완전 탈구된 미성숙 영구치의 치수재생치료 증례 보고 (Outcome of Regenerative Endodontic Treatment for an Avulsed Immature Permanent Tooth: A Case Report)

  • 치아의 완전 탈구는 치조골에서 치아가 완전히 이탈된 것으로 정의되며, 결과적으로 신경혈관 공급의 중단을 야기하는 가장 심각한 치과적 손상 중 하나로 알려져 있다. 완전 탈구는 조직 허혈(tissue ischemia)을 야기하며, 이는 치수 괴사를 초래할 수 있다. 치근단형성술(apexification)은 치수 괴사로 진단된 미성숙 영구치에서 치근단 장벽을 유도하는 전통적인 치료 방법이다. 하지만 치근단형성술로는 치근 길이 및 두께의 증가를 포함하는 치근 발육을 얻을 수 없다. 본 증례는 완전 탈구되어 재식된 이후 치수 괴사로 진단된 치아를 가진 5세 환자에서 ciprofloxacin, metronidazole, cefaclor 및 CollaTape과 Biodentine을 이용하여 시행된 치수재생치료(regenerative endodontic treatment)의 임상적 및 방사선학적 결과를 다루고 있다.

X 염색체 우성 저인산혈증성 구루병: 13년간의 치험례 (X-LINKED DOMINANT HYPOPHOSPHATAEMIC RICKETS: 13 YEARS FOLLOW-UP STUDY)

  • 본 증례에서는 본과에서 발견한 임상적인 특징을 통해 X 염색체 우성 저인산혈증성 구루병으로 진단받아 13년간 의과적 및 치과적인 처치를 받고 있는 환아에 대해 보고하였다. 의과적으로 환아는 저신장 및 휜 다리 등의 전형적인 구루병 증상을 나타내었고, 치과적으로도 넓은 치수강, 상아법랑경계까지 연장된 치수각 및 자발적 치아농양 등의 전형적인 증상을 나타내었다. 본 환아는 정기적인 검진을 통해 유구치와 영구 소구치 및 대구치 모두 치면열구전색 및 수복치료 등을 적극적으로 시행해왔다. 그러나 예방적인 처치에도 불구하고 유치열에서는 총 8개의 치아에서 치아 우식에 이환 없이 자발적 치아농양이 발생하여 치수절제술을 시행하였으며, 조기 상실 등의 합병증 없이 영구치교환 시점까지 유지할 수 있었다. 영구치열에서는 그 빈도가 낮아졌으나 하악 우측 제1, 2대구치에서 발생한 자발적 치아농양으로 치료를 완료하였다. 앞으로 치아교모 등의 원인으로 영구치에서 발생할 수 있는 치아농양을 예방하기 위해 예방적인 전장관수복치료가 필요하리라 생각된다.

Shell teeth 환자의 전신마취 하 치과치료 : 증례보고 (DENTAL TREATMENT OF SHELL TEETH UNDER GENERAL ANESTHESIA: A CASE REPORT)

  • 부산대학교 치과병원 소아치과에 내원한 Shell teeth에 이환된 소아 환자를 전신마취 하에 치료하였다. 임상검사시 다수 치아의 치관 파괴와 치수 노출 및 상악 유전치 잔존치근이 관찰되었다. 방사선검사상 전반적인 유치열에서 정상보다 큰 크기의 치수강과 넓은 치근관 및 얇은 경조직 층이 존재했으며 발생중인 영구치 치배의 형태이상이 관찰되었다. 환자의 언니와 아버지를 포함한 부계 가족 구성원 일부에서 상아질형성부전증 병력이 조사되었다. 치료시 수복이 불가능한 치아는 발거하였고 보존 가능한 치아는 치수치료 및 기성금속관, 복합레진 수복치료를 시행하였다. 치료 결과 환자의 통증이 감소하였고 저작능력이 회복되었다. 주기적인 내원 및 경과관찰이 요구된다.

Effects of Micro-Electrical Stimulation on Regulation of Behavior of Electro-Active Stem Cells

  • Purpose: Stem cells provide new opportunities in the regenerative medicine for human or animal tissue regeneration. In this study, we report an efficient method for the modulating behaviors of electro-active stem cells by micro-electric current stimulation (mES) without using chemical agents, such as serum or induction chemicals. Methods: Dental pulp stem cells (DPSCs) were cultured on the tissue culture dish in the mES system. To find a suitable mES condition to promote the DPSC functions, the response surface analysis was used. Results: We found that a working micro-current of 38 ${\mu}A$ showed higher DPSC proliferation compared with other working conditions. The mES altered the expressions of intracellular and extracellular proteins compared to those in unstimulated cells. The mES with 38 ${\mu}A$ significantly increased osteogenesis of DPSCs compared with ones without mES. Conclusions: Our findings indicate that mES may induce DPSC proliferation and differentiation, resulting in applying to DPSCs-based human or animal tissue regeneration.

Comparative efficacy of photo-activated disinfection and calcium hydroxide for disinfection of remaining carious dentin in deep cavities: a clinical study

  • Objectives: To comparatively evaluate the efficacy of photo-activated disinfection (PAD), calcium hydroxide (CH) and their combination on the treatment outcome of indirect pulp treatment (IPT). Materials and Methods: Institutional ethical clearance and informed consent of the patients were taken. The study was also registered with clinical registry of India. Sixty permanent molars exhibiting deep occlusal carious lesion in patients with the age range of 18 - 22 yr were included. Clinical and radiographic evaluation and set inclusion and exclusion criteria's were followed. Gross caries excavation was accomplished. In group I (n = 20) PAD was applied for sixty seconds. In group II (n = 20), CH was applied to the remaining carious dentin, while in group III (n = 20), PAD application was followed by CH placement. The teeth were permanently restored. They were clinically and radiographically followed-up at 45 day, 6 mon and 12 mon. Relative density of the remaining affected dentin was measured by 'Radiovisiography (RVG) densitometric' analysis. Results: Successful outcome with an increase in radiographic grey values were observed in all three groups. However, on inter-group comparison, this change was not significant (p > 0.05). Conclusions: PAD and CH both have equal disinfection efficacy in the treatment of deep carious dentin. PAD alone is as effective for treatment of deep carious lesion as calcium hydroxide and hence can be used as an alternative to CH. They can be used independently in IPT, since combining both does not offer any additional therapeutic benefits.

전부도재관용 레진시멘트의 생체적합성에 관한 연구 (A STUDY ON BlOCOMPATABILITY OF RESIN CEMENTS FOR ALL-CERAMIC CROWN)

  • Statement of problem : Resin cements were used widely on all ceramic crowns, but the influence of resin cements on biocells was not understood clearly. Purpose : This study was investigated to evaluate the biocompatibility of resin cements for all-ceramic crowns. Material and Method : The resin cements used in this study were Panavia F (Kuraray Co., Ltd. Japan), Variolink II (Vivadent Ets., Schann / Liechtenstein), and Bistite II (Bistite dual cure resin cement-clear Tokuyama Soda Co. Japan). The viability of normal human oral keratocytes, gingival fibroblast, and gingival fibroblast immortalized by Human Papilloma virus 16 was measured in vitro for evaluation of cytotoxicity on resin cements, and the response of pulp tissue was analyzed and evaluated with light microscope after application of cements at cutting edge of incisors. Results : The normal human oral keratocytes was the most sensitive to toxicity of resin cement, and toxicity of cements was higher in Bistite II than in Variolink II. The cell viability of immortalized gingival fibroblast did not affected by type of cement and cultivation period, but there was a tendency that cytotoxicity in Bistite II was higher than in Variolink II. The cell viability of gingival fibroblast was similar to that of immortalized gingival fibroblast regardless of cement type, but Bistite II showed more toxic than others after 5 days cultivation. The responses of pulp tissue according to cement type were similar after 2 days cultivation, but revealed high toxicity in Bistite II after 10 days cultivation. Conclusion : Variolink II was more biocompatible than any other resin cements used in this study.

Effect of pre-operative medication with paracetamol and ketorolac on the success of inferior alveolar nerve block in patients with symptomatic irreversible pulpitis: a double-blind randomized clinical trial

  • Background: The efficacy of local anesthesia decreases in patients with symptomatic irreversible pulpitis. Therefore, it was proposed that the use of premedication with an anti-inflammatory drug might increase the success rate of pulpal anesthesia in mandibular posterior teeth with vital inflamed pulp. Methods: One hundred thirty-four patients who were actively experiencing pain willingly participated in this study. The Heft Parker (HP) visual analog scale (VAS) was used to record the initial pain intensity. Patients were randomly allocated to receive a placebo, 10 mg of ketorolac, and 650 mg of paracetamol. The standard inferior alveolar nerve block (IANB) was administered to all patients using 2% lidocaine with 1:200,000 adrenaline after one hour of medication. After 15 min, the patient was instructed to rate the discomfort during each step of the treatment procedure, such as access to remaining dentin, access to the pulp chamber, and during canal instrumentation on the HP VAS. IANB was considered successful if the patient reported no or mild pain during access preparation and instrumentation. Moderate or severe pain was classified as a failure of IANB and another method of anesthesia was used before continuing the treatment. Results: The rate of successful anesthesia in the placebo, paracetamol, and ketorolac groups was 29%, 33%, and 43%, respectively, and no statistically significant difference was found between the groups. Conclusion: Preoperative administration of paracetamol or ketorolac did not significantly affect the success rate of IANB in patients with irreversible pulpitis. No significant difference was observed between the paracetamol and ketorolac groups.

A new phantom to evaluate the tissue dissolution ability of endodontic irrigants and activating devices

  • Objective: The aim of this study was to introduce a gelatin/bovine serum albumin (BSA) tissue standard, which provides dissolution properties identical to those of biological tissues. Further, the study evaluated whether the utilization of endodontic activating devices led to enhanced phantom dissolution rates. Materials and Methods: Bovine pulp tissue was obtained to determine a benchmark of tissue dissolution. The surface area and mass of samples were held constant while the ratio of gelatin and BSA were varied, ranging from 7.5% to 10% gelatin and 5% BSA. Each sample was placed in an individual test tube that was filled with an appropriate sodium hypochlorite solution for 1, 3, and 5 minutes, and then removed from the solution, blotted dry, and weighed again. The remaining tissue was calculated as the percent of initial tissue to determine the tissue dissolution rate. A radiopaque agent (sodium diatrizoate) and a fluorescent dye (methylene blue) were added to the phantom to allow easy quantification of phantom dissolution in a canal block model when activated using ultrasonic (EndoUltra) or sonic (EndoActivator) energy. Results: The 9% gelatin + 5% BSA phantom showed statistically equivalent dissolution to bovine pulp tissue at all time intervals. Furthermore, the EndoUltra yielded significantly more phantom dissolution in the canal block than the EndoActivator or syringe irrigation. Conclusions: Our phantom is comparable to biological tissue in terms of tissue dissolution and could be utilized for in vitro tests due to its injectability and detectability.

β-glycerophosphate 혼합시 인간 치수 세포에 대한 Portland cement의 생활성에 관한 연구 (A BIOACTIVITY STUDY OF PORTLAND CEMENT MIXED WITH β-GLYCEROPHOSPHATE ON HUMAN PULP CELL)

  • $\beta$-glycerophosphate는 치수의 상아모세포 분화를 촉진하는 물질이다. Portland cement는 수중에서 장기간에 걸쳐 용해되기 때문에 $\beta$-glycerophosphate을 혼합한 Portland cement는 수산화칼슘과 함께 $\beta$-glycerophosphate를 장기간 용출하게 된다. 본 실험에서는 $\beta$-glycerophosphate을 혼합한 Portland cament에 대한 인간치수세포의 반응을 알아보았다. 인간 치수 세포에 대한 $\beta$-glycerophosphate의 효과를 알아보기 위해 다양한 농도의 $\beta$-glycerophosphate와 dexamethasone에 대한 인간 치수 세포의 ALP activity을 측정하였고 alizarin red S로 염색하여 관찰하였다. $\beta$-glycerophosphate가 다양한 농도(10 mM, 100 mM, 1 M)로 혼합된 Portland cement에 대한 인간 치수 세포의 MTS assay, ALP activity를 측정하고 SEM으로 관찰하였다. 치수세포의 석회화 정도를 관찰한 연구에서 $\beta$-glycerophosphate와 dexamethasone 단독으로 적용하였을 때 거의 효과가 없었으나 5 mM $\beta$-glycerophosphate와 100 nM dexamethasone을 혼합 적용하였을 때 가장 높은 ALP acticity를 보였다. 분화제를 첨가하거나 첨가하지 않은 모든 실험군에서 치수세포에 대한 독성은 관찰되지 않았으며 Portland cement에 10 mM $\beta$-glycerophosphate을 혼합한 시편의 ALP activity가 대조군에 비교하여 가장 많이 증가하였다. 결론적으로 $\beta$-glycerophosphate이 혼합된 Portland cement는 세포 독성이 없으며 첨가물이 없는 Portland cement에 비해 치수 분화 및 석회화를 더 많이 일으키므로 임상적으로 $\beta$-glycerophosphate을 혼합한 Portland cement 적용은 재료 하방에 더 많은 상아질을 형성시킬 것으로 추측된다.

골절된 사자 견치의 근관치료 및 Crown 장착 1례 (A Case of Root Canal Treatment and Crown Therapy on Fractured Canine Teeth of Lion)

  • Seven years old male lion in Everland Zoo has three fractured canine tooth. To avoid the risk of a possible fracture with pulp exposure after restroration of largest abration defects, endodontic therapy was performed. The pulp chamber was filled with calcium hydroxide, zinc oxide cement and gutta percha. We also installed the Crown prosthesis was installed in fractured canine tooth to protect the tooth and to improve the cosmetics and function of the tooth. The lion could eat in one day and there were no any signs of dental problem. It is the obvious prophylactic procedure for wild animal is the inclusion of an oral examination at every opportunity to handle the animal. It is considered that crown prosthesis after endodontic therapy was useful for maintaining normal physiological function and for provention of additional fracture or complication.
