Purpose: The increasing demand for esthetically pleasing results has contributed to the use of ceramics for dental implant abutments. The aim of this study was to compare the biological response of epithelial tissue cultivated on lithium disilicate ($LS_2$) and zirconium oxide ($ZrO_2$) ceramics. Understanding the relevant physicochemical and mechanical properties of these ceramics will help identify the optimal material for facilitating gingival wound closure. Methods: Both biomaterials were prepared with 2 different surface treatments: raw and polished. Their physicochemical characteristics were analyzed by contact angle measurements, scanning white-light interferometry, and scanning electron microscopy. An organotypic culture was then performed using a chicken epithelium model to simulate peri-implant soft tissue. We measured the contact angle, hydrophobicity, and roughness of the materials as well as the tissue behavior at their surfaces (cell migration and cell adhesion). Results: The best cell migration was observed on $ZrO_2$ ceramic. Cell adhesion was also drastically lower on the polished $ZrO_2$ ceramic than on both the raw and polished $LS_2$. Evaluating various surface topographies of $LS_2$ showed that increasing surface roughness improved cell adhesion, leading to an increase of up to 13%. Conclusions: Our results demonstrate that a biomaterial, here $LS_2$, can be modified using simple surface changes in order to finely modulate soft tissue adhesion. Strong adhesion at the abutment associated with weak migration assists in gingival wound healing. On the same material, polishing can reduce cell adhesion without drastically modifying cell migration. A comparison of $LS_2$ and $ZrO_2$ ceramic showed that $LS_2$ was more conducive to creating varying tissue reactions. Our results can help dental surgeons to choose, especially for esthetic implant abutments, the most appropriate biomaterial as well as the most appropriate surface treatment to use in accordance with specific clinical dental applications.
The hardening effect by ice-quenching after oxidation of a Pd-Ag-Sn-Au metal-ceramic alloy during porcelain firing simulation was investigated by means of hardness test, field emission scanning electron microscopic observations, and X-ray diffraction analysis. The hardness decreased by ice-quenching after oxidation, which was induced by the homogenization of the ice-quenched specimen. The decreased hardness by ice-quenching after oxidation was recovered from the wash stage which was the first stage of the remaining firing process for bonding porcelain. After wash stage, the hardness of the ice-quenched specimens decreased during the subsequent porcelain firing process. But the final hardness of the ice-quenched specimens after oxidation was higher than that of the specimens cooled at stage 0 after oxidation. The increase in hardness of the specimens during the first firing process was caused by the lattice strains generated at the interface between the face-centered cubic Pd-Ag-rich matrix and the face-centered tetragonal Pd3(Sn, Ga, In) precipitate. The decrease in hardness of the specimens during the remaining firing process was caused by the microstructural coarsening.
The objective of this finite element method study was to analyze the stress distribution induced in a maxillary central incisor Ni-Cr base metal coping ceramic crowns with various margin design. Margin designs of crown in this experiment were knife-edge metal margin on chamfer finishing line of tooth preparation(M1), butt metal margin on shoulder finishing line(M2), reinforced butt metal margin on shoulder finishing line(M3), beveled metal margin on bevelde shoulder finishing line(M4). Two- dimensional finite element models of crown designs were subjected to a simulated biting force of 100N which was forced over porcelain near the lingual incisal edge. Base on plane stress analysis, the maxium von Miss stresses(Mpa) in porcelain venner was 0.432, in metal coping was 0.579, in dentin abutment was 0.324 for M1 model, and M2 model revealed in porcelain was 0.556, in metal coping was 0.511, in dentin was 0.339, and M3 model revealed in porcelain was 0.556, in metal coping was 0.794, in dentin was 0.383 for M4 model. All values of each material in metal-ceramic crown were much below the critical failure values.

Application of CAD/CAM is changing the way partial or full veneer all ceramic restoration is made. CAD/CAM systems, which were used mainly in other industries, have been developed and introduced for the dental purposes recently. It produced a flood of information on the CAD/CAM systems. It also influenced the development of restorative materials and all ceramic is substituting the traditional restorative materials of gold, composite resin and metal. Price increase of gold and other raw materials made the all ceramic more appealing. The introduction of a CEREC 3D system was innovative in several ways. Image of the prepared tooth is captured by camera and impression taking is unnecessary. Restoration can be delivered to the patient on one appointment and it will satisfy the demand of busy patients. One-day treatment with direct CAD/CAM system saves time compared to indirect CAD/CAM system. More superior restoration can be produced if lab work such as the adaptability check and shade selection is cooperated with lab technician. Short working time and comparably superior shade compatibility of color block was close to ideal. In the future, restorations with better quality can be fabricated in less time to busy patients thanks to the development of CAD/CAM system and dental materials.
Purpose: This study aimed to observe the effect of laminate veneer on patient's teeth based on the manufacturing of laminate veneer restorations, which are produced by fabricating a ceramic cast body using IPS Empress, a pressure casting method and then forming the veneer by layering. Subsequently, we assessed the potential of its clinical application. Methods: This study discusses and preserves various treatment plans, such as diagnostic wax-up and treatment room diagnosis, for patients who visit the hospital to improve the appearance of teeth due to diastema of maxillary teeth, inexperienced resin filling, lack of esthetics, and external teeth. A ceramic cast body is constructed using IPS Empress, which is an effective and aesthetic restoration pressure casting method to restore the veneer with a laminate made by layering. Results: Compared with the preoperative state, the frontal view of the patient after the final restoration showed the formation of a natural smile line; the space between the central and lateral incisors was filled in synchronously with the adjacent teeth. In addition, the emergence profile is maintained by reducing the over-contour as much as possible. Conclusion: The patient's quality of life is improved by providing them with a satisfactory natural smile.
The purpose of this study was to investigate the mechanical properities and biocompatibility with crystallization temperature and time of a bioactive glass-ceramic system $41.4wt%SiO_{2}-35.0wt%CaO-3.0wt%MgO-12.0wt%P_{2}O_{5}-8.6wt%Al_{2}O_{3}$ with same molar percent of $Al_{2}O_{3}\;and\;P_{2}O_{5}$. The crystallization behaviors were investigated with DTA, XRD and SEM. Fracture toughness with the change of crystallization temperature and time was measured by indentation fracture method. Also, biocompatibility was evaluated by culture of mouse fibroblast cell line L929. The results obtained were as follows ; 1. The major crystalline phases were apatite and anorthite, and relative intensity of anorthite phase was increased at $1004^{\circ}C$. 2. The hardness and fracture toughness were gradually increased with the increase in ceraming temperature to $1004^{\circ}C$. 3. When the glass ceramic was heat-treated for 4 hours at ceraming temperature of $1004^{\circ}C$, hardness and fracture toughness showed the maximum values $578.84k/mm^2\;and\;2.07MPa\;m^{1/2}$, respectively. 4. The growth rate and cytotoxic of L929 fibroblast cells for bioactive glass ceramic were better than those of stainless steel and titanium.
PURPOSE. The aim of the study was to evaluate the effect of abutment shade, ceramic thickness, and coping type on the final shade of zirconia all-ceramic restorations. MATERIALS AND METHODS. Three different types of disk-shaped zirconia coping specimens (Lava, Cercon, Zirkonzahn: ${\phi}10mm{\times}0.4mm$) were fabricated and veneered with IPS e.max Press Ceram (shade A2), for total thicknesses of 1 and 1.5 mm. A total of sixty zirconia restoration specimens were divided into six groups based on their coping types and thicknesses. The abutment specimens (${\phi}10mm{\times}7mm$) were prepared with gold alloy, base metal (nickel-chromium) alloy, and four different shades (A1, A2, A3, A4) of composite resins. The average $L^*$, $a^*$, $b^*$ values of the zirconia specimens on the six abutment specimens were measured with a dental colorimeter, and the statistical significance in the effects of three variables was analyzed by using repeated measures analysis of variance (${\alpha}$=.05).The average shade difference (${\Delta}E$) values of the zirconia specimens between the A2 composite resin abutment and other abutments were also evaluated. RESULTS. The effects of zirconia specimen thickness (P<.001), abutment shade (P<.001), and type of zirconia copings (P<.003) on the final shade of the zirconia restorations were significant. The average ${\Delta}E$ value of Lava specimens (1 mm) between the A2 composite resin and gold alloy abutments was higher (close to the acceptability threshold of 5.5 ${\Delta}E$) than th ose between the A2 composite resin and other abutments. CONCLUSION. This in-vitro study demonstrated that abutment shade, ceramic thickness, and coping type affected the resulting shade of zirconia restorations.
PURPOSE. To assess the clinical performance of monolithic CAD-CAM lithium disilicate glass-ceramic (LDGC) crowns and metal-ceramic (MC) crowns provided by predoctoral students. This study also assessed the effects of patient and provider-related factors on their clinical performance as well as patient preference for these types of crowns. MATERIALS AND METHODS. Twenty-five patients who received 50 crowns (25 LDGC CAD-CAM and 25 MC) provided by predoctoral students were retrospectively examined. LDGC CAD-CAM crowns were milled in-house using the CEREC Bluecam system and cemented with either RelyX Unicem or Calibra Esthetic resin cements. MC crowns were cemented with RelyX Unicem cement. Clinical assessment of the crowns and the supporting periodontal structures were performed following the modified California Dental Association (CDA) criteria. Patients' preference was recorded using a visual analog scale (VAS). The results were statistically analyzed using log-rank test, Pearson Chi-squared test and Kaplan-Meier survival analysis. RESULTS. Twelve complications were observed in the MC crown group (9-esthetic, 2-technical and 1-biological). In comparison, 2 complications in the LDGC CAD-CAM crown group were observed (1-technical and 1-esthetic). The 6-year cumulative survival rates for MC crowns and LDGC CAD-CAM were 90.8% and 96%, respectively, whereas the success rates were 83.4% and 96%, respectively. Overall, patients preferred the esthetic outcomes of LDGC CAD-CAM crowns over MC crowns. CONCLUSION. The high survival and success rates, low number of complications, and the high level of patients' acceptance of monolithic LDGC CAD-CAM crowns lend them well as predictable and viable alternatives to the "gold standard" MC crowns.

The availability of high-technology systems that use computer-aided design(CAD) and computer-aided machining(CAM) is on the increase. One such system is the Procera system, which fabricates an all-ceramic crown composed of a densely sintered, high-purity aluminum oxide coping combined with a compatible veneering porcelain. Strength, precision of fit, esthetics, cementation, and biocompatibility are among the many factors that concern clinicians when fabricating all-ceramic restorations with this system. This paper reviews the long history and background development of technical, laboratory and clinical applications and presents, in summary form, the data from the many studies on the Procera system.
