The Current Status and Medical Fee Propriety of Psychotherapy and Neuropsychological Test for Dementia in Korean Medicine (한방정신요법 및 치매 검사의 현황, 수가 적절성 연구)

  • Objectives: A large number of patients require psychiatric therapy. We attempted to determine the present situation regarding psychotherapy and neuropsychological tests for dementia in Korean medicine for the benefit of the Health Insurance Review and Assessment Service (HIRAS). The aim of this study was to aware of the current status about psychotherapy and neuropsychological test for dementia in Korean medicine. Methods: We searched the medical practice records for psychotherapy and neuropsychological tests in oriental neuropsychiatry between 2009 and 2013 using the Health Insurance Review and Assessment Service (HIRAS) database. The search categories were: IJeongByunGi (Medical practice code:59001), JiUnGoRoen (59002), Kyungjapyungji (59003), OhJiSangSeung (59004), neuropsychological test for dementia (29005). Results: 1. The number of patients treated with Korean Medical Psychotherapy increased annually by 151%. The total number of patients treated with Korean Medical Psychotherapy was 4,289 in 2013. 2. The total cost for patients treated with Korean medical Psychotherapy in the public health medical insurance budget was 268,032,000 won in 2013. The average medical cost for one therapy was 17,000 won in 2013. 3. The number of patients in local clinics is increasing faster than the number in Korean medical hospitals. 4. The age group between 20~30 years of age, for both men and women, is the group with the greatest density in Korean Medical Psychotherapy. 5. Neuropsychological Testing for Dementia in Korean Medicine is slowly decreasing. Conclusions: The prevalence of mental illness in Korea is increasing, therefore, the demand for Korean Medical Psychotherapy has increased recently. Authorizing Korean Medical psychiatrists to utilize Korean mental Health resources is essential. This study could be helpful in understanding the current status for the purpose of expanding Korean Medical Psychotherapy.

  • As the demand for slim laptops requires low-height optical disc drives, vibration problems of optical disc drives are of great concern. Additionally, with the decrease of a track width and a depth of focus in high density drives, studies on vibration resonance between mechanical parts become more important. From the vibration point of view, the performance of optical disc drives is closely related with the relative displacement between a disc and an objective lens which is controlled by servo mechanism. In other words, to read and write data properly, the relative displacement between an optical disc and an objective lens should be within a certain limit. The relative displacement is dependent on not only an anti-vibration mechanism design but also servo control capability. Good servo controls can make compensation for poor mechanisms, and vice versa. In a usual development process, robustness of the anti-vibration mechanism is always verified with the servo control of an objective lens. Engineers partially modify servo gain margin in case of a data reading error. This modification cannot correct the data reading error occasionally and the mechanism should be redesigned more robustly. Therefore it is necessary to verify a mechanism with respect to the possible servo gain plot. In this study we propose the experimental verification method for anti-vibration mechanism with respect to the existing servo gain plot. Thismethod verifies axial vibration characteristics of optical disc drives on the basis of transmissibility. Using this method, we verified our mechanism and modified the mechanism for better anti-vibration characteristics.

Effect of Roughness and Densification of Precalendered Sheet on Surface Roughening

  • To meet the ever increasing quality demand of coated papers and duplex boards, the local gloss variation and print unevenness must be avoid. To do this, surface roughening phenomenon which affects local gloss and print unevenness was investigated. In this study, the effect of roughness and densification of sheet obtained at various calendering conditions on surface roughening was examined. To evaluate the calendering effect properly we introduced a new value of roughening index by moisture for evaluation of surface roughening. Caliper and roughness of sheet decreased with increasing of calendering temperature and pressure, and the remarkable reduction of those properties occurred at the temperature around Tg. Roughening index increased when the amount of water increased and calendaring temperature and pressure increased. And as the sheet became denser by calendering, the roughening index increased to some extent. But roughening index by moisture is the lowest for sheet precalendered at higher temperature condition. When the surface of calendered sheet is rougher, there is more roughening of surface. The severe condition of calendering made the roughening index by moisture small.

Changes in Phytoplankton Community Structure by Freshwater Input in the Cheonsu Bay, Korea (담수 유입에 따른 천수만 해역의 식물플랑크톤 군집 변화)

  • Environmental factors and changes in phytoplankton community structure before (August 5, 2017), during (August 18 and 25) and after (August 30 and September 15) freshwater input were analyzed to investigate the effects of freshwater input from Ganwol and Bunam lakes located in the upper part of Cheonsu Bay. Due to the large amount of freshwater input in the Cheonsu Bay, the surface salinity of the bay decreased by more than 8 psu, and the thermocline existing in the bay during August weakened. In addition, hypoxic phenomena occurred temporarily in the bay as the low oxygen water mass from the freshwater lakes flowed into the bay, and chemical oxygen demand, nutrients, and N/P increased with freshwater inflow. The density of phytoplankton during the freshwater inflow increased owing to their input from the freshwater lakes. Diatom species (Eucampia zodiacus) dominated the phytoplankton community in the bay before freshwater input; nanoflagellates, chlorophyta, cyanobacteria, and diatoms (Pseudonitzschia delicatissima, Chateocceros spp.) entered during freshwater input; and after freshwater inflow ended, diatoms (Chateocceros spp.) again became predominant indicating a return to previous conditions. The amount of phytoplankton standing crops increased sharply due to the inflow of freshwater species into the bay on the second day of discharge compared to before freshwater input; pre-discharge conditions were restored at most stations except at some sites close to the Bunam Lake three days after discharge. Therefore, the large amount of freshwater flowing into the bay affects not only the geochemical circulation in the bay but also the phytoplankton community structure. In particular, the high concentration of nutrients in the freshwater lake affect the marine ecosystem of the bay during August.

Development of Regional Balanced Index for Administrative Districts and Dongs in Seoul using Factor Analysis (요인분석을 활용한 서울시 자치구/행정동의 지역균형지수 개발)

  • Purpose: Regional gaps and conflicts between regions due to Korea's economic development and industrialization have become important issues, and the issue of balanced regional development at regional level has been discussed as the size of the region has increased recently. Although evaluation of regional balance was attempted through various regional balanced development indexes, it is inappropriate as a standard for determining regional balance in Seoul. Therefore, this study aims to develop objective evaluation methodologies and evaluation indicators for balanced development of administrative districts in Seoul, not existing city and national units. Methods: We looked at existing regional balanced development indexes, and suggested a new regional balanced index reflecting regional development, backwardness, and spatial characteristics in Seoul using factor analysis. Results: As a result of factor analysis, the regional balanced development index for administrative districts and administrative dongs consists of two factors (regional revitalization, financial power) and three factors (commercial density, social security demand, regional retardness), respectively. Then the regional balanced development index scores for 116 administrative districts and 423 administrative dongs are calculated by multiplying each factor by a weight obtained through experts' survey. Conclusion: The proposed regional balanced development index can be used as an objective and quantitative basis for regional balanced development within a city. Further research may include continuously adding new indicators that reflect the direction and scale of development.

Analysis of Electroencephalogram and Electrocardiogram Changes in Adults in National Healing Forests Environment

  • This study analyzed the changes in Electroencephalogram(EEG) and Electrocardiogram(ECG) depending on the healing environment in order to find a way to improve the forest healing program based on the healing environment in response to the demand for qualitative improvement of the program since the program is a charged service. This study selected eight sites running forest healing programs at four national healing forests (i.e., Saneum, Cheongtaesan, Daegwanryeng, and Jangseong) - two routes per national healing forest - considering forest environments. This study chose NUMBER standard sampling plots ($20{\times}20m$) and measured three atmospheric environment items, seven physical environment items, two soil environment items, and eight vegetation environment items including forest sound and anion at each plot to evaluate physiological changes in it. EEG and ECG, which have been widely used in forest healing evaluation, were utilized as criteria. Seventy three subjects were selected with taking the age, drug, caffeine, smoking, and the time of last meal into consideration. As a result, EEG changes were correlated with three atmospheric environment items, six physical environment items, one soil environment item, and two vegetation environment items. ECG changes were significantly correlated with two atmospheric environment items, six physical environment items, two soil environment items, and two vegetation environment items (p<.05). It is expected that 11 environmental factors such as temperature, density, and altitude affecting EEG (e.g., alpha balance and gamma balance) and ECG (e.g., HRV mean) could be used as effective tools in developing more differentiated programs for improving healing effects.

A Study on Integrated Operation of School Bus in Suburbs (교외지역 통학버스 통합 운영 방안 연구)

  • Purpose: Generally, since the population density is lower in suburban areas, the distance to school is inevitably long. Therefore, schools in suburban areas often operate school buses to improve student welfare. However, since school buses are usually used only during going to and from school, the utilization rates are relatively low. Therefore, this study aims to establish integrated operation plan of public school bus covering all schools. Methods: It is necessary to decide which school buses will serve the specific demand node which want to go to certain school in order to design an integrated operation plan for school buses. Therefore, a mathematical model is developed for minimizing the total number of vehicles and the distance of transportation by reflecting the characteristics of school buses and students as constraints. To solve the proposed mathematical model, CPLEX, a commercial solver, is applied. Results: To validate and to confirm the proposed process, numerical example is designed with the comparison between before and after integrated operations of school buses in terms of total operation cost. The result shows that the integrated operation can lead the reduction of the number of school buses as well as the decreasing of the fuel cost. Conclusion: This study provides the quantitative method to perform the integrated operation of school buses in suburban areas. The optimal operation strategy is required because there are more complex decision-making elements considering the integrated operation. It is expected to apply this research result at real situation to expand this services based on an optimal operation.

Large-Scale Production of Cronobacter sakazakii Bacteriophage Φ CS01 in Bioreactors via a Two-Stage Self-Cycling Process

  • Cronobacter sakazakii is an opportunistic pathogenic bacterium found in powdered infant formula and is fatal to neonates. Antibiotic resistance has emerged owing to overuse of antibiotics. Therefore, demand for high-yield bacteriophages as an alternative to antibiotics has increased. Accordingly, we developed a modified mass-production method for bacteriophages by introducing a two-stage self-cycling (TSSC) process, which yielded high-concentration bacteriophage solutions by replenishing the nutritional medium at the beginning of each process, without additional challenge. pH of the culture medium was monitored in real-time during C. sakazakii growth and bacteriophage CS01 propagation, and the changes in various parameters were assessed. The pH of the culture medium dropped to 5.8 when the host bacteria reached the early log phase (OD540 = 0.3). After challenge, it decreased to 4.65 and then recovered to 4.94; therefore, we set the optimum pH to challenge the phage at 5.8 and that to harvest the phage at 4.94. We then compared phage production during the TSSC process in jar-type bioreactors and the batch culture process in shaker flasks. In the same volume of LB medium, the concentration of the phage titer solution obtained with the TSSC process was 24 times higher than that obtained with the batch culture process. Moreover, we stably obtained high concentrations of bacteriophage solutions for three cycles with the TSSC process. Overall, this modified TSSC process could simplify large-scale production of bacteriophage CS01 and reduce the unit cost of phage titer solution. These results could contribute to curing infants infected with antibiotic-resistant C. sakazakii.

Electrocoagulation of Disperse Dyebath Wastewater: Optimization of Process Variables and Sludge Production

  • This study was conducted to investigate the effect of initial pH, current density, and electrolysis time on process performance in terms of decolorization and chemical oxygen demand (COD) removal from disperse dyebath wastewater (DDW) by mono-polar parallel laboratory scale electrocoagulation (EC) process. COD reduction of 51.3% and decolorization of 92.8% were obtained with operating cost of 0.19 €/㎥ treated wastewater for Al-Al electrode pair, while 90.5% of decolorization and 49.2% of COD reduction were obtained with operating cost of 0.20 €/㎥ treated wastewater for an Fe-Fe electrode pair. The amount of sludge production were highly related to type of the electrode materials. At the optimum conditions, the amount of sludge produced were 0.18 kg/㎥ and 0.28 kg/㎥ for Al-Al and Fe-Fe electrode pairs, respectively. High decolorization can be explained by the hydrophobic nature of the disperse dye, while limited COD removal was observed due to the high dissolved organic matter of the DDW based on auxiliary chemicals. Energy, electrode, and chemical consumptions and sludge handling were considered as major cost items to find a cost-effective and sustainable solution for EC. The contribution of each cost items on operating cost were determined as 10.0%, 51.1%, 30.5% and 8.4% for Al-Al, and they were also determined as 9.0%, 38.0%, 40.5% and 12.5% for Fe-Fe, respectively. COD reduction and decolorization were fitted to first-order kinetic rule.

A Study to Set up Guideline for Public Facilities as Infrastructure of Low-rise Residential Community in Seoul (서울 저층주거 밀집지역 공공생활지원시설 설정방향에 관한 연구)

  • Low-rise residential community is the most popular type (51%) of residential neighborhoods in Seoul. Currently, there is a shortage of public facilities needed for living conditions and the quality of life in low-rise residential areas. This study defines 'public facility' as infrastructure to improve the living environment and used by residents jointly in low-rise residential areas. In this regard, this study analyzes current legal and institutional standards, latest trends in public facility of apartments, and residents' demand and satisfaction level in order to find out the criteria for installation of priority public facilities. As a result, the essential facility basically conforms to the number of household which is the standard of the facility supply in apartment. However, considering the limitations on the accessibility due to low density, it should consider two standards at the same time: the number of household and distance (radius of neighborhood). In conclusion, it is necessary to install legally prescribed facilities according to the number of household and distance: 500 households facilities in 250m radius neighborhood and 1,000 households facilities in 400m radius neighborhood. Also, considering the reality of low-rise residential area, it is necessary to integrate some facilities that can be functionally linked to improve level of utility and efficiency of operation and management. It is expected that the output of this study can be applied to institutionalize of the legal basis for the public facility of low-rise residential community.
