A retrospective randomized study of success rates according to abutment selection in DENTIS submerged implant with an internal hex connection (DENTIS 내부연결형 서브머지드 임플란트에서 지대주 선택에 따른 성공률의 후향적 연구)

  • PURPOSE. The purpose of this study is to determine the efficacy of the DENTIS submerged-type implant with an internal hex connection and to build corresponding abutment-selection criteria. MATERIAL AND METHODS. A total of 204 patients received submerged implant fixtures with an internal hex connection at the Dong-A University Hospital Dental clinic in Busan from January 2013 and May 2016. Three specific abutments, UCLA abutments, customized abutments, ready-made abutments, were randomly selected. Implant success was defined as the basis of the International Congress of Oral Implantologists(ICOI, 2007) criteria. The relationship between the implant success rate and the abutment factor was analyzed using the Kruskal-Wallis test(P<.05). RESULTS. A total of 508 implants were placed in 204 patients. After a mean observation period of 38.6 months, 493 out of 508 implants were in normal function, yielding an overall success rate of 97.05%. A total of 15 implants failed: 10 in the maxillary molar area, 4 in the mandibular molar area, and 1 in the mandibular incisal area. All of the implant failures occurred in a single-implant prosthesis, especially high in the maxillary molar area. The Kruskal-Wallis analysis showed that abutment selection has no significant correlation with implant failure(P>.05). CONCLUSION. DENTIS submerged implants with an internal hex connection showed predictable results with a success rate of 97.05%. It is no influence on the success rate in the selection of submerged implant abutment with an internal hex connection.

A Finite element stress analysis of abutment screw according to the implant abutment material (임플란트 지대주 재질에 따른 지대주나사의 유한요소 응력 분석)

  • Purpose: The present study was to determine the stress distribution of an abutment screw according to implant abutment material. Methods: This study was a tightening torque 10 Ncm, 20 Ncm, set to 30 Ncm, and a titanium alloy (Ti-6Al-4V), PEEK (polyetheretherketone), Endoligns (60% Carbon Fiber Reinforced PEEK) material of the custom abutment titanium alloy (Ti-6Al-4V) the stress distribution in the material of the abutment screw will be evaluated by the finite element analysis. Results: Abutment screw most stress has been concentrated on the interface between the fixture and the abutment was also part of the interface that the threads are started. Depending on the abutment of the abutment screw Material von Mises stress values are shown differently. 10Ncm T10 under the tightening torque of 294.2 MPa, P10 is 562.8 MPa, appeared to E10 is 295.8 MPa, 20Ncm tightening torque under T20 is 581.1 MPa, P20 is 1125 MPa, E20 was shown to 585.1 MPa, 30Ncm tightening torque under T30 is 918.2 MPa, P30 is 1795 MPa, E30 has appeared 925.1 MPa. Conclusion: If the abutment is used as Endoligns, it was confirmed that the abutment screw exhibits of von Mises stress value is similar to the titanium alloy abutment.

Correlation between microleakage and screw loosening at implant-abutment connection

  • PURPOSE. This study aimed to evaluate the correlation between microleakage and screw loosening at different types of implant-abutment connections and/or geometries measuring the torque values before and after the leakage tests. MATERIALS AND METHODS. Three different abutment types (Intenal hex titanium, internal hex zirconium, morse tapered titanium) with different geometries were connected to its own implant fixture. All the abutments were tightened with a standard torque value then the composition was connected to the modified fluid filtration system. After the measurements of leakage removal torque values were re-measured. Kruskal-wallis test was performed for non-parametric and one-way ANOVA was performed for parametric data. The correlation was evaluated using Spearman Correlation Test (${\alpha}=0.05$). RESULTS. Significantly higher microleakage was found at the connection of implant-internal hex zirconium abutment. Observed mean torque value loss was also significantly higher than other connection geometries. Spearman tests revealed a significant correlation between microleakage and screw loosening. CONCLUSION. Microleakage may provoke screw loosening. Removing torque values rationally decrease with the increase of microleakage.

  • Aesthetic impression is emphasized in the recent field of implant restoration. However, there is limitation of creating proper shape of soft tissue as well as cervical emergence profile due to the use of pre-existing healing abutment in the process of initial post-operative soft tissue healing period. Designing the shape of abutment into the final customized abutment instead of its original shape helped to achieve more aesthetic implant restoration by applying healing abutment which could minimize the malposition and recession of soft tissue. In this study, soft tissue healing was promoted using the post-operative customized healing abutment and thereby obtained the result of more aesthetic and functional restoration by minimizing displacement of soft tissue in the process of applying final customized abutment.

  • Since $Br^{\circ}anemark$ introduced the osseointegrated implants, they have been granted for useful methods for the restoration of oral function. The original $Br^{\circ}anemark$ protocol recommended long stress-free healing periods to achieve the osseointegration of dental implants. However, many clinical and experimental studies have shown that the osseointegration is no wonder in almost cases and that early and immediate loading may lead to predictable osseointegration. So we are willing to introduce the Satellite Abutment newly invented for immediate loading. We think that it will make the occlusal forces dispersed to surrounding bone and that we can restore the oral function immediately after implant installation not disturbing osseointegration. In case of using Satellite abutment, stress concentrated to bone contact area of implant was distributed not only fixation plate and screws but also superior, middle portion of implant and cortical layer of jaw bone. It was clearly decreased on the bone contact surfaces around dental implants. 1. Stress was decreased more than 76.5% when satellite straight abutment was used. 2. Stress was decreased more than 50% when satellite angled abutment was used. 3. The stress around dental implant was well distributed along the cortical bone surface and the fixation plate and screw. This study concludes that satellite abutment can be used as all immediate loading implant prothesis because it was possible to distribute periimplant occlusal stress through implant contact bone surface and cortical layer of jaw bone.

A Study on the Screw Loosening Torque According to the Type of Tightening the Implant Fixture and Abutment (임플란트 고정체와 지대주의 체결방식에 따른 스크류 풀림토크에 관한연구)

  • Purpose: In this study, the loosening torque test was conducted with three implant products that are produced, approved and sold in Korea, which are manufactured in different fixture and abutment tightening methods (internal submerged type, internal morse taper type, and external type) to examine the loosening torque of the screw according to the method of tightening the implant fixture and abutment. Methods: In the loosening torque test, the three types of fixtures and abutments with different tightening methods were tightened by rotating them clockwise with a $30N{\cdot}cm$ force using a driver equipped with an electric torque meter. The results of the test are as follows. Results: The loosening torque values of the internal submerged type, internal morse taper type and external type implants were $24.10{\pm}0.742N{\cdot}cm$, $29.10{\pm}1.710N{\cdot}cm$, and $26.60{\times}1.636N{\cdot}cm$, respectively. Conclusion: The screw loosening torque values of the three fixture and abutment tightening methods were analyzed via Kruskal Wallis test layout, and they were significantly different (p<0.05).

  • Statement of problem : Several domestic dental implants have been developed since dental treatment being implants are increasing and popularized. However, they have not been used in domestic market like imported goods. Material and methods : This study was carried out to compare domestic and imported implants in aspect of fitness, dental implant fixture and abutment screw using scanning electron microscope. All experiments were performed under dry condition. Results : 1. Only in aspect of relation of dental implant fkxture and abutment screw, except only one group with point contact, good fitness was existed. 2. Home products must presevere in their efforts, so as excellent to fit.

Prosthetic restorations with cementless screw-retained implant systems: a case report (시멘트리스 나사 유지형 임플란트 시스템을 이용한 보철물 수복 증례)

  • This study aims to describe the clinical experience of single and bridge crowns fabricated using a cementless screw-retained implant prosthesis system. In the case of single crown (#37), regular link (HDL) was used, and bridge crowns (#15~#24), (#26~#27), (#17~#14) (#24~#26) were fabricated by selecting regular link and short link considering the vertical height. One abutment was hex shaped to ensure that it could be mounted while preventing insertion and prosthesis rotation. The advantages of cementless implant prosthesis include shorter chair time and periodic care, strong retention with LINK abutment, safety from inflammation, bacterial infection, and complications due to peri-implant cement, and high patient satisfaction. Dentists should double-check the position of the implant fixture and dental technicians should continuously manage the fit of the link and prosthesis with digital equipment to reduce screw loosening and fractures.

Evaluation on machining accuracy according to convergence angle and radius of curvature value used for fabricating custom abutments (맞춤형 지대주 제작에 사용되는 수렴 각과 곡률 반경의 값에 따른 가공 정확도 평가)

  • This study evaluates the machining accuracy of the custom abutment design according to the selected convergence angle and radius of curvature value in the CAD program. Ten custom abutments were designed based on dental CAD. And then, the fabricated custom abutment was scanned ten times using a contact scanner. The data of the scanned custom abutment was saved as "Test STL" file. The Geomagic studio software was used to superposition each exported as an "Test STL" file with the CAD-reference-model STL file (CRM) specified by the same name. In the experimental results, the A8 group (convergence angle $8^{\circ}$) showed lower error than the A4 group (convergence angle $4^{\circ}$) . In addition, the higher the radius of curvature, the less error in the top and chamfer regions of the custom abutment (p< 0.05). Overall, the convergence angle and radius of curvature value in the custom abutment design were found to affect the machining accuracy.
