Tissue conditioner in edentulous patients, depending on the time and the thickness of the changes affect the dimensional stability (무치악 환자에서 조직양화제(tissue conditioner)가 시간과 두께에 따라 체적안정성의 변화에 미치는 영향)

  • This study evaluated the linear dimensional changes with the factor of time and thickness using of three commercially available tissue conditioners (Coe-Comport, Visco-Gel, Soft-Liner). The thickness of materials were changed (1.5mm, 3.0mm) and the percentage changes in dimension were measured at 1h(baseline), 12h, 24h, 3 days and 7days after specimen preparation. The obtained data were analyzed by ANOVA with the SAS/PC statistical package. From the results, large differences appear between the various tissue conditioners. The results suggested that the period recommended for forming functional impression would be 36h to 3days after insertion in the mouth. Depending on the type of tissue conditioner over time, as there were significant differences in the elastic change(p<0.05). Tissue conditioner of the 1.5mm, 3.0mm thickness were significantly different by the dimensional stability(p<0.05). Elastic deformation of the ideal itself, and resilient when compared only the look, Visco-Gel 3.0mm group, stability was the most stable volume. In addition, it is important to select tissue conditioners suitable for functional impression because of the wide range of dimensional stability among the materials.

PROMOTER METHYLATION OF THE CDH-13 GENE IN THE ORAL SQUAMOUS CELL CARCINOMA (구강 편평상피암종에서 CDH-13 유전자의 promoter methylation에 대한 연구)

  • CDH-13(T-cadherin), which is one of a kind among the 20 cadherins, can be found mainly in wall of aorta, neuron, spleen, blood vessel etc. It is also called H-cadherin. This structural difference can explain that CDH-13 is thought to play a key role in maintaining mutual relation between extra and intra-cellular environment rather than in cell adhesion. The main function of CDH-13 is to participate in blood vessel function. Additionally, it is known to regulate cell growth and cell contact inhibition. When cells are proliferating, cell surface perceives other cells so that substance such as CDH-13 can inhibit their growth or proliferation resulting in homeostasis without endless proliferation or invasion of connective tissue boundaries. However, tumor cell itself appears to be different from normal cells' growth, invasion or transmission. Therefore, it can be diagnosed that these characteristics are closely related to expression of CDH-13 in tumor cells. This study is to investigate expression of CDH-13 in SCC and its correlation with promoter methylation. 20 of tissue species for the study are excised and gathered from 20 patients who are diagnosed as SCC in department of OMS, dental hospital, dankook university. To find development of CDH-13 in each tissue samples, immunohistochemical staining, RT-PCR gene analysis and methylation specific PCR are processed. The results are as follows. 1.Immunohistochemical staining: In normal oral squamous epithelial tissue, strong expression of CDH-13 was found in cell plasma membrane of basal cell layer. On the other hand, in case of low-differentiated oral SCC, development of CDH-13 was hardly seen. 2.The development of CDH-13 gene: In 9 of samples, expression of CDH-13 gene could be seen and 2 of them showed low expression compared to the others. And rest of the 11 samples showed no expression of CDH-13 gene. 3.Methylation of CDH-13 gene: Among 9 samples which expressed CDH-13 gene, 7 of them showed unmethylation. In addition, among 11 samples without CDH-13 gene expression, 10 showed methylation. According to the results stated above, promoter methylation were found in 13 samples(65%) among 20 of oral SCC samples. In low-differentiated SCC, suppression of gene expression could be seen accompanying promoter methylation. These phenomenon of gene expression was proved by immunohistochemical investigation. Finally, for development of oral SCC, conclusions can be made that suppression of CDH-13 played a main role and suppression of gene expression was originated from promoter methylation. Considering this, it is expected that suppression of CDH-13 from promoter methylation to be utilized as a good diagnostic marker of oral SCC.

Knowledge and Satisfaction Status of Tooth Bleaching (치아미백에 대한 지식과 만족도 현황)

  • The purpose of this study was to investigate the knowledge and satisfaction of tooth bleaching. From June 1st, 2017, 328 patients who visited a dental clinic in Ulsan Metropolitan City for two weeks were asked to do a self - filling questionnaire. As a result of the study, the tooth bleaching satisfaction according to tooth bleaching knowledge was not statistically significant (p<0.05), and the highest(8-10) degree of satisfaction was 3.86 when the knowledge of tooth bleaching was high, and the lowest(0-3) was 3.00. The tooth bleaching satisfaction according to general characteristics was significantly different in age and gender. The score of women were 3.74 and higher than men's 3.11 The tooth bleaching experience and tooth bleaching satisfaction were statistically significant (p <0.05) and professional tooth bleaching (4.41) was higher than OCT bleaching (2.64). Also, when the tooth bleaching was done by the invitation of the hospital, it was higher than the case of experiencing tooth bleaching through media advertising. Therefore, the satisfaction of tooth bleaching is required not only for general characteristics but also for tooth tooth bleaching, and it is influenced by tooth bleaching method and tooth bleaching motivation. These factors may be useful to establish a treatment plan to improve the satisfaction of tooth bleaching.

Evaluation of orthodontic treatment outcomes and post-treatment changes by the PAR index (PAR 지수를 이용한 교정치료 결과의 평가에 관한 연구)

  • The purposes of this study were to assess the treatment results in orthodontic department, Kyung Hee University, to evaluate the occlusion at 2 year follow-up control, in relation to the original malocclusion, and changes occurring in the follow-up period, and to determine whether treatment and posttreatment changes of the PAR index were related to the original Angle classification Dental study casts of 25 patients in permanent dentition were evaluated using Peer Assessment Rating (PAR) index. The PAR index was measured at the pretreatment stage, immediately after treatment, and at 2 yews posttreatment. The following results were obtained : 1. The treatment reduced the PAR index by 85.05% and it indicates that KHDC clinic has a high standard of treatment 2. The follow-up result of 75.40% PAR score reduction implies a gain of 9.55% of posttreatment PAR score during follow-up period. 3. There is no relationship between the treatment changes of the PAR index and the orignal Angle classification.

Comparison of landmark position between conventional cephalometric radiography and CT scans projected to midsagittal plane (3차원 CT자료에서 선정된 계측점을 정중시상면으로 투사한 영상과 두부계측방사선사진상의 계측정의 위치 비교)

  • Objective: The purpose of this study is to compare landmark position between cephalometric radiography and midsagittal plane projected images from 3 dimensional (3D) CT. Methods: Cephalometric radiographs and CT scans were taken from 20 patients for treatment of mandibular prognathism. After selection of land-marks, CT images were projected to the midsagittal plane and magnified to 110% according to the magnifying power of radiographs. These 2 images were superimposed with frontal and occipital bone. Common coordinate system was established on the base of FH plane. The coordinate value of each landmark was compared by paired t test and mean and standard deviation of difference was calculated. Results: The difference was from $-0.14{\pm}0.65$ to $-2.12{\pm}2.89\;mm$ in X axis, from $0.34{\pm}0.78$ to $-2.36{\pm}2.55\;mm$ ($6.79{\pm}3.04\;mm$) in Y axis. There was no significant difference only 9 in X axis, and 7 in Y axis out of 20 landmarks. This might be caused by error from the difference of head positioning, by masking the subtle end structures, identification error from the superimposition and error from the different definition.

Longitudinal measurements of tooth mobility following orthodontic treatment (교정치료 후 치아동요도 감소에 대한 종단적 계측연구)

  • Objective: The aim of the present study was to evaluate the changes in tooth mobility following orthodontic treatment and to obtain information regarding the guideline of retainer wear duration during the post-treatment period. Methods: The sample consisted of twenty patients who had been treated with edgewise appliances. The mobility of the maxillary teeth from the central incisor to the first molar was measured bilaterally by way of the $Periotest^{(R)}$, a non-invasive, electronic device that provides an objective measurement of the reaction of the periodontium to a defined impact load. Tooth mobility was monitored at the time of the removal of the orthodontic appliances and subsequently at three-month intervals during the two years following appliance removal. Results: Tooth mobility decreased rapidly for the first six months and then decreased at a slower rate during the next six months; no statistically significant decrease in mobility was observed during the second year following appliance removal. Conclusions: The results of the present study suggest that adequate tooth stabilization is critical during the first six months following appliance removal and that continued wearing of retainers is recommended at least until twelve months after the completion of orthodontic treatment.

Study of General Public's Knowledge of TMD : how predominantly is TMD understood by people (일반인에 있어서 측두하악장애의 인지도 조사에 대한 연구)

  • This study was performed to research how predominantly TMD is known among people and how exactly it is understood. The data set up by this study could be used as beneficial references. Based on this data, people can be more knowledgeable of TMD so that they can recognize the signs and symptoms of TMD. Then, the patients can visit eligible clinician, TMD professionals. Nine hundred thirty six people (426 men and 510 women, ranging from 18 to 69 years old) were selected for the subjects of this study and were investigated by use of self-administered questionnaire. The obtained results were as follows : 1. 10.68% of the subjects were reported to have heard of the term, "Temporomandibular disorders". Females occupied significantly higher percentage than males and 18-29 age group occupied higher percentage than 30-49 age and 50-69 age groups. On the other hand, even more people (52.24%) were reported to have heard of the tenn, "Jaw joint disease". It also had same sex and age prevalences, too. 2. As for the route through which people have heard of TMD, more than half (58.38%) of the people mentioned mass media of communication (T.V. radio 40.36%, newspapers magazines 15.86%, internet 2.16%). Other people who had already heard of TMD (25.05%), dentist (7.75%), and other sources (8.83%) such as physicians, physicians of Chinese medicine, pharmacists were also mentioned. There were not significant differences among sex and age groups. 3. When it comes to the cause and concept of TMD, 32.59% of the subjects considered" an inappropriate overuse of the mandible" as the cause of TMD. There were not significant differences among sex and age groups. 4. The most frequently reported presumable TMD signs and symptoms were jaw pain (61.00%), jaw joint sound (57.80%), and difficulty with mouth opening (50.11%). 5. In answer to the question who is the eligible person to treat jaw joint disease, 35.26% answered the dentists are and 41.99%, orthopedicians. Of the people who chose dentist, 30-49 age group occupied the largest part. In the case of orthopedicians, 18-29 age group was the largest. 6. Of the 7.69% of subjects who had been treated of TMD, only 5.02% of them visited the dentist for the treatment of TMD. There were not significant differences among sex and age groups. 7. In answer to the question of how to prevent development of TMD, 58.87% selected "Avoid eating hard food", 58.65%, "Avoid opening the mouth wide", and 51.07%, "Avoid chewing gum frequently".

Effect on the Pain Characteristics of Tension-Type Headache by Buccomucosal Linea Alba (협점막 백선이 긴장성 두통 환자의 통증 양상에 미치는 영향)

  • This study was designed to evaluate the pain characteristics of tension-type headache by buccomucosal linea alba. Patients with tension-type headache visited the Department of Oral Medicine, K University Dental Hospital were recruited to this study. Experimental group (n=79) was composed of tension-type headache with buccomucosal linea alba and control group (n=79) was composed of tension-type headache without buccomucosal linea alba. Evaluation list was pain quality, pain intensity, pain laterality, pain increase by routine physical activity and then it was analyzed statistically. The results were as follows : 1. Pain quality of tension-type headache patient was not significantly different by buccomucosal linea alba. 2. Pain intensity of tension-type headache patient was significantly different by buccomucosal linea alba. (p=.043). 3. Pain laterality of tension-type headache patient was not significantly different by buccomucosal linea alba. 4. Pain increase by routine physical activity of tension-type headache patient was not significantly different by buccomucosal linea alba. Therefore, it was considered that the tension-type headache patient was influenced by buccomucosal linea alba in the pain quality.

A CLINICAL STUDY ON SKELETAL ANCHORAGE SYSTEM USING MINISCREW (구내고정원을 이용한 교정 치료중 miniscrew 탈락에 관한 연구)

  • At orthodontic treatment, we have made every effort to get rigid anchorage which is not stirred when teeth move. As a result, the miniscrew that is rigid anchorage was invented recently, and now it is used widely. Concerning the advantage of miniscrew, it is reduced dependence of extraoral anchorage and it shortens treatment time for rapid tooth movement. In contrast, the defect of miniscrew is falling off it resulted from increasing of the mobility. So the purpose of this research is to be of help to prognose clinical use of miniscrew, which is inserted for intraoral anchorage, by investigating and comparing the failure rate of miniscrew for loading time. This study researches the failure rate of miniscrew for teeth movement at the orthodontic treatment. The failure rate of miniscrew in mid course, after inserting 147 miniscrews in 51 patients, is 13%(20/147). It showed no statistically significant differences as compared man with woman, maxilla with mandible, double-head with uni-head miniscrew, and drilling and non-drilling before inserting the miniscrew. In comparison below twenties with over twenties and the times that we give load to miniscrew, it produced that the failure rate of miniscrew is 9.7% higher in the case of below the twenties than over the twenties. Also, the failure rate of loading immediately is 10.8% higher than loading after 7 days. According to using driver for the insertion of miniscrew, the failure rate of miniscrew is higher in the case of using machined driver than in the case of using hand driver when the level of significance is 95%. According to the research, we can suppose that the failure rate has no concern with using miniscrew on man or woman, maxilla or mandible, the shape of head, and drilling or non-drilling before insertion of miniscrew. Therefore, we can choose eclectic miniscrew as demands. In addition, we must notify the patient, below twenties, to be possibility of high failure rate. And It is strongly recommended to give load after $1{\sim}2$ weeks for healing of the insertion area.

TREATMENT OF DENTOFACIAL DEFORMITY PATIENT WITH CEREBRAL PALSY (뇌성 마비를 동반한 악안면 기형 환자의 치험례)

  • Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.
