclusion : The atopic dermatitis is associated with breast-feeding, Sasang constitution types. More active approach for the treatment and prevention of Atopic dermatitis in children are needed.
