Effective Management Interval Focused on Professional Mechanical Tooth Cleaning (전문가 치면세균막관리 중심의 예방관리프로그램의 효과적인 관리주기)

  • This study was conducted to investigate the optimal management interval in 33 healthy adults by observing changes in oral health on the basis of the plaque control index and gingival index. When plaque control score was compared according to the period, it was found that oral hygiene management was the best in the fourth visit (p<0.05). Improved oral hygiene status was observed during the 3- and 6-month follow-up (p<0.05). Regarding gingival index, the fourth visit showed the healthiest gingival status (p<0.05). At the 3-month follow-up, the gingival index worsened, but a healthy gingival status was attained by 6-month follow-up (p<0.05). The plaque control score according to interest in dental health showed that the 'interested' group had good oral hygiene management (p<0.05). As a motivation for oral hygiene status and gingival health, examination with a 'phase contrast microscope' in the first visit and calculation of the 'evaluation index' in the follow-up visit tended to improve the patients' ability for oral hygiene management (p>0.05). The questionnaire survey showed, that the optimal management interval was 1, 3, and 6 months. As a result, with the effective management interval for the preventive management program focused on professional mechanical tooth cleaning, which was administered weekly, the maximum ability for oral hygiene management was attained at the fourth visit. The effective management period was 1 month. The use of a phase contrast microscope and the calculation of the evaluation index for oral hygiene management could influence the motivation to improve oral hygiene management.

The Jurisdictional Precedent Analysis of Medical Dispute in Dental Field (치과임상영역에서 발생된 의료분쟁의 판례분석)

  • Along with the development of scientific technologies, health care has been growing remarkably, and as the social life quality improves with increasing interest in health, the demand for medical service is rapidly increasing. However, medical accident and medical dispute also are rapidly increasing due to various factors such as, increasing sense of people's right, lack of understanding in the nature of medical practice, over expectation on medical technique, commercialize medical supply system, moral degeneracy and unawareness of medical jurisprudence by doctors, widespread trend of mutual distrust, and lack of systematized device for solution of medical dispute. This study analysed 30 cases of civil suit in the year between 1994 to 2004, which were selected among the medical dispute cases in dental field with the judgement collected from organizations related to dentistry and department of oral medicine, Yonsei university dental hospital. The following results were drawn from the analyses: 1. The distribution of year showed rapid increase of medical dispute after the year 2000. 2. In the types of medical dispute, suit associated with tooth extraction took 36.7% of all. 3. As for the cause of medical dispute, uncomfortable feeling and dissatisfaction with the treatment showed 36.7%, death and permanent damage showed 16.7% each. 4. Winning the suit, compulsory mediation and recommendation for settlement took 60.0% of judgement result for the plaintiff. 5. For the type of medical organization in relation to medical dispute, 60.0% was found to be the private dental clinics, and 30.0% was university dental hospitals. 6. For the level of trial, dispute that progressed above 2 or 3 trials was of 30.0%. 7. For the amount of claim for damage, the claim amounting between 50 million to 100 million won was of 36.7%, and that of more than 100 million won was 13.3%, and in case of the judgement amount, the amount ranging from 10 million to 30 million won was of 40.0%, and that of more than 100 million won was of 6.7%. 8. For the number of dentist involved in the suit, 26.7% was of 2 or more dentists. 9. For the amount of time spent until the judgement, 46.7% took 11 to 20 months, and 36.7% took 21 to 30 months. 10. For medical malpractice, 46.7% was judged to be guilty, and 70% of the cases had undergone medical judgement or verification of the case by specialists during the process of the suit. 11. In the lost cases of doctors(18 cases), 72.2% was due to violence of carefulness in practice and 16.7% was due to missing of explanation to patient. Medical disputes occurring in the field of dentistry are usually of relatively less risky cases. Hence, the importance of explanation to patient is emphasized, and since the levels of patient satisfaction are subjective, improvement of the relationship between the patient and the dentist and recovery of autonomy within the group dentist are essential in addition to the reduction of technical malpractice. Moreover, management measure against the medical dispute should be set up through complement of the current doctors and hospitals medical malpractice insurance which is being conducted irrationally, and establishment of system in which education as well as consultation for medical disputes lead by the group of dental clinicians and academic scholars are accessible.

Fracture strength of tie wings in a newly-developed polycarbonate bracket (국산 폴리카보네이트 브라켓 윙의 파절 강도에 관한 연구)

  • Objective: With an increasing demand for esthetic orthodontic appliances, the interest in polycarbonate brackets has also increased. However, polycarbonate bracket wings are prone to fracture. The purpose of this study was to evaluate the clinical usefulness of a newly-developed polycarbonate bracket by measuring the fracture strength of bracket tie wings. Methods: Alice (K.M. Daicom, Seoul, Korea) and Spirit MB (Ormco, Glendora, CA, USA) brackets were used as an experimental and control bracket respectively. Each bracket group was divided into halves. One half was untreated and the other half was treated with 2,000 times of thermocycling between $5^{\circ}C\;and\;55^{\circ}C$. The fracture strength of the wing was measured by a universal testing machine. Results: Alice bracket wings showed significantly higher strength than Spirit MB bracket wings in both untreated and thermocycled bracket cases. Conclusion: Alice brackets may be used clinically in terms of the strength of bracket wings.

  • The purpose of this study was to perform oral health education using UCC media in consideration of middle school students' characteristics, and to investigate the intelligence area related to knowledge conveyance, affective area related to attitude change, and psychomotor area related to behavioral change. As a result, it tried to develop further the strengths of UCC media based oral health education and overcome its weaknesses in order to increase learners' interest and have any behavioral changes. By letting learners have the education at YouTube at any time, rather than use one-time educational medium material it is possible to keep educating learners. In the future, by applying the education to health education programs and health education, it will be possible to convey education contents to learners accurately, give motivation to learners, and thereby increase educational results.

Investigation of bone formation using calcium phosphate glass cement in beagle dogs

  • Purpose: Among available biomaterials, bioceramics have drawn special interest due to their bioactivity and the possibility of tailoring their composition. The degradation rate and formulation of bioceramics can be altered to mimic the compositions of the mineral phase of bone. The aim of this study was to investigate the bone formation effect of amorphous calcium phosphate glass cement (CPGC) synthesized by a melting and quenching process. Methods: In five male beagle dogs, $4{\times}4$ mm 1-wall intrabony defects were created bilaterally at the mesial or distal aspect of the mandibular second and fourth premolars. Each of the four defects was divided according to graft materials: CPGC with collagen membrane (CM), biphasic calcium phosphate (BCP) with CM, CM alone, or a surgical flap operation only. The dogs were sacrificed 8 weeks post-surgery, and block sections of the defects were collected for histologic and histometric analysis. Results: There were significant differences in bone formation and cementum regeneration between the experimental and control groups. In particular, the CPGC and BCP groups showed greater bone formation than the CM and control groups. Conclusions: In conclusion, CPGC was replaced rapidly with an abundant volume of new bone; CPGC also contributed slightly to regeneration of the periodontal apparatus.

Synthesis of Yttria Stabilized Zirconia by Sol-gel Precipitation Using PEG and PVA as Stabilizing Agent

  • There is increasing interest in zirconia as a dental material due to its aesthetics, as well as the exceptionally high fracture toughness and high strength that are on offer when it is alloyed with certain oxides like yttria. In recent years, many solution based chemical synthesis methods have been reported for synthesis of zirconia, of which the sol-gel method is considered to be best. Here, we synthesize zirconia by a sol gel assisted precipitation method using either PEG or PVA as a stabilizing agent. Zirconia sol is first synthesized using the hydrothermal method. We used NaOH as the precipitating agent in this method because it is easy to remove from the final solution. Zirconium and yttrium salts are used as precursors and PEG or PVA are used as stabilizers to separate the metal ions. The resulting amorphous zirconia powder is calcined at $900^{\circ}C$ for 2 h to get crystallized zirconia. XRD analysis confirmed the partially stabilized zirconia synthesis in all the synthesized powders. SEM was taken to check the morphology of the powder synthesized using either PEG or PVA as a stabilizing agent and finally the transparency was calculated. The results confirmed that the powder synthesized with 10 % PVA as the stabilizing agent had highest percentage of transparency among all the synthesized powder.

Anatomical variations of trabecular bone structure in intraoral radiographs using fractal and particles count analyses

  • Purpose : This study was performed to evaluate possible variations in maxillary and mandibular bone texture of normal population using the fractal analysis, particles count, and area fraction in intraoral radiographs. Materials and Methods : Periapical radiographs of patients who had full mouth intraoral radiographs were collected. Regions of interest ($100{\times}100$ pixels) were located between the teeth of the maxillary anterior, premolar, and molar area, as well as the mandibular anterior, premolar, and molar areas. The fractal dimension (FD) was calculated by using the box counting method. The particle count (PC) and area fraction (AF) analyses were also performed. Results : There was no significant difference in the FD values among the different groups of age, gender, upper, and lower jaws. The mean FD value was $1.49{\pm}0.01$. The mean PC ranged from 44 to 54, and the mean AF ranged from 10.92 to 11.85. The values of FD, PC, and AF were significantly correlated with each other except for the upper molar area. Conclusion : According to the results, patients with normal trabecular pattern showed a FD of approximately 1.5. Based on these results, further investigation would be recommended if the FD value of patient significantly differenct from this number, since the alteration of this value indicates microstructural modification of trabecular pattern of the jaws. Additionally, with periapical radiographs, simple and cost-effective, PC and AF could be used to assess the deviation from the normal.

An Effect of Self-Esteem and Self-Efficacy in Middle-Aged Women's on the Behavior for Preparing Their Old Age (중년여성의 자아존중감과 자기효능감이 노후생활 준비행동에 미치는 영향)

  • This article tried to find how they foster their emotion and promote their health, as examining the effect of self-esteem and self-efficacy on self-esteem and self-efficacy in middle-aged women. This paper conducted a survey for women in 40 50s who live in Daegu Metropolitan for one month from the 1st of May, 2013. As the result of analysis on primary factor, it was classified into 5factors of organizing such as the physical behavior for preparing their old age, the behavior for preparing their old age of participation in social activities, the behavior for preparing their old age of preparing death, the emotional behavior for preparing their old age and the economic behavior for preparing their old age. The self-efficacy had the positive effect on the physical, emotional and economic behavior for preparing their old age, on the other hand, the self-esteem and the self-efficacy had the positive effect on the behavior for preparing their old age of participation in social activities and of preparing death. It can be said to be more important that the middle aged-women have to make an effort to develop self-esteem and self-efficacy for improving the behavior for preparing their old age, in addition, this article suggests to develop a systematic program that may inspire a belief for the behavior that needs to get the desired result, and to require continuing interest.

How to Improve Patients' Satisfaction in Healthcare Organization? - Healthcare Service Quality Classification using Kano Model - (의료기관의 소비자 만족도 향상요소 도출 - Kano 모형에 기반한 의료서비스 품질 분류를 중심으로 -)

  • Objective: This research investigates how to increase the quality of medical service and supply high quality of medical service to patients. By using Kano Model theory we examines what medical service attributes the hospital would be conducted preferentially for patient's satisfaction and provides informations of management strategies for hospitals. Method: To study patients' perception of medical service quality, first we performed pilot test to derive 30 medical service attributes. With 30 medical service attributes, we conducted survey of 300 subjects who have experienced medical services in 6 months. To examine patients' conception of medical services, a modified Kano's questionnaire using 5 scale is applied. Finally we calculated SI(Satisfaction index) and DI(Dissatisfaction index) and PCSI(Potential Customer Satisfaction Improvement) index with Kano's Model analysis results. Key Findings: We found that the quality of medical service categorized in 15 one-dimensional elements, 9 must-be elements and 6 indifferent elements. Moreover the attribute of gives prompt services and have patient's best interest at heart scored the highest SI, whereas the attributes of accurate and precise medical service, exact records, enough explanation and polite attitudes are the highest score of DI. And also good explanation of the bill scored the highest PCSI. In this study findings indicate that while medical service providers try to increase patients' satisfaction by improving hospital's environments, patients' perception of trust and good interpersonal relationships with medical service providers have strong and positive impact on patients' satisfaction.

Development of "L2L Teaching Model" Integrating a Hospital to School Laboratory to Strengthen Practical Education for Preliminary Clinical Laboratory Technologist (예비 임상병리사의 실무중심 교육의 강화를 위한 병원-학교 검사실 융합형(L2L) 교수학습 모형의 개발)

  • The development of teaching models to strengthen practical education and actively participate in the classes of students is now required in recent education situations. This study examined the participation and satisfaction level of students' classes after applying a teaching model-integrated hospital and school laboratory (named L2L). A total of 33 students who took the subject of a clinical microbiology experiments were involved in this study. Each group representative participating in a pre-class field exercise in the hospital microbiology laboratory was asked to conduct the experiment. After applying the L2L teaching models, the academic self-efficacy ($2.87{\pm}0.58{\rightarrow}3.38{\pm}0.55$), class participation ($2.60{\pm}0.92{\rightarrow}3.62{\pm}0.78$), and class satisfaction ($2.48{\pm}1.01{\rightarrow}3.85{\pm}0.87$) increased significantly (P<0.05). This means that pre-class field exercises created interest in the student's class and boosted self-confidence, leading to increased participation and satisfaction for the class. In conclusion, the L2L teaching model is an effective teaching method to enhance the practical training for preliminary clinical laboratory technologists.
