Due to the improvement of the composite resin and esthetic desire of the patient, amalgam restoration has been replaced by composite resin. However, still there are many unsolved problems, for example, technique sensitivity, polymerization shrinkage stress and limited mechanical properties. These factors results in fracture of the restoration and secondary caries of the tooth. Also the use of the dental bonding system should be used for the retention of the restoration. In this paper, I want to talk about the present and the future of the remineralizing component released from dental composite resin to overcome the secondary caries and there possibility in the clinical use.
Oral health projects that cater to the disabled should be more prevailing in order to ensure the maintenance and successful promotion of the oral health of disabled people. 70 public dental clinics that conducted oral health projects geared toward the disabled were examined to get a precise grip on their oral health projects. The findings of the study were as follows: 1. 31 out of 70 public dental clinics investigated(44.3%) were equipped with two or more dental hygienists who were professional human resources in charge of the oral health projects for the disabled. As for the age and disability type of the beneficiaries of the oral health projects, adolescents(74.3%) and people with mental retardation(87.1%) benefited most from the oral health projects. Concerning the most common implementation frequency of the projects, the projects were carried out once to three times a week(62.9%). 2. The most dominant oral disease treatment provided to disabled people was amalgam treatment and resin treatment(68.6%), which were the early dental caries treatment. The most common preventive treatment that was offered to improve their oral health was oral prophylaxis(82.9%). As for reform measures for the oral health projects, education of personnels in charge of the projects and their specialization(58.6%) were most emphasized. 3. Regarding factors related to the preventive oral health projects for the disabled, the implementation of oral prophylaxis and toothbrushing education was linked to the age of the beneficiaries. More oral prophylaxis was offered to teens, and more toothbrushing education was provided to preschoolers and adolescents. The age of the beneficiaries and the number of dental hygienists responsible for the projects had something to do with the application of fluorides. 4. Concerning the relationship of the preventive oral health projects for the disabled to the number of dental hygienists, one of the personnels in charge of the projects, the application of fluorides( 54.4%) and pit & fissure sealing(56.8%) were more prevalent when there were two or more dental hygienists. There was a statistically significant disparity in that regard(p<0.05). The above-mentioned findings illustrated that in order to boost the oral health of the disabled, dental hygienists who are responsible for the oral health projects for the disabled should put ceaseless efforts into fostering their professional knowledge and ability and offering quality service to disabled patients. Every public dental clinic should be equipped with plenty of professional personnels to enlarge the scope of treatment and ensure the efficiency of treatment and the preventive projects.
The price for health service are decided by very complicated process because many of factors are related with them. The RBRVS(resource-based relative value scale) were used to calculate the Korean health service fees including dental fees. This study aimed to compare dental fees of Korea with other countries, such as Japan, Germany, and the US for evaluating the adequacy. Dental fees were categorized as oral evaluation and imaging, dental treatment including restorative, periodontal, and surgical work, and preventive treatment and compared by each country. The official documents about dental fees were collected from Korea, Japan, Germany, and the US. Each fee was presented as their own currency at first. Then they were converted into Korean won (KRW) by applying the market exchange rates at a specific point of time. Finally the fees were adjusted by purchasing power parities (PPPs) which equalize the different currencies. In general, the level of Korean fees were markedly low compared to those of Japan, Germany, and the US. German fees were similar or higher than that of Japan, and the US. The Korean fees were lower than three other countries 1.2~4.1 times for oral evaluation and 2.2~7.3 times lower for panoramic radiography. The endodontic fees of Japan, Germany, and the US were higher 1.8~15.3 times and 4.0~35.9 times for the deciduous teeth extraction compared to the Korean. In Japan the prophylaxis was 3.2 times more priced than the Korean fee. Exceptionally, the fees for re-evaluation, amalgam filling, and scaling were lower priced in Japan than other countries. This study has limitations on the items in definition and contents of dental practices units which were not exactly comparable and differently determined by countries. However, this study is meaningful because it surveyed the price levels to compare four different countries and then applied PPPs adjustment. This finding can be used to develop the dental RBRVs of Korean national health insurance and will contribute to improving the payment systems of health care.

Cracked teeth are a common problem with incomplete fracture that typically shows pain on biting and temperature stimuli. The most common cause of crack is known to be the masticatory accident. Predisposing factors were aging, old restoration, and iatrogenic stress concentration, etc. Accurate diagnosis is needed for detecting the extent of crack and pulp vitality. There are many diagnostic tools like bite test, direct observation with transillumination, dye penetration, and radiographs. Immediate treatment for pain relief and occlusal adjustment would be needed. Composite resin and bonded amalgam showed favorable prognosis. Moreover, cusp protection is reliable permanent treatment for cracked teeth. The purpose of this article is to review the literature about the prevention, diagnosis, and treatment for cracked teeth.
This study analysed the mercury concentration in the hair of 62 students (male : 21, female : 41) of D university in Najusi, Jeon-Nam area. Also, questionnaire, which developed by NIES, were completed by the students. According to the questionnaire, most of the students had a higher intake of meat than that of fish. Also, among the students, there were many smokes and many had dental treatment with amalgam. Based on the gender, female showed 1.024 ${\mu}g/g$ and male showed 0.882 ${\mu}g/g$ of the mercury concentration. This means that female students had higher level of mercury in their hair, but it was not a significant statistic difference. Comparing by the intake frequency of meat, processed food stuffs, protein foods, it shows the more they at the higher mercury concentration was observed. According to the intake frequency of fish, when the number of times they intake increased, the mercury concentration was higher. Under one time was 0.857 ${\mu}g/g$ and twice or more times were 1.152 ${\mu}g/g$, which showed a significant statistical difference (p<0.05). Depending on the Life style, the mercury concentration in the hair was high with workout, outdoor activities, and direct smoking. The mercury concentration in the hair showed no difference for whether they received amalgam treatment but the personal dietary habit and the Living condition affects strongly on the mercury concentration level.
Leakage studies have been performed frequently, since a fluid-tight seal provided by various dental fill-ing materials has been considered clinically important. The leakage of the various root-end filling materials has been widely investigated mostly dye penetration method. These dye studies cannot offer any information about the quality of the seal of a test material over a long period of time The purpose of this study was to evaluate the microleakage of root end cavities in blood contamination filed amalgam, intermediate restorative material(IRM), light cured glass ionomer cement(GI) and mineral trioxide aggregate(MTA) by means of a modified fluid transport model. Fifty standard human root sections, each 5mm high and with a central pulp lumen of 3mm in diameter, were and filled with our commonly used or potential root end fill ing materials after they were contaminated with blood. At 24h. 72h, 1, 2, 4, 8, and 12 weeks after filling, leakage along these filling materials was determined under a low pressure of 10KPa(0.1atm) using a fluid transport model. The results were as follows : 1 MTA group showed a tendency of decreasing percent of gross leakage (20m1/day) in process of time, whereas the other materials showed a tendency of increasing in the process time. 2. At the all time interval, GI group leaked significantly less than amalgam group and IRM group (p<0.05). 3. At the 4 weeks, the percentage of gross leakage in MTA group decreased to 0% thereafter, the low per-centage of gross leakage was maintained in MTA group until the end of the experiment, whereas the percentage in IRM group increased to 100% 4. At the 12 weeks, percentage of gross leakage was significantly low in MTA group(0%), comparison with GI group(40%), amalgam group(90%) and IRM group(100%), but there was no significant difference between latter two materials.
The purpose of this study was to investigate the fracture resistance of crown-root fractured teeth repaired with dual-cured composite resin and horizontal posts. 48 extracted human premolars were assigned to control group and three experimental groups. Complete crown-root fractures were experimentally induced in all control and experimental teeth. In the control group. the teeth (n=12) were bonded with resin cement and endodontically treated. Thereafter, the access cavities were sealed with dual-cured composite resin. In composite resin core-post group (n=12), the teeth were endodontically treated and access cavities were sealed with dual-cured composite resin. In addition, the fractured segments in this group were fixed using horizontal posts. In composite resin core group (n=12), the teeth were endodontically treated and the access cavities were filled with dual-cured composite resin without horizontal posts. In bonded amalgam group (n = 12), the teeth were endodontically treated and the access cavities were sealed with bonded amalgam. Experimental complete crown-root fractures were induced again on repaired control and experimental teeth. The ratio of fracture resistance to original fracture resistance was analyzed with Kruskal-Wallis test. The results showed that teeth in control and composite resin core-post group showed significantly higher resistance to re-fracture than those in amalgam core group (p < 0.05). The resistance to refracture was high in the order of composite resin - post group, control group, composite resin group and bonded amalgam group. Within the scope of this study, the use of horizontal post could be beneficial in increasing the fracture resistance of previously fractured teeth.
The purpose of this study was twofold. One was to evaluate the wettability of 4 elastomeric impression materials on tooth and different kinds of filling materials. The other was to identify the effect of topical surfactants sprayed on the surface of each impression. The elastomeric impression mate rials used in this experimental study were one polyether(Impregum F) and three polyvinyl siloxanes(Provil novo, Zerosil, Imprint). The abutments were prepared for 3/4 crown or onlay on the extracted human first molar. The filling materials used in this study for the duplication of prepared abutment were composite resin, amalgam, and casting metal. Impression was taken by manufacturer's recommendations and the number of voids on the impression surface was counted. The topical surfactants were Spannex $II^{(R)}$ and $Cohere^{(R)}$. The wettability was evaluated by comparing the number of voids between non-treated group and treated groups. The results were as follows : 1. $Zerosil^{(R)}$ showed the least number of voids on the impression surface. The number of voids increased in order of Provil $novo^{(R)}$, Impregum $F^{(R)}$, and $Imprint^{(R)}$. 2. Impregum $F^{(R)}$ and $Zerosil^{(R)}$ showed the least number of voids on the surface of dental stone master cast. The number is inclosed in order of Provil $novo^{(R)}$ and Imprint 3. When abutment material is tooth, the number of voids on the surface of master cast was smallest compared with that of other abutment materials. The number of voids increased in order of casting metal, amalgam and composite resin 4. The number of voids on the surface of the dental stone master cast was smallest when Spannex $II^{(R)}$ was used, followed by $Cohere^{(R)}$ treated group and non-treated group. These results suggest that the difference in wettability caused by the types of rubber base impression materials as well as abutment materials can affect the number of voids. And the use of topical surfactant for rubber base impression materials can improve the wettability of the materials and enhance the accurate master cast which has fewer surface voids.

Packable resins have been introduced in the market with high expectations as an alternative to amalgam. They are characterized by a high-filler load and a filler distribution that gives them a different consistency compared with the hybrid resins. The effect of high filler load on the microhardness and polymerization shrinkage of packable resins was tested. Hybrid resins were also tested to compared with the packable resins. As a result, packable resins showed a much greater microhardness value than hybrid resins. All the tested resins have a correlationship with the microhardness and filler content (vol%). The packable resins showed much less polymerization shrinkage than hybrid resins. The filler content and polymerization shrinkage were inversely correlated in the tested resins.

The spectrum of pediatric dentistry has been changing recently, due to many socioeconomic changes and developments in dental materials. The purpose of this study was to recognize the changing pattern in pediatric dental treatment and to present the direction for pediatric dentistry in the future. Patient distribution and practice trends were reviewed based on the patient records of the department of pediatric dentistry, Seoul National University Dental Hospital, from 2000 to 2005. Results were as follows ; 1. In 2000, preventive treatments comprised 14.5%, restorative treatments 52.6%, surgical treatments 11.4% and orthodontic treatments 21.5%. In 2005, the percentages were changed to 12.1%, 38.3%, 13.7%, and 35.9% respectively, showing a decline in preventive and restorative treatments and an increase in surgical and orthodontic treatments. 2. In restorative treatments, the proportion of amalgam, stainless steel crown restoration and pulp treatments decreased, and that of glass ionomer and resin restoration increased. 3. The number of out-patients increased from 2000 to 2002, and decreased thereafter. 4. In 2005, the average age of patients decreased from that of 2000.
