The Ethanol Extract of Croton Seed Inhibits the Oral Pathogen, Streptococcus mutans

  • It is noted that Streptococcus mutans (S. mutans) triggers dental caries establishment by two major factors: the synthesis of organic acids, which demineralize dental enamel, and the synthesis of glucans, which mediate the attachment of bacteria to the tooth surface. Therefore, it is noted that the development of a more effective, substantial and safe preventive agent that works against dental caries and periodontal disease is required at this time. For this reason, the present study was designed to investigate the effect of croton seed ethanol extracts on the growth, acid production, adhesion, and insoluble glucan synthesis of S. mutans. In this case, the ethanol extract of croton seed showed concentration dependent inhibitory activity against the growth, acid production and adhesion of S. mutans. Especially, it is important to note that it has produced significant inhibition at the concentration of 0.1 and 0.2 mg/ml as compared to the control group. Moreover, these results suggest that the application of croton seed extract may be considered to be a useful method for the prevention of dental caries.

Quantitative Analysis of Oral Pathogenic Bacteria according to Smoking Using Real-Time PCR

  • This study investigates the relationship between smoking and periodontal disease through quantitative analysis of intra-buccal oral pathogenic bacteria detected in smokers and aims to yield objective baseline data for applications in anti-smoking and dental health education programs. From April to May 2016, participants in an oral health management program within an intensive dental hygiene training course at Choonhae College of Health Sciences received an explanation of the study purposes and methods, after which male smokers aged 18~30 years agreed to participate voluntarily. Real-time polymerase chain reaction (PCR) analysis of oral pathogenic bacteria was performed after collecting gingival sulcus fluid samples from 67 smokers. The intra-buccal oral pathogenic bacteria distributions were analyzed based on the subjects' general characteristics, smoking behaviors, and oral care behaviors. The distribution results show that pathogens in the anterior teeth are affected (in this order) by age, toothbrush size, and smoking status; older people had fewer pathogens, those who used larger toothbrushes had more pathogens, and smokers had more pathogens, compared to non-smokers ($_{adj}R^2=19.1$). In the posterior teeth, pathogens were influenced (in this order) by smoking status, smoking duration, and the number of tooth brushings per day; smokers had more pathogens than non-smokers, and those who brushed their teeth more often had fewer pathogens ($_{adj}R^2=25.1$). The overall pathogen distribution was affected only by smoking status: smokers generally had more pathogens, compared to non-smokers. Therefore, it is necessary to provide information about the risk of periodontal disease due to smoking during anti-smoking or dental health education sessions; particularly, the use of smaller toothbrushes for anterior teeth and the need for smokers in their early twenties to quit smoking for dental health should be highly emphasized.

Induction of Prostaglandin E2 by Porphyromonas gingivalis in Human Dental Pulp Cells

  • Cyclooxygenase-2 (COX-2)-mediated prostaglandin $E_2$ ($PGE_2$) plays a key role in development and progression of inflammatory responses and Porphyromonas gingivalis is a common endodontic pathogen. In this study, we investigated induction of COX-2 and $PGE_2$ by P. gingivalis in human dental pulp cells (HDPCs). P. gingivalis increased expression of COX-2, but not that of COX-1. Increased levels of $PGE_2$ were released from P. gingivalis-infected HDPCs and this $PGE_2$ increase was blocked by celecoxib, a selective COX-2 inhibitor. P. gingivalis activated all three types of mitogen-activated protein kinases (MAPKs). P. gingivalis-induced activation of nuclear $factor-{\kappa}B$ ($NF-{\kappa}B$) was demonstrated by the results of phosphorylation of $NF-{\kappa}B$ p65 and degradation of inhibitor of ${\kappa}B-{\alpha}$ ($I{\kappa}B-{\alpha}$). Pharmacological inhibition of each of the three types of MAPKs and $NF-{\kappa}B$ substantially attenuated P. gingivalis-induced $PGE_2$ production. These results suggest that P. gingivalis should promote endodontic inflammation by stimulating dental pulp cells to produce $PGE_2$.

PHP INDEX에 따른 전문가 치주예방관리 환자의 치주질환 원인균 copy 수 변화 (The change of PHP index and copy numbers of periodontal bacteria in patients with professional periodontal prevention management)

  • Background: By regulating the factors that contribute to oral diseases in a healthy way, oral health can be maintained and prevented. Methods: The general characteristics, PHP index, oral health behavior, and clinical periodontal index of each group were calculated by frequency analysis, and a cross-analysis (χ2 test) was conducted to assess the homogeneity of the general characteristics, oral health behavior, and clinical periodontal index of the study subjects. An Oral bacteriological examination was performed by gargling with saliva. Results: The expert periodontal prevention group showed a decrease in the copy number of periodontal disease causative bacteria, and A. actinomycetemcomitans, P. gingivalis, T. forsythus, andT. denticolashowed a significant difference before and after treatment (p=0.021). In the periodontal treatment group, A. actinomycetemcomitans, P. gingivalis, T. forsythus, andT. denticolaall showed a decrease in copy number, but there was no significant difference. Conclusions: This study showed professional periodontal prevention management had some effect on periodontal bacterial reduction.

치과용 유니트 수계의 미생물 오염 및 효과적인 관리 방법 (The Microbial Contamination and Effective Control Method of Dental Unit Water System)

  • 치과치료를 위해 다양한 용도로 DCU에서 배출되는 물이 사용된다. 이 DCU 물의 질에 관한 계속되는 논란으로 인한 환자들의 불안감을 줄이고 양질의 치과 치료를 제공하기 위해서는 DCU 물이 음용수만큼 깨끗하게 유지되어야 한다. 따라서 본 논문은 DCU 물의 세균 오염 수준과 오염된 DCU 물의 임상적 위험성에 대하여 설명하고, 세균 오염을 방지하기 위한 여러 가지 관리 방법들을 검토하였다. 여러 국가에서 DCU 물을 대상으로 오염도를 확인하는 연구가 많이 진행되어 DCU 물이 높은 수준으로 오염되어있다는 것이 입증되었다. 오염된 DCU 물로 인해 감염된 일부 사례도 보고되었고 이는 DCU 물이 감염의 잠재적인 원인일 수 있음을 보여주었다. DCU 물로 인한 잠재적 감염의 위험성을 줄이기 위해서는 효율적인 소독방법을 사용하고 자체적인 모니터링 또한 시행되어야 한다. 하지만 제안되고 있는 여러 가지 수관관리 소독방법으로 인해 치과종사자들의 혼란이 야기되고 있으며, 효율적인 소독방법을 보편화하기 위해서는 관련 연구가 더 진행되어야 할 것으로 생각된다. 치과치료를 받는 환자들을 안심시킬 수 있는 좋은 치과서비스를 제공하기 위해서는 정책적인 개선과 국내 치과 상황에 맞는 수관 관리 지침이 빠른 시일 내에 시행되어야 할 것으로 생각된다.

제 2형 당뇨 환자의 근관 감염에서 나타난 병원균과 그 항생제 감수성 (Pathogens and Their Antimicrobial Susceptibilities of Endodontic Infection in Type II Diabetes Mellitus Patients)

  • Aim: The purpose of this study was to identify dominant pathogens and their antimicrobial susceptibilities of endodontic infection in type II diabetes mellitus (DM) patients to determine effective empirical antibiotics. Methodology: Pathogens from endodontic infection in six patients with DM and in six patients without DM were cultured, identified and their antimicrobial susceptibility was tested using Vitek2 systems ($bioM{\acute{e}}rieux$, Marcy l' Etoile, France). The results were analyzed using Chi-square test and Fisher's exact test at P< 0.05 level. Results: Pathogens of opportunistic infection were dominant in DM patients (P=0.015). However, there was no significant difference of antimicrobial susceptibility between DM and non-DM patients. Relatively high percent (27%) of pathogens showed resistance to penicillin. Conclusions: More cautions should be paid to DM patients because they are prone to opportunistic infection. Penicillin is not effective in the control of endodontic infection.

치주질환의 면역학 (The Role of Immune Response in Periodontal Disease)

  • The periodontal diseases are infections caused by bacteria in oral biofilm, a gelatinous mat commonly called dental plaque, which is a complex microbial community that forms and adhere to tooth surfaces. Host immune-pathogen interaction in periodontal disease appears to be a complex process, which is regulated not only by the acquired immunity to deal with ever-growing and -invading microorganisms in periodontal pockets, but also by genetic and/or environmental factors. However, our understanding of the pathogenesis in human periodontal diseases is limited by the lack of specific and sensitive tools or models to study the complex microbial challenges and their interactions with the host's immune system. Recent advances in cellular and molecular biology research have demonstrated the importance of the acquired immune system in fighting the virulent periodontal pathogens and in protecting the host from developing further devastating conditions in periodontal infections. The use of genetic knockout and immunodeficient mouse strains has shown that the acquired immune response, in particular, $CD4^+$ T-cells plays a pivotal role in controlling the ongoing infection, the immune/inflammatory responses, and the subsequent host's tissue destruction.

Antibacterial Activity of Acanthoic acid Isolated from Acanthopanax koreanum against Oral and Skin Microfloras

  • The (-)-pimara-9 (11), 15-dien-19-oic acid, acanthoic acid was extracted from the roots of Acanthopanax koreanum using bioassay-guided isolation of a MeOH extract. Acanthoic acid was assayed against Streptococcus mutans and Staphylococcus epidermidis causing dental caries and opportunistic pathogen. The minimum inhibitory concentration (MIC) of acanthoic acid against Streptococcus mutans and Staphylococcus epidermidis was 2 and 4 ${\mu}g/mL$, respectively, which was much lower than those of other natural antimicrobial agents such as 8 ${\mu}g/mL$ of tanshinone IIA. Acanthoic acid also significantly inhibited the growth of other cariogenic bacteria such as Streptococcus sobrinus and Streptococcus sanguis, and Streptococcus grodenii in the MIC range of 4${\sim}$32 ${\mu}g/mL$. Our findings suggest that acanthoic acid could be employed as a potential antibacterial agent for preventing dental caries and skin infections.

구강 질환 유발 미생물에 대한 항생작용을 갖는 천연물 추출물 검색 (Screening of the Natural Plant Extracts for the Antimicrobial Activity on Dental Pathogens)

  • 구강청정제에 사용되는 많은 항미생물 성분들은 치주조직의 착색 및 작열감 등의 부작용을 가지고 있다. 이러한 취약점을 해결하고, 좀 더 생체친화적인 항미생물 물질을 찾기 위하여,한국식물추출물은행에서 소장하고 있는 2,125종의 식물추출물을 검색하였다. 항미생물 능력을 확인하기 위해, 잘 알려진 치과 병원균(치주질환 3종, 충치 1종)에 대한 성장억제 효과를 측정하였다. 대표적인 치주질환 유발세균인 Porphyromonas gingivalis에 대해서 19중의 식물 추출물이 항생효과를 나타내었으며, 17종이 Prevotella intermedia에 대해, 45종이 Haemophilus actinomycetemcomitans에 대해, 그리고 85종의 추출물이 충치 유발세균인 Streptococcus mutans 에 대해 효과를 나타내었다. 이중에서 총8종의 식물추출물이 실험에 사용한4종의 미생물 중 3종 이상에 대해 항생작용을 나타내었다. 또한 Cudrania tricuspidata, Morus bombycis와 Mallotus japonicas가 실험에 사용된 4종의 치과 병원균 모두에 대해 항생작용을 나타내어 치과용 재료 혹은 양치액에 항미생물 물질로 사용될 가능성을 나타내었다.
