Prosthetic treatment for Down's syndrome patient with dental cross bite problem using maxillary double crown denture (교차교합을 가진 다운증후군 환자의 상악 이중관 의치를 이용한 수복 증례)

  • Patients with Down's syndrome have several dental complications such as small teeth caused by underdevelopment of dentin and enamel, periodontitis, agenesis of teeth, prolonged retention of primary teeth and malocclusion due to narrow palate. Removable denture with maxillary double crowns would be a good treatment option to solve the problems of the patient with Down's syndrome. Double crowns compensate the insufficient support and retention of denture and easily solve the cross bite problem. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 26-year-old female patient with Down's syndrome and dental phobia had small number of teeth with enamel hypoplasia, prolonged retention of primary teeth and dental cross bite. Prosthetic treatment was done using removable denture with double crowns in the maxilla. In the mandible, teeth preparation was done on enamel margin without anesthesia. Anterior laminate and posterior complete zirconia crown restorations were performed. As a result, the cross bite was effectively corrected by denture with double crowns. Pronunciation and appearance were also improved without extraction of teeth and dental anesthesia.

A study on dental health and physical & psychological health status of the aged (노인 구강보건실태 및 신체·심리적 건강상태에 관한 연구)

  • This study was conducted to investigate the dental health and physical & psychological health status of the aged. The subjects were 61 old persons, women who were over 65, living at Sahagu in Pusan. The data for this study were collected by using direct interviewing method from October 22th, 2004. The data were analysed by using a computerized program named statistical package for social science including frequency, percentage, ANOVA. The results of the study were summarized as follows: 1. The dental status and the use of dental care institute 1) In status of denture use, it was found to be no denture(45.2%), full denture(35.5%), partial denture(19.4%). 2) In the questionaire of "when did you go to the dental care institute recently?", it was found to be uncertain(41.9%), less than one year(38.8%), 2~3years(16.1%), 1~2years(3.2%). 3) In the questionaire of "what type of the dental care institute did you use?", it was found to be in the dental clinic(80.6%), others(9.7%), dental hospital(6.5%), department of dentistry in general hospital(3.2%). 4) In the questionaire of "why did you select the dental care institute?", it was found to be in the nearly distance(80.6%), relative advice(16.1%), advertisement(3.2%). 5) In the questionaire of "how did you go to the dental care institute?", it was found to be walking(71.0%), bus & subway(22.6%), car(3.2%) and others(3.2%). 6) In the questionaire of "how long did it take to the dental care institute?", it was found to be 10~30min(48.4%), less than 10min(38.7%), 30min~1h(6.5%), 1~2h(3.2%), more than 2h(3.2%). 7) In the questionaire of "what kind of dental care did you take?", it was found to be denture making(45.2%), extraction of tooth, dental surgery, general exam(12.9%), caries(9.7%), denture repair(6.5%), others(3.2%). 8) In the questionaire of "how much did you pay for dental care recently?", it was found to be less than 5,000won(35.5%), 20,000~100,000won(19.4%), 1~3million won(16.1%), 5,000~20,000won, 100,000~300,000won, 500,000~1million won, more than 3million(6.5%), 300,000~500,000won(3.2%). 9) Average score of the subject's physical health status was 4.11 and psychological health status, 4.01 in a 5 point Likert scale. 10) The physical and psychological health status showed the significant differences according to the frequency of eating snack(pE0.05) and snack type(pE0.01). Above findings suggest that geriatric oral health program is necessary in improving the dental health & health status of the aged.

Prosthetic treatment for patient with congenital bilateral cleft lip and palate to close oro-nasal communication using maxillary double crown and clasp retained removable denture (선천성 양측성 구순구개열 환자의 구비강 연결 폐쇄를 위한 상악 이중관과 고리 유지형 국소의치를 이용한 수복 증례)

  • Patients with cleft lip and palate have several problems such as oro-nasal communication, dental cross-bite caused by the insufficient growth of maxilla, poor pronunciation and esthetic problem. Removable denture with maxillary double crown near cleft palate and with clasp in the posterior teeth was used to solve the problems of the patient with cleft lip and plate. Double crowns make up for the insufficient mucosal support caused by the mucosa and alveolar bone loss in cleft lip and palate. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 55-year-old female patient had bilateral cleft lip and palate and few remaining teeth on anterior maxilla. Prosthetic treatment was done for patient with bilateral cleft lip and palate to close oro-nasal communication using maxillary double crown and clasp retained removable denture. As a result, oro-nasal communication was effectively closed and the cross-bite was easily corrected by double crown and clasp retained denture. In addition, pronunciation and appearance were also improved.

Number of implants for mandibular implant overdentures: a systematic review

  • PURPOSE. The aim of this systematic review is to address treatment outcomes of Mandibular implant overdentures relative to implant survival rate, maintenance and complications, and patient satisfaction. MATERIALS AND METHODS. A systematic literature search was conducted by a PubMed search strategy and hand-searching of relevant journals from included studies. Randomized Clinical Trials (RCT) and comparative clinical trial studies on mandibular implant overdentures until August, 2010 were selected. Eleven studies from 1098 studies were finally selected and data were analyzed relative to number of implants. RESULTS. Six studies presented the data of the implant survival rate which ranged from 95% to 100% for 2 and 4 implant group and from 81.8% to 96.1% for 1 and 2 implant group. One study, which statistically compared implant survival rate showed no significant differences relative to the number of implants. The most common type of prosthetic maintenance and complications were replacement or reattaching of loose clips for 2 and 4 implant group, and denture repair due to the fracture around an implant for 1 and 2 implant groups. Most studies showed no significant differences in the rate of prosthetic maintenance and complication, and patient satisfaction regardless the number of implants. CONCLUSION. The implant survival rate of mandibular overdentures is high regardless of the number of implants. Denture maintenance is likely not inflenced substantially by the number of implants and patient satisfaction is typically high again regardless os the number of implants.

  • Fiber reinforced materials have favorable mechanical properties. Moreover, the strength to weight ratios of this material is superior to those of most alloys. Comparing to the metals, it showed many other advantages as well, including non-corrosiveness, translucency and easy repair characteristic. Since, it has the potential for the chair-side and laboratory fabrication, it is not surprising that fiber reinforced composites offer the potential for use in various applications in dentistry. To make the well-fitted restorations, Fiber reinforced composite (FRC) has been suggested as an alternative framework material for the implant supported fixed prosthesis. Two fixed partial denture fabrication procedures were tried. Vectris fiber was pressed to the EsthetiCone gold cylinder on the implant positioned cast. And then, Targis were added on it. In the other method, we used the customized component using UCLA abutment. The beads for retaining the Vectris fiber were added on the abutment. If careful laboratory and clinical techniques were done, these two techniques would fulfill the demands of the esthetics and strength.

  • Dr. Walter Wright first presented the results of his studies on acrylic resins in July, 1937. The use of resins for adaptation in inlay and crown and bridge prosthesis was first reported in June 1940 by Harris. There has been now and acceptable list of several physical and mechanical properties of acrylic resins which have been studied to a considerable extent by various researchers, or determined from clinical experience. They include; pleasant esthetics, taste, odor, cleanliness, compatibility with oral tissue, dimensional stability, water sorption by imbibition, hardness, ease and success of repair, weight, thermal coefficient of expansion and strength to resist functional stress. The author carried a series of experiments forward to check the strength. Specimens which were cured at boiling temperature showed weaker strength than those ones which were cured at 72℃.

  • The purpose of this study was to investigate the interfacial elemental change when solding the Ni-Co-Cr dental removable partial denture alloy and Fe-Cr-Ni wrought wire alloy with Ag-Cu-Zu Silver solder, by EDXA, EPMA, to investigate the appropriateness of clinical usefullness for repair the fractured clasps of removable partial dentive. The result of this study was as follows: 1. The Ni element of major component of Ticonium penetrate into the silver solder 2. The movement Age element of silver solder into Fe-Cr-Ni wire was not significant, by EDXA and EPMA.

Retrospective Evaluation of Implant Retained Overdentures Using Two Implants and Locator Attachments in the Edentulous Mandibles (Locator attachment를 이용한 하악 임플란트 유지 피개의치의 후향적 평가)

  • This study compared the marginal bone loss around dental implants that were placed in the canine areas of the mandibles and finded the survival rate of the implants, marginal bone loss around implants and prosthetic complications in 20 patients treated with overdentures retained with Locator attachments. Implants placed in this study showed a 95% survival rate and the average marginal bone loss was 1.21 mm($SD{\pm}0.60mm$) in 94.3 weeks of functional loading. Implant-retained overdentures had various prosthetic complications such as male change, metal cap loosening, Locator attachment loosening, denture teeth change, relining and denture fracture. Implant-retained overdentures using two implants and Locator attachments in the edentulous mandibles is considered as a stable way for long terms because of minimal invasive operation, simple prosthetic process and easy repair.

A Study on demand and supply of dental laboratory technician in Gangwon Province. (강원도내 치과기공소 인력수급을 위한 기초조사 연구)

  • The aim of this study is to propose effective measures for shortage of manpower of dental technology laboratory in Gangwon province by means of examining not only the history of Gangwon association, present condition of transference of members of Gangwon association from 1990, of missing members and annual rate of participation in repair education with setting object of study into Gangwon association under the influence of the Korean Dental Technology Association but also the present condition of opening and closing laboratory, amount of business connection and condition of shortage of manpower and its field with setting object of study into 28 dental technology laboratories. Results from this study are like followings : First, Gagnwon association was established in 1984 as the eleventh in the line of the board, reorganization of board is conducted 8 times up to the eighth in the line of the board in 2001. board is composed of 16 persons including president and operated its term as 3 years. Second, a number of representative and ordinary member of Gangwon association has increased gradually from 1992 to 2001. in aspect of annual rate of participation in repair education, representative's is more higher than ordinary member's overall. the worst rate of whole members' is shown as 47.9% and the its best rate is shown as 92.0%. in addition, 70$\sim$80% rate of participation into repair education is shown most frequently as 8 times of whole 20 times from 1992 to 2001. Third, 29 authorized dental technology laboratories have registered in Gangwon association until march 2002 within 9 cities and counties. in consideration of establishment location. Gangneung city has most amount as 7 laboratories(24.1%), Wonju city has 5(17.2%), Samcheok city and Sokcho city, each has 4(13.8%), Chuncheon city has 3(10.3%), Donghae city and Taeback city, each has 2(6.9%) and Yangyang and Yongwol county, each has 1(3.4%). Fourth, amount of work personnel of laboratory in Gangwon province is 1$\sim$10 persons as the maximum value. 23 laboratories(82.1%) have their own work personnel as amount of 5 and below. in aspect of main business, most of laboratories focus on the Porcelain and Crown. and 16 laboratories(57.1%) have their own expensive equipments such as Milling. its also suggests that 16 laboratories(57.1%) suffer their shortage of working personnel. specifically, 6 laboratories(21.4%) suffer the highest shortage especially in Denture field, Fifth, whole number of Gangwon association is up to 101 members (28 representatives and 73 ordinary members) until December 31th 2001. amount of immune from repair education of Gangwon association until October 2001 is up to 10 persons, and missing members from 1997 is up to 18 persons, suspension of business is up to 4 persons. in aspect of transference from 1990, transfer-in is up to 21 persons and transfer out is up to 16 persons.

RIGID FIXATION AND SPACE MAINTENANCE BY TITANIUM MESH FOR RECONSTRUCTION OF THE PREMAXILLA (상악골 전방 결손부 재건 시 견고 고정과 공간 유지로 사용된 타이타니움 메쉬의 임상 예)

  • Reconstruction of defect in the anterior part of the maxilla to enable implant placement or prothesis is a complicated treatment due to the anatomical position and lack of soft tissues. Two cases are presented in which autogenous iliac PMCB(particulate marrow and cancellous bone) with titanium mesh were used for premaxilla reconstruction and alveolar bone repair of the anterior maxillas prior to denture and implants fixation respectively. Cancellous bone from the anterior iliac crest was compressed and placed against a titanium mesh fixed to the bone of palate in a patient with severe defect of the anterior maxilla. There were no problem in the healing, and the anterior maxillas of two patients had increased height and width during the initial healing and remodeling. The clinical reports describe the use of titanium mesh for reconstruction of premaxilla. Autogenous bone grafts were harvested from the iliac crest and were loaded on a titanium mesh that were left in the patient's maxilla for 6 months before they were removed respectively. The radiographic analysis demonstrated that a 10mm vertical ridge augmentation had been achieved. In guided bone regeneration, the quantity of bone regenerated under the barrier has been demonstrated to be directly related to the amount of the space under the membrane. This space can diminish as a result of membrane collapse. To avoid this problem which involved the use of a titanium mesh barrier to protect the regenerating tissues and to achieve a rigid fixation of the bone segments, were used in association with autologous bone in 2 cases. The aim of this study was to evaluate the capability of a configured titanium mesh to serve as a mechanical and biologic device for restoring a vertically defected premaxilla.
