광주광역시 치과위생사의 직무만족도에 영향을 주는 요인에 대한 연구 (A study on factors affecting the job satisfaction of dental hygienists in Gwangju)

  • Objectives : This study was to examine factors affecting the job satisfaction of dental hygienists. Methods : The subjects in this study were dental hygienists who worked at dental hospitals, dental clinics and general hospitals in the city of Gwangju. A survey was conducted by mail from January 2 to March 2, 2007. Out of the collected data, 208 answer sheets were analyzed. Results : 1. The dental hygienists investigated got a mean of 3.20 on job satisfaction. Among the job satisfaction factors, relationship with patients ranked highest(3.79), followed by relationship with colleagues(3.62), working environments(3.39), future prospects(3.30), professional status (2.89), pay(2.82) and required workload(2.58). 2. Regarding links between general characteristics and job satisfaction, the older dental hygienists were more satisfied with professional status, pay, required workload and relationship with patients. By marital status, the married dental hygienists expressed better satisfaction at professional status than the unmarried ones. By education, those who were receiving college education or received the same or higher education were more gratified than the junior college graduates. By the total length of career, the dental hygienists whose length of career was longer were more gratified with professional status, future prospects and relationship with patients. By workplace, the dental hospital workers were more satisfied with working environments, and the general hospital employees were more gratified with pay. The dental hospital employees were better satisfied with future prospects as well. As to the impact of the length of career at the current workplace, there was a tendency that those who worked at their current workplaces for a longer time expressed better satisfaction with professional status, pay and relationship with patients. By monthly mean income, the larger income earners had a tendency to be better gratified with professional status, pay and relationship with patients. 3. The general characteristics that were selected as independent variables were identified as the factors that exercised an influence on the job satisfaction of the dental hygienists and made an about 14.0% prediction of it. Out of those factors, the total length of career and monthly mean income had a statistically significant impact on that. Conclusions : The above-mentioned findings suggested that out of the seven job satisfaction components, they gave the lowest marks to satisfaction level with pay. As a result of making a multiple regression analysis, it's found that job satisfaction was under the influence of the total length of career and monthly mean income. Therefore there should be an improvement in the pay system in order to boost the job satisfaction of dental hygienists, and they should be paid properly in accordance with total length of career.

Various autogenous fresh demineralized tooth forms for alveolar socket preservation in anterior tooth extraction sites: a series of 4 cases

  • The aim of this study was to evaluate the clinical relevance of autogenous fresh demineralized tooth (Auto-FDT) prepared at chairside immediately after extraction for socket preservation. Teeth were processed to graft materials in block, chip, or powder types immediately after extraction. Extraction sockets were filled with these materials and dental implants were installed immediately or after a delay. A panoramic radiograph and a conebeam CT were taken. In two cases, tissue samples were taken for histologic examination. Vertical and horizontal maintenance of alveolar sockets showed some variance depending on the Auto-FDT and barrier membrane types used. Radiographs showed good bony healing. Histologic sections showed that it guided good new bone formation and resorption pattern of the Auto-FDT. This case series shows that Auto-FDT prepared at chairside could be a good material for the preservation of extraction sockets. This study will suggest the possibility of recycling autogenous tooth after immediate extraction.

  • Odontoma is the most common odontogenic benign tumor, and the treatment of choice is generally surgical removal. After excision, bone grafts may be necessary depending on the need for further treatment, or the size and location of the odontoma. Although the osteogenic capacity of a demineralized tooth was verified as early as 1967 by Urist and many other investigators, the cumbersome procedure, including a long demineralization time, may be less than comfortable for clinicians. A modified ultrasonic technology, with periodic negative pressure and temperature control, facilitated rapid and aseptic preparation of demineralized teeth for bone grafts. This approach reduces the demineralization time dramatically (${\leq}80$ minutes), so that the graft material can be prepared chairside on the same day as the extraction. The purpose of this article is to describe two cases of large compound odonotomas used as graft material prepared chairside for enucleation-induced bony defects. These two clinical cases showed favorable wound healing without complications, and good bony support for future dental implants or orthodontic treatment. Finally, this report will suggest the possibility of recycling the benign pathologic hard tissue as an alternative treatment option for conventional bone grafts in clinics.

치과 내원 환자의 치아 이상(dental anomaly)에 관한 연구 (A study on the prevalence of dental anomalies in Korean dental-patients)

  • 본 연구는 본원에 내원한 환자에 있어서 결손치, 매복치, 과잉치의 빈도 및 형태적 이상 등을 관찰하고자 하였다. 연구대상으로는 2009년 1월부터 2010년 6월까지 한림대학교 성심병원 치과에 내원한 환자를 대상으로 조사하였으며 의무기록지, 파노라마 방사선사진을 토대로 치아 이상에 대하여 성별, 치아 이상의 종류와 발생위치별로 조사하였다. 본 연구에서는 치아의 선천적 결손, 과잉치, 왜소치, 매복치, 변위치에 대한 치아 이상을 연구하였다. 연구기간동안 전체 내원하여 파노라마 방사선사진을 찍은 환자 3,133명 중 치아 이상을 가진 환자의 비율은 11.55%였다. 그 중 선천적 결손치를 가진 환자의 내원율은 5.71%로 가장 높았고, 매복치를 가진 환자는 3.09%였다. 과잉치, 왜소치, 변위치를 가진 환자의 비율은 각각 1.79%, 1.66%, 0.45%를 차지하였다. 치아의 수나 형태의 변화는 환자의 치료시 고려해야 할 필수적인 요소이다. 그러므로 치료를 위해 치과에 내원한 환자뿐만 아니라 구강검진으로 내원한 환자에서도 파노라마 방사선사진을 채득한다면 국민 구강보건 향상에 큰 도움이 될 것으로 생각된다.

치과위생사의 임상치과 핵심역량 개발 (Development of clinical dental competencies in dental hygienists)

  • Objectives: This study was conducted to identify the requirements for the clinical dental tasks in the dental hygienists using the frequency of dental hygienists' tasks in the Korean dental clinics, and to provide them with the core competencies for achieving these. Methods: This study was based on evaluation of a self-reported survey. The dental hygienists were investigated upon classification of their tasks into direct and assistant performances, and the dentists were investigated by the competency level of the dental hygienists. The data from 481 clinical dental hygienists and 67 dentists were used for final analysis in this study, excluding the subjects who provided incomplete or inappropriate responses to the survey. The collected data were analyzed by frequency analysis. Results: The clinical dental core competency tasks of the dental hygienists were divided into 4 tasks for common dentistry, 4 for periodontics, 5 for conservative dentistry, 5 for pediatric dentistry, 5 for dental prosthodontics, and 3 for dental orthodontics. Clinical dental core competency tasks of the dental hygienists were performed more frequently of tasks performance than 8.0, and dentists required competence level was similar to that of most dental hygienists clinical dental core competencies. Conclusions: It is necessary to check whether the curricula of the universities include the competencies for the students enough to perform the corresponding tasks and the core competencies need to be reflected in the curricula. The clinical dental core competencies need to be agreed by dental hygienists, and it is necessary to be organized as the evaluation guide of the institute of dental hygiene education and evaluation and utilized as the national examination.

안양시에서 치과용 파노라마방사선사진의 참고 선량 수준 (Reference dose levels for dental panoramic radiography in Anyang City)

  • Purpose : To measure dose-width product (DWP) values used for dental panoramic radiography in Anyang city, Korea. Materials and Methods : Thirty-six panoramic dental radiographic sets (17 analogue panoramic sets and 19 digital panoramic sets) in 36 dental clinics in Anyang city were included in the study. Each patient's panoramic exposure parameters were simulated and the panoramic radiation doses were measured at the secondary collimator using a Mult-O-Meter (Unfors Instruments, Billdal, Sweden) at each dental clinic during 2006. The third quartile DWP was determined from 310 surface dose measurements on adult. Results : The third quartile DWP for adult panoramic radiograph was 106.7 mGy mm. For analogue and digital panoramic radiograph, 3/4 DWP were 116.8 mGy mm and 72 mGy mm respectively. The overall third quartile DWP of panoramic radiography was 106.7 mGy mm. Conclusion : The measured 3/4 DWPs were higher than the 3/4 DWP of 65 mGy mm recommended by NRPB. Dentists who are operating above the reference dose should lower their panoramic exposure doses below the recommended reference value by changing the exposure parameters and/or their panoramic equipments.

사용자 중심 서비스디자인을 활용한 치과예방서비스 개선 사례연구 (A Case Study on The Improvement of Dental Clinic Preventive Service Using User-Centered Service Design)

  • Purposes: The objective of this study is to analyze service design process of dental clinic preventive service by using 'Double Diamond Design Process' which is user-centered for improving dental clinic management efficiency. Methodology: We examine case study that dental clinic preventive service of local dental clinic. Following the double diamond design four steps(①discover, ②define, ③develop, ④deliver), we use suitable analytic tools in each steps. In discover step, we perform observation and desk research. Next, in define step, affinity diagram, user persona, and user journey map are performed. We then make service blueprint in develop step. Finally, in deliver step, virtual scenario and confirm some outputs which are based on results of former steps are suggested. Findings: As a result of this study, waiting and consulting phases turn out that these phases are in the most trouble than others. Users(patients) have lots of complain and dissatisfaction in these phases. These touch-points will have to be modified Practical Implication: After checking this study result, it could suggest improvement schemes. To apply these, we try to make some scenarios, and finally realize some outputs. Service design has arisen to strengthen the competitiveness by reflecting user's experience and needs. Especially, service design is not typical, it is so flexible that can apply to various situation and objects. We hope that result of this study is made good use for dental clinics in need.

전문치과위생사제도에 대한 견해 (Opinions of dental hygienists on the advanced dental hygienist system)

  • Objectives: This study aimed to provide basic data to lay the groundwork for the introduction of an advanced dental hygienist system by sampling dental hygienists' views about the system. Methods: A nationwide questionnaire survey was conducted targeting 857 dental hygienists working at dental hospitals and clinics, local health institutions, and educational institutions. The collected data were analyzed using frequency analysis, t-test, one-way analysis of variance (ANOVA) (Duncan as post-analysis), and crossover analysis. Results: The average interest level in the advanced dental hygienist system was 3.83±0.95 points. The necessity by field was confirmed to be the highest during dental hygiene for the elderly and persons with disabilities. The working experience necessary for becoming a specialized dental hygienist is 5.56±2.99 (years). The education period necessary for becoming a specialized dental hygienist is 77.30±77.61 (hours). The work authority level for an advanced dental hygienist was indicated to be 50 respondents (5.8%), who said they required direct guidance from a dentist, 313 respondents (48.2%) who said they needed indirect guidance from a dentist, 200 respondents (23.3%) who said OK when given an advanced dental hygienist's separate judgment, 194 respondents (22.6%), who said that the authority must be varied depending on the work. Conclusions: The interest and need of the advanced dental hygienist system were proven to be high and are expected to be applied to basic data for the introduction and settlement of the system.

일부지역의 치과병·의원 선택요인의 중요도와 영향 (Importance and Influence in Factors of Selecting Dental Clinics in Some Regions)

  • 본 연구는 일반치과환자의 대상으로 일반적 특성에 따른 치과 의료기관 선택요인에 관한 중요도와 치과의원과 치과병원의 선택에 중요한 영향을 미치는 정보를 제공하고자 2009년 8월부터 9월까지 조사 한 것으로 SAS(ver 9.1)분석결과 다음과 같은 결론을 얻었다. 1. 일반적 특성에 따른 치과의료기관 선택요인에 관한 중요도를 비교한 결과 연령대에 따 라서 거리, 교통, 주차편리 같은 거동이나 이동성의 편리를 중요하게 생각하고, 최종 학력에 따라서는 치과소문요인의 중요도가 높으며,(p<0.05) 직업에 따라서는 치과소문, 주차편리의 중요도에 유의한 차이가 나타났다.(p<0.05) 소득에 따라서는 전문의 수련 여부요인이 200만원 미만인 집단보다 500만원이상인 집단이 통계적으로 높게 나타났다.(p<0.05) 2. 대상 치과의원을 방문한 환자들의 방문경로는 소문이 23.7%로 가장 많았으며 다음으로 의료진 및 직원의 소개, 가족의 권유가 각각 17.7%로 나타났는데 이는 인터넷이 발달해 있는 지금도 소문이나 가족의 권유 등 구전의 효과가 크다는 것을 알 수 있다. 3. 추천도에 영향을 미치는 요인에서는 Model I에서는 의료진 실력 우수(p=0.013), 의료 정보 획득 편리(p=0.014), 예약제도 편리(p=0.029), 최신시설(p=0.001), 고가 치료비(p=0.027)은 통계적으로도 유의한 결과를 보였으며, Model II에서는 인테리어(p=0.009), 치과 이름(p=0.027)은 통계적으로도 유의한 결과를 보였다. 4. 재이용 의사에 영향을 미치는 요인에서는 Model I에서는 의료정보 획득편리(OR=0.64)와 고가의 치료비(OR=1.74)가 통계적으로 유의한 결과를 보였으며, Model 에서는 야간 및 공휴일 진료여부(OR=1.47)가 통계적으로 유의한 결과를 보였다.

Noise Exposure Assessment in a Dental School

  • Objectives: This cross-sectional study was performed in the Dental School of Prince of Songkla University to ascertain noise exposure of dentists, dental assistants, and laboratory technicians. A noise spectral analysis was taken to illustrate the spectra of dental devices. Methods: A noise evaluation was performed to measure the noise level at dental clinics and one dental laboratory from May to December 2010. Noise spectral data of dental devices were taken during dental practices at the dental services clinic and at the dental laboratory. A noise dosimeter was set following the Occupational Safety and Health Administration criteria and then attached to the subjects' collar to record personal noise dose exposure during working periods. Results: The peaks of the noise spectrum of dental instruments were at 1,000, 4,000, and 8,000 Hz which depended on the type of instrument. The differences in working areas and job positions had an influence on the level of noise exposure (p < 0.01). Noise measurement in the personal hearing zone found that the laboratory technicians were exposed to the highest impulsive noise levels (137.1 dBC). The dentists and dental assistants who worked at a pedodontic clinic had the highest percent noise dose (4.60 ${\pm}$ 3.59%). In the working areas, the 8-hour time-weighted average of noise levels ranged between 49.7-58.1 dBA while the noisiest working area was the dental laboratory. Conclusion: Dental personnel are exposed to noise intensities lower than occupational exposure limits. Therefore, these dental personnel may not experience a noise-induced hearing loss.
