알카자이트 수복재와 자이오머의 상아질 접착제를 투과하는 불소 유리 평가 (Assessment of Fluoride Release through Dentin Adhesive in the Alkasite Restorative Material and Giomer)

  • 이 연구의 목적은 알카자이트 수복재와 자이오머에서 방출되는 불소가 상아질 접착층을 투과하는 유리되는 양을 측정하고 비교하는 것이었다. 알카자이트 수복재와 자이오머 및 불소 미함유 복합레진의 시편을 각 재료 당 20개씩 제작하여 그 중 10개의 시편에 상아질 접착제를 도포하였다. 만들어진 시편을 2.0 mL의 탈이온수가 들어있는 폴리에틸렌튜브에 넣고 37.0℃ 항온수조에 연구 기간동안 보관하였다. 보관 후 1일, 3일, 7일, 14일, 21일, 28일째에 불소 유리량을 측정하였으며 매 측정 후 탈이온수를 교체하였다. 상아질 접착제층 두께의 적절성은 시편을 추가로 제작하여 전계방사형 주사전자현미경으로 관찰하여 평가하였다. 알카자이트 수복재와 자이오머에 상아질 접착제를 도포한 경우 접착제가 불소의 유리를 완전히 차단하지는 않았지만, 유리된 불소의 양이 현저히 감소하였다. 28일간 측정한 알카자이트 수복재의 불소 유리량은 상아질 접착제의 유무와 관계없이 자이오머보다 크게 나타났다.

불소도포한 상아질면의 산처리가 상아질접착에 미치는 영향 (EFFECTS OF ACID TREATMENT OF FLUORIDE APPLIED DENTIN SURFACE ON DENTIN BONDING)

  • The purpose of this study was to investigate the effect of acid treatment of fluoride applied dentin surface with various concentrations of phosphoric acid for various periods of time on dentin bonding. Dentin specimens prepared from freshly extracted bovine mandibular anterior teeth were divided into fluoridated and nonfluoridated groups. Specimens of nonfluoridated group were pretreated with 10% phosphoric acid for 15 seconds. Those of fluoridated groups were treated with 2% sodium fluoride or 2% stannous fluoride solution for 5 minutes and stored in $37^{\circ}C$ distilled water for 3 days, followed by phosphoric acid treatment. The concentrations of phosphoric acid were 10%, 32% or 50% and the treatment periods of time were 15, 30 or 60 seconds. All the specimens were bonded with All Bond$^{(R)}$ 2 and Bisfil$^{TM}$ composite resin. After bonded specimens were stored in $37^{\circ}C$ distilled water for 24 hours, tensile bond strengths of each specimens were measured and the pretreated dentin and the fractured dentin surfaces were examined under the scanning electron microscope. The results were as follows : The tensile bond strengths from the fluoridated groups were significantly lower than those from the nonfluoridated group when the concentrations of phosphoric acid and the treatment periods of time were equal in all the groups (p<0.05). In general, the higher the concentration of phosphoric acid and the longer the treatment period of time for acid etching on the fluoride applied dentin surface, the higher were the bond strength values. Recovery of bond strength of the dentin bonding agent was better in the NaF applied group than in the $SnF_2$ applied one. SEM findings of NaF applied and $SnF_2$ applied dentin surfaces demonstrated reaction product-covered and partially or completely obstructed dentinal tubules. SEM findings of dentin surfaces fluoridated for 5 minutes followed by etching showed wider tubular openings and more clean dentin surfaces when dentin was etched with higher concentration of phosphoric acid for longer period of time. On the SEM observations of the fractured dentin-resin interface, the etched specimens of fluoridated group showed an adhesive failure mode when the concentration of phosphoric acid and the treatment period of time were same as in the nonfluoridated group. As the concentration of phosphoric acid and the treatment period of time increase during acid etching, the cohesive failure area increased. However, excessive acid etching caused adhesive failure.

Is dentin biomodification with collagen cross-linking agents effective for improving dentin adhesion? A systematic review and meta-analysis

  • Objectives: The aim of this investigation was to evaluate the effectiveness of collagen cross-linking agents (CCLAs) used in combination with the adhesive technique in restorative procedures. Materials and Methods: In this systematic review, the authors followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. An electronic search was performed using PubMed, Scopus, Web of Science, Cochrane Library, LILACS, and DOSS, up to October 2020. The gray literature was also researched. Only randomized clinical trials were selected. Results: The selection process yielded 3 studies from the 838 retrieved. The addition of CCLAs in the retention of restorations increased the number of events. The postoperative sensitivity scores and marginal adaptation scores showed no significant difference between the CCLA and control groups, and the marginal pigmentation scores showed a significant increase in the CCLA group. There were no caries events in any group throughout the evaluation period. Conclusions: This systematic review showed that there is no clinical efficacy to justify the use of CCLAs in the protocols performed.

  • Before resin composites can serve as durable substitutes for amalgam restorations, the adhesive system used to retain them and add to seal dentin must be improved. The recent observations, using the newly developed LVDT (linear variable differential transformer) probe, that all commercial bonding systems cause a collapse of the demineralized dentin matrix, simulated a re-evaluation of the effects of these formulations on the matrix. The experiment was begun by examining solvents such as water-HEMA mixtures. These produced the same shrinkages as did the complete formulations.(omitted)

간접수복용 복합레진과 자가 접착 레진 시멘트의 전단결합강도에 레진코팅법이 미치는 영향 (Shear bond strength of a self-adhesive resin cement to resin-coated dentin)

  • 연구 재료 및 방법: 본 연구에서는 간접 레진 수복시에 상아질 접착제와 저점도 레진을 이용한 상아질 레진코팅법을 자가 접착 레진 시멘트에 적용해 전단결합강도가 증가하는지 비교하고 기존의 레진 시멘트와의 전단결합강도 차이가 어떠한지 평가하고자 하였다. 연구 목적: 사람의 발거된 건전한 구치 36개의 치관부를 삭제하고 상아질을 노출시킨 후 3개의 군으로 분류하였다. 1군과 3군은 상아질에 아무런 처리 과정을 거치지 않고 2군만 Clearfil SE bond와 Metafil Flo를 사용해 상아질 레진코팅법을 시행하였다. 24시간의 임시수복 기간을 둔 후 Tescera를 이용하여 제작한 레진 인레이 블록을 1군과 2군에는 Unicem, 3군에는 Panavia F를 사용해 접착하였다. 그 후 증류수에 48시간 동안 보관한 후 전단결합강도를 측정하였다. 측정된 전단결합강도는 one-way ANOVA를 이용하여 비교 분석하였으며 유의한 차이가 있는 경우 Tukey method를 이용하여 multiple comparison를 시행하였다. 결과:상아질에 레진코팅법을 시행한 경우의 Unicem의 결합강도가 상아질에 아무런 처리를 하지 않은 대조군의 결합강도보다 높게 나타났고(P<.0001), 레진코팅을 하지 않고 Panavia F로 접착한 실험군의 결합강도보다 높게 나타났다(P<.0001). 결론: 상아질 레진코팅법은 자가 접착 레진 시멘트의 접착 강도를 증가시켰다.

5급 와동에서 수종의 상아질 접착제와 복합레진의 종류에 따른 미세누출 (The Microleakage in Class V Cavity Based on Different Kinds of Several Dentin Adhesive and Composite Resin)

  • 5급 와동에서 상아질 접착제와 레진의 종류에 따른 미세누출을 알아보고자 64개의 건전한 상 하악 소구치 및 대구치에 5급 와동을 형성하였다. 형성된 와동에 4종의 상아질 접착제를 각각 적용하고 2종류의 복합레진을 수복하여 법랑질과 상아질 변연부 색소침투 정도를 실체현미경하에서 23배율로 관찰하여 미세누출 점수를 기록한 후 각 군 간의 유의성을 검정하였다. 연구 결과, 5급 와동 수복 시 상아질 접착제와 레진의 종류에 따른 미세누출은 통계적으로 유의하지 않았다. 그러나 복합레진 Filtek Z350XT Universal 적용 시 1단계 자가부식 접착제 Adaper Easy Bond에서 미세누출이 낮게 나타났고, 유동성레진 Filtek Z350XT Flowable 적용 시 3단계 전부식 접착제 Adaper Scotchbond Multi-Purpose, 2단계 전부식 접착제 Adaper Single Bond 2에서 미세누출이 낮게 나타났다. 따라서 복합레진 및 상아질 접착제의 선택적 적용이 가능하며 특히, 1단계 자가부식 상아질 접착제 적용 시 조작의 편이성과 시술시간의 단축에 유리할 것으로 생각된다.

한 단계 자가부식 접착제를 이용한 복합레진 수복 시 타액오염 후 처치 방법에 따른 미세인장강도 비교 (Effect of Saliva Decontamination on Bond Strength of 1-step Self-etching Adhesives to Dentin of Primary Posterior Teeth)

  • 이 연구의 목적은 유치에서 한 단계 자가부식 접착제를 이용한 레진수복 시 타액오염 후 처치과정에 따른 미세인장강도를 비교하는 것이다. 건전한 유구치를 접착제 사용에 따라 각각 $Scotchbond^{TM}$ Universal Adhesive (SBU), All-Bond $Universal^{(R)}$ (ABU), and $Tetric^{(R)}$ N Bond Universal (TBU)의 3 그룹으로 나누어 무작위로 분배하였다. 각 그룹은 접착제 도포 후 처치과정에 따라 7가지 하위분류로 나누었다. Subgroup I은 대조군으로써, 타액오염 없이 제조사 권고사항에 따라 접착제를 적용하였다. Subgroup II - IV는 접착제 광중합 전에 타액오염을 시행하였고, subgroup V - VII는 접착제 광중합 후에 타액오염을 시행하였다. 그 후 subgroup II, V는 건조, subgroup III, IV는 수세 후 건조, subgroup IV, VII는 수세 후 건조, 그리고 접착제 재도포를 시행하였다. SBU와 ABU에서 subgroup I, IV, VII의 미세인장강도가 subgroup II, III, V, VI의 미세인장강도보다 유의하게 높았다. TBU에서 subgroup I, IV의 미세인장강도가 subgroup II, III, V, VI의 미세인장강도보다 유의하게 높았다. 이 연구 결과, 임상가는 한 단계 자가부식 접착제를 이용한 레진 수복 시 타액과 수분의 오염으로부터 철저하게 격리를 지켜야 하지만 그럼에도 불구하고 타액오염이 발생하였다면, 수세 및 건조 후 접착제를 재도포할 것을 권장한다.

복합레진 수복시 상아질 표면 처리가 미세 변연 누출에 미치는 영향에 관한 연구 (THE EFFECT OF MOISTENING OF ETCHED DENTIN AND ENAMEL SURFACE ON THE MICROLEAKAGE OF COMPOSITE RESINS)

  • The purpose of this study was to evaluate the effect of moistening and air-drying of acid conditioned dentin and enamel on the marginal microleakage. In this study, Class V cavity were prepared on both buccal and lingual surface of sixty extracted human premolars with cementum margin. These specimens were randomly devided into three groups and three dentin adhesives(Scotchbond Multi-Purpose, All bond 2, Prisma Universal Bond 3) were applied to each group. The specimens in each group were subdevided into four groups (Wet/primed, Dry/primed, Wet/not primed, Dry/not primed) and the etched dentin and enamel surface were treated these four surface treatments prior to the placement of a bonding agent or adhesive. Wet/primed group was simply blot-dried with a damp facial tissue before primer placement ; Dry/primed group was air dried for 30 seconds before the placement of a primer ; Wet/not primed group and Dry/not primed group were not primed after blot dried and air dried for 30 seconds each group. The bonding agent and composite resin were applied for each group. All specimens were exposed to 500 cycle of thermal stress. Specimens were placed in a silver nitrate solution and then sectioned buccolingually through the center of the restoration. The dye penetrations of the specimens were observed with a stereo microscope. The statistical test were applied to the results using a one way analysis variance (ANOVA) and Duncan's multiple range test. The aspects of silver ion penetration into the resin/dentin interface were examined under scanning electron microscopy. The results were as follows. 1. In all groups, the enamel margin showed significantly lower leakage value than the cementum margin (p<0.05). 2. Regardless of various surface treatment and dentin adhesives, there was no significant difference at the enamel margins (p>0.05). 3. At the dentin margins, the leakage values of Dry/not primed group showed significantly higher than that of the other groups (p<0.05). The leakage values of Wet/primed group showed significantly lower than that of the other groups, but, there was no significant difference between Wet group and Dry group. 4. There was no significantly difference between the dentin adhesives regarding the surface treatments in all groups(p>0.05). 5. On the backscatterd scanning electron microscopy observation, the penetration of the silver ion occured at the bonding resin/dentin interface. In the Wet/primed group, resindentin hybrid zone and resin penetration into the dentin was observed. The resin tags were compactively formed to a thickness of $3\sim4{\mu}m$ at the upper part of dentinal tubules. In the Dry/primed group, the thickness of the hybrid zone and the diameter, depth of the resin tags diminished. In the Non-primed groups, the hybrid zone was not identified and few resin tag was observed. There was the gap formation in the resin/dentin interface.

복합레진 수복시 복합용기 및 단일용기 상아질 접착제의 미세변연누출에 관한 연구 (MICROLEAKAGE OF CURRENT DENTIN BONDING SYSTEMS)

  • The purpose of this study was to evaluate the microleakage of 5 current dentin bonding systems which are composed of 2 multi-bottle systems(Scotchbond Multi-Purpose, All Bond2) and 3 one-bottle systems(Single bond, One-Step, Prime & Bond). In this in vitro study, class V cavities were prepared on buccal and lingual surfaces of sixty extracted human premolars and molars on cementum margin. The experimental teeth were randomly divided into six groups of 10 samples (20 surfaces) each, Group 1 : Scotchbond Multi-Purpose ; Group 2 : All Bond 2 ; Group 3 : Single Bond ; Group 4 : One-Step ; Group 5 : Prime & Bond ; Group 6 : no bonding agent(control). The bonding agent and composite resin were applied for each group following the manufacturer's instructions. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 60 teeth were placed in 2% Methylene blue dye for 24 hours, then rinsed with tab water. The specimen were embedded in clear resin, then sectioned buccolingually through the center of restoration with a low speed diamond saw. The dye penetration on each of the specimen were then observed with a stereomicroscope at ${\times}20$. The results of study were statistically analyzed using the Student-Newmann-Keul's Methods and the Mann-Whitney Rank Sum Test. The resin/dentin interfaces were examined under Scanning Electron Microscopy. The results of this study were as follows. 1. None of the dentin bonding systems used in this study showed significant difference in leakage values at both the enamel and the dentin margins (P>0.05). 2. In all groups except the control, leakage value seen at the enamel margin was significantly lower than that seen at the dentin margin (P<0.05). 3. Compared to the control group, all the groups treated with dentin bonding systems showed significantly lower leakage value at both enamel and dentin margins (P<0.05). 4. In the SEM view, gaps were observed in the composite resin / dentin interface in group 6 where no dentin bonding agent was used, and in all the other groups (group 1, 2, 3, 4, 5) composite resin, hybrid layer, and dentin were seen to be closely adhering to each other where there were no leakages. Well-developed resin tags 3~100${\mu}m$ in length infiltrated dentinal tubules past the hybrid layer and a hybrid layer 1~5${\mu}m$ thick had developed between the dentinal surface and the composite resin surface.

상아질 지각완화제가 레진시멘트의 전단결합강도에 미치는 영향 (EFFECT OF DENTIN DESENSITIZERS ON SHEAR BOND STRENGTH OF RESIN CEMENTS)

  • The purpose of this study was to investigate the effects of desensitizer on shear bond strength of adhesive resin cements for all ceramic crown. For this study, Gluma $desensitizer^{(R)}$(Heraeus Kulzer, Germany) and MS $Coat^{(R)}$(Sun medical, Japan) were used as desensitizer, and Bistite $II^{(R)}$ resin cement(Tokuyama, Japan) and Variolink $II^{(R)}$ resin cement (Ivoclar, Liechtenstein) were used. IPS Empress ceramics were bonded to dentin surfaces after application of desensitizer and shear bond strength of specimens were measured using UTM(Zwick 1456 41. Zwick, Germany) at a crosshead speed of 1mm/min. The obtained results were as follows : 1. The shear bond strength of both resin cements were not affected by Gluma $desensitizer^{(R)}$ containing HEMA 2. The shear bond strength of Bistite $II^{(R)}$ resin cement was significantly decreased by MS $Coat^{(R)}$. 3. The failure mode in MS $Coat^{(R)}-Bistite \;II^{(R)}$ group was mainly adhesive type between dentin and resin cement. but that of Gluma $desensitizer^{(R)}$-Variolink $II^{(R)}$ group was mainly mixed type, combined adhesive failures between dentin and resin cement or porcelain and resin cement.
