본 연구는, 보호자가 바라보는 진정법의 모습과 이를 통해 보호자들이 얻고자 하는 것과 원하는 것이 무엇인지를 알고 보호자들의 높은 만족도와 능동적인 협조를 이끌어 내기 위한 목적으로 시도되었다. 2006년 7월부터 9월까지 경희대학교 병원 소아치과에 내원한 신환 보호자 168명과 의식하진정법을 경험한 환자의 보호자 50명을 대상으로 설문조사를 실시하였다. 설문지는 신환 보호자용과 진정법 경험 보호자용으로 제작하여 설문을 실시하였다. 조사결과 다음을 알 수 있었다. 1. 일반적인 의식하진정법에 대한 정보의 제공이 주로 주변 사람들에 의해 제한적이고 수동적으로 이루어 졌으며, 비전문적인 성향임을 알 수 있었고, 또한 소아치과 진료영역에서의 의식하진정법에 대한 정보의 제공이 부족함을 알 수 있었다. 2. 신환 보호자와 진정법을 경험한 환자 보호자 모두에서 환아의 공포감 감소에 가장 큰 관심을 가지고 있었다. 3. 행동 개선보다는 환아의 심리적 개선에 있어 보호자가 느끼는 만족감이 크며, 따라서 진정법 시 행동 개선의 목적 보다는 환아의 공포감 및 불안감 감소에 초점을 두어야 겠다. 4. 신환 보호자의 경우 진정법의 안정성에 대한 인식이 정립되어 있지 않았고, 진정법을 경험한 보호자의 경우 모르겠다는 것과 불안하다는 대답이 대부분을 차지하여 경험을 하였음에도 올바른 안전성에 대한 인식을 하지 못하고 있었다.
The purpose of the study was to examine health status and mini nutritional assessment (MNA) according to self-esteem status of elderly in care hospital. The survey was carried out from $13^{th}$ to $31^{st}$ of September, 2014 in five care hospitals. Analysis was performed for 226 subjects. Evaluation criteria included demographics, clinical status, MNA, health-related quality of life (EQ-5D) and self-esteem. The SE I, SE II and SE III groups were divided by self-esteem scores. Data were analyzed one-way ANOVA, Duncan' multiple range test and Pearson's correlation using SPSS 15.0. Education level, religion, dental condition and medical endurance type were significantly different in all groups. Many elderly people were normal BMI ($20.94{\sim}21.67kg/m^2$). Self-esteem significantly increased in order of SE I, SE II and SE III. Mobility, usual activity and anxiety or depression were significantly different in all groups, and EQ-5D of SE II and SE III groups were significantly higher than that of SE I group. Comparison of MNA screening score in elderly patients was as follows : Score for food intake, psychological stress or acute disease, neuropsychological problems, pressure scores or skin ulcer, protein intake, mode of feeding, nutritional status and health status in SE I group were significantly lower than those of SE II and SE III groups. Mobility and drug intake of SE I group were significantly lower than those of SE III groups. Fruit and vegetable intake SE I group were significantly higher than those of SE II and SE III groups. MIS (Malnutrition Indicator Score) was significantly different among the SE I, SE II and SE III groups. We conclude that self-esteem score is positively correlated with protein intake, nutrition status, health status and MIS in elderly care hospital. To improve nutritional status of elderly in care hospitals, systematic nutrition management and self-esteem education program should be implemented.
Purpose: The purpose of this study was to explore nurses' experience with caring for COVID-19 patients in a negative pressure room amid the spread of the pandemic. Methods: This study was a qualitative research, and focus group interviews were used to collect data. Three focus groups comprising 19 nurses were interviewed from February 17 to 25, 2021. All interviews were recorded and transcribed verbatim with the consent of the participants. The verbatim transcripts were scrutinized using thematic analysis. Results: Two main themes emerged from the analysis: 'Struggling in an isolated space' and 'Limitations of nursing infrastructure and system'. The nurses caring for COVID-19 patients experienced anxiety and fear about the infection, physical exhaustion, emotional burnout, and a sense of duty as a nurse. They also acknowledged the lack of guidelines, increased task and burden, limitations of nursing care, and the demand for improving the limitations of the nursing system. Conclusion: The results of this study demonstrate that nurses caring for COVID-19 patients encounter physical and emotional problems within the limited healthcare system. The study suggests that comprehensive interventions are needed for nurses. Furthermore, detailed guidelines, strengthening of nursing personnel, and improvements to the nursing system are vital to effectively cope with the pandemic. The government and medical institutions should be aware of the needs of nurses and what they are going through, and make efforts to improve the quality of life of healthcare workers and create a safe healthcare environment.

Objectives: To compare the vital sign stability and cost of two commonly used sedatives, midazolam (MDZ) and dexmedetomidine (DEX). Patients and Methods: This retrospective study targeted patients who underwent mandibular third molar extractions under intravenous sedation using MDZ or DEX. The predictor variable was the type of sedative used. The primary outcome variables were vital signs (heart rate and blood pressure), vital sign outliers, and cost of the sedatives. A vital sign outlier was defined as a 30% or more change in vital signs during sedation; the fewer changes, the higher the vital sign stability. The secondary outcome variables included the observer's assessment of alertness/sedation scale, level of amnesia, patient satisfaction, and bispectral index score. Covariates were sex, age, body mass index, sleeping time, dental anxiety score, and Pederson scale. Descriptive statistics were computed including propensity score matching (PSM). The P-value was set at 0.05. Results: The study enrolled 185 patients, 103 in the MDZ group and 82 in the DEX group. Based on the data after PSM, the two samples had similar baseline covariates. The sedative effect of both agents was satisfactory. Heart rate outliers were more common with MDZ than with DEX (49.3% vs 22.7%, P=0.001). Heart rate was higher with MDZ (P=0.000). The cost was higher for DEX than for MDZ (29.27±0.00 USD vs 0.37±0.04 USD, P=0.000). Conclusion: DEX showed more vital sign stability, while MDZ was more economical. These results could be used as a reference to guide clinicians during sedative selection.
부산 사하구내 경로당과 D 노인대학을 이용하는 65세 이상 노인의 고혈압과 당뇨병에 대한 실태를 비교 조사하여 이를 바탕으로 노인의 구강건강행태 이론을 개발하도록 돕고 보다 전문화된 구강건강행위 전략 구축을 위한 하나의 기초자료로 활용하고자 하였다. 설문도구는 인구사회학적 특성, 건강행태, 건강검진, 고혈압 실태, 당뇨병 실태, 삶의 질(EQ5D) 문항으로 구성하였다. 2009년 8월 중순에서 9월 중순까지 약 한 달 동안 사하구내 경로당 10곳과 D 노인대학을 직접 방문하여 설문내용을 자세히 읽어드리고 협조에 응해주시는 65세 이상인 경로당 노인 100명과 D 노인대학 학생 74명으로 총 174명을 대상으로 자료수집 하였다. 자료분석은 SPSS 12.0을 이용하여 빈도분석, 교차분석을 통한 ${\chi}^2$검정, 독립표본 t 검정을 하였다. 연구결과는 아래와 같다. 1. 건강검진관련 항목 중 최근 1년 동안 혈압을 한번이라도 측정한 경우가 94.8%로 높았으며, 최근 1년 동안 혈당을 측정한 경우도 91.4%로 높게 나타났다. 건강관련 정보 취득원의 경우 경로당 노인은 방송을 통해서가 50.0%, 노인대학 노인은 전문가 강좌 및 상담이 64.8%로 약간 높게 나타났다(p < 0.05). 2. 고혈압인 경우가 32.2%로 나타났으며, 최초 고혈압 발견은 병원 진료 시 83.9%로 가장 많았다(p < 0.05). 고혈압 진단 장소는 병의원 50.0%로 나타났으며(p < 0.05), 고혈압을 진단받은 시기는 5년 이하 50.0%로 가장 많게 나타났다(p < 0.05). 정기적 혈압점검은 월 1회 이상 76.8%로 가장 많았고(p < 0.05), 혈압조절여부에서는 잘 조절된다가 75.0%로 가장 높게 나타났다(p < 0.05). 혈압약 복용은 경로당 노인은 매일 복용함이 85.7%로 가장 많았으며 노인대학 노인은 매일 복용하지 않는 것으로 나타났다(p < 0.05). 3. 당뇨병인 경우가 14.4%로 나타났으며, 당뇨병 최초 발견은 병원 진료 시 80.0%로 나타났다(p < 0.05). 개인용 혈당측정기 보유에서는 아니오가 64.0%로 약간 높게 나타났다(p < 0.05). 4. 삶의 질의 측정 항목 중 운동능력에서 경로당과 노인대학 노인 모두 걷는데 지장이 없다가 62.7%로 많았으며, 두 군을 비교해 보면 경로당 노인이 노인대학 노인보다 걷는데 다소 지장이 있는 분들이 조금 더 많았다(p < 0.05). 통증과 불편여부에서는 경로당 노인은 다소 통증이나 불편감이 있다가 다소 많았으며 노인대학 노인은 통증이나 불편감이 없다가 조금 더 많게 나타났다(p < 0.05). 5. 경로당과 노인대학 노인의 오늘 건강상태를 비교한 결과 노인대학 노인이 경로당 노인보다 유의하게 높은 건강상태를 나타냈다(p < 0.05). 이상의 결과에서 대체로 경로당 노인들보다 노인대학 노인들이 건강상태가 비교적 양호하였으며 삶의 질의 항목인 걷는데 지장이 없다와 불안/우울하지 않다라고 생각하는 것이 더 많았으나, 고혈압, 당뇨관리는 경로당과 노인대학 노인 둘 다 어느 정도 잘 되는 것으로 나타났다. 본 연구에서 노인의 삶의 질을 향상시키기 위해서 건강행태 및 고혈압·당뇨병 등 주요 질환관리 실태를 기초로 다양한 구강보건교육 및 구강건강증진 프로그램을 개발하여 노인 개개인에게 맞춤형 치과 의료서비스를 제공하여야 한다는 점에서 현재 본 대학에서 실시하고 있는 노인대학 구강건강관리프로그램 운영이 노인의 구강건강행위 실천에 긍정적인 영향을 주는 것으로 생각된다. 이번 실태조사에서 노인대학 노인중 고혈압 환자와 당뇨 환자의 수가 적었다는 제한점을 보여 신뢰성 있는 결론을 도출하는 데는 한계가 있다고 생각하나 후속연구에서 보다 많은 대상환자를 확보하고 체계적인 실태조사가 이뤄진다면 그 결과가 노인의 구강건강증진을 위한 프로그램 계획시 필요한 기초자료로 활용될 수 있으리라 생각한다.
Crosstalk between G-protein signaling and glutamatergic transmission within the brain reward circuits is critical for long-term emotional effects (depression and anxiety), cravings, and negative withdrawal symptoms associated with opioid addiction. A previous study showed that Regulator of G-protein signaling 4 (RGS4) may be implicated in opiate action in the nucleus accumbens (NAc). However, the mechanism of the NAc-specific RGS4 actions that induce the behavioral responses to opiates remains largely unknown. The present study used a short hairpin RNA (shRNA)-mediated knock-down of RGS4 in the NAc of the mouse brain to investigate the relationship between the activation of ionotropic glutamate receptors and RGS4 in the NAc during morphine reward. Additionally, the shRNA-mediated RGS4 knock-down was implemented in NAc/striatal primary-cultured neurons to investigate the role that striatal neurons have in the morphine-induced activation of ionotropic glutamate receptors. The results of this study show that the NAc-specific knock-down of RGS4 significantly increased the behaviors associated with morphine and did so by phosphorylation of the GluR1 (Ser831) and NR2A (Tyr1325) glutamate receptors in the NAc. Furthermore, the knock-down of RGS4 enhanced the phosphorylation of the GluR1 and NR2A glutamate receptors in the primary NAc/striatal neurons during spontaneous morphine withdrawal. These findings show a novel molecular mechanism of RGS4 in glutamatergic transmission that underlies the negative symptoms associated with morphine administration.
