Adherence of Candida albicans(C. albicans) to the surface of a denture is believed to be an initial and essential step in the formation of denture-induced stematitis. Previous studies have provided enormous infomation on the relationship between composition of palatine gland/parotid saliva and upper denture stomatitis. Relatively little information is available on the correlation between lower denture stomatitis and sublingual-submandibular ( SLSM ) saliva. The plaque samples were collected from the two sites($100mm^2$) on the inner surface of lower partial denture corresponding to the stematitis and healthy region of the lower partial dentures of 12 denture stomatitis patients and 6 nor-mal persons who wore lower partial dentures. The samples were plated to isolate C. albicans on a selective Saboraud's dextrose agar plate and the isolates were identified by germ tube test and gram staining. The subjects were divided into group I (stomatitis with C. albican), group II (lesion without C. albicans), group III (no lesion but C. albicans), and group IV (normal and healthy denture wearer). Individual SLSM saliva($20{\mu}g$ of protein) was analyzed by SDS-PAGE (SDS -poly-acrylamide gel electrophoresis) with Coomassie brilliant blue and PAS(Periodic Acid Schinff) stain-ing. The salivary proteins separated in the polyacryamide gels were subjected to immunoblot anaysis using anti-lactoferrin, anti-sIgA, and anti-secretory component of sIgA. In this study using custom made acrylic denture resin beads(5mm in diameter) coated with stimulated individual SLSM saliva, the binding ability of individual C. albicans strains to the beads was observed. Levels of C, albicans adhered to the acrylic resin beads were determined by measuring the optical density of the bound C. albicans to the beads at 580nm. The results showed that a higher number of C. albicans was observed in the lesion site than healthy site. The saliva of group I contained more high molecular weight glycoprotein(mucin, MGI) as compared to group II, III and IV. And lactoferrin and sIgA affected to the binding ability of C. albicans to acylic resin beads. Binding ability of individual C. albicans to the acrylic resin coated with respective individual saliva was found to be greater in group I than the other 3 groups. And when bound cells of C. albicans isolated from individual subject #2 to the saliva coated beads were used binding ability of subject #2 saliva coated beads was founed to be greater than the other sutjects. These results suggested that denture induced stomatitis is related to individual patient's salivary protein composition, especially MG-1. Future studies will be directed toward saliva exam-ination of patients who have general disease and analysis of pellicles formed on prosthesis with respect to oral disease.
심한 의치성 구내염의 경우, 항진균제의 처방이 효과적이지 않은 경우가 있다. 이번 증례는 5개월간 fluconazole을 처방받았으나, 증상의 개선이 없었던 72세의 환자에 증례이다. 본원의 재진단을 거쳐 약물치료를 시행하였고, 임시의치를 제작하여, 구강회복을 기하고자 하였다. 그러나 계속되는 약물치료에도 구강상태가 호전되지 않는 증례에서 미생물학적인 분석 및 보조요법이 도움이 될 수 있음을 본 증례를 통해 살펴보고자 한다.
목적: 60세 이상의 의치사용자를 대상으로 구강 건강 상태, 의치 사용 및 관리 실태를 파악하여 올바른 의치 사용과 관리 중요성의 대국민 홍보 및 인식 제고를 위한 기초 자료로 활용하는 것이다. 대상 및 방법: 의치사용자의 구강건강상태, 의치성 구내염 현황 및 의치관리상태에 대해 2017년 8월 21일부터 9월 8일까지 서울 및 4대 광역시(부산, 대구, 광주, 대전)에서 60세 이상 남녀 중 현재 의치(총의치, 국소의치)를 사용하고 있는 자를 500명을 대상으로 조사하였다. 연구는 각 항목들에 대해 개별 면접 조사를 시행하였으며, 사용된 조사 항목은 응답자의 특성 8문항, 틀니 사용 현황 및 불편 경험 6문항, 구강 건강 상태(의치성 구내염 여부) 12문항, 틀니 관리 현황 7문항으로 구성되었다. 결과: 연구대상 의치사용자 중 161명(32.2%)은 본인의 구강 상태가 건강한 편으로 인식하고 있었고, 111명(22.2%)은 보통이라고 인식하고 있었다. 의치성 구내염은 208명(41.6%)에서 인지하고 있었다. 의치건강보험제도에 대해서는 조사대상자의 26.2%(131명)만이 인지하고 있었으며, 조사대상자의 65.4%(327명)의 의치사용자가 잘못된 의치 관리 방법을 사용하고 있었다. 결론: 본 연구에 따르면 의치사용자의 많은 수가 구강건강, 의치관리 및 의치건강보험에 대한 올바른 인식이 부족함을 알 수 있다.
목적: 본 연구의 목적은 효모형 또는 균사형 Candida albicans에 대한 표고버섯 추출물의항진균효과를 살펴보기 위함이다. 연구 재료 및 방법: 표고버섯 추출물은 표고버섯을 물 또는 에탄올에 닮근 후, 상층액을 건조시켜서 얻었다. 효모형 C. albicans에 대한 추출물의 항진균활성은 마이크로플레이트를 이용한 감수성 시험을 이용하여 조사되었다. C. albicans 생물막을 CO2 배양기에서 Ham's F-12 배지를 이용하여 12-well 플레이트에 형성시키고 에탄올 추출물로 처리하였다. 또한 C. albicans 생물막을 의치상용 레진 디스크에 형성시키고 의치세정제를 에탄올 추출물이 포함 또는 포함되지 않은 조건에서 처리하였다. 두 조건의 항바이오필름 효과 시험에서의 바이오필름내 살아있는 C. albicans를 조사하기 위해서 한천고체배지에 접종한 후 집락 형성 단위(CFU) 값을 측정하였다. 결과: 효모형 C. albicans에 대해서 표고버섯으로부터 물 추출물보다 에탄올 추출물이 강한 항진균력을 보였다. 에탄올 추출물은 균사형 C. albicans 바이오필름에 대해서도 유의적인 항진균력을 보였다(P < 0.05). 또한 에탄올 추출물은 의치세정제와 의치에 형성된 C. albicans 바이오필름에 대한 항진균력에 대해서 동반상승효과를 보였다(P < 0.05). 결론: 표고버섯 에탄올 추출물은 구강 칸디다증 예방뿐만 아니라 의치관련 구내염에 대해서 예방할 수 있는 후보물질로 사료된다.
Tissue conditioners have been used for treatment of denture stomatitis caused by wearing of dentures. Early studies pointed out Candide albicans (C. albicans) as main etiologic factor, and antifugal agents were added for control of the species. But there is a little information about broad comparison on the effect of tissue conditioners and antifungal agents added. The purpose of the present study was to compare the inhibiting effect of four tissue conditioners and one temporary soft liner on the growth of C. albicans for treatment of denture stomatitis using gel diffusion method by measuring diameter of the zone of growth inhibition. Three antifungal agents were added to each material for evaluation of the effect of added agents. Finally, observation was made to evaluate the effect of the loss of antifungal elements by aging of the specimen. The results of this study were obtained as follows : 1. Tempo had remarkable antifungal effect showing the zone of growth inhibition as 2.35 mm at 1st day, and was most effective on End: 4th and 7th day from incubation (p<0.05). But Coecomfort, Dura conditioner, Visco-gel, Coe-soft had little antifungal effect from the 1st day of incubation. 2. Nystatin was most effective showing 9.60-12.04 mm of zone of inhibition at the 1st day from incubation. The antifungal properties were reduced to amphotericin B, chlorhexidine and materials without agent (p<0.05), and the effect was diminished by time. 3. As pretreatment with amphotericin B, nystatin, chlorhexidine, Tempo was very effective at the 1st day from incubation showing zone of inhibition as 3.65, 12.04, 4.78 mm with addition of each agent. Dura conditioner had strongest antifungal effect at the next day as 2.86, 5.33, 1.29 mm of zone of inhibition, and yielding results of Coe-comfort, Tempo, Coe-soft was shown at 4th and 7th day from incubation (p<0.05). Taken all together, tissue conditioners have little antifungal effect except Tempo. Formation of the zone of growth inhibition was due to agents amphotericin B, nystatin, chlorhexidine and nystatin was most effective. Conclusively, it is advisable to select material which is effective on the growth of C. albican and consider addition of antifungal agents for treatment of denture stomatitis.
For the purpose of evaluating the cleansing efficiency against Candida albicans detected frequently in patients with denture stomatitis, two denture cleansers with or without enzymes were studied under the same conditions. The results were as fellows: 1. Enzyme-contain denture cleanser was showed more Candida albicans lytic ability than non-enzyme-contained denture cleanser. 2. It was observed that Candida albicans lytic activity in further diluted manufacturerers' recommended concentration was decreased. 3. In fungicidal test, the enzyme-contained denture cleanser sterilized Candida albicans, and the non-enzyme-contained denture cleanser did not sterilize Candida albicans. 4. Sterilizing time of Candid albicans was needed for at least 60 minutes in enzyme-contained denture cleanser solution which was diluted with manufacturerers' recommended concentrations., and was needed for more times with further diluted manufacturerers' recommended concentrations. 5. In vitro growth test of Candida albicans on acrylic resin surface, the only enzyme-contained denture cleanser inhibited growth of Candida albicans, and it was observed that inhibiton ability of growth of Candida albicans on arrylic resin surface was decreased in further diluted manufacturerers' recommended concentrations.

PURPOSE. The goal of this study was to compare the adhesion of Candida albicans to the surfaces of CAD/CAM and conventionally fabricated complete denture bases. MATERIALS AND METHODS. Twenty discs of acrylic resin poly (methyl methacrylate) were fabricated with CAD/CAM and conventional procedures (heat-polymerized acrylic resin). The specimens were divided into two groups: 10 discs were fabricated using the CAD/CAM procedure (Wieland Digital Denture Ivoclar Vivadent), and 10 discs were fabricated using a conventional flasking and pressure-pack technique. Candida colonization was performed on all the specimens using four Candida albicans isolates. The difference in Candida albicans adhesion on the discs was evaluated. The number of adherent yeast cells was calculated by the colony-forming units (CFU) and by Fluorescence microscopy. RESULTS. There was a significant difference in the adhesion of Candida albicans to the complete denture bases created with CAD/CAM and the adhesion to those created with the conventional procedure. The CAD/CAM denture bases exhibited less adhesion of Candida albicans than did the denture bases created with the conventional procedure (P<.05). CONCLUSION. The CAD/CAM procedure for fabricating complete dentures showed promising potential for reducing the adherence of Candida to the denture base surface. Clinical Implications. Complete dentures made with the CAD/CAM procedure might decrease the incidence of denture stomatitis compared with conventional dentures.
When exposed to different types of bacteria in the oral cavity, denture based resins are prone to bacteria attachment. The purpose of this study was to investigate the physical, biological, and antimicrobial properties of denture base resins coated with Peony extract (200, 400, and 600 ㎍/mL). Specifically, the surface properties (microhardness, contact angle, and color change of the coated specimens), cell cytotoxicity (measured using MTT assay), and antimicrobial activity (against S. mutans (Streptococcus mutans) and C. albicans (Candida albicans) using a growth inhibition assay) were evaluated. The polyphenol content was measured using ultraviolet-visible (UV-vis) spectrometry. The experimental groups (specimens coated with Peony extract) and a control group (specimens coated without Peony extract) were statistically compared using a one-way analysis of variance and Tukey's post-hoc tests. No statistically significant differences in surface properties or cell cytotoxicity were observed, which demonstrated their biocompatibility. Conversely, a statistically significant difference in antimicrobial activity was observed between the experimental and control groups after 48 h. This confirms the antimicrobial activity of the denture base resin coated with Peony extract and demonstrates that it is a promising dental material for preventing stomatitis.
Candida albicans is now well recognized among the denture stomatitis patients. The broth macrodilution test is the most widely used technique for antifungal susceptibility testing. The purpose of this study was to determine the C. albicans carrier rate of the denture patients in Iksan, chonbuk. To determine the C. albicans carrier rate of denture patients, culture were made from 227 sample taken in Iksan, Chonbuk during July 1997 to August 1997. Also activities of amphotericin B and miconzole against isolates of denture patients of C. albicans were tested by broth macrodilution test using RPMI medium 1640. The results were as follows : First C. ablicans was isolated from 6.6% of denture patients samples and the frequency of isolation fo C. albicans was highest(50%) in the age group of 71-year-old to 80-year-old denture patients. Second, against C. albicans, the MIC range of amphotericn B was $0.06{\sim}0.25{\mu}g/ml$. MIC50 and MIC90 were $0.13{\mu}g/ml$ and $0.25{\mu}g/ml$, respectively. Third, the MIC range of miconazole was $10-{\ge}20{\mu}g/ml$ and MIC50 and MIC90 were $20{\mu}g/ml$ and ${\ge}20{\mu}g/ml$, respectively. It was concluded from this study that C. albicans acrriages from healthy denture individuals only over 60-year-old ages were isolated, they remain susceptible to amphotericin B and not rarely resistant to miconzole.
Purpose : Colonization of denture soft lining materials by Candida albicans can result in clinical problem, and deterioration of the materials. This study aimed to compare the retention and penetration of C. albicans into four denture soft lining materials commonly used. Materials and methods : Four denture soft lining materials (Coe-comfort$^{(R)}$, Coe-soft$^{(R)}$, GC soft liner$^{(R)}$, and Tissue conditioner$^{(R)}$) discs were prepared to glass slide and dental stone. Adherence of yeast to surfaces was monitored after one hour incubation of standardized washed cell suspension with test disc surfaces. Adherent cells stained with acridine orange were counted fluorescence microscopy. Penetration of yeast into materials bonded with acrylic resin after 1, 2, 3,4, 5, 6 and 7 days incubation was observed through sections stained using acridine orange and estimated to quantitative analysis using radioisotope. Results : There was statistical significance in cell numbers between smooth and rough surfaces(p<0.05). Higher numbers of cells were observed on rough surfaces. There was statistical significance in adherent cell numbers into smooth and rough surfaces individually(p<0.05). According to the increase of incubation periods, the cells penetrated into denture soft lining materials were shown to increase. The differences among all kinds of soft liner were statistically significant(p<0.05),and the largest number of cells penetrated into soft liners was observed in the Coe-soft$^{(R)}$. Conclusion : Initial adherence and penetration of yeast into denture soft lining materials has been influenced by surface roughness and chemical composition of them. The selection of appropriate materials and their fabrication may promote clinical performance.
